DERELICTION OF DUTY: UNDERSTANDING ERECTILE DYSFUNCTION Just so we are in the same library; Erectile Dysfunction (ED) is defined as the inability of the penis to respond to sexual stimuli to achieve sufficient rigidity either to begin or complete the act of intercourse. This event may be transient or a chronic indictment going forward.

But in either case, ED can be the source of significant relationship anguish and loss of male confidence each time it occurs.

To ensure we’re reading from the same book, the penis becomes rigid from a combination of delicate nervous (parasympathetic) and vascular actions which allows blood to enter and become trapped in the two corpus cavernosum chambers on either side of the organ.

The purpose of this article is to appreciate the possible causes of ED and to offer some traditional and nontraditional solutions for the problem.

Let us start at the lowest level where ED might be transitory and mentally reversible. If the male is in an emotionally anxious frame of mind, either from fright, guilt, or tension. His sympathetic nervous state, the one which calls for aggression or retreat, may dominate over his parasympathetic pathways and the correct message is ‘blocked’ at the level of the external pudendal (penis) arteries.

Merely being on certain types of prescribed medication can usher in the start of ED. Likely candidates are hypertensive and anti-depressive medications.

Higher levels of causes that have the potential of a chronic reoccurring ED condition include cardiovascular issues like hypertension, hyperlipidemia, and arteriosclerosis. These two are likely the result of ED becoming more prevalent in the younger male population. But the granddaddy of them all which is responsible for the lion share of the middle aged and senior ED, is from the metabolic damage to small blood vessels in diabetics. Whether you’re a type I or type II diabetic, the pervasive changes to the inner lining of one’s blood vessels (Endothelium) are unforgiving.

Smoking and chronic alcohol intake round out the major causes of ED. Just so we are on the same page, the penis will not get and stay erect if there is inadequate engorgement from blood conduits. It is time for the introduction of a most valuable player. A potent but elusive molecule: NITRIC OXIDE. Nitric Oxide is an immediate and effective vasodilator. A frequent question asked surrounds the role of testosterone levels in ED. The fact is that while testosterone is necessary for ‘maleness’ and for libido, and yes there is some vasodilation of the arteries involved in erections, it pales in comparison to the effect of Nitric Oxide. Remember earlier we spoke about the parasympathetic nervous involvement. It so happens that the endothelium of the penis vessels respond by releasing Nitric Oxide thus dilating and allowing said engorgement.

The solution may seem obvious now; provide more Nitric oxide. Indeed, this solution is the backbone of the mechanism-of-action of the very successful (and expensive) ‘blue pill’ Viagra. The problem is Nitric Oxide is elusive. It’s a flighty gas which does not hang around and it’s made indirectly after a cascade of precursors in the body which are complex. One must induce its release rather than give It directly.

SOLUTIONS With the groundwork taken care of, we can advance to some simple and more esoteric solutions to ED. As a general guideline, solutions emerge when the offending agent/condition is removed. Weight loss, improved physical fitness, and ceasing to smoke and drink high levels of alcohol are inexpensive and attainable fixes.

We introduced Viagra in a prior paragraph. The active ingredient is Sildenafil Citrate. Its cousins, Cialis and Levitra are also effective. These medications have few serious side effects and, in some situations, can have an effective duration of up to 36 hours. The drawbacks include the cost of having to see a physician and the cost of the medication itself especially if you have an active sex life. It’s reasonable to ask if one can increase Nitric Oxide levels by natural dietary methods. The answer is yes. Foods rich in Nitrates like spinach, Bok choi, and beets can help. As an aside, the breakdown of Nitrates begins in the mouth on the tongue. That specific bacteria is sensitive to fluorides in toothpastes and mouthwashes so be informed if you go this route.

There are anecdotal reports about the effectiveness of pomegranate juice, watermelon, olive oil, caffeine, and dark chocolate which one can experiment with.

For those who are resistant to the previous remedies, there are surgical interventions that a urologist can advise you further. Vacuum devices and penile implants all have their pros and cons. Prostaglandin injections directly into the penis have the effect of vasodilation also, but this is clearly not for everyone.

There are two more interventions utilizing modern devices that have the possibility of a long-term solution.

The first is known as penile low-intensity focused shockwave therapy. A device using sound waves intended to break up vascular obstructions leading to the penis. The drawback of this therapy is that it takes some weeks before results are seen. The second uses Electromagnetic waves in combination with redlight therapy. This painless procedure might require multiple applications, but it achieves the essentials for robust erections; release of Nitric Oxide from the Endothelium and increased vascular perfusion.

All is certainly not lost when it comes to failing to meet the moment of intimacy. The key is understanding the origin of one’s issue and addressing the source.

Sadly that is not the case for some people, not everyone likes or enjoys the holiday season! Feeling depressed or anxious at this time is real, especially for persons who live alone, have no family members, are living overseas, or who relive past `memories.

Depression or anxiety can be linked to changes in seasons commonly called seasonal affected disorder (for those who live in Northern cold climates) usually tends to appear in the late fall and can last until early spring however just because we live in the tropics doesn’t mean that it there are not persons right here in Barbados that may dislike or even hate this time of the year.

The holidays are a time to share, reach out, and comfort those in need, we should look around our neighborhood and identify those who live alone, who are elderly, and those whom we have not seen for some time. Reaching out can break the monotony and can raise the spirits of the lonely.

According to the professionals, certain indicators let you know that your friend or loved one is in crisis. Baking Ham Like a Bajan

The sweet and savory aromas of baked ham are a staple during the holidays in Barbados (or Bajan culture).

This mouthwatering Bajan-style ham brings a taste of the Caribbean to your holiday table. This baked ham recipe will add some Bajan flair and festive spirit to your next celebration. Both simple and elegant, ham baked "like a Bajan" makes for an unforgettable main course that your guests will rave about.

RECIPE ½ cup brown sugar 1 cup pineapple juice ¼ cup whole cloves 1 tablespoon dry mustard 1 smoked picnic ham (weigh to determine lbs) 1 can pineapple rings, drained METHOD 1. Preheat the oven to 300 degrees F 2. Whisk pineapple juice, brown sugar, and mustard (honey mustard if desired) in a bowl until brown sugar has dissolved.

3. Place ham in a baking dish cut side down and arrange pineapple rings onto the ham. Push cloves, as many as desired, into ham and baste ham and fruit with pineapple juice glaze.

4. Bake in the preheated oven until glaze has baked onto the ham, giving 1/2 hour to every pound. 2 1/2 to 3 hours for a 7 lb picnic ham.

Bajan Great Cake

It isn’t Christmas in Barbados unless you have the traditional ‘Great Cake’ or ‘Black Cake’. This is a cake unlike any other because of its potency. It’s contents, apart from the normal cake ingredients must have, or dare I say, must be drowned with alcohol, rum, wine and vodka. From the fruits which are added, having been soaked for weeks or the entire year to the added alcohol which is poured over the cake when it is finished.

So fill your house with the aroma of this Bajan Cake, give it a go and enjoy it warmed with a scoop of ice cream. It’s an excellent dessert after your Christmas meal. everyone.

METHOD 1. Mince the rum fruit with spices, essences, nuts, and sugar. Mix ingredients well and put in a jar, cover with rum, and allow the mixture to steep for 3 weeks or more.

2. When ready to bake, cook fruit over low heat with 1 cup of water for 15 minutes. Cream the butter/margarine and remaining sugar well and add eggs beating in one at a time. Then add this to the fruit mixture. Stir in enough browning to make the mixture dark brown. Add the flour and baking powder last.

3. Place mixture into greased baking pans lined with 2 sheets of wax paper. Fill the pans, and bake slowly in the oven at 250 degrees for 2 to 3 hours. Test with skewer before removing from oven making sure skewer is dry and without cake batter. As soon as the cakes are removed from the oven, prick them all over with a skewer and pour a mixture of Rum and wine or port slowly over the cakes to absorb. Let stand in baking pans adding more liquor as desired before serving.
