This manual contains information about using the RehaCom therapy system. Our therapy system RehaCom delivers tested methodologies and procedures to train brain performance . RehaCom helps patients after stroke or brain trauma with the improvement on such important abilities like memory, attention, concentration, planning, etc. Since 1986 we develop the therapy system progressive. It is our aim to give you a tool which supports your work by technical competence and simple handling, to support you at clinic and practice.

Training description

1.1

Training task The training "Overview and Reading" consist of two separate modules. Module 1 searching (overview): In the module searching, the task of the client is to find out a certain character (referred as target or target stimulus in the further text) from a variety of other characters (referred as NonTarget or distractor in the further text) on the screen. Depending on the presence of this target stimulus, a certain button must be pressed as fast as possible. If a target stimulus is indicated, the button "arrow left" on the RehaCom panel is to press. If no target stimuli is indicated, the button "arrow right" on the RehaCom panel is to press. Each level consists of a defined number of runs with a variable part of Target- or NonTarget runs. The target runs are characterized by a target stimulus, which is shown on the screen. In contrast are no target stimulus seen during NonTarget runs. Before every run, a black screen with a blue cross is shown for a certain time. The trainee's task is to fixe the cross.

Subsequently, actual stimuli are displayed on the screen. The number and variation of the distractors (NonTargets) increases with the level of difficulty.

After a certain number of runs, either a higher level starts or the same level continues. This depends on reaction speed and incorrect reactions. The following varieties are classified as errors: left arrow key pressed, although no target stimulus was visible (false positive reaction) right arrow key pressed, although a target stimulus was present (false negative reaction) Module reading: In the module Reading, a word, a number or a word group can be seen for a certain duration. Depending on the setting in the parameter menu, the presentation happens on the left half of the screen, in the center or on the right half of the screen. The client has to read the presentation on the screen aloud. The therapist decides at the touch of a button, whether the client read the word correctly or not. The pressing of the RehaCom button "arrow left" is rated as correct and the pressing of the RehaCom button "arrow right" as incorrect. The next word, which is read aloud, only appears after the therapist has evaluated the reading performance of the previous word at the

touch of a button.

After a certain number of runs, either a higher level starts or the same level continues. It depends on the number of errors. Assignment of the buttons: The pressing of the buttons of the RehaCom panel can be simulated with the following key combinations on the computer keyboard and the mouse. Performance feedback The performance feedback during a tasks occurs neither in module "Searching" nor in module "Reading". Only after the completion of the task, the client is informed about eventually incorrect reactions.

Levels of difficulty The module works adaptive. For the module "Reading" 51 levels are available. The module "Searching" has 18 levels. In the module "Searching" is an extra distinction between the presentation of the letter type and the shape type. In the shape type, different shapes (triangles, circles, arrows) are represented as stimuli in 6 different levels. A distinction is made in the training between 3 different stimulus modes (parallel, mixed, serial). The differentiation between Target and Non Target Stimuli decreases more and more from the parallel- to the serial stimulus.

The patient reaches the next higher level, if at least 90% of the performance were correct and the stated maximum power value was not exceeded. A demotion to a lower level does not occur. With it the following special rules apply to get to a higher level: A patient gets to a higher level, if one already solved the task at first try. If the patient does not solved the task at first try, one has to solve the task 2-times in a row correctly in order to get to a higher level. Was a change in the stimulus mode (in the letter variant from level 6 to 7 and from level 12 to 13, in the shape variant from level 2 to 3 and from level 4 to 5) the patient must solve the level 3-times in a row correctly. Training parameters In the Basic manual RehaCom general hints are given about the training parameters and their effects. These hints shall be taken into consideration subsequently.

Training mode: In the group training mode, the module, which has to be trained, can be chosen. Chosen end of the training: It is differentiated between the automatic completion of the training after the given consultation time (we recommend 25-30 min) and the completion of the training after

performing a given number of tasks. Module Searching therapy: Analogous to the diagnostics a therapy with shapes is possible in addition to the therapy with the letter type (standard). The size of the target stimuli and the distractors are determined with the parameters Size target and Size NonTarget. There are 4 different sizes to choose from. The color of the letters can be changed if required. The set color then applies for targets and NonTargets. It has no influence on the shape type because predefined colors are used. The maximum duration of a run is defined with the time limit. During training, runs with target stimulus (Target run) and runs without target stimulus (NonTarget run) are shown randomly. The number of runs with target stimulus can be increased or decreased with the button Percentage target runs. The interstimulus duration is the period between the reaction of the client and the presentation of the next stimuli. The sum of all Target and NonTarget runs within a level, can be set with the "Number of runs". Module reading therapy: For the presentation place of the words, which have to be read, the following possibilities are available: - left: presentation only in the left half of the screen - center: presentation only in the center of the screen - right: presentation only in the right half of the screen - random: presentation randomly per run (left, center, right) The font color determines the color of the words, which have to be read. The font size can be varied between 1 and 4. Caution: In higher levels, too long words or word groups can not be displayed in too large fonts. They are decreased automatically by the software, so there is no loss of text. The interstimulus duration is the period between the evaluation of the reading performance of a word (touch of the button by the therapist) and the representation of the word, which is to be read next. The number of runs can determine the number of words, which have to be read per level. The presentation duration of words or word groups is described in section Levels of difficulty. The there given presentation times can be increased or decreased by means of the parameter "factor presentation time". This factor has no influence on the presentation time in level 1 (diagnostics). Data analysis The various possibilities of analyzing the data in order to find strategies how to continue the training are described in the Basic manual RehaCom. In the graph as well as in the charts you find - beside the setting of the training parameters - the following information:

Reaction times are indicated in milliseconds. Only the reaction times, which were measured by the correct reaction, get included

in the statistic. Course: The chart below shows the course of reaction times for single runs of a level. For each Target- and NonTarget run a graph with statistic information is available.

Due to a detailed analysis of the training, it is possible to indicate deficits to the client and to draw conclusions for further training.

Information about the reading texts In this help file only the following sections are available in English: - Training task - Performance feedback - Levels of difficulty - Training parameters and - Data analysis. Information about the reading texts (Parallel forms for pre- and retest) The texts have an extend of 200 words each and they are taken from the fable "Der Rangstreit der Tiere" by G.E. Lessing. To improve the comprehensibility and to achieve an equal distribution of word length, both texts were appropriately modified. Tahoma 14 pt was chosen as font with double line spacing; hyphenation was deliberately avoided. Both parallel forms contain the same amount of words of equal length (number of letters); when preparing own texts (e.g. to have several parallel forms available), this distribution should be considered due to comparability. Instructions: Please read this text without any mistake, but as fast as possible. Evaluation: Reading performance in words per minute (WpM): needed reading time (in minutes)/200 - error Error: not corrected, wrongly read words Error: The number of reading errors of healthy control persons was altogether very low; the mean value (M) was 0.96, the standard deviation (S) 1.4. The factor education level had a significant influence on the error rate (without A level: M=1.4, S=1.8; with A level: M=0.57, S= 0.75). The factor age, however, did not have any

influence. This means, that persons without A level are considered as "normal" when making up to 3 errors, persons with A level when making up to 2 errors. This note is especially important for the classification of the reading performance after therapy. For both text variants 1 and 2, there were no significant differences regarding the errors. Cut-off values for WpM: The Cut-off values are based on survey data of healthy subjects (mean value - 1 standard deviation). The Cut-off values depend on age and education level; for the factor sex no significant differences in the reading performance could be found. Since healthy subjects showed a slightly different reading performance in both parallel forms, the values given below are indicated separately.

Der erste Schritt der visuellen Informationsaufnahme unter ökologischen (d.h. natürlichen) Bedingungen ist das Gewinnen eines Überblicks über die aktuelle Umgebung oder „Reizvorlage". Dieser „erste Blick" beinhaltet auch eine globale Kodierung der Außenwelt in Form einer grob strukturierten Szene, d.h. die wesentlichen Informationen über die räumliche Struktur dieser Szene als Grundlage für das Finden und die darauf folgende Verarbeitung von relevanten Reizen (z.B. von Objekten oder Personen) sind darin bereits enthalten (vgl. Abb. 1). Die „Relevanz" von Reizen kann entweder extern aufgrund der Auffälligkeit eines Reizes (bottom-up) oder intern (aufgrund von Intentionen; top-down) vorgegeben sein. In der Regel handelt es sich wohl um eine Kombination bzw. Interaktion extern und intern gesteuerter Verarbeitungsprozesse, da Vorwissen über die aktuelle bzw. zu erwartende Umwelt und in einem gewissen Umfang auch Intentionen praktisch immer gegeben sind. Das Gewinnen eines Überblicks erfolgt auf der Basis der globalen Verarbeitung einer Szene. Bei gegebenem Überblick kann ein Objekt („Zielreiz") dann leicht und unabhängig von der Anzahl der zusätzlich in der Szene vorhandenen Reize („Ablenker" oder Distraktoren) gefunden werden, wenn der Unterschied zwischen den beiden Reizklassen ausreichend groß ist. In diesem Fall spricht man von paralleler Suche. In der Regel genügt ein Blick, um den Zielreiz schnell und sicher zu entdecken, weil er aus der Szene sozusagen herausspringt („pop out"). Im Gegensatz dazu ist ein serieller Suchmodus erforderlich, wenn sich Ziel- und Ablenkerreize ähnlich sind (Müller, 2003). Das Aufmerksamkeitsfeld muss entsprechend verkleinert werden; die Suchzeit steigt entsprechend mit der Anzahl der Distraktoren an (Abb. 2). Der parallele Suchmodus kann somit als Form der globalen, der serielle Modus als Form der lokalen visuellen Informationsverarbeitung angesehen werden.

Eine Schädigung posterior parietaler Strukturen kann zu einem kontralateralen (visueller Neglect) oder bilateralen (Balint Syndrom) Verlust des Überblicks führen. Die Schädigung dieser Strukturen kann aber vermutlich aufgrund des Verlusts des globalen visuellen Verarbeitungsmodus auch das simultane Erfassen einer Szene beeinträchtigen, ohne dass ein visueller Neglect oder ein Balint-Syndrom vorliegen (Zihl, 2003; Zihl & Hebel, 1997; Zihl & Nelles, 2004). Die Einengung des Aufmerksamkeitsfeldes führt zu einer mehr oder weniger ausgeprägten ein- bzw. beidseitigen Einschränkung des Wahrnehmungsfeldes und zu einer unsicheren Orientierung innerhalb der Szene, sodass ein planmäßiges Abtasten erschwert wird. Die Folge sind (teilweise) Auslassungen von Teilen der Szene und -viel häufiger- der Verlust des globalen Verarbeitungsmodus. Die Szene wird bereichsweise und unsystematisch abgetastet, was z.B. bei visuellen Suchtests als teilweise sehr ausgeprägte „Verlangsamung" auffällt. Die wahre Ursache für den deutlich erhöhten Zeitbedarf ist das aufwändige Blickbewegungsmuster des Patienten (Zihl, 2000). Eine andere, sehr häufige Ursache für den Verlust des Überblicks stellen homonyme Gesichtsfeldausfälle dar. In diesem Fall ist die visuell-sensorische Grundlage für die simultane ganzheitliche Erfassung der aktuellen Umwelt nicht mehr gegeben. Diese Einschränkung des Überblicks wird vor allem durch eine Einengung des Aufmerksamkeitsfeldes verursacht; sekundär wird dadurch das vollständige Verarbeiten und damit Erkennen von Szenen, Objekten, Gesichtern, Wörtern und Zahlen behindert. Die ein- oder beidseitige Einschränkung des Überblicks wirkt sich auf die Lesefähigkeit insbesondere dann aus, wenn durch den Verlust parafovealer Gesichtsfeldbereiche das ganzheitliche Erfassen von Textmaterial (Wörter, Zahlen) als wesentliche Voraussetzung für eine kontinuierliche, flüssige Textverarbeitung nicht mehr möglich ist (Zihl, 1995). Abhängig von der Seite des parafovealen Gesichtsfeldausfalls haben Patienten Schwierigkeiten, den Anfang der Zeile bzw. eines Wortes zu finden oder das Wort- bzw. Zeilenende aufzusuchen. Der Prozess der Textaufnahme und -verarbeitung wir dann meist unter- oder sogar abgebrochen; durch (mehr oder weniger gelungene) Wort- und Textergänzungen wird in der Regel versucht, dem unvollständig und teilweise auch fehlerhaft verarbeiteten Text einen Sinn zu geben (Zihl & Nelles, 2004; vgl. Abb. 3). Meist sind sowohl die Lesegeschwindigkeit als auch die Lesegenauigkeit herabgesetzt. Noch auffälliger sind die Schwierigkeiten beim Verarbeiten von Zahlen, da hier selten eine durch den Kontext geleitete Kontrolle bzw. Korrektur möglich ist. Die durch die Einengung des Aufmerksamkeitsfeldes resultierenden Veränderungen der Informationsverarbeitung manifestieren sich auch in den Blickbewegungsmustern (Zihl & Hebel, 1997). Homonyme Gesichtsfeldstörungen stellen mit ca. 75% die häufigsten zerebralen Sehstörungen dar. Etwa 60% der Betroffenen haben einen reduzierten Überblick und ca. 80% eine beeinträchtigte Leseleistung; in den meisten Fällen liegt beides

vor. Da weder eine ausreichende Spontanrückbildung des Gesichtsfeldausfalls noch eine ausreichende Spontananpassung an den Gesichtsfeldausfall zu erwarten ist, besteht somit eine Indikation zur Behandlung, um die Alltagsbehinderung zu reduzieren. Der Erwerb okulomotorischer Kompensationsstrategien (Ersatz des Gesichtsfelds durch Blickbewegungen) hat sich dabei als effiziente, ökologisch valide und auch unter Kostengesichtspunkten günstige Behandlungsmethode erwiesen. Diese okulomotorischen Kompensationsstrategien können durch systematisches Training erworben werden (Zihl, 2000; Zihl & Nelles, 2004).

Diagnostik Homonyme Gesichtsfeldausfälle lassen sich nur mit Hilfe standardisierter perimetrischer Verfahren valide bestimmen. Andere Verfahren erlauben bestenfalls eine grobe Abschätzung des Ausfalls. Der Überblick kann mit Hilfe sog. Suchtests (z.B. Durchstreichtests; Zahlen-Verbindungs-Test; Trail-making-Test) überprüft werden, wobei neben der Suchzeit auch die Suchstrategie (z.B. systematische oder unsystematische Abfolge der bearbeiteten Zeichen) erfasst werden sollte. Für die Bestimmung der Leseleistung eignen sich standardisierte Lesetests mit definierter Schriftgröße und definiertem Umfang. Für die Verlaufskontrolle (z.B. Vergleich vor und nach Behandlung) sind Parallelformen erforderlich (vgl. Texte im Anhang). Für die Einordnung der Leseleistung sind auch Alter und Bildungsstand zu berücksichtigen (vgl. Leistungswerte im Anhang).

Für die sachgerechte Anwendung von Verfahren zur Erfassung der visuellen Suche und der Leseleistung sind folgende Ausschlusskriterien zu beachten: unzureichende Sehschärfe, vermindertes Kontrastsehen, Aphasie, reine Alexie, visueller Neglect, unzureichende Aufmerksamkeits- und Gedächtnisleistungen; Doppelbilder, Störungen der Konvergenz und Akkommodation. Bei Verdacht auf eine zusätzliche peripher bedingte Sehbeeinträchtigung (Gesichtsfeld, Visus, Farbsehen) sowie bei unzureichender optischer Korrektur sollte eine augenärztliche Untersuchung veranlasst werden (vgl. Übersicht bei Zihl, 2006).

Die wesentlichen Trainingsziele sind der (Wieder-) Erwerb eines ausreichenden und raschen Überblicks sowie einer ausreichenden Lesefähigkeit. Da die zugrunde liegenden Verarbeitungsprozesse unterschiedlich sind und zusätzlich auch unterschiedliche Blickbewegungsmuster als Kompensationsstrategie gelernt werden müssen (vgl. Abb. 4), ist ein spezifisches Vorgehen erforderlich. Zielvariable für die Verbesserung des Überblicks ist die Steigerung der visuellen Suchgeschwindigkeit beim Abtasten von Vorlagen bei gleichzeitig hoher Genauigkeit (möglichst keine Auslassungen) auch für komplexe Szenen (z.B. bei Kombination von globaler und lokaler Verarbeitung). Zielvariable für die Verbesserung der Lesefähigkeit ist die Steigerung der Lesegeschwindigkeit bei gleichzeitiger Abnahme der Lesefehler. Für beide Funktionsbereiche gilt, dass mit steigender Aufgabenschwierigkeit eine zunehmende Annäherung an reale Szenen bzw. die reale Lesebedingung erfolgt und damit ausreichend effiziente Kompensationsstrategien gelernt werden, die dann als Routinen verfügbar sind. Das Training hat somit zum übergeordneten Ziel, effiziente okulomotorische Kompensationsstrategien für den Überblick, die visuelle Exploration und das Lesen sicher zu etablieren. Der Transfer auf die verschiedenen Alltagsbedingungen, in denen natürlich weitere Einflussgrößen (z.B. Anspannung, Leistungsdruck, usw.) ins Spiel kommen, muss dann in eben diesen Situationen erfolgen. Ein kritischer Faktor für den effizienten Erwerb der beschriebenen Kompensationsstrategien ist die dem Patienten über die Instruktion vermittelte Vorgehensweise. Grundsätzlich gilt: Überblick vor Detailanalyse. Dies sollte deshalb auch das grundsätzliche Ziel des Trainings darstellen und im Laufe der Trainingsphase zur sicheren Routine werden.

Für das Explorationstraining bedeutet dies, dass der Gewinn eines globalen Überblicks der lokalen Analyse immer vorausgehen sollte. Der Patient sollte zuerst einen möglichst vollständigen Überblick gewinnen, bevor er eine Szene nach Details absucht. Dies geschieht mit Hilfe großer Blickwechsel, die systematisch erfolgen sollten, z.B. zuerst einer möglichst großen Blickbewegung nach links und dann nach rechts, unabhängig von der Seite des Ausfalls. Dabei ist unbedingt darauf zu achten, dass die Augenbewegung der Kopfbewegung vorausgeht, da sonst zwar eine Kopfbewegung generiert wird, das Auge jedoch nicht oder nur unzureichend in die gleiche Richtung bewegt wird. Entscheidend ist somit die Blick- und nicht die Kopfrichtung.

sowie die „Normalisierung" der Lesebewegungen nach dem Lesetraining vor allem hinsichtlich der Fixationsdauern (Pfeile in P-vor). Nach dem Lesetraining finden sich zahlreiche Regressivsakkaden (Pfeile in P-nach), d.h. die Fixation wird zu einem bereits gelesenen Wort oder Wortteil zurückgeführt. Suchzeiten in der Explorationsaufgabe: N: 7.4 s, P-vor: 24.6 s, P-nach: 11.2 s (keine Auslassungen vor und nach Training). Lesen: N: 172 Wörter pro Minute (WpM), P-vor: 64 WpM, P-nach: 137 WpM. Die Verlaufskontrolle nach 8 Wochen ergab für die Exploration ähnliche Werte; die Leseleistung betrug 156 Wpm (der Patient war gebeten worden, regelmäßig zuhause zu lesen und so die neue Lesestrategie weiter zu üben).

Für das Lesetraining gilt als Grundprinzip ebenfalls das ganzheitliche Erfassen von Worten bzw. Textpassagen. Dies bedeutet, dass Patienten mit linksseitigem Ausfall immer zuerst den Wortanfang aufsuchen, Patienten mit rechtsseitigem Ausfall immer zuerst das Wortende, bevor der „Leseprozess" einsetzt. Das zu frühe Einsetzen der Textverarbeitung führt dazu, dass der Prozess der Informationsaufnahme gestoppt oder zumindest unterbrochen wird. Ein flüssiges Lesen ist somit kaum mehr möglich. Durch den systematischen Erwerb von Kompensationsstrategien lernen die Patienten auch, ihren Überblick selbst zu kontrollieren, d.h. die exekutive (intentionale) Komponente des Blickverhaltens wird verbessert. Zusätzlich können durch das Aneinanderreihen von Trainingseinheiten auch Daueraufmerksamkeit und Belastbarkeit gesteigert werden.

Der Grossteil der Patienten mit homonymen Gesichtsfeldausfällen braucht sowohl ein Explorations- als auch ein Lesetraining. Patienten mit einem Restgesichtsfeld von mindestens 5 Grad links bzw. mindestens 8 Grad rechts weisen in der Regel keine nennenswerte Beeinträchtigung der Lesefähigkeit auf; in diesem Fall ist ein Lesetraining nicht indiziert. Die Entscheidung, ob ein Explorations- oder ein Lesetraining oder beides erforderlich ist, hängt ausschließlich vom Ergebnis der Eingangsuntersuchung ab. Liegen die erzielten Leistungswerte eindeutig unterhalb der Cut-off Werte, so sollte eine Behandlung durchgeführt werden. Im Einzelfall kann – und muss – manchmal von dieser Vorgehensweise abgewichen werden; dies hängt von der subjektiven Einschätzung des Patienten ab. Manche Patienten

besitzen bereits bei der Erstuntersuchung eine altersentsprechende Leseleistung, berichten aber trotzdem, dass ihre Leseleistung im Vergleich deutlich schlechter sei. Aufgrund der Art der ausgeübten beruflichen Tätigkeit erscheint diese Aussage in der Regel als durchaus zutreffend; die „prämorbide" Leseleistung lag offensichtlich deutlich über dem Durchschnitt. In diesem Falle würde ein Lesetraining durchaus indiziert sein. Die bisherige wissenschaftlich fundierte Erfahrung (Zihl, 2000; Zihl & Nelles, 2004) zeigt, dass mit den beiden Trainingsverfahren eine auch ökologisch relevante Reduzierung der durch Gesichtsfeldausfälle verursachten Störungen des Überblicks und der Lesefähigkeit erreicht werden kann. Es ist durchaus möglich, dass auch andere Zielgruppen (z.B. Patienten mit kognitiver Verlangsamung nach SchädelHirn-Trauma und damit assoziierter Minderung der visuellen Such- bzw. der Lesegeschwindigkeit) von diesen Verfahren profitieren können.
