Adelaide PHN recently undertook a review of our well known Integrated Care website and is pleased to announce that it’s getting a refresh in 2022! Like the original site, our newly named Integration Hub website will be a one stop shop for all things integration. The new site • Showcases our integration programs and services, including

HealthPathways, Priority Care

Centres and our Central Referral

Unit • Houses a range of resources for integrated care • Includes pathway navigation for each of the Local Health

Network’s It is expected that the new site will be launched in the coming months - stay tuned! Adelaide PHN will ensure to communicate the new website to all stakeholders when available.
