Our Day Services offer a warm, friendly and engaging environment for all adults over 50, including those living with dementia. We have centres across Lancashire, and pride ourselves on providing a safe and supportive space where people can socialise, enjoy activities, and feel included. A Warm, Welcoming Community
Our centres are safe and inclusive spaces where adults aged 50+ can laugh and feel at home.
From crafts to music, reminiscence, or gentle movement, our activities are tailored to meet physical, social, and cognitive needs. Members are encouraged to take part in activities that support theirwell-being. Our activities are adapted to the individual needs and abilities of members and shaped around things that they enjoy.
Shared mealtimes are an important part of our day. Freshly prepared, nutritious meals are enjoyed together.
Whether it’s work, errands, a quiet cup of tea, or even attending one of our carers groups, you’ll have space to do whateveryou need to do.
We don’t want the families we support to feel like they’re handing over responsibility You’re gaining a team. Every member is supported by experienced, compassionate staff, with a dedicated keyworker and a personalised care plan tailored to their needs, preferences and choices. When you or a loved one comes home after a hospital stay or faces a major life change, things can feel uncertain. Who’s going to help? What needs doing? And how will you stay safe?
Our Living Well Support Service is here to help you stay in control and feel confident at home, whether you ’ re returning from the hospital, dealing with illness, or just need support to avoid being readmitted. Age UK Lancashire offers practical help and emotional reassurance, exactly when and where you need it.
We offer two flexible levels of support because we understand that no two people need the same service.
Up to 3 hours of personalised support This may include help with transport, a friendly face to help settle you back at home, and a home assessment to make sure everything’s safe and manageable from day one.
A follow-up support package tailored to you. We can help you access:
✔ Home safety checks and minor adjustments
✔ Falls prevention and mobility support
✔ Medication collection
✔ Benefits and financial advice
✔ Help to access local services and community links
✔ And someone to talk to, because emotional support matters

Our Support at Home service is personal, flexible, and respectful. We’re here to make life easier, not take over. Whether it’s a bit of cleaning, help with the shopping, setting up a video call with your grandchildren, or just someone friendly to share a cuppa with, we ’ re here to help.
We tailor each and every visit to your needs. Some of the ways we can support you include:
✔ Trips out for a tea or coffee or to attend a hobby.
✔ Friendly chats and companionship.
✔ Light cleaning, laundry, and meal prep.
✔ Running errands and doing the shopping (or coming with you).
✔ Help with letters, bills, or online tasks.
✔ Getting ready for holidays (like putting up decorations).

Our Community Engagement Manager is the friendly face you’ll see at local events, community gatherings, and networking meetings, making sure that older people, their families, and our communities know exactlywhat support is available and how to get it

Your wishes, your way.
Making a Will isn’t just paperwork; it’s a promise to the people and causes you care about most. It ensures that your home, your savings, and your personal possessions are passed on exactly as you intend, relieving your family of the stress and uncertainty often found during an already difficult time.
Through Age UK Lancashire, you can have a basic Will written for a suggested donation of just £100 for one person or £180 for two, which is about half the usual cost. Every penny ofyour donation goes straight back into helping fund ourvital work supporting older people in the community.

Living well with dementia starts with the right support.
Dementia support should feel personal and consistent, and here in Lancashire, it does. At Age UK Lancashire, we are committed to improving the lives of people living with dementia across our County. We offer a range of support that empowers those with the condition to live well, for as long as possible.
A FREE 14-week programme for people with mild to moderate dementia, designed to maintain/improve an individual’s cognitive function (memory and thinking) as well as boost mood and improve social connections. (Click here to find out more)
Working in collaboration with a number of Community partners across the County, our FREE monthly drop-in social groups are for people with dementia and their carers, offering an opportunity to access information, advice and support and benefit from meeting others in a similar position. As part of our ongoing commitment to the provision of post-diagnostic support, we work in close partnership with our colleagues at the Alzheimer’s Society and can support with a referral to their Dementia Navigator Service.
Dementia Awareness Training for local businesses and groups
Dementia-friendly events like tea dances and outings. Partnerships with organisations and councils to strengthen local support.

If there’s something we can’t directly help with, we’ll make sure you ’ re connected to someone who can through trusted partners, or our own specialist services.
We also work alongside local authorities to help distribute the Household Support Fund, ensuring vital help reaches those who need it most Our friendly helpline is open Monday to Friday, 9:30am – 4:00pm.

We operate five days a week, and in some areas, depending on staff availability, we can also offer home visits.

Independence isn’t about doing everything alone; it’s about having the right help when you need it.
This service supports those living in East Lancashire aged 50 and over who are living with longterm health conditions, a high frailty score, or those at risk of unplanned hospital admissions. We help you access the right services and support to help you manage your health and wellbeing and stay independent and connected in your communities
How it works
Our Personal Independence Coordinators (PICs) work alongside you to understand what truly matters to you, your goals, priorities, and aspirations. Together, we create a personalised plan around the individual’s lifestyle designed to promote wellbeing every step of the way. Over a typical 12-week support period, we focus on practical steps that make a lasting difference, addressing both physical and emotional wellbeing, and helping people regain independence.
It’s about more than care, it’s about empowerment. We help people take control, make choices that suit them, and live as independently and meaningfully as possible.
Who can access the service?
This service is available to East Lancashire residents aged 50+ who are identified as being at increased risk of an unplanned hospital admission
This may include those who:
Have long-term health conditions.
Have had recent unplanned admissions.
Have a frailty score of 5 or above.
Need a high level of intervention.
Are being discharged through the Integrated Discharge Service (IDS) or directly from hospital wards.
The service operates Monday to Friday, 9:00am – 5:00pm and referrals can be made by Health and Social Care professionals involved in your care. If you feel this service is for you, please chat to your GP, nurse, social worker, or other professionals involved in your care. They can refer by email (or EMIS) via: ageukintegrated.carereferrals@nhs.net
