Achieving Accreditation 2009 saw a huge achievement for the hospital in securing JCI accreditation. Maintaining our accreditation in 2012 has shown our commitment to quality patient care and safety.

The Team The Best Practice team is made up of experienced clinical personnel with a wealth of knowledge in quality and safety. Sarah O’Sullivan explains that this experience guarantees international standards are adhered to and exceeded. “Our aim is to guarantee patients recieve a quality service in a safe environment, complimented by strong team work and the continuous professional development of all our staff, we feel that this provides constant reassurance to patients.”

Quality & Risk Management Through numerous quality initiatives around the hospital, staff are reminded that quality and patient safety are their responsibility and Louise Heffernan says that the team’s job is to keep the hospital at a consistent ‘audit-ready’ level. “There are always challenges when it comes to ensuring standards are met. We conduct audits in different criteria to ensure preparedness in areas such as risk management and infection control,” she says. Risk management in terms of patient care is of paramount importance in Barringtons. “Our approach is very much leadership driven with strong lines of communication between all

departments in the hospital,” adds Eileen Hayes. “This ensures we take into account the entire patient cycle. This has an impact on both patient treatment and patient satisfaction.”

Aims Patient safety is achieved with a consistently proactive approach, from the audits and surveys that are conducted to education days and other quality initiatives rolled out across the hospital. Strong team work from all facets of the hospital ensures that patient safety is everyone’s goal and this can be seen from audit results. The hospital has just secured accreditation from the Irish National Accreditation Board (INAB) for the Laboratory which represents procedures in accordance with EU and other international standards.

P&C activities are co-ordinated by the IP&C team which consists of disciplines from Theatre, Laboratory, Hygiene Services, Consultant, Nursing and Specialist Nursing in the field of IP&C, the team is further supported by the IP&C committee with representation from Accreditation/ Best Practice, Risk Management, Central Sterile Supply Department (CSSD), Procurement and Facilities Management.

The aim of the IP&C team is to ensure the effective delivery of the infection prevention and control programme within the hospital, acting as a source of specialist advice to all disciplines and to support their endeavours in the prevention and control of health care associated infections.

Policy/Procedure/Guidelines in relation to IP&C activities are in place and are in accordance with national and international best practice.

The team, supported by the committee, have a comprehensive programme in place which includes such activities as: Staff Education: There is a mandatory training programme in place for all new staff on induction and yearly thereafter. This year the team co-

Audit: There is a comprehensive audit programme in place throughout the hospital in key areas like the safe use and disposal of sharps, the appropriate segregation of healthcare waste and hand hygiene.

Surveillance of healthcare associated infection is undertaken in areas such as Surgical Site Infection, Blood Stream Infection and Intravascular Device Related Infection. A root cause analysis is always undertaken. What is important is while we realise that infections cannot be prevented completely, is that we do everything that is possible to prevent the spread of infections.

arringtons Hospital pharmacy service is provided in conjunction with Ballinacurra Late Night Pharmacy. We provide a pharmaceutical service, the aim of which is to achieve excellence in the pharmaceutical care of patients by ensuring the safe, effective and economic use of drugs. As pharmacist for the hospital I am heavily involved in the implementation of pharmaceutical policies which have been integral in the achievement of Joint Commission International Accreditation. The accomplishment of Barringtons Hospital achieving the JCI accreditation is reflected by a consistent quality service and practice in all services that the hospital provides and as pharmacist I work within this established framework. I am available round the clock to provide advice and training to clinical staff on dosage, side-effects and interactions relevant to the use of medication in the hospital. I provide a procurement service for difficult to source medicines and imported medicines. As pharmacist I also review the prescriptions of patients and monitor this during and throughout the course of their treatment. My primary concern is patient safety so my work also involves analysing a patient’s medical history and tracing any medications that they have previously been prescribed. Medication

is also reviewed in terms of appropriateness for the patient’s condition, taking into account the frequency, dosage and method of administration to ensure the patient benefits to the maximum from any prescriptions. When required I interact with the patient and explain the need for various medications. The pharmacy staff work closely with other members of the multidisciplinary team to ensure that patients receive optimal pharmaceutical care while in the hospital and after discharge. There is a dedicated medication management committee which meets regularly to review drugs being used in the hospital along with new drugs which are to be added to the hospital formulary. Regular audits are carried out on the ordering, dispensing, storage, prescribing and administration of drugs in the hospital. There is constant monitoring of drug use in Barringtons with the primary aim of improving the outcome of treatment and the quality of life of patients who attend the hospital. The pharmaceutical expertise on hand at Barringtons Hospital is another example of the hospital’s dedication to providing a service which meets and exceeds the expectations of patients.

he Laboratory facilities at Barringtons are an excellent barometer of the ambition of the hospital and the skills of its highly trained staff. Utilising the latest technology in a modern environment, the laboratory provides a comprehensive and ever expanding range of services for the hospital and its patients. The lab at Barringtons offers a full, multidisciplinary facility which provides the following primary services to the hospital and its patients: • Histopathology • Haematology • Biochemistry • Serology

• Microbiology Medical Scientist Pamela Colbert has been working at Barringtons hospital for twelve years and has been manager of the laboratory since 2003. “The laboratory has a strong working relationship with all other medical professionals within the hospital. One of our primary roles is in pre and inter-operative assessments. We then conduct post-operative blood sampling to ensure satisfactory surgical outcomes. Our histology and pathology services are also widely used, in all cases where tissue samples are taken. We are also the only laboratory providing andrology (semen) testing in the MidWest region.” The laboratory provides a blood sampling service following referrals from GP’s nationwide. The laboratory has expanded to a large extent over the past decade, now employing seven people to deal with the hospital’s increasing caseload. Blood testing alone has increased from 20 patient samples a week to over 100 per week. “Our histopathology services have also increased due to the increase in patient volume and the more advanced procedures now being carried out at the hospital. The laboratory has also just been awarded ISO 15189 accreditation by the Irish National Accreditation Board (INAB). INAB is the national body with responsibility for the accreditation of Testing and Calibration laboratories, ISO 1702, ISO 15189.

Nursing at Barringtons Director of Nursing at Barringtons, Helen Power, explains her role in the hospital and vision for the future.

B

eginning life with Barringtons Hospital as a staff nurse, Helen Power later became the Theatre Manager before promotion to ber current position as Director of Nursing. “I’ve seen a lot of changes over the years,” she explains. “When I started eighteen years ago we only had seven beds and we were only operating three days a week. Now we have forty four beds and thirty five of the seventy three consultant specialists attending are surgeons providing elective surgical, diagnostic procedures and treatments under thirteen specialties – the place has grown phenomenally over the years. It is fantastic to be a part of the continual growth here at Barringtons Hospital.” The overarching philosophy at the hospital, which drives all of the staff, is the provision of high quality care. We aim to provide a comprehensive personalised service appropriate to our patient’s individual health care needs,” she says. As Director of Nursing, patient care and the delegation of her nursing team falls under Helen’s remit of responsibility as she works on the frontline of care. Patients feedback, Helen believes, is what shows that Barringtons is a first-class hospital. “We have a very good patient rating,” she says. “We have a patient satisfaction questionnaire which is given to every patient on the day of their procedure. Each area is touched upon and that’s the best way to improve – through patient feedback.” In tandem with patient care is the philosophy of efficiency. One such example of this, Helen says, is the endoscopy unit. GPs can refer a patient for an endoscopy utilising a direct access booking system, ensuring

they are completely prepped with the result of minimal fuss when they present at Barringtons. GPs form a large part of Helen’s work; informing them about the services and diagnostics we provide and liaising with them regarding urgent patient results and referral pathways. “We have open GP nights,” she says. “We might have a group of new specialists that have joined so we would invite the GPs in to a medical meeting, the new specialists would talk on an area that would be applicable to primary care and it is a great opportunity to get to know the new consultants personally.” Taken on a walkthrough of the hospital, these GPs can get to know the facility and any new services and developments. “They know then where they are sending their patients,” says Helen. As with any other high level institution, Barringtons Hospital is not content with resting on its laurels, and has big plans for the future. Besides developing the building adjacent to the hospital and the construction of a new endoscopy unit, operating theatres and increasing bed capacity, Helen envisages a future where Barringtons works hand in hand with the public service, being part of an integrated health care system within the Mid west region, offering the use of their elective surgery capabilities to relieve some of the huge strain the public hospitals in Ireland are under. “If we can help take the pressure off, that’s great” says Helen. “We’re all working together in providing better access to care at a higher quality standard for the people of the Mid-West and surrounding areas.”

Here are five very important questions for patients to ask their insurance company: • What is the start date of my cover? • What is the name of my plan? • Do waiting periods apply to my policy at the moment? • Is there an excess/shortfall payable by me to the hospital on the day of my treatment? • Am I covered for my treatment ? For example, I am having Orthopaedic Surgery in Barringtons in two weeks time, am I covered for this treatment at this time? This amount will be verified by our Admissions Team and/or your insurance provider for you. The hospital will accept cash, cheque, laser and visa as payment options. In most instances health insurance patients are covered in full for their hospital treatment and consultant care, meaning that your treatment will be paid by the provider and there will be no additional cost to you.

r Paul O’Byrne is passionate about what Barringtons Hospital means in terms of providing a new level of healthcare for patients in the Limerick and Mid West region. He was instrumental in the reopening of Barringtons Hospital and its establishment as the only independent medical facility in the Mid West region in 1994. aving trained on the Halsted Surgical Residency at the John Hopkins Hospital in Baltimore US, in the UK and on the Higher Surgical Training Scheme, Mr Raphael Keane was appointed as a surgeon to St. John’s and the Regional Hospitals in Limerick in 1990. fter more than 20 years working in the public health system in the Mid-West area, Mr John Drumm, along with his colleague Mr Imtiaz Ahmed, are focusing on developing the urology services at Barringtons. “We are working on the development of a comprehensive urology service here at the hospital. As the main private hospital in the region and we’re hoping to build upon the reputation of Barringtons and provide a wider range of services.” In terms of the procedures at the hospital, Mr Drumm and Mr Ahmed focus on the following areas: • Prostate Pathology (benign and neoplastic) • Urinary tract infections • Bladder problems including neoplastic and overactivity • Renal Calculi • Female urinary incontinence • Sexual health problems “The day care model is already very well established here and we now have overnight facilities for more comprehensive or advanced procedures, which, in terms of urology is very much how we are seeking to develop the service. We work closely with our colleagues in the MWRH and hold regular multidisciplinary conferences with them to discuss cases and to plan the best course of treatment for the patient,” Mr Drumm adds. The waiting times for cases at Barringtons Hospital are measured in weeks and everything is geared toward providing

r Dartee has worked at Barringtons since 1998, and he has seen the hospital develop into the multi-disciplinary facility it is today. “When I started working in Barringtons I was the only Orthopaedic Consultant working in the hospital. I think it’s wonderful to see where the hospital is today and indeed where it can go in the future, it provides an excellent service to the people of Limerick and Mid-West region. Mr Dartee is one of five orthopaedic surgeons currently working at Barringtons hospital. His colleagues are Mr Maha Lingam, Mr Dermot Farrell, Mr Breandán Long and Mr Michael Gilmore. “One of the strongest aspects of Barringtons Hospital is the strong working relationships between the departments. For example, I can consult and discuss cases with my colleagues in the radiological department during the course of a patients treatment, the whole system is designed to move at a pace which benefits the patient. We also work closely with our physiotherapy colleagues in particular.” Mr Dartee’s particular specialty is in relation to problems concerning the hips, knees and ankles and he also focuses on sports injuries. “Some

of the common procedures we would undertake here include arthroscopy of the knee, shoulder and ankle. Arthroscopic surgery is a minimally invasive procedure which allows the surgical team to perform keyhole procedures on our patients. We also provide treatment for anterior cruciate ligament injury, cartilage and meniscus problems and treating foot deformities problems like bunions. At present we do not carry out more advanced orthopaedic procedures such as hip replacements, but we are optimistic about the development of a new laminar flow orthopaedic operating theatre, which will allow such procedures to be undertaken.” A laminar flow operating theatre means that the air which is

here are over 200 distinct rheumatologic conditions that can present and while every case that presents is different, the most common conditions that we treat would be predominantly arthritis, such as osteo-arthritis, psoriatic arthritis and rheumatoid arthritis. I would also encounter and treat conditions such as dupuytrens contracture, lupus and I would also provide treatment for those suffering from sports injuries,” explains Dr El Rafie. The reputation of the rheumatologic treatment facilities and services at Barringtons has grown and clinics specialising in sports injuries in the Mid-West region now frequently refer patients to the hospital for further treatment.

Dr El Rafie adds that along with hospital management, he and his fellow medical professionals are constantly seeking ways to streamline and improve the experience for the patient at Barringtons hospital. “There is very good communication here at the hospital and if necessary, a patient can be moved between departments should their condition be diagnosed as requiring a different procedure. There are also regular multi-disciplinary conferences here at the hospital between the different consultants and this provides for discussion on any difficult or challenging cases. Further possible developments at the hospital would include a 5-day ward, in which patients would be admitted and receive the full range of treatments necessary during the course of their stay. We are constantly seeking to refine and improve what we do here in line with the hospital’s mission statement and the ever changing nature of modern healthcare.”

Dr El Rafie is very happy to be able to provide a rapid-access clinic for patients, working off a very short waiting list which normally does not exceed two weeks. “The structure of Barringtons hospital means that I can focus on treating cases and it allows me to see more patients during my twice weekly clinics.” As one of the longer serving medical professionals at the hospital, he has seen the facilities and services at the hospital expand and indeed he has been directly involved in one of the impressive recent additions to the hospital. The hospital has recently opened a dedicated IV infusion facility for patients suffering from conditions such as osteoarthritis or rheumatoid arthritis. “The infusion process provides very effective treatment and relief for patients suffering from arthritic conditions. It is a more cost effective way to provide the treatments also. The IV infusion clinic has been over two years in development and without doubt it’s a very impressive addition to the hospital.”

MF surgery is unique in requiring a dual qualification in medicine and dentistry, and is often seen as the bridge between medicine and dentistry. It treats conditions that require expertise from both backgrounds, such as head and neck cancers, facial skin cancers, salivary gland diseases, facial disproportion, facial pain, temporomandibular joint disorders, impacted teeth, cysts and tumours of the jaws as well as numerous problems affecting the oral mucosa, such as mouth ulcers and infections.

The Service There has been an Oral and Maxillofacial Service in Barringtons Hospital since 2004. This is currently provided on a daycase basis. There is a regular outpatient and review clinic. It is provided by Mr Michael Gilbride FDS, FFD, FRCSI, FRCSI(OMFS), Consultant OMF Surgeon. He is also a Consultant Surgeon at the Midwest Regional Hospital and this provides a link for more complex surgery.

• Dentoalveolar surgery is treatment of the teeth and jaws. This includes cysts, tumours, abscesses and infected teeth, particularly wisdom teeth. • Facial Skin Cancer • Head and Neck Cancer - the surgical treatment of tumours affecting the head and neck, from removal of the tumours to subsequent reconstruction. Ms Sylvia Jungkim is a Consultant Ophthalmologist at Barringtons Hospital where she works alongside three other specialists; Mr Sean Chen, Mr Raymond Niland and Mr David Kent, to deliver excellent ophthalmological care.

“A

t Barringtons we provide a service focusing on general ophthalmology. My own area of speciality is in the treatment of glaucoma and cataract surgery,” she explains. “At Barringtons we can focus exclusively on providing treatments and procedures for our patients who are assured of ease of access, rapid consultation and treatment.” Cataract surgery, one of the areas in which Sylvia specialises, has been the subject of considerable advances in terms of both procedures and technology. Changes in techniques performed during procedures, such as making smaller incisions, make recovery times much swifter and advances in technology complement this to ensure safer procedures, better results and quicker recovery. Imaging Department at Barringtons Hospital Having worked at Barringtons for over 22 years, radiologist Dr Alex Stafford is well positioned to appreciate the expansion in facilities and services which the hospital has undergone in that time-frame.

MRI: The hospital’s MRI centre is a bright and accessible facility which provides rapid access to patients. MRI is a proven and established technology, in operation for almost forty years, which utilises magnetic technology, radio waves and computer imaging to form pictures of the patient’s body. The images form via the magnet emitting radio signals, which interact with the hydrogen in the human body, much of it being water. The computer then translates this data into images. MRI is particularly useful for musculoskeletal, abdominal, neurological and vascular imaging. MRI scans are very safe and the lengths of time scans take depend on the part of the body which is being scanned.

The most common cases treated at the hospital include: • Eczema • Psoriasis • Lesions • Acne Some of the other treatments at the hospital include excisions in the cases of skin cancer, phototherapy and patch testing.

Phototherapy This light-based treatment is particularly effective in the treatment of Guttate and plaque psoriasis resistant to topical therapy. The treatment involves exposing the skin to ultraviolet light three times

a week for 6-8 weeks under medical supervision. Phototherapy treatment at Barringtons is done in a dedicated phototherapy suite at the hospital, using the latest technology.

Patch Testing A patch test is a method used to determine if a specific substance causes allergic inflammation of the skin. Any individual with eczema suspected of having allergic contact dermatitis and/or atopic dermatitis needs patch testing. Patch testing helps identify which substances may be causing a reaction in a patient. It is intended to produce a local allergic reaction on a small area of your back where the diluted chemicals were planted. The chemicals included in the patch test kit are the offenders in approximately 85-90 percent of contact allergic eczema and include chemicals present in metals (e.g. nickel), rubber, leather, hair dyes, formaldehyde, lanolin, fragrance, preservative and other additives. “All dermatology cases are seen on a day case basis and we have a very rapid consultation and treatment programme for patients,” says Dr Khan. “I review all patients subsequent to procedures to ensure there are no complications and I offer a same day service to my patient’s if there are any questions which they might have. If I feel there is a need for a patient to be seen by other specialty team in relation to any condition, we work with our colleagues in Barringtons Hospital to ensure the patient receives the care required.”

Gynaecological services at Barringtons Hospital offer an increasing range of procedures to its patients, explains Consultant Surgeon, Dr Cathy Casey.

r Casey has been providing gynaecological services at Barringtons Hospital for over four years, providing day case procedures and investigations to patients. “We provide a very efficient service in terms of day case procedures for patients here at Barringtons Hospital,” says Dr Casey. “Primarily our work here is for investigation and treatment of the vast majority of gynaecological conditions other than complex cases.” At Barringtons Hospital, gynaecological patients are assured of first class procedures, carried out by extensively qualified personnel in a caring environment. “One of the great strengths of Barringtons Hospital is that in the course of my treatment of patients I can call upon a host of other disciplines and areas of speciality, should that be necessary,” says Dr Casey. “The fact that the hospital has developed excellent imaging facilities is of particular benefit, giving me access to image guided diagnostics and investigations. This provides access to a far broader range of procedures for my patients.” Dr Casey adds that it is not just access to technology that has benefits for the cases she treats. “I also have the benefit of the knowledge

and expertise of my clinical colleagues at the hospital she adds.” Dr Casey says that the majority of gynaecological procedures have changed from major surgical procedures to minimally invasive laparoscopic procedures, which have become very common in this field. This allows surgeons to access the abdomen and pelvis through a key-hole incision. This minimally invasive procedure has greatly reduced post-operative pain, the duration of a patients stay at a hospital and it also greatly reduces the necessary recovery time for the patient. She adds that the experience of the nursing staff at Barringtons Hospital is a fantastic asset to both consultants and ultimately the patients. “The quality of the nursing at the hospital is excellent and this is augmented by very good administrative staff also. The thorough standards at the hospital, in which everything is tracked and documented is of immense benefit and comfort to a visiting consultant, this allows us to focus on the most important thing, providing excellent care. The management of the hospital also deserve great credit for how they have expanded the services at the hospital and worked closely with surgical staff to develop very good services for patients.”

Dietetics and Nutrition at Barringtons Hospital Barringtons Hospital, as part of its dietetics services, is now offering consultations the low FODMAP diet a new dietary approach to the management of irritable bowel syndrome.

Elaine McGowan M.Sc., B.Sc., Human Nutrition, Dip. Dietetics, MINDI Elaine established her first dietetic clinic in Barringtons Hospital in 1992 and has gained vast experience in providing patients with individual dietetic consultations for patients in her clinics. Her main areas of interest are weight management, obesity, diabetes, heart disease, nutrition and gastroenterology. She is also a very active member of the Irish Nutrition & Dietetic Institute (INDI). Elaine was one of the first Dietitian’s to practice the low FODMAP diet in Ireland and since 2010 has experienced exceptional results with a large patient base and is very pleased to be able to offer the diet to patients at Barringtons Hospital. It has proven a most effective treatment for functional gut disorder particularly IBS. She has provided dietetic consultancy to a wide range of public and private sector companies, including the implementation of workplace wellbeing programmes. These include Aughinish Alumina in Limerick. Elaine has a Masters in Sports Nutrition (A Nutritional Evaluation of Elite Irish Athletes) from the University of Limerick and has worked for the Olympic Council of Ireland for the 1992, 1996 and 2000 Olympic Games. With her expertise in this area she has worked in establishing

a nutrition programme for the National Athlete Player Support Network. She has worked with the national Rugby team and inter-county GAA team. Elaine’s interest in functional gut disorders led her to Melbourne, Australia, the hub of research in this area, in 2010. Whilst in Melbourne, Elaine spent time training with Dr Jaci Barrett one of the senior researchers working on the low FODMAP diet research at Monash University. She decided to further expand her knowledge base in this area and in May 2011 she completed training on ‘The Low FODMAP Diet in Functional Gastrointestinal Disorders’ at King’s College London. The ‘low FODMAP’ diet is a relatively new dietary treatment option available in Ireland, now accepted and recommended as one of the most effective dietary therapies for IBS and may help reduce functional symptoms in other gut disorders such as Colitis and Crohn’s disease. Elaine has adapted the diet to Ireland and has done continuous research in updating information for her patients. “Carbohydrates can be present in different forms of food and most are easily digested and absorbed to produce energy. But new research shows that a group of shorter chain carbohydrates (FODMAPS) are poorly absorbed in the small intestine and can trigger the symptoms of irritable bowel syndrome (IBS),” says Elaine.

What is the low FODMAP diet?

Ingestion of foods high in FODMAPs leads to alterations in fluid content and bacterial fermentation in the colon triggering functional gut symptoms in susceptible individuals. The aim of the diet is to identify which FODMAP foods, if any, cause symptoms. It involves restricting certain foods for up to four to six weeks, and then re-introducing them systematically back into the diet. Removing foods high in FODMAPs from the diet is effective in improving such symptoms as diarrhoea and/or constipation, bloating, wind, nausea and abdominal pain. This dietary intervention takes into account that there is a threshold for the amount of global FODMAPs an individual can tolerate at one time. However, the diet is most effective when FODMAP-trained dieticians provide dietary advice. It has been shown that patients who had seen a FODMAP-trained dietician reported significant improvement in symptoms after being on the diet. In practice, by modifying your diet, you will be able to know what FODMAP-containing foods you can tolerate, and which ones will produce symptoms.

Results of the Diet

According to researchers at King’s College London, the low FODMAP diet is effective for greater than 70 per cent of people with IBS who try it. After four to six weeks your progress is reviewed. Many people can return to their normal diet and will have identified the high FODMAP foods that need to be avoided or consumption quantity reduced. While FODMAPs do not cause the underlying functional gut disorder, they do represent an opportunity for reducing symptoms in 75 per cent of cases. It is certainly a clinically proven

option for anyone suffering from chronic digestive disorders. With the low FODMAP diet and other services, Elaine is committed to providing simple, practical nutrition and dietary advice to patients at Barringtons Hospital.

There have been a lot recent advances in research in this area. Information on the low FODMAP diet available in books and on the internet may be misleading and inaccurate. Research into the dietary strategy continues and information requires continuous updating.

“The main culprits and major FODMAPs are fructans, galactans, fructose, polyols and lactose. These carbohydrates can create gut symptoms when consumed. When the molecules are poorly absorbed in the small intestine of the digestive tract, these molecules then continue their journey to the large intestine, where they act as a food source to the bacteria that live there normally. The bacteria then digest/ferment these FODMAPs and can cause symptoms of IBS. They can also cause symptoms by exerting an osmotic effect and drawing fluid into the gut,” she adds. In people with gastrointestinal symptoms, foods high in FODMAPs such as pasta, bread, beans and dairy can cause: • Abdominal pain/discomfort • Abdominal bloating/distension • F latulence/wind • Belching/burping • Urgency to open bowels • Poor stool consistency • Constipation
