The Affordable Care Act: How new Obamacare mandates affect you and your business
By JONATHAN COPSEY GEORGIA – Beginning in 2010, the state of health care has taken a new course. The Affordable Care Act (ACA), commonly known as “Obamacare,” went into effect in phases, spanning several years. The intent of the ACA was to slow the rapid rise of health care costs throughout the nation. To do this, everyone in the nation must be covered with some form of health insurance. Insurance could be obtained through employers, as has traditionally been the case, or in situations where employerprovided coverage was not available or affordable, through state-run health care exchanges. Consumers could purchase insurance plans through these marketplaces, and in cases receive monthly tax credits to offset the cost of the premiums. States could also expand their Medicaid programs to include a wider swath of citizens to increase coverage. However, the federal government did not mandate that states establish their own health care exchange, nor did it mandate an expansion of state-run Medicaid programs. As a result, many states, including Georgia, chose not to build their own exchange, and 21 (to date) have chosen not to expand their Medicaid programs. In those states, citizens can purchase insurance through a federal exchange. Despite numerous challenges to its legality, the law is now largely in place and will remain so for some time. In Georgia, as of March 31, 2015, more than 450,000 Georgians have signed up for the ACA through the federal marketplace. Georgia was listed as among the top 10 states where consumers received financial assistance through advanced premium tax credits. Figures say 91 percent of those receiving health insurance through the marketplace received subsidies. “Georgia has one of the largest numbers of people on the [ACA],” said David Howard, an associate professor of health policy and management at Emory’s Rollins School of Public Health. Howard says there are benefits to the new health care legislation, such as increasing the continuity of insurance coverage and funding for compara-
tive effectiveness research, but he is concerned about the fiscal impact of the legislation. Because the state opted out of expanding Medicaid, the federal government has had to pick up much of the slack in the form of subsidies for monthly health care premiums through the exchange. Howard said that, despite the ACA’s tumultuous initial years, things should begin to settle down by 2015-16. “In the initial years of the exchanges, insurers set premiums based on a lot of guesswork,” he said. “Now they have a complete year of claims under their belt and they know people are signing up for plans.” Unfortunately, he said, the first and largest group to sign up are the sick and elderly. This causes premiums to rise until more healthy people sign up. Once that happens, the industry should even out. But what does the ACA mean for individuals and businesses in 2015-16? For individuals, everyone must be covered. The first penalties for not enrolling began going into effect in 2014 and increase annually. The penalties are based on family income and are meant to deter a failure to enroll. For businesses, most of the provisions of the law have gone into effect and are largely the same as those for individuals. The employer mandate was delayed until 2015 and 2016. Ultimately, 95 percent of employees must be covered by some form of health care. A new change will force employers of small and medium-sized companies to begin complying by the rules. Up until now, it was only large companies. Employers that do not meet the requirements will face penalties. “It’s hard for people who don’t work in the industry to figure out what they will be liable for,” Howard said. This leads to confusion both for the employer and the insurer. And this confusion can lead to higher premiums. “It may work itself out,” Howard said, “but insurers are saying that hasn’t happened yet.”

What is Gamma Knife surgery? Gamma Knife radiosurgery is a type of radiation treatment performed with the Leksell Gamma Knife, a noninvasive neurosurgical tool. Gamma Knife radiosurgery involves focusing small, highly precise doses of radiation into the brain. This shrinks small brain tumors or blocks abnormal blood vessels and nerves that cause pain or seizures. Gamma Knife surgery can also treat some brain disorders, such as Parkinson’s disease. The name is derived from the knife-like precision of the radiation delivered to the target area. The procedure is an effective alternative to neurosurgery for many patients. Gamma Knife surgery can help patients who cannot tolerate neurosurgery because of a medical condition, age or simply preference. It is less invasive and requires less hospitalization and recovery time than neurosurgery. Most Gamma Knife patients are treated on an outpatient basis and require no overnight stay. Anyone interested in Gamma Knife should discuss all procedures and surgery options with their doctor and consider getting a second opinion about all treatment choices before having the procedure.

medical science Noninvasive procedure uses high-tech precision to treat brain maladies By HATCHER HURD SANDY SPRINGS, Ga. – A new noninvasive surgical procedure is showing great success at Emory-Saint Joseph’s Hospital for tricky brain surgeries and goes by the name of Gamma Knife. Dr. Peter Rossi, a radiation oncologist at Emory-St. Joseph’s Hospital in Sandy Springs, is trained on one of four Gamma Knife machines in the state of Georgia. This new technology allows for extremely precise radiation surgery that is unattainable through most conventional brain surgeries. It is almost like science fiction than medical science, but Rossi says it is a game-changer for certain types of brain surgeries. He often works with Dr. Shannon Kahn, assistant professor in the Emory
School of Medicine Department of Radiation Oncology. “The machine is only used on the brain. It allows you to target the precise surgical spot you want to get at without damaging the surrounding areas. It is a great alternative to surgery. It is a halfday outpatient procedure,” Kahn said. The success rate on tumors is about 90 percent to 95 percent. “These are the most advanced machines in the world for many kinds of cranial surgery,” Rossi said. “The gamma rays are specific to treating certain kinds of tumors. The machine gets us to the exact spot we need to be,” he said. Because the Gamma Knife expertise is known to just a few doctors, they act as a peer group of world experts on the device, Kahn said.

There are also conditions where a vein and artery grow together, creating a high risk of aneurism (where the vessel just balloons with the catastrophic risk of bursting). “It is virtually the same procedure as surgery but without hospitalization and a longer recovery,” Kahn said. “It’s a half-day outpatient procedure with virtually the same results as surgery, but [Gamma Knife] is safer and shorter.” The Gamma Knife can operate within a 4-millimeter area that an MRI can’t even “see,” Rossi said. Rossi and Kahn both are part of the Gamma Knife team at Emory-St. Joseph’s.
“One of the advantages of the Emory system is the Winship Cancer Institute. Kahn is a clinical member of the Cancer Prevention and Control Research Program at Winship Cancer Institute,” said Emory-St. Joseph’s Hospital interim Chief Executive Officer Dane Peterson. Emory University Hospital through Winship is ranked among the top 25 cancer hospitals in the nation, Peterson said. One of Kahn’s research interests includes examining newer technologies and treatment of breast cancer to minimize cosmetic changes while maximizing long-term tumor control. The use of the Gamma Knife is strictly monitored and controlled, so that it is available to get at brain tumors as soon as possible, Rossi said. The amount of actual time on the device itself is short, but it takes a lot of preparation first. Insurance companies like the Gamma Knife because it keeps costs down. “In a standard case, the patient never opens his checkbook. The costs are covered in most insurance plans,” Rossi said.

Resurgens Orthopaedics provides free physicals for students By JOE PARKER NORTH FULTON, Ga. — Resurgens Orthopaedics, Georgia’s largest orthopaedic practice with 21 offices in the metro Atlanta area, provides free physicals for high school students every April. Resurgens provides physicals for athletes, band members, Boy Scouts and any other student that may be in need of a physical. The program has run since 2011. According to Dr. Jeffrey Albert of Resurgens Orthopaedics, providing the free physicals is a way to give back to the community. “Providing the physicals helps out so many young athletes,” Albert said. “It is definitely worthwhile. We can provide this service for athletes who can’t afford the physicals, and Resurgens Orthopaedics and North Fulton Hospital are happy to work with coaches that need this type of service for their schools.” Since 2012, Resurgens has partnered with North Fulton Hospital to provide the physicals. According to Albert, the North Fulton Hospital location makes for a smooth process for physicals, given that they provide all necessary equipment. In addition to Resurgens Orthopaedic surgeons, family practice physicians and ER physicians from North Fulton Hospital, who can check heart and lung functionality, also volunteer their time for the event. Local optometrists also donate their time to make sure that the athletes’ vision is at peak performance. Since the program began, Albert said more and more students are taking advantage of the service every year. In 2015, over 300 students received the pro bono physicals, up from 232 in 2014. “We are really happy to be able to provide the service, and the program will continue for the foreseeable future.” Albert said. By KATHLEEN STURGEON FORSYTH COUNTY, Ga. — Forsyth County has been recognized once again for being the healthiest county in Georgia, according to the sixth annual County Health Rankings compiled by the Robert Wood Johnson Foundation University of Wisconsin Population Health Institute. Overall, Georgia ranked 38 out of all the states. “When we look at how long people live, we examine what we call ‘premature death,’ i.e., deaths before age 75, since many of these deaths are preventable,” the report said. “Specifically, we look at the years of life lost so that deaths at an earlier age are given more weight than deaths among those closer to age 75. To estimate how healthy people feel, we use measures of people’s reported health status and how often they feel healthy each month. And, our final measure of health outcomes gauges the share of a community’s youngest members that have an unhealthy start to life — we look at the percent of babies born with low birth weight.” Forsyth recorded 4,234 years of life lost due to premature death, with Georgia’s overall being 7,314. For-
syth also had a 7 percent rate of poor or fair health for quality of life compared to Georgia’s 16 percent, and 6.9 percent low birth weight rate, compared with Georgia’s at 9.5 percent. The county reported 12 percent of adults as smokers. In Georgia, smokers account for 18 percent. In Forsyth County, 25 percent of adults were reported as being obese, compared with 29 percent for Georgia. “The purpose of the County Health Rankings is to serve as a call to action to improve health within all states and local communities by comparing counties within states and highlighting their differences in health,” the key findings said. “Identifying the healthiest or least healthy county in the United States would not serve this purpose. However, the rankings do include data on top performing counties across the nation for each of the 35 measures, so communities can see how they compare to other counties in their state and how they compare to these top performing counties in the nation.” Six percent of the county is unemployed. In Georgia, that figure is 8.2 percent. Forsyth ranked first for health outcomes, including length and quality of life, first for health behaviors, ninth for clinical care, second for social and economic factors, 100 for physical environment and third overall for health factors.

Although flawed, Affordable Care Act important key in taming rocketing costs
By HATCHER HURD ALPHARETTA, Ga. – John Bardis is a maverick in the health care industry. Recently retired as president, chief executive officer and now chairman emeritus of MedAssets (see box), he sounds a loud and clear call for supporting the Affordable Care Act. A 30-year veteran of managing health care costs, Bardis says it is not only important to the health of everyone, but if we don’t get a grip on the true cause of skyrocketing health care costs, an economic crisis of epic proportions is unavoidable. So why is Bardis so sure he is right?

“Well, what is clear and irrefutable is there are 40 million uninsured Americans, many of whom have no chance of ever being able to afford health insurance, and that number is growing for the same reasons,” Bardis said. Those reasons are founded in how the government originally began paying for health care with Medicare in the 1960s with a cost-plus system of payments, he said. But in the mid-80s, there was a cost split. The government capped the rising costs in Medicare of what are called diagnostic related groups (DRGs) and linked any rising costs to a cost-of-living figure. But
DRGs in the private sector had no such cap, “So private sector costs continued to rise, until companies could no longer absorb those costs and passed them on to employees,” he said. “Now premium costs are so high, individuals have been driven out of the insurance market altogether.” The situation has become so bad, those who don’t have insurance cannot afford premiums on their own, and people with pre-existing conditions are in an even worse situation. Not only were their premiums much higher, but often insurance could not be procured at any price. Today, health costs are growing at a much faster rate than wage growth or even economic growth. “It was only a question of time before the affordability of health care versus wage and wealth growth would segment out the poor and the middle class. And that is exactly what has happened to us,” he said. The Affordable Care Act is an attempt to rectify that, but Bardis says the law is “by no means perfect,” nor was the way the law was legislated. The disparity in costs are so great now between what wage earners can pay and what care costs, many of that 40 million uninsured do not seek care until they are at such a point of deterioration that they must walk into an emergency room. “There couldn’t be a more expensive place to get your primary care than the ER,” Bardis said. The reality is the country needed an answer to the insurance issue. An estimated 15 million people are covered today who were not before the law. Conservatives say the number of newly insured is nowhere near 15 million, but closer to 5 million. Bardis says no matter the number – and he says it is nearer the upper range – the next step is clear. “We have to manage health, not health care,” he said. “We’re in a much better place if we have an educated consumer in a transparent market.” In a market today of high deductibles, the economic forces should reward the person who chooses health care at the most reasonable cost. But Bardis says the consumer is snookered because there is an “opaqueness” on the cost side. Costs can be hidden in so many ways consumers can’t make an informed comparison decision. Meanwhile, the onset of the presidential campaign has brought more cloudiness to the issue. Conservatives agree health care needs fixing, but say first Obamacare must go. “That is unproductive, and there is

nobody who believes that is going to happen. But it is a tear-down political strategy – just tear down the other guy,” Bardis said. Rather, the conversation should be about what adjustments can be made to make the act better. States that rejected the Affordable Care Act have only hurt themselves, Bardis said. Those states have left more uninsured on the outside. “So I am waiting for someone to come up with a better idea,” he said. Bardis says he is a fiscal conservative, but 40 million uninsured is a looming social problem that cannot be ignored. MedAssets manages $400 billion in claims for U.S. hospitals, so this is a problem Bardis has wrestled with for the last 16 years. “We have a pretty good bird’s eye view of how payments and costs don’t coordinate with one another,” he said. Medicaid and Medicare today have 67 million on the rolls. That number will

grow to 85 million by the 2020s. Part of this is the product of the good health care that Americans receive today. Life expectancy has gone up every year for 100 years except 1919 and its worldwide flu epidemic. That is an actuarial nightmare as more people are living longer. When Bardis compares the economic growth of about 1.2 percent to 1.5 percent and health care costs cruising at 5 percent a year, the economic train wreck is only a question of when. “We can’t get a handle on costs. And we can’t survive with wage and earnings growth at under 2 percent and health care growing at 5 percent. That’s nearly a 500 percent disparity,” he said. The great challenge is to bring health costs into line with economic growth. America is 4.1 percent of the world population and represents 40 percent of health care dollars. The world GDP is $77 trillion, and the United States accounts for $18 trillion of

that, or 22 percent. But America spends 40 percent of the health care GDP. “That tells you we’re spending more than we should,” Bardis said. “We have 4 percent of the population consuming 40 percent of the global health care GDP.” Another disconnect in the economic balance is the United States has the largest pharmaceutical expenditure in the world, yet Americans also pay the highest price for drugs. In almost any other market, the largest buyer should be getting a discount. “These numbers tell me we can do better,” Bardis said. “And we have the resources to do it. We have the best medical infrastructure in the world. But by any measure, you have to conclude that America’s costs for health care are too high. “Costs will be the rule of thumb going forward,” he said. “You can’t have health care costs growing at five times that of the economy. That’s why costs have to be addressed.”

Hub: New resource
center connects needy to help By JONATHAN COPSEY JOHNS CREEK, Ga. – When Kate Boccia’s son, Daniel, 23, was found to have a drug problem, she didn’t know what to do. Daniel is currently serving a 15-year mandatory sentence in prison for armed robbery. “He had no priors, no weapons and stole nothing,” Boccia said. However, she said he was a heroin addict when he went to prison, where he was forced to get clean. “The reason my son is alive is because he is in prison,” she said. Only afterward did she find out an addiction specialist lived almost in her backyard. She is convinced her family would not be where they are now had she had a resource to help connect her to the help she needed. “My son wouldn’t be in prison if I had the Hub,” she said. The Hub Family Resource Center is a newly-formed nonprofit on State Bridge Road in Johns Creek. It aims to be the primary connecting point for families and the community in times of crisis with entities that can help. When someone is in need of mental wellness help, or rehab from a drug problem, it can be difficult to find the appropriate entity that can help. Executive Director Remco Brommet and the people at the Hub want to be that clearinghouse, pointing people to the resources they need. Working with local police departments for several years, Brommet said he met people in all stages of distress. “I met a lot of families and individuals in a wide range of crises,” Brommet said. “Everything from suicides to house fires and court issues. I began to hear a common theme – people tell us to get help, but we don’t know where to get it and who to trust.”
So the idea for the Hub was formed. The Hub hopes to help people and families find substance abuse recovery programs, psychological and counseling services and mental wellness programs, connecting them to nonprofits and educational programs that can help. “We want to be the central community place where families can come and be connected to service providers,” Brommet said. The need is evident, he said. One recent issue is the one with which Boccia is familiar – heroin use. Prescription use of opioid drugs among teenagers has skyrocketed in recent years along with the similar drug, heroin. Brommet said most children are able to become addicted to the drug first through their own home’s medicine cabinet. Sending addicted teens and the mentally ill through the judicial process can be daunting and expensive. Public safety and legal service policy is changing across the country, Brommet said. It is becoming clear that many of society’s ills – drug use, domestic violence, crime – often have a mental

health aspect. Milton Police Chief Steven Krokoff came to his post from Albany, New York. He said when he first started policing, the policy was to arrest drug offenders and fine them. If they cannot pay the fine, they get jail. “For a decade, the mantra was ‘the more you lock up, the better,’” he said. He then realized he was arresting the same people repeatedly, with jail having little effect on them. One day, he tried to assist one drug user in finding help. He spent a day trying to get help from services and was told it would be 60 days before any rehab center could accept the man. Krokoff failed to help him because it was too difficult to find the right support. “The easiest way [to deal with an offender] is to arrest them,” he said. “We need to find an easy way to direct offenders to services.” Brommet said the Hub aims to be that easy way. needed is dictated by how crowded or spaced out your teeth are at the beginning of treatment. Most treatments completed with aligner therapy involve less movement and shorter treatment times ranging between 6 and 12 months. However, Invisalign can be used in more difficult situations utilizing elastics to improve the bite and has also been used for more serious problems that require extractions or even reconstructive jaw surgery. Clear or ceramic tooth colored braces and wires have made orthodontic treatment more discreet and aesthetically pleasing while new super elastic memory wires have made treatment more efficient and comfortable. Heat activated super elastic memory wires are very flexible at room temperature and can be placed into the braces on extremely crooked teeth. Then, as the wire warms in the mouth, the wire exerts a light continuous force on the teeth causing them to move a
little every day. Surprisingly, the lighter forces move the teeth faster but with considerably less discomfort. Bulky braces and heavy forces are a thing of the past. Another recent innovation involves customized orthodontic appliances that are computer generated to work with each patient’s unique situation. This all-inclusive orthodontic solution includes customized brackets, wires, and placement trays. A computerized scan of the patient’s mouth is initially taken to diagnose and create a treatment plan. Customized appliances improve the efficiency of the treatment and minimize the time required to detail the bite and complete the treatment. Because of these innovations most patients finish treatment in significantly less time. Another innovation that cuts treatment time is Acceledent which can be used with any orthodontic treatment
modality. Acceledent is an intraoral device that generates gentle micropulses to accelerate bone remodeling. With just 20 minutes per day, the tiny vibrations caused by Acceledent can expedite tooth movement, and therefore treatment time, by up to 50% according to one clinical study. Telehealth is a new buzz word being thrown around in healthcare. What does telehealth mean to you? Your provider and you discussing your health via the internet. Programs such as Skype and FaceTime have made it easy to stay in contact with your family and friends. Now the health field is stepping up and using these new technologies to give you a virtual visit. If you are discussing medication side effects, trying to coordinate complex health issues, or mental health issues, a virtual visit sounds great. You and your provider can discuss face-to-face what is troubling you. However, what if your ear hurts? Or you are coughing? How does your provider look in your ears or listen to your chest via a virtual visit? Harmony Pediatrics is partnering with MedZed to address these real issues. Once a virtual visit is scheduled, a MedZed nurse comes to your house. She will weigh your child and take their vital signs. Then she will contact Dr. Chheda thru a secure video link. You and Dr. Chheda can discuss your child’s symptoms. Dr. Chheda will then perform an exam with the nurse’s help. The nurse will look in your child’s ears, nose, and throat with
an electronic otoscope. You will be able to view the image on the monitor along with the nurse and Dr. Chheda. The nurse will then place an electronic stethoscope on your child’s chest and Dr. Chheda can hear the lungs, heart, and abdomen. An actual visit will take place with a real person placing their hands on your child. Dr. Chheda can then diagnose and prescribe medication to help your child feel better. The nurse can even perform simple tests for strep throat, flu, and urinary complaints. This is an ideal situation for many families. One parent may be traveling and the at-home parent doesn’t want to take multiple children out in the cold. Some children are very sensitive to their environments and would be much more comfortable staying at home rather than go to a doctor’s office. You can continue to work at home and wait for the nurse to come to you, rather than facing traffic and waiting in a busy waiting room. The cost of the visit would be your regular office cost (co-pay/co-insurance/ deductible) plus an additional fee to offset the cost of the nurse and equipment. What a wonderful concept: competent care by your own pediatrician in the com-

common sports injuries that you see in your practice? A: In a typical day, an orthopaedic surgeon can see several different sports injuries such as rotator cuff tear injuries, ACL tears, meniscal injuries, sprains and strains of all joints, labral tears, shoulder injuries, golfer and tennis elbow, broken bones, ankle injuries, groin and hip injuries, and achilles tendon tears. The list of sports injuries can go on and on.

ties such as tennis and heavy gardening that can damage the rotator cuff. The rotator cuff tendon is vulnerable to injury in individuals over the age of 40 years due to diminished elasticity of the tendon. Minor trauma such as a ground level fall or lifting a heavy box can damage the shoulder. Repetitive muscle contractions from overhead activity can lead to inflammation and micro-tears of the tendon which can ultimately cause significant structural damage. The initial treatment is to relieve the inflammation and irritation of the rotator cuff tendons and bursa. Rest takes the form of activity modification which means stopping the offending activity such as sports. As the pain subsides with everyday activity, then one must begin a strengthening program to rehabilitate the rotator cuff. These exercises are performed below shoulder level usually with rubber tubing. Once strength has returned and pain has resolved with normal daily activities, then you can resume sporting activities. There needs to be a gradual resumption of sports to prevent recurrence of symptoms

injuries include Ligament tears and strains (ACL, MCL, PCL, and LCL), Cartilage injuries (meniscus tears), as well as Arthritis. There are many proactive ways to prevent knee problems. Maintaining normal body weight decreases stress across the knee and decreases risk of injury. Maintaining a strength training program that focuses on quadriceps and hamstring strengthening also protects the knee. Knee braces during athletic activities also act like an added “insurance policy” against injury. elping others through medicine is something Dr. Catherine “Cathy” Emeruwa, was born to do. “I was good in science and medical subjects in school,” Dr. Cathy said. “It was natural for me to fall into that area of study. Once I got into it, I saw I really had a gift for diagnosing diseases. It was almost like I had an instinct for being able to connect the dots and finding out pretty relatively easy how to not only diagnosis but treat medical illnesses.” Medicine may come easy to her because it’s in her genes. Her mother was a registered nurse and her father was a doctor focusing on microbiology. Born in Canada, Dr. Cathy moved to Nigeria as a child with her family after her father accepted a position heading the University of Nigeria’s microbiology department. In 1989, Dr. Cathy graduated with her medical degree from the University of Nigeria’s medical program. After working in Nigeria, London, Toronto and New York, Dr. Cathy moved to Atlanta in 2000 where she eventually started her own practice focusing on preventative medicine, integrative and holistic family medicine, bio-identical hormone therapy, IV vitamin therapy, medical weight loss and sex therapy. With these specialties, Dr. Cathy is able to help a wide range of people from birth to age 99. “I wanted that flexibility and to be able to treat anyone of any age and at any stage,” Dr. Cathy said. “I wanted variety including delivering babies and doing a lot of different treatments. I love the fact I can do anything within that wide range of medicine.” She opened two practices in Fairburn and Buckhead, but soon realized she needed to move north of Atlanta to Alpharetta. Currently, Dr. Cathy works with

Eventually, she wants to open more offices in other states and countries. “Patients from up north were coming down south to Buckhead,” Dr. Cathy said. “I thought ‘why don’t I open a shop in the north where I’ll get more patients’. It’ll also increase the range for patients to be able to get access to me.” With so many specialties, Dr. Cathy is currently trying to find her niche. Having more patients helps. Her philosophy is greater sex leads to greater health, and she knows the powerful link between sexual function and heart health lead to optimum overall health. “I’m currently treating patients holistically and conventionally by integrating both worlds,” Dr. Cathy said. “Now I’m focusing on sexual enhancement and trying to make sure patients understand sexual function is connected to the heart. They should not ignore their sexual functions because it means
they’re ignoring their heart. Improving the heart functions using sex helps create greater sex which creates greater health.” Part of helping patients with their health and sex life is being openminded. Dr. Cathy said she looks for patients who are ready to make a change or difference in their lives. “I still welcome those who aren’t ready, but I wait for them to be ripened and get ready to make some changes in their life,” Dr. Cathy said. “I try to make them understand they have control of their health. Ideal patients are willing to learn, inquire, ask questions and do research. I love patients who come in and say they want to learn and will be dedicated to learning more about themselves.” In doing so, Dr. Cathy said patients can expect her to go in depth to find the root of their problems. She says she will not give patients a cookie-cutter description, but will piece together the puzzle to best address their individual concerns.
“I like to customize their treatment and care because everyone is unique and has their own way of dealing with illness and getting better,” Dr. Cathy said. “They have to listen to their body so they’re able to have an individualized therapy plan to help transform them.” Aside from her medical practice, Dr. Cathy said she hopes to get more involved with medical advocacy. “I’m planning on going into more countries to let people understand sex is not a taboo, it’s healthy,” Dr. Cathy said. “It’s something women, men and couples should enjoy. I’m hoping to be able to talk more about sex education. A lot of young girls now don’t know what a lot of the sexually transmitted diseases are and they’re not protecting themselves. I want to be able to talk not only to women who are having sexual problems in their bedroom with their partner, but people who have been hurt, injured or raped. I want to be able to reach out to them also and give them options.”

loss in today’s society? A: The data is clear. Hearing loss among Americans is steadily increasing in prevalence. Estimates are that over 34 MILLION experience some form of hearing loss. Six in ten are men and of all people with loss, six in ten are UNDER 65 years old.
Q: How do I know if I have a hearing loss?
A: Trust your senses- if you
think you are not hearing like you should be, you probably have an issue. Asking for people to repeat themselves, trouble following conversations in various environments, having the TV on a loud volume, or an annoying ringing sensation are all warning signs. Your friends and loved ones know it too. The average person has been having trouble for 7 to 10 YEARS before deciding to get help.
Q: How is the overall health

Q: I know I have some hear-

Like fingerprints, no two ears are the same – even your own two. 9 out of 10 first time users do not consult with an actual Audiologist and/or choose devices based solely on price. Many also buy sound amplifiers marketed as “hearing aids” that simply just turn up the volume without any programmed help in situational hearing environments. These

Most people don’t look forward to having a hip replaced. But thanks to the less invasive anterior approach, patients who undergo hip replacement surgery now can expect a faster, less traumatic recovery. Using the anterior approach, Dr. Garrett is able to access the hip joint from the front as opposed to the back as many other surgeons do. “Surgery from the front is less invasive and less damaging to the surrounding muscle and hip joint,” Dr. Garrett says. “As a result, patients experience less pain, recover quicker and leave the hospital sooner.” In many cases, patients are able to go home the day of surgery. Other advantages to anterior approach include decreased risk of dislocation as well as more accurate restoration of natural anatomy and leg length. Dr. Garrett also specializes in less invasive total and partial knee replacement. “After a joint replacement, whether it’s a hip or a knee, our goals are really to allow patients to get back to the things they want to do,” Garrett says. “We do hip replacement and knee replacement surgery to help patients take care of the pain of arthritis, but we also want them to get back to their activities of daily living.” In addition to joint replacement, Dr. Garrett specializes in orthopaedic trauma and many other musculoskeletal conditions. age of 50 need a screening colonoscopy? A: YES. Men and women have a significant risk of colon cancer that is preventable. In the African American population colon cancer screening is recommended to begin at age 45. A colonoscopy identifies polyps before they become cancer so they can be removed, and finds colon cancers early so they can be successfully treated. Here are a few things you need to know: According to the American Cancer Society, next year almost 150,000 people will be diagnosed with colorectal cancer; the third most commonly diagnosed cancer in the US; and the second leading cause of death in the US. Having said that, it is also one of the most preventable cancers BUT only when detected early.
Q: How long does the pro-
cedure take? A: A colonoscopy is divided into 2 parts, putting the scope into the colon, insertion, and then pulling back the scope to examine the lining of the colon. The insertion time is usually 5 to 15 minutes and the withdrawal time is at a minimum 6 to 8 minutes or longer if the colon is twisty or if polyps are found that have to be removed. Q: Am I awake or asleep during the procedure? A: Some colonoscopists do keep the patient awake and administer small doses of sedatives. However, in my practice, we believe in patients remaining comfortable during a colonoscopy. In our center we use a licensed certified registered nurse anesthetist (CRNA) to administer medications to achieve conscious sedation with primarily Versed and Propofol, two short-acting
medications that will keep you comfortable and without pain, and with little to no memory of the procedure. Q: What are you looking for when you perform a screening colonoscopy? A: The first priority is to find polyps; the second is to find cancers. Q: Well, what is a polyp anyway? A: It is a growth on the lining of the colon. There are lots of types. What you need to know is that all colon cancers typically start out as a polyp, so it is important they are removed and that a repeat colonoscopy is done, typically in two to five years to make sure they have not come back. Q: So tell us about the dreaded “prep”? A: First of all, think of it as a cleanse! The bowel prep or

patients physician the dentist creates an appliance that moves the lower jaw down and forward to encourage the airway to open up and prevent the cessation of breathing. We create mouthpieces or ‘Mandibular Advancement Appliances’ that will allow you to protect a more open airway all night – and that will allow you to either see dramatic improvement with that treatment alone – or if the CPAP is still beneficial, you will be able to turn the pressure of the air way down as the obstruction is significantly less. That makes sleeping with CPAP much more likely.

A: I only give low dose chemotherapy through a delivery system known as insulin potentiation therapy (IPT). This innovative approach allows us to use about 10-20% of the typical chemo doses. The strategic use of insulin allows us to safely and effectively use much lower doses of chemotherapy because the drugs are believed to be more targeted to the cancer cells. A major advantage of this approach is that the body’s healthy cells are relatively spared, so the toxic effects of high dose chemotherapy are reduced. Another benefit to this approach is that unpleasant side effects such as nausea, vomiting, fatigue, and hair loss occur at a much lower rate, if at all.

do you typically use in your protocols? A: My protocols focus on high quality nutrition. My eat-
ing plan, The Stegall Protocol, is the culmination of years of research into cancer cells’ unique physiology. addiction. Many people have a hard time separating their work life from their home life, and with the influx of new technology, the workday seems to never end as our cell phones continu-
ously ping with new emails and messages. Although being committed to your job is great, being a workaholic is not. As the old adage says, if you take time to “sharpen the saw”—to renew your focus— your work will go smoother when you come back to it. Here are some signs that you might be a workaholic—and tips for creating a healthy work-life balance. You might be a workaholic if… ■ You can’t remember the last time you took a day off. The Fix: Everyone needs a break – whether you’re feeling sick or thinking about a vacation, there’s a reason you are given those days off. Take them. ■ You’re always saying yes. The Fix: Learn to say no. Think of it as an opportunity to let someone else shine in that role. And make sure you set aside time in your schedule for yourself. Learn to choose and prioritize the activities that will fill your day. It is important to spend time doing things that relax you and make you happy. ■ You see your coworkers more than your family. The Fix: If you’re constantly missing family commitments, coming home late at night and working on the weekends, you and your family will benefit from some time off, even if it’s just coming home earlier one night a

week for a family evening. ■ You do everyone’s job. The Fix: Learn to delegate responsibilities – you have coworkers for a reason. Trust that your colleagues will follow through. Check your work brain at the door when spending time with your friends and family. Studies show that stress can be a factor for many physical and mental health conditions. And being a workaholic certainly adds to your stress! So put down the phone and turn off the computer. ttention-deficit hyperactivity disorder (ADHD) is the name of a group of behaviors found in many children and adults. People who have ADHD have trouble paying attention in school, at home or at work. They may be much more active and/or impulsive than what is usual for their age. These behaviors contribute to significant problems in relationships, learning and behavior. For this reason, children who have ADHD are sometimes seen as being “difficult” or as having behavior problems. ADHD is common, affecting 4 to 12 percent of school-age children. It’s more common in boys than in girls. You may be more familiar with the term attention deficit disorder (ADD). This disorder was renamed in 1994 by the American Psychiatric Association (APA).
What are the symptoms of ADHD?
The child with ADHD who is inattentive will have six or more of the fol-
lowing symptoms: • Has difficulty following instructions • Has difficulty keeping attention on work or play activities at school and at home • Loses things needed for activities at school and at home • Appears not to listen • Doesn’t pay close attention to details • Seems disorganized • Has trouble with tasks that require planning ahead • Forgets things • Is easily distracted The child with ADHD who is hyperactive/impulsive will have at least six of the following symptoms: • Fidgety • Runs or climbs inappropriately • Can’t play quietly • Blurts out answers • Interrupts people • Can’t stay in seat • Talks too much • Is always on the go • Has trouble waiting his or her turn
What can I do to help my child? A team effort, with parents, teachers and doctors working together, is the best way to help your child. Children who have ADHD tend to need more structure and clearer expectations. Some children benefit from counseling or from structured therapy. Families may benefit from talking with a specialist in managing ADHD-related behavior and learning problems. Medicine also helps many children.
What medicines are used to treat ADHD?

Children who have ADHD may be difficult to parent. They may have trouble understanding directions. Children who are in a constant state of activity can be challenging for adults. You may need to change your home life a bit to help your child. Here are some things you can do to help: Make a schedule. Set specific times for waking up, eating, playing, doing homework, doing chores, watching TV or playing video games and going to bed. Post the schedule where your child will always see it. Explain any changes to the routine in advance. Make simple house rules. It’s important to explain what will happen when the rules are obeyed and when they are broken. Write down the rules and the results of not following them. Make sure your directions are understood. Get your child’s attention and look directly into his or her eyes. Then tell your child in a clear, calm voice specifically what you want. Keep directions simple and short. Ask your child to repeat the directions back to you. Reward good behavior. Congratulate your child when he or she completes each step of a task. Make sure your child is supervised all the time. Because they are impulsive, children who have ADHD may need more adult supervision than other children their age. Watch your child around his or her friends. It’s sometimes hard for children who have ADHD to learn social skills. Reward good play behaviors. Set a homework routine. Pick a regular place for doing homework, away from distractions such as other people, TV and video games. Break homework time into small parts and have breaks. Focus on effort, not grades. Reward your child when he or she tries to finish school work, not just for good grades. You can give extra rewards for earning better grades. Talk with your child’s teachers. Find out how your child is doing at school--in class, at playtime, at lunchtime. Ask for daily or weekly progress notes from the teacher.
Will my child outgrow ADHD? We used to think children would
“grow out” of ADHD. We now know that is not true for most children. Symptoms of ADHD often get better as children grow older and learn to adjust. Hyperactivity usually stops in the late teenage years. But about half of children who have ADHD continue to be easily distracted, have mood swings, hot tempers and are unable to complete tasks. Children who have loving, supportive parents who work together with school staff, mental health workers and their doctor have the best chance of becoming well-adjusted adults.

What should I do if I think my child has ADHD?
Talk with your child’s doctor. A diagnosis of ADHD can be made only by getting information about your child’s behavior from several people who know your child. Your doctor will ask you questions and may want to get information from your child’s teachers or anyone else who is familiar with your child’s behavior. Your doctor may have forms or checklists that you and your child’s teacher can fill out. This will help you and your doctor compare your child’s behavior with other children’s behavior. Your doctor will do vision and hearing tests if these tests haven’t been done recently. Your doctor may recommend trying medicine to see if it helps control your child’s hyperactive behavior. A trial of medicine alone cannot be the basis for diagnosing ADHD. However, it can be an important part of evaluating your child if ADHD is suspected. It might be hard for your doctor to tell if your child has ADHD. Many children who have ADHD aren’t hyperactive in the doctor’s office. For this reason, your doctor may want your child to see someone who specializes in helping children who have behavior problems.
What causes ADHD?
Children who have ADHD do not make enough chemicals in key areas in the brain that are responsible for organizing thought. Without enough of these chemicals, the organizing centers of the brain don’t work well. This causes the symptoms in children who have ADHD. Research shows that ADHD is more common in children who have close relatives with the disorder. Recent research also links smoking and other substance abuse during pregnancy to ADHD.

What are the options for bariatric surgery? Gastric Bypass Roux-en-Y The surgeon divides the stomach in two parts; the large portion of the stomach is closed off with staples to create a smaller pouch. The smaller stomach, your new pouch, restricts the amount of food you can eat at one time. The small intestine is cut below the duodenum and reattached to the new stomach pouch, leaving a shortened path for food to travel through so less food is absorbed. Laparoscopic adjustable gastric banding An adjustable gastric band is placed around the top part of the stomach dividing it into two parts: a small upper pouch and a lower stomach. The size of the band is adjusted using a port placed under your skin. Adjusting the band changes how much you can eat and how quickly food leaves the new pouch so patients feel full sooner and longer.
Gastric sleeve procedure The sleeve limits food intake by reducing the size of the stomach. A stapling device is used to divide the stomach vertically, leaving behind a thin vertical sleeve of stomach. The excised portion of the stomach is removed. *Talk to your doctor about specific risks associated with each of these procedures and which one might be right for you.
How do you prepare for bariatric surgery?
Like all major surgeries, the best preparation is to understand the risks and potential benefits, and to closely follow your doctor’s orders. To mentally prepare yourself: • Understand the surgical process and what to expect afterward. • Talk to people who have had weight loss surgery. • Write a letter to yourself and your surgeon explaining your reasons for having the surgery and outlining your plans to maintain your weight loss after surgery.
• Start a journal about your experience. Record how you feel now, the obstacles you encounter, the things you hope to be able to do after surgery. • Get support from friends and family. It helps to know you have people behind you.
What is life like after bariatric surgery?
The modifications made to the gastrointestinal tract will require permanent changes in eating habits that must be followed for the rest of one’s life to keep the weight from returning. According to statistics, about 80 percent of patients have long-term success following bariatric surgery. Newborn
Within just days of birth and discharge from a hospital, a child should visit a pediatrician. This type of physician specializes in the care of children, whose bodies are different from adults. A pediatrician will tell you if a newborn has any medical problems that cause concern, what their initial weight is and what you can expect for their growth, development and eating habits.

Toddler Years
During this time period, a child often sees a doctor more frequently than not, simply because of their developing immune systems and the exposure to childhood germs. However, in the absence of germs or injuries, it is a good idea to have a well visit once per year until the age of 10. Doctors will usually recommend the standard continued course of immunizations, including the chicken pox vaccine, which is not given until the child is at least 1 year old.
5 years Old
A child that is ready for Kindergarten will need to go to the pediatrician to be screened and immunized, in most cases. The screenings consist of hearing, vision and sometimes blood pressure or cholesterol tests. Also, the doctor’s office can give you the necessary forms for enrollment in a public school. Not crazy about vaccines, but still want to enroll your child in school? Schools now accept waivers (available at your school office) that allow parents who do not immunize to request enrollment for their child in school without having such vaccinations.
Early Childhood
From the ages of 6 to 10, annual well visits to the pediatrician are sufficient.
10 Years Old
Children usually go for another round of vaccinations.
11-18 Years Old
Keep in mind that at any point in the teen years when a child is participating in sports, most schools will require the student to undergo a routine physical to make certain that the student is healthy enough to participate in athletics.
18 Years Old
Many colleges require a physical examination upon acceptance to their school, and the paperwork involved with those tests may be required in order to attend classes. Along with proof of a physical exam, colleges will want
to verify that a collegeaged student has had all the required childhood immunizations. Young women at this age may also begin to schedule visits to a gynecologist, in order to schedule an annual Pap smear test. Women should begin breast self-examinations as well. At this stage of early adulthood until senior care, most adults will begin to fill out questionnaires that detail their health history. Questionnaires help physicians to pinpoint risk factors, and evaluate family history issues and other general health concerns. Once an adult enters the workforce and obtains health care coverage, a questionnaire is mandatory and must be filled out in order to qualify for health care. Typical questions refer to past medical history, family history, whether the person is a smoker, consumption of alcohol, drug use and any prior surgeries.
20s to 40s
ommendation may come earlier. But between the ages of 45 and 50, most men will receive the test for prostate cancer. The American Urological Association recommends a first PSA (prostate specific antigen) test at the age of 40 for most men. In a man’s mid-40s, doctors recommend a screen for Type-2 Diabetes as well.
40s for Women
Guidelines for mammograms change often, but as of now, doctors recommend a mammogram every year or two for women who are 40 and older. Beginning at age 45, doctors also recommend a cholesterol check for women and a blood pressure check every other year. A bone density scan may be performed if the patient is concerned about their specific risk of osteoporosis.
Adults should undergo cholesterol and blood pressure checks every 5 years, along with Tetanus and diphtheria boosters every 10 years. There are really no other recommended tests, unless a patient has an increased risk factor or family history of a specific disease or disorder. Skin checks for irregular moles and changes in the skin are recommended for everyone. Regular dental check-ups (twice per year) also provide optimal dental health. The influenza vaccine is now recommended for all ages.
By the age of 55, doctors begin to recommend testing for glaucoma, an eye disease that damages the optic nerve. Both men and women at this stage will most likely be checked for thyroid imbalances, eyesight deficiencies and blood sugar issues.
40s for Men
50s for Women
For men, 40 is the age at which doctors may begin to recommend a prostate cancer screening. Based on risk factors or family history, this rec-

60s
Doctors are beginning to recommend the Shingles vaccine around the age of 60 for both men and women. This is a once-in-a-lifetime shot that provides protection against the emergence of Shingles. Hearing evaluations are also important at this age, since nearly 33 percent of the population age 65 and older experiences hearing loss.
65 and Older – Men
Men are now encouraged to undergo testing for abdominal aortic aneurysms. This screening only occurs once, and is typically done after the age of 65.
Women
In addition to all of the previous tests, doctors may now begin to perform a bone density test. With age, women lose calcium, which causes bone breakage and weakness. A bone density scan will provide information on the strength of a woman’s bones. Since the incidence of falls also increases at this age, doctors highly recommend this test.
