Welcome to the Alberta School Employee Benefit Plan

Hello, and welcome to the Alberta School Employee Benefit Plan (ASEBP)! We are thrilled to be supporting you and your dependants in the months and years ahead.

Over the coming weeks, we want to ensure the transition to your new ASEBP benefits is as seamless as possible. The information in this brochure will introduce you to ASEBP benefits, organize the steps you will need to take for a smooth onboarding, and give you a head start on completing some initial paperwork.

After reviewing the content of this brochure, we encourage you to use this page as a helpful checklist. Completing each item, including the applicable forms, will ensure you are ready to start accessing your ASEBP benefits on September 1.

Forms: Complete and submit the following forms, if applicable. All members must complete the Group Insurance Enrolment form:

• Appointment of Beneficiary(ies) form: Deadline to submit – August 31.

• Change Application form: Complete if you want to change who is covered under your benefits (e.g., single to family coverage). To avoid a lapse in coverage, please submit prior to October 1.

• Coordination of Benefits form: Complete if you wish to coordinate coverage between two or more plans.

• Group Insurance Enrolment form: Submit to your employer by October 1, 2024.

Review the Your Benefits content in this information package or at asebp.ca/rockyview, where we share your coverage details and how to find valuable information about your ASEBP benefits.

Consider filling prescriptions and refills with your current provider in late August so that you have a bit of breathing room during the transition to ASEBP.

Your Personal Benefits Portal

The following information will help get you ready to successfully access your ASEBP benefits starting September 1.

As early as August 19, you will want to:

Register for My ASEBP.

Here are some other helpful tasks, and more reasons, to register for your My ASEBP account:

Download the app.

Once registered for your My ASEBP account, scan one of the QR codes below to download the My ASEBP app. You are now set to review your benefits or submit a claim where and when it’s convenient for you!

Create your My ASEBP account (it’s best to use your personal email address), following the details listed in your initial onboarding email (that we will send to your work email on or about August 19). Your My ASEBP account is where you will view and manage your benefits.

Verify your ID.

Once signed into your account, click on the circle with your initials (top right) and select ASEBP ID. Check the spelling of your name and whether your dependants (if applicable) are listed. If anything looks off, let your employer know ASAP and they will get it corrected.

Enter your banking information.

Click again on the profile icon and select My Profile. Enter and save your bank account information so we can deposit your claim reimbursements.

Confirm your address.

While still on the My Profile page, check your address and contact information. Again, please contact your employer to have this information changed. 1 2 3 4

Set up direct billing.

Present your paper or electronic ID card (available on My ASEBP or the My ASEBP Mobile App) to your health care providers so they can bill ASEBP directly for eligible products or services.

Submit your claims.

Quickly submit claims for drugs, dental, vision, other medical services and supplies, and spending accounts.

Stay informed.

Easily check your benefit coverage and usage and get account notifications for things you need to attend to, like drug special authorizations, dental pre-determinations, spending account allocation deadlines, and more.

Your Benefits

The following is a brief outline of your ASEBP benefits. You can also find a full explanation at asebp.ca/rockyview.

Extended Health Care

Extended Health Care includes coverage for drugs, travel emergencies, and other medical services and supplies (such as massage therapy, physiotherapy, and psychology, among others).

Drugs

To better understand your prescription medication coverage, visit asebp.ca and view the Drugs page under the Benefits tab.

If you have an existing drug special authorization, you can find more information in our Frequently Asked Questions section of the Rocky View Schools’ private web page.

Be sure you are familiar with the health conditions listed under Therapeutic Alternatives and Maintenance Medication Program web pages, along with Generic Alternative Pricing.

In late August, you may wish to fill any existing prescriptions and refills with your current provider so that you have a bit of breathing room during the transition to ASEBP.

Travel Emergencies

Other Medical Services and Supplies

Dental Care

Your Dental Care plan helps you to proactively manage your oral health through basic preventative and restorative treatments, and also covers major restorative dental work and orthodontic treatments. You can find more information at asebp.ca and view the Dental page under the Benefits tab.

Basic preventative/restorative treatments

100% of the ASEBP Dental Benefit List fees for basic treatments per person per calendar year.

Major restorative treatments

60% of the ASEBP Dental Benefit List fees up to $2,500 for major treatments per person per calendar year.

Orthodontics

100% of the ASEBP Dental Benefit List fees for examinations and diagnostics and 60% of the Dental Benefit List fees for all other treatments (e.g., appliances, banding, etc.) up to $3,000 per person per lifetime.

Vision Care

Your Vision Care benefit—$400 every (rolling) two years per person—includes coverage for a variety of products and services such as eye exams, prescription eye glass lenses and frames, prescription contact lenses, contact lens fitting fees, prescription sunglasses, corrective eye surgery, and lens implants. Life Insurance (under 70 years of age)

Your Life Insurance plan provides two times your annual earnings to a maximum of $800,000. For more information on this benefit, including basic criteria and full coverage details, visit asebp.ca and view the Life Insurance page under the Benefits tab.

Accidental Death & Dismemberment (under 70 years of age)

Your Accidental Death & Dismemberment plan provides coverage in the event your death is accidental, and it also includes various coverage levels if an accident leads to dismemberment or loss of use of a body part or function. Extended Disability Benefits (under

65 years of age)

ASEBP provides coverage if you become sick or injured and are unable to work.

Pre-September 1

If you are on disability leave prior to September 1, your existing General Health Benefits (which may include Extended Health Care, Vision Care, Dental Care) and Accidental Death and Dismemberment insurance will transition to ASEBP. However, your existing Life insurance and Long-term Disability benefits will continue with your existing provider, Desjardins. Please reach out to your employer for further information.

Post-September 1 Sick Leave Support

For absences greater than two weeks (including gradual return to work longer than 14 days but less than 90 days)—allow us to get to the heart of your health needs. We aim to streamline supports so you can get the right medical attention early, helping you heal and get back to feeling like yourself and return to work quicker. Please contact ASEBP for more information.

Extended Disability Benefits

If you have a serious or prolonged illness that makes you unable to work for more than 90 days, you may be eligible for ASEBP’s Extended Disability Benefits (EDB), which provides 70% of your monthly earnings. This coverage is available for members under 65 years of age. Please contact ASEBP for more information or visit asebp.ca and look for Extended Disability Benefits under the Benefits tab.

Spending/Savings Accounts

On or around August 19, you will receive an ASEBP email—to your work email account—about your spending/savings account options and allocation deadlines.

To access your spending/savings account credits, you will need to create a My ASEBP account (your personal benefits portal). You’ll receive more information about creating your My ASEBP account in a separate welcome email (sent to your work email) during the week of August 19.

Teachers

You must allocate your credits—choosing between your Health Spending Account (HSA) and Wellness Spending Account (WSA)—no later than November 30, 2024. If you miss the allocation deadline, your spending accounts credits will be deposited into your HSA account.

Other (non-teaching) education workers

Eligible non-teaching staff can allocate their spending account credits between an HSA, WSA, RRSP, and TFSA. You must allocate your credits no later than September 15, 2024. If you miss the allocation deadline, your spending accounts credits will be deposited into your HSA account.

Note: Starting 2025, your annual credit allocation deadline becomes August 31.

Employee and Family Assistance Program (EFAP)

Your confidential EFAP, administered through GreenShield Health, connects you and your dependants with clinical mental health professionals (e.g., Master of Social Work or registered counsellors), registered nurses, legal and financial experts, and more. With your EFAP, you can access free 15-minute consults and four hours of services per calendar year with a health or wellness professional. Plus, access free proactive online tools, resources, and support services (24/7) to help you navigate life’s journeys, including topics such as:

• Childcare and eldercare

• Nutritional well-being

• Stress and anxiety

• Relationship concerns
