What We Do Northwest Human Services has been providing residents of Marion and Polk counties with compassionate and professional medical, dental, mental health, and wraparound services for 50 years. Our programs are designed to maximize support for those in our community who need it most - uninsured individuals, families, the homeless, and migrant workers.

Our Medical Home medical | dental | counseling & psychiatry Our West Salem Clinic and Total Health Community Clinic are Patient Centered Medical Home Certified federally qualified health centers, offering a full range of primary care services for patients of all ages, across a broad range of socioeconomic and cultural backgrounds.

Our medical, mental health, and dental services are fully integrated, allowing us to provide our patients with exceptional coordinated care. We employ highly qualified medical professionals including physicians, physicians assistants, nurse practitioners, registered nurses, a psychiatric team, counselors, behaviorists and an on-site pharmacy consultant, each bringing with them a passion for serving those most in need.

Transitional Programs HOAP | HOST | crisis& information hotline Homeless and at-risk youth and adults at our HOST and HOAP Programs access basic needs services such as laundry, showers, meals, case management, peer support, and linkages to medical, dental, and mental health care at our West Salem Clinic campus.

The Hotline provides 24-hour crisis support and suicide intervention as well as limited emergency financial assistance for rent, utilities, and prescription medications. Our Crisis & Information Hotline is accredited by the American Association of Suicideology and is a member of the National Suicide Prevention Lifeline network.

NWHS’ Indirect Cost Rate is just 15%. That means that

85 cents of every dollar generated as revenue or donated to NWHS goes directly back into patient and client care.

Charity Care refers to the discounted Medical, Dental, and Mental Health services offered to those who are uninsured or under-insured and are on the sliding fee scale, and unable to pay for the services we provide.

But at NWHS “unable to pay” does not mean “unable to access care”.

During the 2019 - 2020 fiscal year a total of 4,285 medical, dental, and mental health visits were provided at no cost to our patients. At an average rate of $155.50 per visit, this amounts to over $666,000 in services.

Our History | 50 Years | One of a Kind Northwest Human Services began in 1970 with the establishment of the Switchboard for Help, now known as the Crisis & Information Hotline. The following year in the summer of 1971, two UCLA medical students arrived on the scene as part of an internship with Marion County Health Department. Appalled by the lack of health care services available to the low-income and homeless, the students, Phil Yule and Paul Kaplan, requested the assistance of the health department in opening a clinic to serve disadvantaged populations. With the department’s support, The Cry of Love Free Clinic was opened.

Cry of Love shared a building with the Switchboard for Help, and soon the two merged into a single entity. In 1979, The Cry of Love Free Clinic became the West Bank Health Network, and later the West Salem Clinic, as it is currently known. In 1982 the agency as a whole took on the name ‘Northwest Human Services’.

Job Summary As a physician at NWHS, you will provide exceptional, compassionate, and trauma informed primary medical care to patients across a broad range of socioeconomic and cultural backgrounds.

YOUR ROLE at NWHS: In addition to providing effective and patient-centered medical care, our Physicians lead, coach, and counsel our care teams based on established policies, procedures, protocols, and guidelines. In that capacity, you will work collaboratively to achieve the philosophy, goals and objectives of Northwest Human Services. You will also help us to design, implement, and update department and function orientations, training processes, and competency certification systems for all levels of department personnel.

CORE COMPETENCIES: 1. Deliver professional, effective and patient-centered quality medical services to our community and clinic patients. You will examine, diagnose, and treat medical problems, prepare appropriate treatment plans and provide follow-up and treatment as needed while adhering to clinic policy and procedure.

2. Provide consultation when needed to nurse practitioners and physician assistants. You may also serve as the supervising physician to no more than two mid-level providers and proctor medical students as assigned.

3. Maintain appropriate and up-to-date medical records documentation on all patients in accordance with organizational policy. You will strive to meet expectations of above 90% on weekly documentation report and maintain your PAQ at less than 75 items at any given period.

4. Actively and enthusiastically contribute to the care team by providing coverage to absent providers, being accessible to support staff, and serving on improvement teams.

5. Actively engaged in organizational initiatives such as 340b usage, Patient Centered Medical Home Certification, and Meaningful Use requirements.

Comprehensive Medical | A multi-choice plan to fit your needs, as well as emergency care, in and outpatient hospital, x-ray and lab services, pharmacy, and mental health.

Comprehensive Dental | 100% coverage for preventative dental care including routine cleanings and exams, 80-90% coverage for more complex dental work such as endodontics, extractions and fillings. After six month waiting period, 5060% coverage for major dental work such as crowns and bridges.

Vision | Vision exam available after a $10 co-pay. Limited maximum benefits allowance available on lenses, contacts, and frames.

403(b) Retirement Plan | Our 403(b) Plan has many investment options to suit individual employee retirement needs. Northwest Human Services also monthly employee contributions dollar for dollar up to 3% of annual wages (highly compensated employees may differ).

Flex Spending Account | This savings plan offers employees the opportunity to save in advance on a pre-tax basis for family health care expenses or child-care expenses. Health care premiums also may be deducted on a pre-tax basis with enrollment in this plan. This plan also allows an annual roll-over of up to $500 for unspent FSA funds.

Group Life & Long Term/Short-Term Disability Insurance | Northwest Human Services pays 100% of the cost for the AD & D benefit program for our employees. The AD & D benefit is equal to 1x your annual salary. Additional voluntary life and disability insurance benefits are also available.

Paid Time Off (PTO) | Immediately following date of hire, eligible employees begin accruing PTO leave of up to 10 hours per month based on FTE. Holiday Pay | Employees are paid for 8½ holidays each year.

2019 NWHS Year in Review 2020 Annual Report West Salem Clinic | Total Health Community Clinic | HOAP | HOST | Crisis & Information Hotline COVID-19 Response In March 2020, a global pandemic was declared. As a community health center providing care to over 12,000 members of Marion, Polk and Yamhill Counties, NWHS rapidly adapted our practices. In a matter of days, we modified our service delivery to include comprehensive COVID-19 screening protocols at each site; telehealth for medical, mental health, and dental appointments; and enhanced our sanitation protocols across the board.

NWHS continues to partner with local service providers as well as our local and state governments to provide COVID testing to our patients, staff, and vulnerable unsheltered populations. We are also thankful to have Board members, staff, and community groups supporting our ongoing need for personal protective equipment by sewing and delivering hundreds of masks for our staff, patients, and clients.

Community Outreach Because we operate both medical clinics and homeless service sites, our multidisciplinary teams are able to conduct outreach to homeless camps offering not only hygiene items, food supplies, and connections to resources, but also on the spot nurse consults and access to healthcare. In recent months, our team has ramped up outreach efforts, visting homeless encampments multiple times a week. We regularly partner with other service providers to boost our outreach impact.

With people struggling now more than ever, our patients and clients are experiencing increased rates of food insecurity. In prior months, these individuals and families could access our farmer’s market style Neighborhood Fresh Connect Program to pick up free, fresh and healthy produce at our Clinics. Due to covid-19 restrictions, we had to modify this critical food security program. but, thanks to the partnership of local food banks, our team has been able to distribute and in some cases even deliver food boxes and fresh produce boxes to our patients and clients in need.

Social Justice & Equality Response In response to local and national unrest due to systemic violence and oppression towards people of color, NWHS took a strong public stance in solidarity with the movement calling for justice and an end to violence against communities of color. The leadership and staff at NWHS recognizes that organizational and/or systemic biases lead to inequitable access to care and must be abolished to ensure optimal health for all people regardless of ones identified color, gender, culture, age or socioeconomic status.

Our pledge as an organization is to lead with compassion for social justice, make bold decisions and changes to break down barriers for people of color, and work together to build a community that ensures all voices are heard, validated, and respected, and that the humanity of all people is upheld.

We support the “Black Lives Matter” movement and believe that the lives of all people of color in our community have value. It is time to learn from our history and break the systemic cycles of oppression and violence so all people have the ability to live prosperous, healthy and fulfilling lives.

By the Numbers We saw 12,417 patients for a total of 39,946 medical appointments at West Salem Clinic & Total Health Community Clinics.

15,562 client appointments were scheduled at our West Salem Mental Health Clinic.

Positive Outcomes 647 individuals and families received emergency financial assistance from our Hotline; 1,178 individuals received crisis and suicide intervention support.

HOAP provided 29,672 meals to program guests.

On average, 70 young adults facing homelessness accessed resources at our HOST Program each month for a total of 8,478 visits.

Our West Salem Dental Clinic provided children and adults with 4,591 cleanings, exams, and dental procedures.

NWHS staff enrolled 2,175 patients & clients in the Oregon Health Plan, and provided OHP assistance to 6,695 individuals.

231 NWHS patients & clients received free fresh produce and other food items at our Neighborhood Fresh Connect Program

Over 19,800 phone calls were handled at our 24-hour Crisis & Information Hotline.

3,167 homeless & at-risk individuals received services at our HOAP Program for a total of 24,118 visits. *2019 data

21 homeless adults & 30 young adults accessing resources at our HOAP & HOST Programs were transitioned into stable housing with the help of our staff.

1,125 homeless individuals accessed our free medical bus transportation to the West Salem Clinic for primary medical, dental, and mental health care.
