Training description

1.1

Training task The training of the responsiveness happens by means of simple-, simple choiceand multiple-choice reactions. The training contains visual and acoustic stimuli. After the appearance of a defined visual stimulus or the reproduction of an acoustic stimulus, a certain button of the RehaCom panel is to press, as fast as possible.

Before starting the training, the loudspeaker symbol below on the Windows menu bar can be used to regulate the volume according to the client's needs. A slider appears, which can be changed. Check the volume in the RehaCom main window after pressing the button System. The window System Parameter opens up where you have to press the button Volume RehaCom (see Basic manual RehaCom).

This is particularly important when using headphones. Loudspeakers often have a turning knob to regulate the volume.

During the acquisition phase (figure 1), the patient familiarizes himself with the

specific reaction task. The patient memorizes the assignments of the relevant stimuli to the panel keys. The irrelevant stimuli probably used in the training are not presented. The client completes the acquisition phase by pressing the OK button.

If the option Practice is activated in the parameter menu, the acquisition phase is followed by a practice phase. In this phase, all different stimuli are shown twice. The practice phase is like the training, but historical data (reaction times and -qualities) are not registered. The practice phase is completed, when all reactions of the client were considered to be "correct". After pressing the OK button, the actual training starts.

The reaction training starts (figure 2). In the course of a task, the number of stimuli defined with stimulus number (see Training parameters) is shown or acoustically reproduced. After the appearance of a relevant stimulus, a certain button of the RehaCom panel has to be pressed as fast a possible. To minimize the memory components, the relations signal to button is always visible on the edge of the screen. If irrelevant stimuli appear, the client may not react. Incorrect decisions are indicated by a visual Feedback.

If several clients train in one room, headphones should be used.

The following types of error are differentiated: reactions in case of irrelevant stimuli no or too late reaction in case of relevant stimuli (reaction time is longer than

the maximum reaction time), incorrect reaction in case of relevant stimuli (wrong button pressed).

Before every training task instructions are given depending on the level, because each level of difficulty consists of specific tasks, which have to be performed (see Levels of difficulty). During instruction phase, "Learning by doing" helps to quickly learn the training task.

Performance feedback The feedback happens visually and acoustically.

Visual feedback: The following options are available for the visual feedback: · Reaction times: The reaction times of the last 10 signals are displayed as a bar graph. If the reaction was incorrect, the bar appears in red. This feedback can be irritating and should be used only for high-performance clients. Richtige Reaktionen innerhalb der max. If reactions were delayed, the phrase "too slow" appears. · No visual feedback.

Additionally, a performance bar is available. With every correct reaction increases a white bar. When the bar reaches the green mark, the level of difficulty is increased in the next task. If the mark is not reached, the client keeps on training with a task in the same level of difficulty.

At the end of each task, the client is informed how many errors were made and whether he/she has to work faster or not in case of an insufficient reaction speed. At the same time it is referred to the level of difficulty of the next task.

Acoustic feedback: After qualitative incorrect reactions, a warning signal will be heard. Correct reactions are not reported. If several clients train in one room and thus interferences are caused, the acoustic feedback should be turned off. Only the visual feedback should be used.

When a module with acoustic stimuli is used (e.g. module2), an acoustic feedback should always be avoided.

Levels of difficulty The module works adaptively. The next task will be activated, when the results of the previous task are below the set limits for the reaction rate and the error rate.

The levels of difficulty are based on the following criteria: Usage of four level types, Use of simple-, choice- and multiple choice reactions, Central and peripheral (randomly distributed across the training field) signal performance and Usage of relevant and irrelevant signals.

At level type 1 (new stimulus after reaction) the next stimulus appears not before the patient's reaction (maximum 60 seconds). The patient determines the processing speed. After a reaction a new signal appears. A relevant stimulus is constantly given up to the reaction. Irrelevant stimuli are not available in this type of level.

At level type 2 (stimulus interval constant) stimuli are shown in a fixed time interval (determined by the parameter stimuli interval). The computer determines the processing speed.

At level type 3 (stimulus interval stochastic) the next stimulus is shown after a random interval. The computer determines the processing speed. After a reaction, a stochastic time elapses up to the next stimulus. It is determined by the parameter stimulus interval 0 to - 75%. A stimulus is reproduced up to the expiry of the set time with stimulus duration.

At level type 4 the time changes between two stimuli adaptively in dependence of the reaction quality. The stimulus interval is reduced in case of correct reaction and

extended in case of incorrect reaction. There are significantly higher requirements on reaction speed rate and on discrimination ability.

For the therapist, 3 stimuli modules are available.

Module 1: Module 1 has 20 levels. In one level 5 tasks of one task type are always trained. In stimulus module 1 no acoustic signals are trained and only task type 2 and 3 get applied. The 5 tasks of one level differ in the assignment of signal and panel key. In default setting, the variation in chart 1 is mostly used:

As relevant stimulus symbols arrows are used, which match in direction with the key assigned arrow on the RehaCom panel (associative link).

Task 2 The OK button is used in connection with a cross symbol. In case of several relevant stimuli the procedure is like in task 1.

Task 3 Symbols without directional information are used (circles, rectangles etc.).

Task 4 Arrows are used as symbols, which are contralateral to the arrows on the panel keys.

Task 5 Symbols with and without directional information are used without reference to arrows of the penal keys (no associative link).

Module 2: The stimuli module 2 consists of 20 levels, each with just one task. In stimuli module 2 all 4 level types are used as well as visual and acoustic stimuli. The training in module 2 is only possible, if the footpadels are connected with the RehaCom panel.

Training parameters In the Basic manual RehaCom general hints are given about the training parameters and their effects. These hints shall be taken into consideration subsequently.

Current degree of difficulty: The degree of difficulty is adjustable from 1 to 20.

Duration of session in min: We recommend a length of 30 minutes.

Change of level: At the beginning of the training is checked, whether the set maximum reaction time or the maximum error percentage was exceeded during the last consultation. If this is the case, the client continues training in the same level and with the same task number. The next task with a higher level of difficulty is used if both criteria have not been exceeded.

Max. reaction time (ms): Besides the maximum error rate, the "maximum reaction time" is used as a criteria for the change of the difficulty. After the client completed a task, the median value of the existing reaction time is calculated. If the median value is above the maximum reaction time, the training is continued with the same task, despite of the not reached maximum error rate.

The increase of the "maximum reaction time" is displayed, when the focus of the training is put on reaction quality and when a time stressor is not used. As soon as

the client's performance is stabilized, each parameter should be reset to the default value. A decrease of this parameter functions as time stressor.

No. of stimuli / task: The sum of the relevant and irrelevant signals, which are displayed in the course of the task, is determined. When working in level type 4, the stimulus number should not be less than 100, to ensure a "stabilization" of the "medium stimulus interval".

Stimulus interval (ms): The parameter is of different importance for the four level types.

In level type 1 (new stimulus after reaction) the stimulus interval is not important.

In level type 2 (stimulus interval constant) the stimulus interval determines the time from stimulus start to stimulus start. If there is no reaction after the time set with the stimulus interval, the reaction is evaluated as incorrect and the next stimulus appears.

In level type 3 (stimulus interval stochastic) the stimulus interval describes the time from the client's reaction to the appearance of the next stimulus (interstimulus interval = stimulus interval 0 to - 75%).

In level type 4 (stimulus interval stochastic) the stimulus interval is the start value for the "medium stimulus interval". When the reactions were correct, the stimulus interval is reduced by 3%. When the reactions were incorrect or omitted, the stimulus interval increases by 3%.

Stimulus duration: The stimulus duration describes the display duration of a stimulus on the screen.

Feedback: The possibilities of the feedback have already been described in chapter "Performance feedback".

Help: For the stimulus reaction training, various help options are available. When the box practice is marked, the client passe through every task in practice mode first. The practice mode is like a training, but without saving the results.

When choosing "with imagination of stimuli", all relevant stimuli of the task are represented successively with the corresponding button. The client has to press the correct button for each stimulus once to make himself familiar with the task. If a stimulus module includes sounds, the option "with imagination of stimuli" has to be activated.

If the option "start level with task 1" is marked, the client always starts the consultation with task 1 of the selected level. This option should be used for clients who had a longer pause since the last consultation in order to avoid overwhelming them with too difficult tasks.

When newly defining a patient, the system automatically uses the following default values:

With irrelevant stimuli: Besides relevant stimuli, irrelevant stimuli are displayed as well. When irrelevant stimuli are shown, the client may not press any button.

Correlation stimulus reaction: Various stimulus modules can be selected from the selection menu "stimulus reaction assignment". Stimulus modules can be created, edited or deleted again in an Editor via the buttons "new stimulus module", "edit stimulus module", and "delete stimulus module". Stimulus modules which are marked with a belong to the default modules and thus can not be changed or deleted.

High contrast: The module Responsiveness also supports the setting "High contrast" which can be defined in the client data. If the parameter "High contrast" is activated, the stimuli in module 1 are displayed enlarged.

Stimulus module editor In this help file only the following sections are available in English: - Training task - Performance feedback - Levels of difficulty - Training parameters and - Data analysis.

In the stimulus module editor, the user can create new stimulus modules. Already existing modules can also be changed. Every module consists of 20 levels with 5

tasks each. Different combinations of stimuli can be assigned additionally to every task.

Different task combinations are chosen randomly by the computer during the training and ensure an extremely varied training for the patient.

Level: Select the level, which needs to be edited by the user, from the "Level" list. This list is always filled with a number of 20 entries.

Chosen level: In the group "Chosen level", the level is always displayed, which is selected in the list "Level". The respective level type (1 to 4) can be assigned via the selection box "Type of level".

Combinations: The user has to chose the task type (1 to 5) in the group "Combinations" first, which has to be edited or viewed. Any number of stimulus combinations for the selected task can be added or deleted by the user via the buttons "Add combination" and "Delete combination". In the table columns "Relevant images", "Non-relevant images", "Relevant sound" and "Non-relevant sound", the number of relevant and irrelevant stimuli of the selected combination is automatically displayed. The respective stimuli of the combination are visible in the table "Images of the combination" or "Sound files of the combination" on the tabs "Images" and "Sounds".

Combination background: Another background image and (or) background sound can be added to every combination via the button "Combination background".

Via the buttons "New image", "Delete image", "New sound" and "Delete sound" the background of the combination can be modified acoustically and (or) visually. If no background has been selected, all stimuli are presented on the usual green RehaCom-background. Only image files with the format "*.bmp" and sound files with the format "*.wav" may be used as file format. The respective background image or the background sound are only activated in the training, if the option "Use background image" or "Use sound file" is marked.

Additionally, the volume (loud, medium, quiet) can be set for the background sound.

The parameter "Loud" refers to the current setting of the Windows system. The parameter "Medium" and "Quiet" corresponds to half or quarter of the volume "Loud". To play a background sound permanently during training, the option "Endless" has to be selected.

Images: On the tab "Images", visual stimuli can be additionally added to or removed from the chosen combination (marked combination from the list "Combinations").

In the list "All images", all images are listed which can be added to a stimulus combination. Besides the actual stimulus, to which the patient has to react later (active image), this stimulus may contain a second image (passive image). The passive image can be used as advance notice for the active stimulus during training.

The passive image is displayed during the entire duration of the task and determines the position, where the active image will appear later. In the image table, the columns for the active and passive images are marked with an "A" (active) or a "P" (passive).

Add / Remove image: I order to add an image to a stimulus combination, you first have to select the searched image from the table "All images". Subsequently, the requested parameters for the picture need to be chosen from the group "Image properties".

The image position can be set or changed in the yellow grid by a simple mouse click. This grid corresponds to the later training field for the patient. If the image position shall be randomly chosen by the computer during the training, the user has to mark the option "Stochastic position".

The image size can be selected from the selection box "Image size". 3 different image sizes are available: big: 160 x 160 Pixel medium: 80 x 80 Pixel small: 40 x 40 Pixel From the selection box "Choose button", a button of the RehaCom panel can be assigned to an image. The client has to push this button later in the training, when the respective image will appear. When using RehaCom panels with only one "OKbutton" both, the left and the right OK-button, can be selected. For subsequent training, only one OK-button is related to the chosen stimulus.

The assignment of one button to a certain image is only necessary in case of a relevant stimulus.

The user has to enable the option "Relevant stimulus" if the chosen image shall be a relevant stimulus. If the advance notice (passive image) shall be additionally displayed for this stimulus, the field "With advance notice" has to be enabled.

Furthermore, only the passive image of a stimulus can be displayed with the option "Only advanced notice". Those pictures are visible for the client during the entire task duration.

To consider all visual stimuli of the marked combination in one overview, the user has to press the button "Preview". This preview corresponds to the later display of the images in the training.

taken over in the table "Images of the combination".

In order to display different images with different frequencies, the same images can be added in the table "Images of the combination" as often as required.

If the user wants to remove an image from the table "Images of the combination", he has to press the button " ".

Add / Delete sound file: In order to add a sound to a stimulus combination, you first have to select the searched sound from the table "All sound files". Subsequently, the requested parameter for the sound files need to be chosen from the group "Sound properties".

The volume of the sound file can be selected from the selection box "Volume". 3 different volumes are given (Loud, Medium, Quiet). The parameter "Loud" refers to the current setting of the Windows system. The parameter "Medium" and "Quiet" corresponds to half or quarter of the volume "Loud". To play a sound file during the stimulus duration of training permanently, the option "Endless" has to be selected.

From the selection box "Choose button", a button of the RehaCom panel can be assigned to a sound. The assignment occurs according to the key allocation for an image.

The user has to enable the option "Relevant stimulus" if the chosen image shall be a relevant stimulus.

By pressing the button " ", the selected sound file with the set sound properties is taken over in the table "Sound files of the combination".

In order to display different sounds with different frequencies, the same sound files can be added in the table "Sound files of the combination" as often as required.

If the user wants to remove a sound from the table "Sound files of the combination", he has to press the button " ".

Modify stimulus from combination In order to change a selected stimulus from the tables "Images of the combination" and "Sound files of the combination" in its properties for the training, the user has to adjust all settings in the group "Image properties" and "Sound properties" to the requested parameters. Then, the button " Modify" has to be pressed.

Caution: A maximum of 8 different relevant stimuli (images or sound files) can be added to one combination.

Add / Edit / Delete new stimuli In order to add or delete new images or sound files to/from the master data, the

button "Modify stimuli" on the tab "Images" or on the tab "Sound" has to be used.

The dialog "Create stimuli" consists of the group "Images" and the group "Sound files".

The field "Images" contains a list of all image stimuli which are currently available for the different stimulus modules. Besides the actual stimulus, to which the client has to react to later (active image), this stimulus may contain a second image (passive image). The passive image can be used as advance notice for the active stimulus during training. The passive image is displayed during the entire duration of the task and determines the position, where the active image will appear later. In the image table, the columns for active and passive images are marked with an "A" (active) or a "P" (passive).

In order to add a new image to the table, you have to press the button "New image" on the tab "Active image". Only image files with the format "*.bmp" may be used.

After an "active image" was added, a passive image can be assigned to this image.

Therefore the button "Change image" has to be confirmed on the tab "Passive image". There is no obligation to assign a "passive image" to the "active image".

As image name, the file name of the image is used as standard. If you want to change the name of the image, you have to enter the new image name in the input field "Image name". After that press the button "Change name".

The currently used images of the selected list entry can be exchanged or removed via the buttons "Change image" and "Delete image".

In the field sound files, all sound stimuli, which are currently available for the different stimulus modules, are listed. Via the button "New sound", "Change sound" and "Delete sound", new sounds can be added, exchanged or deleted. Only sound files with the format "*.wav" may be used. The name change of a sound file occurs in the same way like the change of the image file name.

Each selected sound can be played, paused or continued via the buttons "Play", "Stop" and "Next". If the option "Endless" is marked, the sound file is permanently played. The option "Endless" is only used for testing the sound file.

Caution: The images or sound files, which are marked with a in the column "Standard", belong to the standard stimuli and therefore can not be changed or deleted.

Data analysis There are various possibilities of analyzing the data in order to find strategies how to continue the training which are described in the Basic manual RehaCom. Only reactions with relevant stimuli are used for calculation.

So it is possible to indicate certain deficits to the client and to draw conclusions for the further training.

Course: The graphic below shows the course of the reaction time for single tasks of a level. In the history chart, different error types and right reactions are displayed in varied colors.

2.1

Das Reaktionsvermögen setzt komplexe psychophysiologische Leistungen voraus, die es erlauben, auf externe Reize in bestimmter Weise zu reagieren. Phasische Aufmerksamkeitsparameter spielen eine große Rolle beim Reaktionsverhalten.

Phasische Aktivierung ist definiert als die Fähigkeit, auf einen Warnreiz hin rasch das Aktivierungsniveau für eine nachfolgende Reaktionssituation zu steigern (Reaktionsbereitschaft, Alertness), während ein über längere Zeit relativ stabiles Aufmerksamkeitsniveau als tonische Aktivierung bezeichnet wird.

Die in diesem Zusammenhang relevante Fähigkeit zur selektiven Aufmerksamkeit bezeichnet die Fokussierung auf bestimmte Aspekte einer Aufgabe, die es ermöglicht, schnell auf relevante Reize zu reagieren und gleichzeitig irrelevante Reize zu ignorieren (vgl. Sturm et al., 1994).

Voraussetzung für eine allgemeine Leistungsfähigkeit hinsichtlich verschiedener kognitiver Anforderungen dar.

Die Aufmerksamkeit gegenüber relevanten Umweltreizen ist von internen Organismusvariablen (physiologischer Status, kognitive Prozesse, Emotionen) und äußeren Faktoren (Reizintensität, Kontrast, Farbigkeit, Konturierung, räumliche Beziehung usw.) abhängig. Durch besonders intensive oder neuartige Reize (mit hohem Informationsgehalt) kann automatisch, d.h. unwillkürlich die Aufmerksamkeit durch eine Orientierungsreaktion fokussiert werden.

Sternberg (1969) (vgl. Keller & Grömminger, 1993) unterscheidet in seinem handlungsorientierten Aufmerksamkeitsmodell vier Phasen: Wahrnehmung, Identifikation der relevanten Reize, Wahl der Reaktion und Starten eines motorischen Programms als Reaktion auf den Reiz.

Diese Prozesse laufen teilweise automatisiert ab. Bei der Erfassung spezifischer Situationsaspekte werden aktive Analyseprozesse in Gang gesetzt. Automatisierte Prozesse laufen mit wenig Kapazität parallel ab, während alle anderen eine serielle Verarbeitung erfordern, die mit größerer Aufmerksamkeitskapazität generiert werden und somit langsamer zu bewältigen sind.

Bei jeder Reaktion lassen sich mehrere Phasen unterscheiden: Steigerung des Aufmerksamkeitsniveaus in Erwartung eines Reizes.

Reizpräsentation.

Latenzphase Entscheidungszeit motorische Handlung.

Reaktionszeit ist die Bezeichnung für ein Zeitintervall zwischen der Reizdarbietung und der Ausführung einer einfachen motorischen Reaktion. Sie setzt sich aus der Latenzzeit (Dauer der Erregungsleitung im Nervensystem) und der Entscheidungszeit (Dauer der Informationsverarbeitung) zusammen (Fröhlich, 1987).

Die Reaktionsgeschwindigkeit wird im Zusammenhang mit der Geschwindigkeit der Informationsverarbeitung gesehen, deren häufigste Operationalisierung die Untersuchung mit einfachen und komplexen Reiz-Reaktionsversuchen darstellt (Säring, 1988).

Münsterberg (1924) unterschied zwischen Einfach- und Wahlreaktionen. Zu den Wahlreaktionen zählt er: einfache Wahlreaktionen, bei denen mehrere Reize dargeboten werden, aber nur auf einen kritischen Reiz reagiert werden soll

Die Reaktion auf kritische Stimuli bei Mehrfachwahlreaktionen wird außerdem durch weitere Faktoren beeinflusst: Art des Stimulus / Signal (akustisch, optisch, thermisch, etc.), Art und Grad der Signaldifferenzierung, Auftrittshäufigkeit relevanter, kritischer Stimuli, Möglichkeit assoziativer Kopplung zwischen Reiz und Reaktion.

Die Fähigkeit, durch eine motorische Handlung auf akustische Reize zu reagieren, erfordert zwei Basisfähigkeiten: ein intaktes Hörvermögen und ein intaktes Reaktionsvermögen. Die richtige Verknüpfung von akustischem Reiz und motorischer Reaktion kann nur auf der Grundlage der richtigen Interpretation der Bedeutung des Reizes erfolgen.

Hörstörungen können periphere oder zentrale Ursachen haben.

Zerebrale Hörstörungen lassen sich nach anatomischen , psychoakustischen und neuropsychologischen Kriterien unterteilen.

Man unterscheidet: Psychoakustische Defizite können neuropsychologisch unter dem Begriff "Störungen der akustischen Diskriminationsleistung" zusammengefasst werden.

Erst über die Phonemdiskriminierung hinaus gehende Störungen der Sprach- oder Geräuschwahrnehmung sollten als akustische Erkennensstörung bezeichnet werden (Scherg, 1988).

Bei unilateralen telenzephalen Läsionen betrifft die Beeinträchtigung hinsichtlich einfacher psychoakustischer Parameter unabhängig von der Hemisphärendominanz jeweils das Ohr kontralateral zur Läsion; diese Beeinträchtigungen können je nach Läsion verschiedene Profile aufweisen (Efron et al., 1985). Die Fähigkeit, relevante akustische Signale heraus zu filtern, fällt auch gesunden Personen ab einer gewissen Anzahl und/oder Lautstärke von Reizen schwer, diese Grenze scheint bei der unilateralen telenzephalen Hörstörung deutlich verschoben zu sein.

Leitsymptom zentraler Hörstörungen ist oft die erhebliche Schwierigkeit von Patienten, Unterhaltungen unter starken Hintergrundgeräuschen zu folgen oder wenn mehrere Personen durcheinander sprechen. Häufig wird auch von einer veränderten oder asymmetrischen Wahrnehmung von Geräuschen berichtet (vgl. , Blaettner & Goldenberg, 1993).

Im Unterschied zu Patienten mit peripheren Hörstörungen können nach schweren telenzephalen Läsionen leise Geräusche oft besser diskriminiert werden als laute und folglich Sprache auch bei leisem Sprechen besser verstanden werden (vgl.

Blaettner & Goldenberg, 1993).

können schwere zerebrale Hörstörungen jeweils unterschiedlich ausgeprägt sein für verbales oder nonverbales Material (vgl. Scherg, 1988). Während linksseitige temporale Schädigungen meist mit Aphasien einhergehen und - im Falle der sensorischen Aphasie - Sprache bei erhaltener Geräusch- und Tondifferenzierung nicht mehr verstanden wird, scheinen rechtsseitige Schädigungen schlechtere Leistungen in komplexeren akustischen Diskriminationsaufgaben nach sich zu ziehen (vgl. Blaettner & Goldenberg, 1993). Verminderte Diskriminationsleistungen kommen auch bei linksseitigen Schädigungen und Hirnstammläsionen vor; bei letzteren führt die Unterbrechung afferenter Verbindungen im Hirnstamm vermutlich zu einer drastischen Reduktion der im Schallsignal redundant vorhandenen Information und bewirkt, dass auf der intakten kortikalen Ebene keine eindeutige Signaldiskrimination mehr möglich ist.

Aufmerksamkeitsstörungen betreffen meist nicht nur die akustische Modalität. Eine Sonderform dieser Störung stellt der Neglect, also die Nichtbeachtung von Reizen auf der Seite konterlateral zur Läsion, dar. Wenn die Vernachlässigungsphänomene einer Raumhälfte ausschließlich in der akustischen Modalität auftreten, sind sie als Ausdruck einer telenzephalen Hörstörung zu werten; meist treten sie aber auch in der visuellen und taktilen Modalität als Bild einer übergeordneten Funktionsstörung auf (Heilman & Valenstein, 1972) Durch Aufmerksamkeits- und Konzentrationsstörungen, welche sich in reduzierter Aufnahme- und Verarbeitungskapazität, reduzierter Informationsverarbeitungsgeschwindigkeit, rascher Ermüdbarkeit vor allem unter Belastung, aber auch erhöhter Ablenkbarkeit äußern können, werden intellektuelle und praktische Tätigkeiten in erheblichem Maße beeinträchtigt.

Aufmerksamkeitsstörungen beinhalten Parameter wie Reaktionsverlangsamung und erhöhte Fehleranzahl in verschiedenen Aufgaben. Beeinträchtigungen der Aufmerksamkeitsfunktionen stellen die häufigsten neuropsychologischen Leistungsdefizite nach erworbener Hirnschädigung unterschiedlicher Lokalisation und Genese dar (Van Zomeren & Brouwer, 1994). Beeinträchtigungen des Reaktionsvermögens finden sich bei etwa 70% der Patienten. Vor allem eine Verlangsamung der Reaktions- bzw. der Informationsverarbeitungsgeschwindigkeit wird häufig beobachtet (Poeck, 1989; Sturm, 1983; Säring, 1988; Benton, 1986). Regel (1981) betrachtet die kognitive Verlangsamung als Hauptsymptom der zerebralen Beeinträchtigung.

In der psychologischen Leistungsdiagnostik, insbesondere in der klinischneuropsychologischen Diagnostik, haben Tests zur Aufmerksamkeitsprüfung und Reaktionsfähigkeit (Zimmermann & Fimm, 1989) einen festen Platz. Diagnostisch lassen sich die zu Anfang genannten Aufmerksamkeitsbereiche durch unterschiedliche Aufgaben abgrenzen.

Das Reaktionsverhalten wird häufig mit Determinationsaufgaben erfasst.

die benötigte Zeit, die Anzahl und Art der Fehler, die Fehlerentwicklung in Abhängigkeit von der Zeit oder die bearbeitete Menge des vorgelegten Materials bei der Bewältigung definierter Aufgaben.

Die Vorteile eines solchen diagnostischen Vorgehens liegen in der Gewinnung von Messgrößen, die sowohl intra- (Krankheitsverlauf, Therapieevaluation) als auch interindividuelle Vergleiche (Orientierung an den Werten einer Standardgruppe) ermöglichen.

Ziel des Trainings Reaktionsfähigkeit ist eine Verbesserung der Reaktionsgeschwindigkeit und –genauigkeit auf visuelle Reize und akustische Reize. Durch Einfach- und Mehrfachwahlreaktionsaufgaben soll eine möglichst rasche, differentielle Reaktion auf Signale trainiert werden.

Bei diesem Training wird - als Voraussetzung für eine Reaktion - die selektive Aufmerksamkeit, d.h. die Fähigkeit zur Fokussierung der Aufmerksamkeit unter Nichtbeachtung irrelevanter Informationen, in der visuellen und akustischen Modalität geschult. Durch das Training Reaktionsfähigkeit kann auch die Geräuscherkennung unterstützt und mit trainiert werden.

Neuere Forschungsergebnisse sprechen für differentielle Trainingsansätze, welche gezielt spezifische Aufmerksamkeitsstörungen behandeln, da unspezifische und wenig theoriegeleitete Trainingsverfahren für die Aufmerksamkeit nicht in allen Aufmerksamkeitsbereichen erfolgreich sind (Gray & Robertson, 1989; Sohlberg & Mateer, 1987; Poser et al., 1992; Sturm et al., 1994; Sturm et al., 1997). Das Verfahren Reaktionsfähigkeit ist jedoch auch bei Defiziten der selektiven Aufmerksamkeit ohne allgemeine Reaktionsverlangsamung indiziert.

Das Training stellt darüber hinaus Anforderungen an die kognitive Flexibilität und kann durch Übung die motorische Bewegungs- und Handlungssicherheit günstig beeinflussen. Ferner stellt das Training - wie alle kognitiven Aufgaben - nach einer bestimmten Zeit auch Anforderungen an die Daueraufmerksamkeit.

Ergänzend kann das Reaktionsvermögen mit visuellen und akustischen Reizen durch die Programme Akustische Reaktionsfähigkeit (AKRE) und Reaktionsverhalten (REVE) trainiert werden. Programme zum Training Verschiedener spezifischer Aufmerksamkeitsdefizite sind Geteilte Aufmerksamkeit (GEAU), Aufmerksamkeit und Konzentration (AUFM) sowie Vigilanz (VIGI).

Die Anwendung des Verfahrens Reaktionsfähigkeit wird bei Patienten mit diagnostiziertem Defizit der Reaktionsgeschwindigkeit und der Reaktionssicherheit nach zerebralen Läsionen, bei Störungen der selektiven Aufmerksamkeitsleistungen, Störungen der akustischen DiskriminationsErkennens- und/oder Behaltensleistung empfohlen. Beeinträchtigungen des Reaktionsvermögens können modalitätsspezifisch- oder unspezifisch bei allen neurologischen Erkrankungen vorkommen. Das gilt ebenso für Aufmerksamkeitsstörungen.

Bei telenzephalen Läsionen hat der Patient oft keinen Zugang mehr zu seiner Hörwahrnehmung, so dass ein Training der Geräuschdiskrimination nach groben Merkmalen hilfreich sein kann (vgl. Scherg, 1988). Bei diesen Störungen ist unbedingt darauf zu achten, dass die akustischen Stimuli in einer für den Patienten angenehmen Lautstärke dargeboten werden; oft führt beispielsweise lautes Sprechen zu einer Reizüberflutung.

Konzeptuell wird von verschiedenen Aufmerksamkeitsfunktionen ausgegangen, die selektiv gestört sein können. Diffuse Hirnschädigungen nach traumatischen oder hypoxischen Ätiologien ziehen häufig unspezifische Aufmerksamkeitsdefizite wie rasche Ermüdung, erhöhtes Schlafbedürfnis und einen allgemeinen Initiativeverlust nach sich, während nach lokalisierten Insulten z.B. vaskulärer Genese oft spezifische Aufmerksamkeitsdefizite zu beobachten sind. Grundsätzlich können Insulte jeglicher Cortexareale zu Beeinträchtigungen der Aufmerksamkeit führen. Insbesondere nach Läsionen des Hirnstamms im Bereich der Formatio reticularis und nach parietalen rechtsseitigen Läsionen sind Störungen der phasischen oder tonischen Alertness sowie der Vigilanz zu beobachten, während linksseitige Parietalinsulte eher die selektiven Aufmerksamkeitsleistungen beeinträchtigen; insbesondere bei Aufgaben, in denen Entscheidungen zwischen mehreren Reiz- oder

Reaktionsalternativen getroffen werden müssen (Covert Shift of Attention) (vgl.

Sturm ,1990).

Unter der Annahme spezifischer Defizite verschiedener Aufmerksamkeitsaspekte sollte auch die spezifische Trainierbarkeit dieser Funktionen postuliert werden.

Vorliegendes Verfahren ist insbesondere geeignet bei Störungen der phasischen Aktivierung und der selektiven Aufmerksamkeit.

Patienten mit motorischen Einschränkungen (z.B. Paresen) haben die Möglichkeit, mit diesem Verfahren ihre Reaktionsschnelligkeit mit der gesunden dominanten oder nicht-dominanten Hand zu trainieren.

Unter der Prämisse maximaler Spezifität und um eine möglichst hohe Effizienz des Trainings zu erreichen, sollte der Erstellung des Therapieplans mit computerunterstützten Verfahren eine differenzierte neuropsychologische Diagnostik vorausgehen.

Diagnostisch auszuschließen sind u.a.: ausgeprägte visuelle Defizite und stark ausgeprägte Aufmerksamkeitsstörungen.

Das Verfahren unterstützt die Anwendung bei Kindern ab dem 11. Lebensjahr, indem bis zu einem Alter von 14 Jahren kindgerechte Instruktionen verwendet werden.
