Jesuits urgently plead for aid

MAJOR FAMINE HITS POOREST OF THE POOR! India has been hit by one of its most severe droughts. In the Australian Jesuit Mission Region, both Hazaribagh and Palamau Districts have been declared famine areas. The monsoon failed and so did the crops. The water and food situation is now very grave, and is rapidly getting worse. Wells have very little water — many have dried up and very little useful rain can be expected before the next monsoon in June. The bulk of the five million people in the Mission Region are the poorest of the poor. They have always had a very tough life . . . deprived of education and health care . . . aggravated by malnutrition and sickness. The whole family works just to survive. This famine could spell complete disaster for them, as poor people are more prone to disease. The old and the very young, the sick and the lepers, are the ones who suffer most. Please have charitable concern for them. F unds are urgently needed to provide —

i f. Food for those in greatest need. * Milk for mothers and children. * Medicines for the sick and lepers. * Food- for - work schemes. * Machinery for the desperate search for water.

From all parts of the Mission Priests and Nuns appeal to Australians who have an abundance of good things to be extra generous to these very deprived people threatened with hunger and thirst, sickness and death. • ••••1111•15•0111•111•••111•8111•1111•••••111•10•••••11111•11••••••••111111111•1116•111•••••••••••••••••••••• •

NEW-TYPE MISSION NAPLES, Italy. 1750

A new style of parish Mission in vogue here in the kingdom of Naples is proving very popular. "We are flat out" says Redemptorist Fr. Salvator Rimbombo. "Scarcely do we finish one mission when we are called to do another" "Popularly known as the 'Redemptorist Fathers' these priests are members of the new Congregation of the Most Holy Redeemer. The Congregation was founded by Fr. Alphonsus Liguori in 1732 at the town of Scala near here. Pope Benedict 14 gave approval to the Congregation last year.

For every day Fr. Rimbombo explained the new style of parish Mission: "Fr. Liguori wanted to reach the fellow who usually misses Masses or goes outside when a sermon starts. "He has designed a Mission for ordinary people. "He is specially concerned about the poor in rural areas. "Our Mission sermons follow a plan and are adapted to the everyday lives of our listeners. "We try to show them how religion is connected with their daily work and problems and fun.

Around the Churches this week An up-to-the-moment report by Father JOSEPH RUSSELL Theology in which he opposes both laxity and rigorism, has been widely acclaimed by theologians. Besides the Redemptorist style of parish mission, there is also a style of mission used by the Passionist Fathers.

Priest's challenge These missioners were approved by Pope Benedict in 1741. They are led by Fr. Paul Francis Danei (56). This Italian priest challenges Catholics at his missions to accept life's crosses as a means of their salvation. The Passionist Mission revolves around the central idea of Christ's redeeming Passion and Death.

'THE ONES' The pope asked everyone ,o pray for "the education and sustenance of candidates for the priesthood." Seminary Day began early for Pope John Paul, who celebrated the 7 a.m. Mass in the Pauline Chapel with students and faculty from the Molfetta seminary. "You are the ones who are called," he told the students. "You are the ones whom the church and society awaits w ith anxiety, given the extreme need for ministers of God who are enlightened and upright, balanced and wise, convinced and courageous priests."

ST. JOHN Noting that the church observes the feast of the baptism of Jesus by St. John the Baptist on January 13, the pope told the seminarians to take a lesson from the "total and absolute confidence in Jesus" which St. John had. "Everything passes, but the truth remains; the things of this world pass, but the church remains,' Pope John Paul added. Pope John Paul spent the evening with students and faculty of the Pontifical Irish College, the community of St. Isidore's Franciscan College and St. Patrick's Augustinian College. "Today, once again, in a very special way, the pope belongs to Ireland,' he told the group, which included Cardinal Tomas O'Fiaich of Armagh, primate of all Ireland.

IRELAND Discussing the practical requirements of the call to conversion, Pope John Paul said, "To walk in the truth of our vocation means . . . a lifestyle that corresponds to the reality of our role in the church today, a lifestyle that takes into account the needs of our brethren and the ministry that we shall exercise tomorrow."

The Reverend Keith Bricknell, Queensland State secretary for the Bible Society in Australia, stated this after a recent tour t hrough the German Democratic Republic. At last weekend's Charistmatic rally, it was wonderful to see many young people fearlessly praising the Lord. applications for anulments. He also praised the lifting of excommunication for divorced and remarried Catholics.

THE POPE? When asked about the statements of Pope John Paul 11 on divorce, the bishop said that the pope was talking about divorce as a social evil. "Divorce is something that breaks down the social fabric. "It's not only a private horror; it erodes society," the bishop said. "Yet we have to do something to minister to divorced people." "I don't think it condones divorce."

PROBLEM The most difficult problem. he said, was the question of divorced and remarried Catholics who could not receive an annulment. "The church is faced with the dilemma of following Christ's teaching and yet bringing pastoral healing to this second union," he said. Bishop O'Donnell says that he doesn't know the answer to the problem. But he urged divorced and separated Catholics to pray and offer their sufferings "to aid and support others."

The future of the Catholic hospital? What can the individual Catholic hospital do in the face of the problems we spoke about last week? Let me sketch two scenarios. WhileIpaint these two pictures as extremes, there is nothing to prevent their simultaneous existence. One scenario would see the hospital follow what has become the traditional path of unremitting protest at all levels of the political system and very little internal reform. The second scenario would see our hospital adopt a corporate planning strategy for internal reform which also recognised that many of these problems won't go away. The traditional path: protest without reform. Some hospitals will mount individual campaigns to resist bed reductions and seek changes which improve private health insurance coverage. Unfortunately these campaigns will not necessarily solve the problems of the declining numbers of Religious, the increasing bill for wages and medical technology and the problems of meeting new demands in communities that lie outside the hospital walls.

Bed Closures Even then, in offering resistence to bed closures or bed reductions, some socially aware hospitals would admit that health authorities are now emphasising one issue very clearly — namely the inconsistencies in the positions of some hospital board members. Catholic hospital boards are extremely fortunate to have attracted the best of business brains available in Australia. These board members, who offer a long standing Christian service to the Catholic health system, generally run the day-to-day business and industrial affairs of their corporations on a for-profit basis. If they don't make a profit they go out of business.

Govt. Subsidies If they run up a deficit in successive years, they do not enhance their reputation, and they would probably go out of business even if governnient subsidies supported their industry. Most public hospitals run on a deficit-financed basis, which means that if their costs exceed their budget, government subsidies fill up the gap. All private hospitals in Australia receive a subsidy of $16 per patient per day. For some hospitals that government subsidy is the difference between profit (and so a continued ability to meet Australia's needs) or loss (and so closure). And here is the paradox for these hospital board members.

Cost Control Another hospital, realising that its method of cost control was not based on the sound accounting practice of identifiable cost centres, called in consultants to develop such a system. With the knowledge gained in this exercise, this particular hospital will probably be in a far better position to confront the 1980s than most other hospitals in Australia. There is a widespread belief in Australian hospitals that the existing methods of budgeting and cost control are pre-historic and that there is a need for indivudual hospitals to present the case to State (and Commonwealth) health authorities for significant changes in the existing methods. One particular "solution" that is being carefully evaluated both in other nations and in Australia is "prospective incentive budgeting." The concept is deceptively simple in four respects.

Budgeting It says that: First, all publically-financed hospitals ought to have their budget set in advance of the budget period, so that they have a fixed expenditure ceiling that they cannot exceed. For each year, each hospital and the State health authority reach mutual agreement on a target expenditure that will not be exceeded. Allowances are made in the budget allocation for estimated changes in patient "throughput" and patient "casemix." and for the effect of wage increases and price inflation. Second, if by efficient administration, the hospital is able to provide the services at a lower cost than this allocation a fixed proportion of the savings (up to 50 p.c.) ought to be returned to the hospital for its own use (say, for acquisition of cost-effective equipment or for refurbishing of the hospital building). The remainder - a saving to tax payers -- returns to the State health authorities for re-allocaticn to projects that meet an obvious social need. 10411.440440-(to

This is the second of a series of three articles based on the memorial oration given to the 40th anniversary meeting of the Sacred Heart Hospital, Coburg (Victoria), in November 1979. By PAUL GROSS So there is a positive incentive for good managers and staff. Third, if the hospital exceeds the budget ceiling, it ought to be forced to find the deficit from its own funds or the deficit ought to be debited to the next budget year. In other words, sloppy management is not instanteously rewarded by the type of deficit financing that is now all too prevalent. Fourth, unlike the present system of hospital financing, the budgeting process recognises the differences in costs that can legitimately occur between hospitals differing in size, bed to staff ratios, the number of specialist units, teaching activities and other factors.

Justifying Deficits How do they justify a continuous run of deficits in hospital activities without seeking to change the causes of the deficit, while in their day-to-day business, they would not tolerate a deficit? If parts of the hospital are like a business operation and other parts (for example, the care function) are not, the average board member requires keen perceptions to assess where his expertise will be most useful. Whatever he does, he must ask questions and seek reforms which diminish the deficit. The statement's response: accountable management Protest without reform is a fairly hollow weapon these days. Many members of hospital boards perceive this problem very clearly. Within their own hospitals. many have initiated major reforms. The following are a non-representative but illustrative sample.

Reforms A : Assessing the results of surgical services. One hospital has participated in a comparative study of surgical procedures on its patients and patients of a large public teaching hospital in the same city. Two aims of the study are: To assess what causes the observable differences in length of stay, diagnostic test ordering and costs between the two hospitals. To assess what effect the treatments have on the patients. (In other words, there is no explicit assumption that all patients benefit from all medical treatment and there is sufficient concern to find out why some patients don't benefit.) B : Assessing the adequacy of budget and control systems.

mpact Assessed I C : Assessing the impact of a new medical technology. A third hospital was concerned with the impact of an expensive medical technology which can provide diagnostic evidence of the presence or absence of internal disorders. The machine costs nearly $800,000 to pruchase and about $350,000 per year to operate. The hospital wanted to know whether it reduced the use of other machines that had been used before its arrival, whether it changed the prognosis for the patient, whether it reduced unnecessary surgery and length of stay in the hospital, and whether it reduced the overall costs of the hospital. D : Assessing the efficiency of various hospital units.

Efficiency

Other hospitals have taken a leadership role in assessing the efficiency of various parts of the hospital, including outpatient and casualty departrbents. food services. pharmacy. cleaning services and maintenance. These studies have clearly shown that hospitals can demonstrate improvements in costs without reducing the quality of care. E : The hospital and the community.
