When conditions change, assisted living at Woodlawn Commons offers three levels of 24-7 support for health and medication management along with socialization and engaging activities. After a hospital discharge, or when you need an inpatient level of health monitoring and care, Wesley Health Care Center helps you stay close to home, family, and friends. Our excellent physicians and nursing staff develop a personalized care plan with each resident and their family. Spiritual services and hospice care are also available without leaving the nursing home.

From outpatient therapy and short-term rehabilitation, to senior apartments that blend total independence with useful conveniences, to home care and assisted living that support your health and lifestyle, to long-term care that keeps you close to family and friends when you most need them, our services deliver peace of mind, close to home.
