Service Plan Brief for Diabetes Education Services Service Idea Diabetes education service is a program designed to help people with diabetes learn more about taking care of themselves. Additionally, the main focus of this clinical practice service is to enhance self-management of diabetes, which includes self-monitoring the blood glucose and adjustment of doses of insulin of an individual. The diabatic education service reflects on the possible hinders attainment and maintenance of glycemic targets (Burke et al., 2014). In the recent past, the development of diabetes has been ignored within the population, and the patient who does not receive formal education of diabetes have knowledge gaps. Moreover, these patients also tend not to receive any recommendation on preventive services that are more likely to develop chronic complications. Diabetes is considered one of the world’s largest, noninfectious chronic diseases, with its complications threatening individual health to different families and society (Gagliardino et al., 2019). The primary concern is that diabetes complications are the established psychological disorders that are mainly neglected by patients and their families. Therefore, the development of diabetic self-management is deemed to be the cornerstone leading to the provision of its education service plan. The diabetic education service is deemed a collaborative, ongoing process in an interactive form where people at risk need to gain the skills and knowledge to modify behavior and successful individual management of the disease and related conditions. Additionally, the diabetic education process integrates the goals, life experiences, and needs of people affected by the evidence-based standards (Burke et al., 2014). The overall objectives of diabetes education services in nursing are critical in supporting the informed decision-making process, problem-

solving, and active collaboration with health care professionals. The focus is to improve the clinical outcomes, health status, and quality of life for the patients affected by the conditions. Partnership with another clinic that offers health care education services would be significant to complete the process. A clinic such as Tampa Bay Community Hospital is one of the health care centers with established hospitals that can be useful for partnership. The partnership will be critical in offering diabetes education services through the clinic’s settings to achieve awareness of the disease. The comprehensiveness of the education program is established to determine and collect diabetic-related symptom information and hones the lifestyle factors of an individual, exercise habits, and possible barriers to self-management. The fundamental outcome of diabetes education service is to help address the changed behavior among the affected individuals. The assessment is one of the ways to attain the outcome through the process of designing an effective individualized program. The aim is to detail a comprehensive aspect relating to diabetes management and self-care of the patients (Gagliardino et al., 2019). Setting a goal is another step that involves a process that helps empower a driven model where clinicians guide people when choosing an area of focus. The aim is to establish measurable, specific, realistic, and attainable goals for every affected individual. There are various benefits associated with the development of diabetes education services in the health care system. The cost-benefit is one of the major concerns associated with the establishment of the service in clinics. Diabetic education service has proven to be cost-effective in many healthcare settings since it minimizes the hospital admissions and possible readmission for the affected individuals in the long term (Zheng et al., 2019). In addition, the overall lifetime healthcare costs related to diabetes people may be lowered significantly due to knowledge of different situations. The current costs involved in managing diabetics in the U.S. are about $350

billion, and the establishment of diabetic education services would be significant in minimizing such costs incurred (Powers et al., 2017). The other importance of diabetic education services within clinical settings is to provide shared decision-making for both providers through collaboration and patient information. Similarly, sharing decision-making is critical in health care to develop a patient-centered care system critical in improving patients’ psychological, clinical, and behavioral outcomes. For instance, the service can enhance sharing information on improving hemoglobin by about 1% for individuals with type 2 diabetes (Powers et al., 2017). The development of diabetic education services can also enhance collaboration and partnership of providers in different sectors. Health care providers require information from different departments to better understand the condition a patient may be experiencing based on the analysis and evaluations (Gagliardino et al., 2019). The patients would also benefit from the development of diabetes education services in many ways. Clinicians can teach them ways of self-management, such as knowing how and even when to take medication and monitor blood sugar levels (Powers et al., 2017). These skills are vital in minimizing complications that would be otherwise risky in the long run. In general, based on the importance of diabetes education service provided, providers’ primary focus is to improve access to quality care, awareness, and collaboration to deliver services effectively to the affected patients (Gagliardino et al., 2019). Similarly, a collaboration of the involved providers is also relevant in advancing and creating a specialized treatment. Market Analysis The prevalence of diabetes in the contemporary world is rising, and the next 20 years’ projection will double the figure. The global number of affected people with diabetes is about 450 million people (Zheng et al., 2019). The targeted people for diabetic education services

range from those already affected to those who do not know whether they have the condition. Further, for affected individuals, the target for diabetes education services provides for a formal program where patients and family members should participate to have self-management skills for any possible risks or complications that may be involved (Zheng et al., 2019). Additionally, the diabetes service’s potential referrals can involve the availability of centers for Medicaid and Medicare services across the country. The office-based health care teams can explore a necessary partnership to provide coordination of program management. This approach’s success needs time, resources, knowledge, and effective provision of education programs designed to serve as a resource for the health care team. The level of competition in diabetes education service provision is high, with different providers implementing new and integrated measures to meet the patients’ demands. For instance, the New York State Health Foundation provides the service and has a structure laid down to enhance diabetic education and management programs. The different competition levels come in through various priority focus that needs to be considered (Van Durme et al., 2014). These consist of improvement of clinical care and patient outcomes established within different primary care settings. There are various competitors for the planned service that include Bayfront Health Hospital, New York State Health Foundation, and Mayo Clinic hospitals. SWOT Analysis The SWOT analysis is a critical model that assists in examining the conditions and development of appropriate plans and strategies by assessing the available proficiencies and attributes that can foster a change of evidence in the condition’s practice and management (Van Wijngaarden et al., 2012). Further, SWOT analysis is an essential tool to evaluate external and internal environments to make systematic approaches in the decision-making process.
