A new seminar on transition of care
Patients born with congenital conditions like spina bifida or heart defects grow up with the same care team from day one. Then when they turn 18, that entire team changes which can be distressing, frustrating, and confusing for patients transitioning from pediatric to adult care.
Our own Dr Dan Wood is pioneering Transitional Care to make sure these patients don’t have to struggle as they enter adulthood.
