he Christmas season is probably my favorite of all. I love the Holiday Cheer the season brings… gives me a little extra bounce in my step and a smile on my face. I love all the photo cards from friends I don’t see very often, and ones I see frequently. I love all the holiday baking…special recipes we only use once a year and new ones you're dying to try. I love all the classic Christmas movies, like “Rudolph”, “A Christmas Story” and “It’s a Wonderful Life”. I love all the festive holiday parties and cookie swaps, lunch with long time friends and Holiday Music on SiriusXM piped into my car. I love all the beautiful lights…everything seems to glisten more during the season. I love the smell of pine and wood burning fireplaces. I love Christmas Eve Candlelight Service and getting together with the family after for a night of celebration. I love the look on my kids’ faces when you get them the perfect gift -- and the look I get when they get the strangest one! I love lounging around all day in my new Christmas PJS’s while my husband spends the day putting some toy or flying contraption together. I love taking the time to reflect on all that is good, and look forward to ringing in the New Year! Happy Holidays to everyone, may your holidays be merry and bright! ■

ach year as a family we enjoy a holiday tradition that does not involve any sort of gift or reward ~ only the joy of participating in an annual event as a family. For the past 20 years or so, my mother-in-law has given me pieces of Christmas china and decorative items to dress up our Christmas table. Almost 20 years later, you can imagine what a beautiful table we have. From putting down the table pads and linens to the last beautiful dish and ornament, she has supplied everything and the boys have certain places and ways of putting out each gift from her that is tradition. There’s lots of giggles and arguing, making this a special time for our (now grown) boys & our family.

Thanksgiving weekend we “set the table” and we end the holidays with an elegant New Year’s Day traditional family meal with all the special china, stemware, and beautiful decorative items. Funny, it’s not tradition for anyone to help me pack away everything! What makes a family tradition is that it involves everyone and the memory that it creates in our brain never leaves us. While your children are young and while you have loving family and friends around, create your own special family traditions. They will soon forget that game, special pair of designer jeans, or tech device that they received as a gift, but the gift of family traditions is a gift that lasts a lifetime. Merry Christmas and Happy Holidays. ■

A

pril Olsen has a passion for her job, mainly because of the people she gets to help. Every day she gets to help the 125 adults with developmental disabilities who use enAble of Georgia, a nonprofit agency. The group was founded in 1979 by a group of concerned parents seeking a place in their community that could provide a nurturing home and quality support services for children. Now, the 125 adults are supported with residential services including 18 group homes, day services and supported employment. Olsen was hired in 2014 while interning with the organization during her senior year at Kennesaw State University studying human services — nonprofit administration. “I have a passion for the individuals we support,” Olsen said. “The development team fundraises year round to help support the folks that we serve. Ninety-nine percent of our individuals are on Medicaid waivers and anything ‘extra’ they might need comes from the funds raised by development.” With the increasing numbers of people affected by autism, Down syndrome, cerebral palsy and other developmental disabilities rising, Olsen said the need for these programs is huge. “There are thousands of Georgians on the waiting lists for waivers,” she said. “Often after high school these 8 | northsidewoman.com | december2017

individuals sit at home dreaming of the support and services needed to live a fulfilled life, something that many of us take for granted. By offering the services we do, it gives the individuals the opportunity to live, work and play in their community.” The adults take educational classes to teach skills in home management, communications, socialization, advocacy, rights, self-preservation and community living. Support is also provided to help them participate in leisure activities of interest. Community inclusion activities vary from participating in volunteer work, attending a local senior center to maintaining health and fitness at a local gym. Pre-vocational services are geared toward preparing individuals to join the work force. Support consists of teaching skills needed to find and maintain employment. Community Living Support Services are provided to those either living at home with their family or in the community in their own homes. In either case, the goal is to support people to live as independently as possible by providing coaching and assistance in the areas requested by the person and/or the family. Areas of training include meal preparation, bill paying, grocery shopping, personal hygiene and using public transportation. Additionally, enAble offers residential services

within group homes located in typical neighborhoods. These homes consist of no more than five people living together who have similar interests and/or support needs. Trained professionals provide 24-hour support to coach and mentor in all areas of daily living activities such as personal grooming and outward appearance, health and safety, communication and socialization, and home management skills such as meal preparation. Olsen said she often jokes the adults have a bigger social life than anyone in the office. “They’re very active in the community playing softball, bowling, doing arts and crafts or enjoying a meal out with friends,” she said. “The local parks and recreation have added great events for our folks. Many churches have adopted our homes and make monthly or annual volunteering events possible. We are lucky to have many supporters who embrace our individuals and donate throughout the year.” The group is currently planning its 30th annual gala for next spring, potentially held at the new hotel at Avalon. Cloudland Coffee finds a niche in the booming coffee world By CANDY WAYLOCK

L

ike many “accidental” entrepreneurs, Kristina Madh never envisioned her talent in creating the perfect cup of coffee would expand beyond her kitchen. Her growing list of clients thought otherwise. “This really started out as a hobby,” said Madh, who operates Cloudland Coffee from her home base in Johns Creek. “I became interested in roasting my own coffee because it was hard to find a coffee that was freshly roasted and not too dark.” She began experimenting with roasting techniques in her own kitchen, drawing upon the experience she acquired working in a coffee shop in college. Soon, Madh perfected the intricacies of roasting the temperamental beans, and the idea of a possible career began to take shape. A graduate of Eastern Michigan University, with an MBA from the University of South Florida, Madh had the business acumen to take on the challenge of small business ownership. She worked in the financial field till the economy crashed in 2008, then redirected her energy to raising her two children. In 2012, the family moved from Tampa to Atlanta, and was drawn to the Johns Creek area for its great schools, community feel and proximity to mountains and lakes. Once settled with her busy husband, two kids and two dogs, Madh faced a career crossroad: resuming her climb up the corporate ladder, or trying something entirely new. “I decided I would pursue starting my own business instead of going back into the banking world,” Madh said. “Ultimately I started Cloudland Coffee.” She named the company after her favorite state park, Cloudland Canyon in the northwest corner of Georgia, as well as after Arabica coffee, grown in the high altitudes where the mountains meet the clouds.

Madh secured a Cottage Food License and launched the business by selling directly to friends and Kristina Madh, Founder and Head Roaster of Cloudland Coffee neighbors. She learned all she could about the world of coffee, taking classes in roasting and brewing, networking through industry events such as the Coffee Fest and the Global Coffee Expo, and becoming a member of the Specialty Coffee Association. Coffee is big business – the average person drinks two cups a day – with a growing number of consumers searching for options beyond the industry giants. “Business starting picking up quickly because I think people really noticed how much better freshly roasted coffee tastes compared to the coffee that has been sitting on the shelf for a while,” Madh said. Soon her “little home roaster” could not keep up with the demands for Cloudland Coffee, especially with her move to the Alpharetta Farmer’s Market. Madh purchased a commercial roaster for her home, then an even bigger one when she moved the company out of her home kitchen into shared kitchen space at Prep ATL. She is a mainstay at the Alpharetta Farmer’s Market, which operates in in downtown Alpharetta from April through October. The market has basically become Cloudland Coffee’s storefront where she interacts directly with her customers. During the off season, she takes orders on the company’s website (www.cloudlandcoffee.com) and tends to her growing list of corporate clients. Madh only uses beans responsibly sourced by importers through programs including Fair Trade and Women Coffee Producers. She says more consumers want to know where their


food is coming from. “Ultimately, in order for my business to be successful, I need to offer a high quality coffee. The fair trade and sustainable farming practices play an important part in helping the farmers benefit financially, [allows] them to purchase better equipment to produce higher quality products, make improvements to their communities and provide more for their families.” She says this approach is a “win-win” for the farmers and the environment, and especially the consumers. “Most of the coffee that my importers bring in comes from smaller lots or cooperatives of small farmers,” she added. Her most popular coffees are from Ethiopia, and she also uses beans from Colombia, Ethiopia, Sumatra, Costa Rica and Mexico. The number of women involved in the coffee industry is growing, with an industry group formed that works exclusively with women-owned farms and cooperatives in

various countries, including Sumatra, Mexico, Guatemala, Brazil and others. “There are a lot of women producers in these regions,” Madh said. “Some may just own an acre or two, or manage a farm. The WCP (Women Coffer Producer) Sumatra that I have been offering comes from 282 coffee producers, all of whom are women.” Looking ahead, Madh has grand plans for the future of Cloudland Coffee. “I would love to be known as the ‘Official Coffee Roaster for the Alpharetta/ Johns Creek area’,” she mused, noting she is expanding her wholesale business segment to get into more restaurants and cafes. “It would be great to see my sign on several restaurants and cafes that says ‘proudly serving local coffee roasted by Cloudland Coffee Company’,’ she laughed, noting the company will have a presence in a downtown bakery beginning this month. For someone surrounded by the aroma of coffee and roasting beans all day, Madh said

her own coffee drinking habits are actually quite limited. “Really just one, maybe two cups in the morning. Sometimes I will have the occasional cappuccino in the afternoon. Shocking, right?” she said, laughing. ■

Black tie, Oh my! What the Black Tie dress code really means By LORI WYNNE Style Contributor ‘

T

is the season to receive holiday cards in your mailbox and invitations to the season’s most sought-after events. What? There’s a dress code? If the definition of the dress code has you flummoxed, think of your host/hostess and try to imagine what setting and vibe they are hoping to achieve at the event. Whether it’s a winter evening wedding or a holiday gala on your schedule this season, here are a few definitions to help you feel confident when choosing your formal occasion attire. Remember, when you look good, you feel good!

White Tie

The most formal of attire. This is your chance to shine. Full-length gowns are a must. Pair them with your best jewels. High slits in the skirt or overt showing of skin is not appropriate. If it is cool in the evening, wear a fur or faux fur. Men don white ties, white shirts with studs and winged collars, black jackets with tails, matching black pants, and high sheen shoes.

Black Tie Traditional black dinner jacket tuxedos for men, and fulllength gowns for women. When choosing a gown, keep in mind that your man will be dressed-up as he ever will be and you should match him. This means, no Hawaiian print muumuus. Choose velvet, lace, satin and silk in rich colors. Do not wear white at the risk of upstaging the bride. (Insider tip: If you do not want to invest in a fulllength formal gown, shop at consignment or thrift shops. It will save you a ton of money and you can donate it right back to the thrift shop after the event to save space in your closet. It’s even less expensive than renting the gown.)

Black Tie Optional This one can be tricky because men may want to wear their regular suit and risk looking underdressed. I recommend tuxedos for men and full-length gown or knee-length cocktail dresses for women. It’s better to be overdressed than underdressed at these types of festivities. Do not wear too short a skirt or too plunging a neckline or you will look like you are ready for the nightclub rather than a wedding.

Creative Black Tie Men, this is your chance to show your family heritage with your best Scottish kilt. Pair it with a short black jacket and bow tie. Ladies, if you are lucky enough to be out with a Scotsman this holiday season, this is your chance to match his creativity with your own plaid taffeta gown with a bit of sheen and a “fascinator” headpiece. Not a fan of kilts? Men can wear a tuxedo jacket made of velvet and pair with a traditional white tuxedo shirt, and black trousers. Ladies, consider a fanciful glittery top and statement skirt with dimensional details such as fabric petals, or shimmery fringe and glitzy statement shoes.

Final Tip Men have an easier time with “black tie” because the dress code’s name essentially tells them what to wear, a tuxedo. If the man in your life is invited to these types of soirees on a semi-regular basis, purchasing his own custom

fitted tuxedo is less expensive than renting one. He will feel confident and more comfortable in clothing he owns. Confidence in a man is such an attractive feature. With you on his arm in full holiday regalia, he will feel and look like your very own secret agent man. ■

areer opportunities brought Art McNaughton to metro Atlanta in 2005 where he applies his graphic arts and creative side as the brand equity director for O’Sullivan Industries. But his lifetime love of music and performing – planted when he saw the Beatles on the Ed Sullivan show – are frequently unleashed as a member of the Hard Luck and Trouble rockabilly group. During the group’s high energy performances, get ready for a show that could take you anywhere from Southern rock to Taylor Swift.

Where did your musical journey begin?

It really began when my aunt exposed me to her collection of Big Band and jazz music. Growing up in the ‘60s with the “British Invasion” combined with mass media and television as a platform for these performers, music became much more accessible and influential. In Chicago my twin brother, Roy, and younger brother, Bob, formed a band which really started our musical journey. In college, we played in the Jazz Lab band. After graduation we went down different paths but have all continued to perform — not letting corporate life interfere.

Define rockabilly and how it differs from other music genres?

Rockabilly is really the roots of rock and roll. It originally combined country music with rhythm and blues. The fact that it can be vocal and harmony-oriented like bluegrass supported by an infectious, energetic groove is very appealing to me. The songs tend to be simplistic in their orchestration and usually tell a story. Sometimes tragic, but usually fun and more than likely upbeat, which gets both the performer and audience moving.

What is it about live performances that you enjoy most?

How important is it to include music education in schools?

Music programs in school goes way beyond teaching kids to play an instrument. On an individual level, kids learn to master a skill in reading music and playing an instrument which requires discipline and perseverance. On a larger scale, music programs foster team building – kids learning to work together and perform as a single cohesive unit. It encourages diversity. My high school band was a melting pot of races and cultures. Music is an international form of communication and transcends all barriers.

people think painting is relaxing but it tends to be just the opposite. There is a certain excitement level generated when I paint. Just like playing music, a conversation starts up. At the end of the day, it can be very exhausting. In the coming months, I plan on devoting more time to painting. A few of my pieces have just recently been picked up by Kai/Lin Gallery in Atlanta with a few of them being displayed in a special showing at The Boiler Room of Ponce City Market. The exhibition will run now through January 14, 2018. ■

I think as a performer if you don’t interact with the audience you might as well pack it up and go home. Playing live offers its own set of challenges and opportunities. We receive requests and actually encourage it when we perform. There are certain songs, for one reason or the other, we really don’t care to play, but if someone desperately wants to hear it we try to accommodate them. weet Kalon is all dressed up for the holidays, but still has a forever home on his wish list. He is just three months old, and is one of 10 puppies born in a foster home after his mother was rescued from a shelter. The whole family has been living in a loving foster home while they await their new life in forever homes. Kalon’s pedigree is a bit of mystery. His mother appears to be a pointer/terrier mix, about 35 pounds, and Kalon will likely grow to be a medium to large dog in adulthood. He is one of the smaller puppies in the litter, with beautiful gray and brown brindle markings. Kalon is one of more than 200 dogs still waiting in a foster home or shelter who would love to ring in 2018 with a permanent family. Flying with Fido A guide to holiday travel

he fur will be flying this month as a growing number of pets take to the skies over the holidays. According to the American Pet Product Association, roughly 40 percent of pet owners would not dream of leaving their pets behind when traveling over the holidays. And the hospitality and travel industry is welcoming these four-legged companions with more accommodations, incentives and rewards to lure their business. But traveling, especially flying, with your pets takes advanced planning — both for their comfort and safety as well as to abide by the rules and regulations that are in place. The Humane Society of the United States (HSUS) recommends taking your dogs only if you have the time and attention they require – and urges caution for your feline companions. “Unless you'll be able to spend a lot of time with your dog, they'll probably be happier at home than tagging along on your trip,” advises Wayne Pacelle, president of HSUS. “As a rule, cats are almost always better off in their own home.” But if you are flying with your fur kids, here are a few suggestions from the Humane Society to make the trip easier (and safer) for your pets. Transport your pet in the cabin. Air travel can be risky. Always choose to transport your dog in the main cabin of the aircraft if possible. Some breeds, especially those with “pushed in” faces that limit oxygen flow, like bulldogs and pugs, should never travel in the cargo hold.

Does the airline have any special pet health and immunization requirements? Does the airline require a specific type of carrier? Most airlines will accept either hard-sided carriers or soft-sided carriers (which may be more comfortable for your pet), but only certain brands of soft-sided carriers are acceptable to certain airlines. If you can't take your pet in the cabin, does the airline have any restrictions on transporting your pet in the cargo hold?

Be aware of the dangers of flying your pet in the cargo hold While most animals flown in the cargo area of airplanes are fine, you should be aware that some animals die, are injured or lost on commercial flights each year. Excessively hot or cold temperatures, poor ventilation and rough handling are often to blame. Study the performance record of any airline before choosing to fly your pet in a cargo hold If you MUST put your pet in the cargo hold, follow these tips: • Use direct flights and travel on the same flight as your pet when possible. • Upon boarding, notify the captain and at least one flight attendant that your pet is traveling in the cargo hold. • If traveling during the winter months, choose an afternoon flight if possible to avoid extreme weather • Affix two pieces of identification on your pet’s collar: a permanent ID with your contact information, and a temporary travel ID where you or a contact person can be reached. • Affix a travel label to the carrier with permanent contact information, and where you or a contact person can be reached as soon as the flight arrives. • Make sure that your pet's nails have been clipped to protect against them getting hooked in the carrier's door, holes and other crevices. • Give your pet at least a month before your flight to become familiar with the travel carrier. • Do not give your pet tranquilizers unless they are prescribed by your veterinarian for flying. • Do not feed your pet for four to six hours before the trip. • Carry a current photograph of your pet to make it easier for airline employees to search if your pet is lost during the trip. • When you arrive at your destination, immediately examine your pet. Johns Creek cafe relies upon shared input of seven sisters By CANDY WAYLOCK

or more than 20 years, Hala Yassine worked to perfect her recipe for the perfect scone, eventually taking the staple of English tea times from her kitchen to her own cafe. Along the way, she relied upon the encouragement and business advice from her six sisters, scattered around the world from Boston to Dallas to Beirut. Seven Sisters Scones in Johns Creek reflects the continued support Hala relies upon to run her successful cafe. “I always loved cooking,” said Hala, who lives in Johns Creek. “When my husband was looking for a portable breakfast, I baked him a scone and he loved it!” Growing up in a family with seven sisters and two brothers, Hala learned to cook by both necessity and enjoyment. While attending college in California, she worked at restaurants as a chef and mastered the kitchen skills that serve her well in her own business. Her first attempt at baking scones came straight from a box, with an end product Hala found good, but too dry and crumbly; the typical scone. She started experimenting with other ingredients and soon landed on scone recipes that earned rave reviews from her family and friends. “The requests [for different flavors] soon started pouring in, especially from my sisters,” she laughed. “Can you make me an apple one? What about savory? Before I knew it, I had 30 flavors.” The decision to make her scones a business was a family affair. “While Hala was visiting her sisters in Boston, we all had a heartto-heart with her and encouraged her to make this into a business,” explained sister Farrah Haidar, who also lives in Johns Creek. “She didn’t believe anyone would actually pay for the scones!” In 2013, Hala started small – first baking under a cottage license and selling primarily at a local farmer’s market. The initial plan, she noted, was to focus on markets, special events and wholesale accounts. But within a year, she went from one farmers market to six, and soon outgrew the shared kitchen she was renting. In September 2015, Hala and Farrah opened the cafe, Seven Sisters Scones in Johns Creek, with a menu that began primarily with scones, and which remains the foundation of the restaurant. Hala explains the scones found at Seven Sisters Scones are different from traditional scones in a number of ways.

“First, these are not your average scones – dry and brittle. Instead, they are moist and filled with flavor,” she noted. “They are meant to be eaten as an entire meal – no need to add jam, butter or clotted cream.” The scones are baked every day for use that day, using only real butter, eggs and cream, and the freshest ingredients available. “When we look for ingredients, we are really looking for three things – quality, flavor and originality,” added Farrah. “We get as many of our ingredients as we can from local farmers and markets. We are really focused on the entire tasting experience – do the flavors blend well? How does it feel in your mouth? Does it have the appropriate texture? And is this something we would gladly eat?” Scone flavors include traditional blueberry, cinnamon, cranberry and orange, and vanilla to more exotic flavors of fig and goat cheese, and jalapeno, cheddar and olive. To draw in a lunch crowd, Seven Sisters Cafe began adding more menu items, including “sconewiches,” paninis, soups and salads. Soon, customers were asking for even more hot meals, so breakfast items, including omelets and quiches, as well as several types of pot pies, found a spot on the growing menu. “The expansion was gradual and really driven by what our customers wanted,” Farrah said. Hala still does the majority of the baking to keep the quality consistent, although more staff has been hired to help with production, under Hala’s careful supervision. Although only Farrah and Hala work full time in the company, all of the sisters play a role in the business success, whether through advice or

support. They often fly in from their homes across the globe to lend a hand during busy times and special events. The business also has evolved into the second generation, with Hala’s children, Teema and MJ, now taking a role in Seven Sisters Scones. They are especially involved in the continued presence of the company at area farmer’s markets. “We still love the markets and do somewhere between four and seven every weekend, depending on the season,” Farrah said. “They are a great way to connect with our customers and spread the word about our scones.” The scones can be found at farmer’s markets in Alpharetta, Johns Creek, Chattanooga, Fayetteville, Peachtree City, Marietta, Suwanee and at the Green Market in Piedmont Park. Looking forward, Farrah said Seven Sisters Scones has big plans. “Next year we are planning on doubling our retail and production space, allowing us to serve even more customers in the store. It will also give us the capacity to expand other services such as QVC and custom baked goods for other cafes,” she said. ■

Have a deliciously healthy holiday season A

Blueberries can help. “Blueberries are a small, tasty package with big health benefits,” advised registered dietitian and spokesperson for the U.S. Highbush Blueberry Council Patricia Bannan. “They’re sweet, tart and delicious but, at only 80 calories per cup, they make a nutritious addition to a holiday dish or an easy, on-the-go snack. Just rinse and go. Blueberries are also an excellent source of vitamin C, which can give the immune system a boost.” Blueberries are low in sodium with virtually no fat. They’re a good source of dietary fiber (3.6 grams per serving), which aids digestion, keep you feeling fuller longer, and help you resist some of those tempting seasonal treats. Blueberries complement different flavors and cuisines and add depth to sweet and savory dishes alike. For a festive twist, add them to favorite holiday recipes such as this:

BLUEBERRY GINGER JAM−FILLED SCONES Makes 16, 3-inch scones Blueberry Ginger Jam: 3 1/2 cups fresh (or frozen) blueberries 1/4 cup maple syrup 3-inch piece fresh ginger (about 2 tablespoons finely grated) 1 tablespoon lemon juice

Scones: 3/4 cup toasted pecans, ground into flour 1 1/2 cups all-purpose flour 1 cup whole-grain spelt flour 2 tablespoons sugar, plus 1 tablespoon for topping 4 teaspoons baking powder 1 teaspoon cinnamon 1 teaspoon kosher salt 3/4 cup cold low-fat buttermilk 1/4 cup cold unsweetened applesauce 1/3 cup cold unsalted butter, cut into ½-inch cubes 1 egg, for egg wash To a medium saucepan, add 2 cups of blueberries and maple syrup and cook over medium-high heat. Grate the ginger into the mixture. Cook for 8 minutes, letting the mixture come to a light boil. Once the fruit has cooked

down and the jam has thickened, add the remaining 1½ cups blueberries and stir in the lemon juice. Remove from heat and transfer to a bowl. Chill covered in refrigerator until ready to use. In a large bowl, whisk the ground pecans, flours, sugar, baking powder, cinnamon and salt. Set aside. In a medium bowl, combine the buttermilk and applesauce and whisk until smooth. Set aside. Add the cubed butter to the dry mixture. Using your hands, gently rub the butter into the flour until pea-sized pieces form. Pour half of the wet mixture over the dry mixture and gently combine, incorporating more of the wet mixture with the second addition. Pat the dough to quickly flatten and then gather it back into a mound and repeat. After two or three repetitions, the dough should be well formed. Form each scone by packing 4 tablespoons of dough, spoonful by spoonful, to form 16 balls. Transfer to two large plates. Dip the back of a 1-tablespoon measuring spoon in flour and press into each ball to form a well for the jam. Scoop 2 heaping tablespoons of the jam into each well. Freeze the prepared scones for at least two hours before baking to let the pastries set. Preheat the oven to 375° F and line two large baking sheets with parchment paper. For the egg wash, beat an egg with a splash of water in a small bowl and set aside. Remove the scones from the freezer and evenly space them over the sheet pans, at least 2-inches apart. Brush the outer rims of the dough with the egg wash and sprinkle with the remaining tablespoon of sugar. Bake from frozen until cooked and lightly browned, about 30 minutes. HALT Stress Management I

t’s a given. We all have stress. Somedays we are pretty good at managing it. Other days we are not. Whether we damage ourselves or those around us, whether the damage is immediate or sneaky & slow, the clincher is identifying that you are stressed out before you act crazy. Here is a quick and easy acronym to help you see the yellow flag before it becomes red: HALT. Hungry. Angry. Lonely. Tired. Hungry: Oh yes, we all know this one and can spot it in friends who are normally nice but just blew their top over something small or manageable. It’s as if those nerves bypass the many filters we have and have a short cut to regretful words and actions. Lonely: This one is surprisingly common. If your life feels like a hurricane, then you are probably lonely sitting in the eye. It seems calm, but you feel empty. It’s difficult to make decisions at this point because your perspective becomes warped. Go to the park and walk around. Say hi to everyone you meet. Smile at the children. Then make your decisions. People who drink alcohol and smokers are more likely to grind their teeth. However, Invisalign can be used in more difficult situations utilizing elastics to improve the bite and has also been used for more serious problems that require extractions or even reconstructive jaw surgery. Clear or ceramic tooth colored braces and wires have made orthodontic treatment more discreet and aesthetically pleasing while new super elastic memory wires have made treatment more efficient and comfortable. Heat activated super elastic memory wires are very flexible at room temperature and can be placed into the braces on extremely crooked teeth. Then, as the wire warms in the mouth, the wire exerts a light continuous force on the teeth causing them to move a little every day. Surprisingly, the lighter forces move the teeth faster but with considerably less discomfort. Bulky braces and heavy forces are a thing of the past. There are many different treatment options for today’s patients! Orthodontists are using everything from clear aligners to clear, metal or lingual braces in order to produce exceptional results. changing mole is a common complaint that leads a patient to a dermatologist. And it’s also one of the most important issues that a patient can have because a changing mole could be a melanoma. Melanoma is the most deadly common form of skin cancer and can spread even when it is the size of a pencil eraser or smaller. That is why dermatologists are so focused on catching melanoma early. If a skin cancer has not yet spread, then a simple surgery and a few stitches are often sufficient to provide a complete cure. If a cancer has spread, then lymph node biopsies, radiation, chemotherapy and other measures are sometimes necessary to try to save someone’s life. A dermatologist can help determine if a growth requires a biopsy. In general, a biopsy is a procedure intended to sample (not eliminate) a growth. A biopsied specimen is submitted to a pathologist who will determine if the growth is clearly benign, malignant, or somewhere in between. For atypical moles, pathologists typically grade how abnormal the mole is by grading them – mild, moderate or severe. This sounds simple, but there exists tremendous variation in how dermatologists manage moles that are assigned these grades. Some dermatologists watch almost all abnormal moles. Some dermatologists recommend cutting out (excising) almost all abnormal moles. Who is right, and why is there variation? Historically, some dermatologists believed that all atypical moles were precancerous. Such moles were (and often still are) called “dysplastic” nevi, although some dermatologists favor labeling them Clark’s nevi to avoid confusion over malignant potential. The term “dysplastic” connotes a tendency towards becoming a cancer. However, research now supports that many so-called dysplastic nevi will never amount to anything and are not “precancers.” There is a pattern of growth that some healthy moles can take that can make them look a bit disorderly, but for many moles “dysplastic” is a misnomer. As a result, a debate exists in the dermatology community over whether dysplastic nevi are precancers and whether they should be excised. A multitude of studies have followed patients whose dsyplastic nevi were excised versus observed. From these studies, we know that people who have dozens of dysplastic nevi still have a very high chance of developing melanoma even if they have every single mole on their bodies removed. We also know that removing mildly atypical nevi is generally not necessary and does not seem to decrease one’s chance of developing melanoma. What about moderate and severely atypical nevi? Mounting evidence now suggests that nevi with a grade of “moderate” probably also do not have to be removed and can be safely observed.

Some studies have even suggested similar observation for severely atypical nevi, but the norm for them continues to be excision. There are exceptions to the TAYLOR above rules: For some moderately atypical nevi, a pathologist may issue a recommendation that might read, “re-excision is recommended to ensure complete removal of this lesion.” This is pathologists’ way of signaling that the pathologist is still uncomfortable enough with the microscopic appearance that the mole should be removed. Alternatively, the mole might be graded as moderate atypia, but the dermatologist may have noted that clinically the mole looked severely atypical. Clinical impressions matter, and a mole that is moderately abnormal under the microscope but severely abnormal to the naked eye would also likely warrant complete removal. Patient factors such as immunosuppression, a history of melanoma, etc., all also might play a factor. However, in a world where profits sometimes drives people’s actions, patients are wise to be informed and be their own advocates. Unfortunately, multiple mildly or moderately atypical nevi are too often removed. Based on current evidence, many of these “dysplastic” nevi in all likelihood could have been safely left in place. Patient sometimes receive a few or, sadly, many scars unnecessarily. No dermatologist wants to leave in place a mole that becomes a melanoma, but neither should we subject patients to procedures and scars unnecessarily. By agreeing on regular skin exams, partnering with our patients to be vigilant for changing growths, and practicing evidence-based medicine, dermatologists can often safely leave mildly and moderately atypical nevi in place. Patient-centered, individualized medicine taking into account the patient’s entire history and risk factors can help guide the decision of how best to protect patients from the dueling perils of missing a melanoma and performing unnecessary procedures. If you have a changing growth or even a recently biopsied growth, seek an opinion from a dermatologist who you trust. Do not hesitate to ask why a procedure needs to be performed. If the answer does not seem reasonable or easily explained, then never hesitate to seek a second opinion. One additional tip: if you are worried about a possible melanoma, then ask your dermatology practice about adding an emergency “spot check”. Almost all dermatologists can and should allow a patient to come in quickly to rule out melanoma. oday there seems to be a cosmetic tattoo artist on every street corner. You can find them in beauty shops, spas and even nail salons. When searching for a permanent makeup artist, the consumer should research and do extensive homework. This is your appearance. You will be displaying the results for quite some time. You will be looking at your appearance in the mirror on a daily basis. You want to love it and be pleased that you made the right decision. One of the first things to consider is safety. Your health is valuable and you want to protect it. The public assumes that all providers RAHN are licensed and approved by “some” government agency. This is far from the truth in many locations. For example, microblading has fallen through the legal cracks in many states. It is marketed as “semi permanent” and therefore not governed under existing tattoo law or permanent makeup laws. It is important to be aware that “microblading” IS tattooing. Anytime the skin is broken and pigment is placed into the tissue, a tattoo has been created. Is your provider licensed? Does your provider attend annual Bloodborne Pathogen Training? Is your provider aware that the Hepatitis C virus can survive outside of the body for several days? This is long enough to pass the virus from client to client. Ask these questions and protect one of the most valuable assets you have, your health. Other things to ask consider: 1. Has your artist taken the time to answer all of your questions? Were you provided written pre and post care directions? 2. Will the artist schedule appropriate time for you? I have had clients tell me their previous provider was finished in 30 minutes. I block 2-3 hours per procedure. 3. Did your artist share an extensive portfolio of his/her OWN work? Do you both share the same vision of what you expect from the service? 4. Do you feel comfortable with your provider? 5. Was the office/studio clean? 6. Does your provider attend regular continuing education courses? Is he/she certified by The Society of Permanent Makeup Professionals or The American Academy of Micropigmentation?
