2018: A Liver Year In Review The year 2018 was an exciting year in liver diseases. It saw much progress to diagnosing, treating and providing improved access to care.

Hepatitis C

There were no new treatments introduced for hepatitis C this previously killer disease is simple to cure in almost all infected patients. Instead, focus has been shifted towards public policies aimed at eliminating hepatitis C and on improving access to its curative, life-saving therapies. New York State become the first state in the nation to set up a Department of Health Hepatitis C Elimination Task Force to develop strategies to diagnose and link to care the many people in the state who are as yet undiagnosed. Barriers to treatment such as patient drug and alcohol screening and limiting which providers can prescribe curative therapies have been removed in New York State.

disease, the importance of assessing disease severity is being disseminated to health care providers and patients. While liver biopsy had been the sole modality available to assess for degree of steatosis and fibrosis, readily available blood tests and advanced imaging techniques such as transient elastography and special MRIs are making it easier to rapidly assess and stage this condition. While diet, exercise and weight loss remain the current treatment, 2018 saw several new innovative, oral therapies being used reporting positive results in clinical trials. These results should lead to the first medications being approved by the FDA in early 2020.

Once thought to have a horrible prognosis, survival from primary liver cancer is improving. 2018 saw new chemotherapeutic agents being approved for its treatment which are better tolerated and have improved efficacy. These new agents, in combination with refined radiological therapies and liver transplantation, have made this condition manageable and curable in many patients who previously were not treatment candidates.

Primary sclerosing cholangitis

For the first time, this deadly condition with no adequate approved therapies is seeing several new agents being evaluated for treatment. While it is still early in development, just the fact that there are potential therapies being considered is exciting, groundbreaking and hopefully will bring much needed relief to the many people affected with this condition.

Alcoholic Liver Disease

Here the news is not good. Alcoholic

liver disease is on the rise in the United States, especially among people under the age of 30. The combination of increased alcohol use and advancement in hepatitis C treatments has made liver disease from alcohol the number one indication for liver transplantation in our country. There are no medications to reverse the effects of alcohol on the liver, only abstinence may lead to improvement. For this, we need the health care system to cover alcohol prevention programs to a greater degree and this is not happening.

While 2018 saw progress, there are still certain specific liver diseases where much work needs to be done. Hepatitis B can be suppressed but cure remains elusive and access to suppressive therapies is complicated by high cost. Autoimmune hepatitis has not seen much progress or even clinical studies with new agents making long term steroid and immunosuppressive agents the treatments of choice, despite their potential longterm side effects. For now, I would like to wish everyone a happy and a healthy New Year.

Parker Jewish Institute Resident Mary Luniewski Celebrates 101 Year Birthday the loving arms of his beloved wife, and the Luniewski’s raised a family, as many returning veterans did. Julius secured a good job working for Brooklyn Union Gas for the next 42 years. Meanwhile, Luniewski took care of the home while raising her family. She had three daughters and one son, and was blessed with 11 grandchildren and 12 great-grandchildren. In later years, the Luniewski family moved to their first home in Richmond Hill, Queens. Devoted to her Catholic faith, Luniewski became deeply involved with the Holy Child Jesus Church in Richmond Hill. Unfortunately, her beloved husband passed away in 1995. Luniewskiremained steadfastly independent, living alone in her Richmond Hill home, while receiving frequent visits

from her extended family. As it happens with all family matriarchs, the decades took their toll on Luniewski, who came down with pneumonia two years ago. With her loving family coming to her aid, she was brought to the Emergency Room at Long Island Jewish Hospital (LIJ) and admitted there for treatment. After she was stabilized, Luniewski was transitioned to Parker for long term skilled nursing care. “To this day, she frequently plays Scrabble with family visitors and her newfound friends at Parker,” according to her son Richard Luniewski. “Mom also enjoys games of 500 Card Rummy and Rack-O. I remember playing these games with my mother as a youngster.” Luniewski’s pneumonia was cured during her stay at Parker, and she’s remained

there ever since, as she is too frail and her legs too weak for her to live alone at home. Despite her age, Luniewski has undertaken several special projects. She enjoys crocheting Baby Hats for preemies, which are donated to LIJ. She created dozens of colorful Baby Hats up until last year. Many of the long term care residents have become Luniewski’s friends, and she enjoys taking part in the many activities Parker has to offer its patients and residents. Her experience at Parker has been very warm and supportive, according to her son Richard, adding that she looks forward to those special days of quality time with friends, loved ones and grandchildren, who frequently visit the centenarian-plus one. —Submitted by Parker Jewish Institute

ary Luniewski, one of Parker Jewish Institute’s longest-living residents, celebrated her 101st birthday last month. This joyous occasion brought family, friends and fellow residents together for a birthday bash on Parker’s sixth floor. Luniewski was born in Brooklyn on Dec. 18, 1917. It wasn’t easy for a young woman growing up in 1930s Depression-era Brooklyn, but in spite of that, Luniewski was able to find work, first as a bank teller and then as a secretary at the Brooklyn Navy Mary Luniewski, one of Parker’s Longest-Living Residents Yard. Cervical Cancer Awareness Month Nearly 13,000 women in the United States are diagnosed with cervical cancer each year, but what some may not know if that the disease is preventable with vaccination and appropriate screening (Pap and HPV tests). January marks Cervical Cancer Awareness month, a time to highlight issues related to cervical cancer, HPV disease and the importance of early detection. Regular check ups can help prevent cervical cancer from going unnoticed,

including receiving the HPV vaccination to prevent infection from both high risk HPV types that can lead to cervical cancer and low risk types that cause genital warts. Although it may seem a disease predominantly found in women, the CDC recommends that all boys and girls get the HPV vaccine at the age of 11 or 12 as the vaccine produces a stronger immune response when taken during the preteen years. A test strictly for

women, a Pap test can discover cell changes to the cervix caused by HPV and is recommended for women over 30 along with the HPV vaccine.
