President’s Report Dear ADAWA family, As the adage goes, ‘the only thing constant in life is change’. There have been new variants of Omicron emerging that require us to constantly adapt to these dynamic circumstances – and now there’s a further change with the introduction of a new CEO for ADAWA. Let’s look at the Omicron landscape first. There have been increasing cases of infection with the new variants of Omicron. This means that we must continue to practice risk assessment and risk mitigation principles for COVID-19. There have been additional side-effects from this pandemic. We are aware of the increasing shortage of staff, which has affected all industries. The shortage of Dental Assistants has been highlighted at a Federal ADA level and this is on their priority list.

Focusing on ADAWA, I am pleased to announce that a new CEO has been appointed. The search and selection process for the new ADAWA CEO was a thorough and exhaustive process with the help of a CEO search company, Gerald Daniels. A selection panel was selected, not only from ADAWA Councillors but also from External Stakeholders, with the aim of representing our community as a whole. After multiple rounds of interviews, the selection panel was successful in the selection of our next ADAWA CEO, Trevor Lovelle. ADAWA Council has ratified the selection panel’s choice and Trevor will be starting as the new CEO of ADAWA from August 15.

Trevor has been the CEO of the Aged and Community Services Western Australia (ACSA) and has considerable experience in leading both State and National not-for-profit, member-based

organisations. His vast experience in Corporate Governance, along with Human Resources Management, will be an exceptional addition to achieving ADAWA’s future goals, objectives and vision. Once Trevor is in office, our media team will undertake a comprehensive cover, which I am sure will satisfy the curiosity of all our members, in regard to our new CEO. ADAWA council thanked our outgoing CEO, Dr David Hallett, at his last council meeting for his tremendous work and stewardship over the past eight years. It was a pleasure to also see the many members who especially came to say goodbye to Dr Hallett at our last General Meeting.

It is the strength of the ADAWA membership which has ensured smooth transitions.

Dr Hallett has kindly agreed to stay on for the next three months with ADAWA to ensure a smooth transition but also to continue to assist with certain dental-associated matters as required. ADAWA will soon be progressing to formulate a strategic plan for the future benefit of our members and our association as a whole. We are always going to encounter changing times and it is the strength of the ADAWA membership which has ensured smooth transitions.

On the eve of his retirement, we celebrate the dedication and service of our CEO, Dr David Hallett.

In his eight years as ADAWA CEO, Dr David Hallett has seen changes, both big and small. Some, he can accept personal responsibility for; others, he has overseen and quietly guided to successful completion.

One thing, however, is certain: ADAWA is now, more than ever, a strong and united membership association, thanks in no small part to David’s caring and committed leadership.

There can be few members who haven’t had the benefit of David’s sage advice and assistance over the years. ADAWA members know that he’s never more than a phone call, email or coffee away, available at short notice when crisis hits or when a friendly chat is required.

COMMUNICATION WITH MEMBERS David, with the support of his progressive executive team, was instrumental in implementing a media and communications strategy at ADAWA. This has been significant in opening the lines of communication with staff, members, stakeholders in health and the wider community.

Taking on the role eight years ago, David had an inkling of what being CEO of ADAWA truly meant. He only knew that he was taking over from the much-loved and admired CEO, Dr Stuart Gairns. While David, at this point, had been an ADAWA office bearer for 15 years, Stuart had 30 years’ experience, and was hugely respected both within the profession and the broader health community.

I realised that if we supported our members enough in their practise, by stealth, patient outcomes would naturally improve.

Stuart handed to David a membership association that was already strong. Membership numbers were high, the branch was running successfully, and members were satisfied. It was, on the whole, a much lauded membership model. And, as the saying goes, if it’s not broken, don’t fix it.

Having already been ADAWA President, David felt confident taking on the role of CEO, and did so with a determination to simply continue Stuart’s legacy.

“I saw my role primarily as supporting the members,” David recalls.

“If there’s such a thing as a KPI in this job, it was simply to improve on member services and maintain member satisfaction.”

In doing so, David hoped to be an advocate for better patient outcomes in the community. “I realised that if we supported our members enough in their practise, by stealth, patient outcomes would naturally improve.”

Over the past eight years, ADAWA has quietly improved and advanced in many other ways – all to the benefit of its members and thanks to the leadership of Dr David Hallett.

The Western Articulator magazine bears little resemblance to the newsletter of a few years ago, receiving widespread acclaim for its innovative design and quality, content-driven articles.

Similarly, the association is now active on social media, with regular communications going out to members both in the private Facebook group and via email.

At no point was this more important than during the early months of the COVID pandemic. “There was a period in 2020 when we thought the profession was going to be disseminated,” David recalls. “It was a testimony to our strength that we were able to support members both emotionally and financially.”

WA DENTAL CPD David says it has been great to watch the WA Dental CPD program under the direction of Dr Jenny Ball and her committee develop into what is now a world-class program. “In particular, the development of programs for recent graduates has been a real feature to keep young members engaged,” he says.

VOLUNTEERISM “Dentists have always provided pro bono care within their own practices, whether it be free treatment or discounted treatment to people in need, but we have never been able to understand the significance or the magnitude of that,” he says. “Whereas now we have a range of structured programs, providing greater insight as to what is really being done.

“It’s also wonderful to see so many young dentists being involved and viewing it as an attribute of being a health professional,” he adds.

BUILDING RENOVATIONS ADA House has been transformed immeasurably over the past few years, with David taking on the role of 'pretend' project manager in the huge renovation.

The project saw the tired, outdated building on Havelock Street in West Perth undergo a dramatic transformation.

“The old building wasn’t particularly welcoming. The AV was outdated and the lecture theatre impractical. Now, it’s perfect for CPD events and general meetings, with quality catering facilities.”

In eight years, David has achieved his “KPI” – and much more besides. As he begins his retirement, he can do so knowing he did his best in trying to make a difference – to Western Australian dental professionals on an individual basis, and the community at large.

We’ve really been able to connect with members on an individual level. Whether you’re a student, in private practice or government health services, part time or full time, retired or still practising, our members know they can reach out to us for individual support.

This, he says, is his proudest achievement as CEO. “We’ve really been able to connect with members on an individual level. “This might be for broader professional support, professional indemnity support, volunteering opportunities or CPD; we are here to offer support on an individual level.”

STRONGER TOGETHER

PLANS FOR RETIREMENT

Regardless of who takes over as CEO of ADAWA, David is confident the association will remain strong, provided the members continue to be united.

It may be a tired cliché, but David is genuinely looking forward to spending more time with his family.

“Whether we’re talking to Government, to health funds, or advocating on behalf of our patients, we can only do that if the branch remains strong,” David says. “The branch can only remain strong if we are unified.”

He also stresses the benefits of being an engaged member, from first-hand experience. “Like many members, for the first 20 years of my ADAWA membership, I just paid my fees and rarely engaged.

Whether we’re talking to Government, to health funds, or advocating on behalf of our patients, we can only do that if the branch remains strong. The branch can only remain strong if we are unified.

“It wasn’t until Peter McKerracher, who was CEO at the time, tapped me on the shoulder and suggested I think about giving something back, that I decided to get involved with the association.”

It was, of course, one of the best decisions David ever made – and he encourages other members to do the same.

“You don’t have to become a councillor,” he says. “You just have to support each other, be a good role model and seriously consider being a mentor for younger graduates. If we ask your opinion, speak up for what’s best for the profession and what’s best for your patients.”

“The cliché has become the reality,” he says. “My two daughters have come back to live in Perth after 15 years or so away with their families, including a new grandson. My perspective on life has started to change and I’m ready to appreciate the simpler things in life.”

When asked if those retirement plans include working on a lawn so green it will be the envy of his competitive, turfloving neighbours, he laughs. “About 20 years ago a well-known dentist in Perth was retiring and he was asked the question what he was going to do during his retirement,” David recalls.

“He said: ‘I’m going to spend more time with my gerberas, creating the perfect colour and the perfect shape and form.’

“I remember I turned to my colleague and said if I ever get like that, just put me out of my misery,” he laughs. “Yet now I’ve reached this age and this point, I don’t think it’s such a bad option.”

Dr Amanda Phoon Nguyen Where do I begin? Dave has been part of my dental journey for the last 15 years, since having the great pleasure of working alongside him at OHCWA. His calm nature and attention to detail quickly saw him earmarked for greatness, sadly not with us at OHCWA but on to the CEO of the ADA. He was and is a wonderful colleague, a gentleman, knowledgeable and admired by the whole profession and his patients alike and I am always grateful that we crossed paths that fateful day he decided to take up a position up on first floor at the Oral Health Centre all those years ago. Tributes David, I recall as if it were yesterday, our catch up over coffee in Swanbourne, where you enlisted me to talk on orthodontics to the new graduates.

For me it was a real paradigm shift, as you always feel as if you have just graduated, and have yet so much to learn. I think this occurs all through our careers, the constant desire to keep learning and keep improving is always present. I felt humbled and had a real professional mindset change. That passing on the guidance and leadership offered to me through years to the next generation, is in fact the continual evolution of our profession and in particular our dental community here in WA. Thank you for offering me that experience, and engaging me to then continue to contribute my experiences and skills to the profession through Women in Dentistry and then Healing Smiles.

We wish you only the most relaxing of retirements, filled with adventure, joy and travel. Thank you for being such an instrumental part of our WA dental community David.

The last three years of my life have been some of the most turbulent and challenging times. Without David’s unwavering support throughout it all, I don’t think I would be where I am today. He has done so much for me selflessly not only as the CEO of ADAWA but as a confidante and a counsellor. He is an amazing man.

I have been fortunate enough to have Dr Hallett’s assistance for various reasons and very much appreciated his kindness, empathy and professionalism at times when I was under a lot of pressure.

David, I can prescribe him with one statement: loving to all.

Not only helping all members professionally, but I’ll never forget when the massacre occurred in New Zealand, with that monster, ruthless man who killed 55 children and women and old people while they were praying in the most safe environment and very vulnerable in the mosques, three years ago, on Black Friday, March 2019.

His will be big shoes to fill and I hope that his successor can rise to the challenge.

David offered us, as Australian Muslims dentists, his condolences in the Journal, and he even offered to come with me for praying.

We were all very scared to go and pray. He not only shared his kindness and care. He offered to join me for a prayer in the mosque. This is only something we will never forget.

I can call David a true Australian and a great professional man. We will miss you, for sure.

David, You are and always will be the best tea maker on the planet, and let’s face it, who else gets tea delivered to their office by their CEO? All of my memories of David Hallett are fond.

My favourite, which summarises his character: An 86-year-old lady, whom he had seen for many decades, was referred to me for Periodontal work.

Given her anxiety about seeing another clinician, David took the afternoon off and came in with her for her consultation appointment to be with her. David was in Claremont at the time, and the practice I was at was in West Perth.

Such an amazing man on so many levels. A real treasure and asset for our profession.

So sorry to hear that Dr Hallett is retiring. I have been very fortunate to have met him many years ago as a new graduate dentist. I had a difficult experience at a private practice, therefore I reached out to the ADAWA for assistance and Dr Hallett and Andrea have been so helpful. I remember having regular meetings with Dr Hallett so he could check my resume and contracts. He would even do follow up calls to ensure I was doing okay. I eventually did a country stint for a couple of years and when I attended the Australian Dental Congress, he would always say hi and ask me about the experience.

Even recently when I contacted him about a new situation, he wouldn’t hesitate to help. I’m sure I am not the only dentist that had received this kind of help from Dr David Hallett and I honestly do believe that he has helped me with my career, especially during difficult times. He will be missed.

This has included introducing and providing ongoing support for the process of sending out emails to members to request dental records, to assist with the Identification of the deceased. This does not happen in any other state or territory in Australia and it is much appreciated.

You have enabled the WA Forensic Odontology Team to provide conclusive evidence to the Coroner of the identification of many hundreds of people and allow their loved ones to gain some closure in some pretty awful situations.

Dr Robert Nedelcu Dr Robert Nedelcu is looking forward to getting to know the WA dental community after making the move from Sweden last year and accepting a position as a Senior Lecturer in Restorative Dentistry at UWA Dental School.

Like many in the profession, Dr Robert Nedelcu’s early interest in dentistry stemmed from admiration of his own dentist.

“When I was six- or seven-years-old, I was riding on my bicycle and I had quite a bad fall and hit my mouth and teeth,” he recalls. “My dentist was very kind. She was extremely empathetic and understanding and she turned the incident into something less traumatic.”

Later, while in high school, a techsavvy Robert spent some time helping a dentist (who was a colleague of Robert’s father) with his computers.

“I worked there (at the practice) in the summer, and I got to see the other side of dentistry almost as a fly on the wall,” he says. “I saw the interaction between patients and the dentists, the nurses and reception, and I enjoyed it; it was a great experience.

“I think I disappointed my father, who comes from a long line of engineers.

Everyone thought I would go into computer science. I chose dentistry because it felt right. I knew I wanted to

work with my hands, I wanted to work with patients, and I didn’t want to sit behind a computer all day long.”

After graduating and working for a few years, Robert says he knew prosthetics was where his heart was and decided to specialise in oral prosthetics at Karolinska Institutet and Stockholm Dental Implant Center.

It was during his specialisation training that Robert had to produce a research project and he chose intraoral scanning. This was when his passion for digital dentistry was sparked. “I found intraoral scanning fascinating – maybe because it appealed to my technical side,” he says. “I felt there was so much more to do with intraoral scanners, but I couldn’t find the right platform.”

My colleagues in prosthetics are all open-minded professionals of similar age, and we have excellent dialogue where we can combine knowledge from four different continents.

Robert began working with Professor Andreas Thor, from the Department of Surgical Sciences, Odontology and Maxillofacial Surgery at Uppsala University Sweden.

Professor Thor took Robert under his wing and became Robert’s academic mentor.

“The Department of Surgical Sciences, Odontology and Maxillofacial Surgery had some fascinating research on virtual planning and maxillofacial reconstructions, in collaboration with plastic surgery, radiology and engineers at Ångström laboratory. They needed a clinician who understood intraoral scanners. I was offered a PhD position in digital dentistry with a focus on the topic of intra oral scanners, which I finished in 2020.”

A NEW START “When I came to the end of my PhD, we decided as a family it was the right time to make a change,” Robert says. “We wanted to experience a country which was culturally and climate-wise different from Sweden.

“As a family, we’ve travelled extensively and visited Australia a number of times,” he adds. “We like the lifestyle here, and one of the best assets you have is the weather – the sun, the beaches, and the blue skies.

“Glen Liddelow (Specialist Prosthodontist) connected me and the UWA Dental School, and after an interview, I was offered the position.”

After some delays because of COVID, Robert arrived in Western Australia in August last year, along with his wife (who is a general dentist) and his children.

Now he is settled at UWA, Robert would like to build a new platform and network. “I want to build a platform here for research and for collaboration with colleagues,” he says. “My colleagues in prosthetics are all openminded professionals of similar age, and we have excellent dialogue where we can combine knowledge from four different continents. To build on that experience and skill set is an excellent path forward.”

He invites other members of the WA dental community to get in touch if they share his passion for intraoral scanners and digital dentistry. “It is undeniable that digital dentistry has come to the stage where it is a must to introduce to DCDs and DMDs but also part of the general practitioner’s everyday use. I think there is still a lot to do on the subject so if people have any questions or want to reach out, I’m available.”

Robert is finding teaching in Australia very rewarding.

“There are differences between Sweden and Australia – here students pay for their education, while in Sweden, university education is free. That to me makes the students here feel like they really care for their education. On the other hand, the training as a dentist is shorter with only four years instead of five, making the curriculum much more compact.”

When he’s not at the university, you’ll likely find Robert at the beach. “I try to see the sunset every time I get the chance to; you don’t know how blessed you are to be able to take a stroll down to the beach. The beaches and the weather are amazing here. You don’t have this kind of blue sky everywhere in the world – it’s like therapy. I’m defrosting from living in a cold country for many years,” he laughs. “It’s also a new cultural experience being here in terms of food and things to do, so there’s always a lot on the to-do list every weekend.” adawa.com.au

love your teeth DENTAL HEALTH WEEK 1-7 AUGUST 2022 Dental Health Week has long been a way for dental practitioners to spread the word about the importance of oral health to the community. We take a look at Dental Health Week campaigns from years gone by – and how your practice can get involved this year.

Dental Health Week is ADA’s annual oral health campaign, which takes place in the first full week of August. However, it hasn’t always been this way. The campaign used to be known as ‘Dental Awareness Month’.

The key messages of Dental Health Week’s campaigns aim to help patients keep their teeth for life. The messages include: • Brush your teeth twice a day with fluoride toothpaste.

We found some examples of Dental Health Week (and Dental Awareness Month) campaigns from previous years.

• Clean between your teeth daily using floss or interdental brushes.

Some over the years have included:

• Visit the dentist regularly for check-ups and preventive care.

Although the themes have changed (and the campaign designs have become more modern), the aim of each campaign has always been the same – to promote oral health in the community.

St Pat’s Stories Volunteering at St Pat’s has opened Drs Amanda Lau and Michael Nguyen's eyes to the world of homelessness and social disadvantage – and they encourage other dentists to step forward to make a difference.

DR AMANDA LAU MAKING A DIFFERENCE, ONE PATIENT AT A TIME Dr Amanda Lau had her first taste of volunteering on a trip to Vietnam several years ago, which she found hugely rewarding. However, with COVID curtailing her plans to continue volunteering overseas, Amanda began looking for opportunities closer to home.

“I was looking at the volunteering opportunities on the ADA website to find something suitable that I could get involved with, and soon after I came across an article about St Pat’s receiving a grant,” she recalls. “It was something that I thought I could work into my busy schedule, so I got in contact with them towards the end of 2020.”

Amanda now volunteers at St Pat’s once a month. “It’s opened up my eyes to the world of homelessness and financial and social disadvantage and made me realise that, especially with the effects of COVID, a lot of these people are regular people who have had jobs and homes, but due to unfortunate circumstances, have found themselves unable to afford rent or the basics,” she says.

At the clinic, Amanda does a lot of extractions, but also restorations, exams, dentures and the odd root canal treatment – and says the majority of patients are thankful and really appreciative. “A lot of them come in quite nervous but leave pleasantly surprised at how painless the procedure was, and are really happy to be out of pain,” she says.

Amanda says there have been many satisfying moments. “The very first patient I saw at St Pat’s was a man who was extremely grateful and said I was the first person who had given him attention,” she recalls. “He was about to go for some interviews and said that by fixing up his teeth, he now had confidence to find a job and get back on track.

“There was another woman who had been a victim of domestic violence and she had been in a few times to have a front tooth extracted that had been causing pain, but didn’t have the courage on the day,” she adds. “On

the day she saw me, her upper lip was really swollen, and on discussing with the nurse, we thought it was a result of being punched, as she had been in the previous week with a swollen eye. After spending time patiently talking her through the procedure (and a lot of swearing on her part!) we managed to get the tooth out, along with a lot of pus. I couldn’t believe all that lip swelling was from the infected tooth!”

Being able to alleviate people’s pain is wonderful, but for Amanda, what she enjoys most about volunteering at St Pat’s, is being able to spend time chatting with people and hearing their stories.

“A lot of people are open about their struggles, and it’s nice to be able to share in that and give them some encouragement,” she explains. “It’s also really nice to be able to give treatment for free and not have to worry about patients not being able to afford the treatment.”

Amanda encourages other dentists to get involved at St Pat’s. “There is absolutely no pressure to commit to a regular schedule, and the clinic is well-equipped, and the staff are really friendly,” she says. “There are also some specialists and hygienists who volunteer.

“If anyone is unsure about it and wants to have a chat about what is involved in a typical day, I’m happy to answer any questions (and I’m sure the other volunteers will too) – just get in touch with the clinic.”

DR MICHAEL NGUYEN FINDING FULFILLMENT “I always wanted to volunteer,” Dr Michael Nguyen says.

“There are a lot of groups that travel overseas but I never had that luxury with a young family and a practice. I contacted Dr Dominic Longo a few years ago and he mentioned something was being set up, but I fell off the radar. Then, by chance, last year someone put me in touch with ADAWA, and Andrea Paterson replied asking if I would like to volunteer at St Pat’s.”

Although he hasn’t been at St Pat’s for long, the experience so far has been very rewarding. “You are directly involved in the community and helping people,” he says. “Day-to-day, I’m at a private practice where most people are quite affluent, so you don’t get to see the other facet of the industry.

“At my practice, some people will be making the decision of whether they should get a veneer or change their smiles. At St Pat’s you see patients that wish just to be out of pain, to eat, or to look presentable to get a job.

“Helping these patients makes me feel valuable because this is what I’m trained to do. I get back much more than I give; you go home and feel like you have done something worthwhile. The patients at St Pat’s are fantastic, they are very grateful and appreciate everything you do. Since I started at St Pat’s I really feel there is a sense of purpose there, an inner peace. It’s hard to describe because I’m not a spiritual person, but there is a fulfilment. I’ve also been inspired by many other volunteer dentists who have been there for so many years.”

Over the years, Michael has been involved in other dental volunteering efforts – his practice hosted a Dental Rescue Day, and also loans its clinical space to treat Healing Smiles patients. “I wish I knew about this type of thing years ago. On a weekend our clinical space is just sitting there and not being used, so being able to contribute it is great.”

Michael says his staff are very giving. “During COVID, staff were reaching out to elderly patients and volunteering to go shopping for them,” he says. “None of that was my initiative, but things members of my staff did themselves,” he says. “Collectively, we always think about what we can do for the community. We have also seen six or seven patients from CARAD – the Centre for Asylum Seekers, Refugees and Detainees – and will fit them in during lunchtime, when we can. One of our nurses is also liaising with the Ukraine refugee community to see how we can help.”

What Michael enjoys most about volunteering is the immediate impact you see. “Getting a patient out of pain and being involved in the community, you feel like you’re giving something back rather than just taking,” he explains. “It gives you a different perspective of the world.

“I want to do my bit and I’m trying to teach my kids to do the same,” he adds. “I have to walk the walk and not just talk the talk. I cannot tell my kids to be kind, be charitable and be humble and then do nothing myself.”

To other dentists thinking about getting involved, Michael highly encourages it. “You will feel quite fulfilled – this is why we do dentistry in the first place,” he says. “It isn’t just about bringing bread home and providing for your family, but it is the broader picture.

“Charity begins at home,” he adds. “For me, as a refugee, when I came here, we were given a lot of opportunity. We were supported by the community and even though we didn’t speak a word of English people welcomed us and were very patient with us. I think my generation owes giving back to Australia at large. I feel good giving back the kindness that was given to me when I was young.

“We are all busy. Today I didn’t have time to reply to any emails. But as busy as you are, you can always give one or two hours. When we did the Dental Rescue Day it felt good and it was a big event to look forward to, but then it was over, and I wanted to do something regularly.

“It’s like exercise; you don’t want to go to the gym once for three hours – you want to do something sustainable.

Something sustainable for me is fortnightly at St Pat’s – there’s always time to give back.”

Dr Samy Francis

We caught up with Dr Samy Francis in the leadup to his upcoming WA Dental CPD courses.

Dr Samy Francis graduated with a Bachelor of Dental Science with Distinction in 2008, and then worked in Egypt, New Zealand, Queensland and New South Wales.

“At the time, my plan was to go back to New Zealand after I finish the training and work as a periodontist there, but Perth became my home, so I never moved back.”

In 2016, he decided to move to Perth to complete a Postgraduate Doctor of Clinical Dentistry in Periodontics.

Samy will be presenting a number of WA Dental CPD courses in August, including ‘Posterior Crown Lengthening’ with Dr Armand Putra on August 11, ‘When Hopeless Teeth Are Not So Hopeless’ for the New Practitioner Program on August 18, and will also be one of the presenters at ‘Specialists in Darwin’ on August 27.

Knowledge is enabling, and it makes people better dentists so they can provide better service to their patients. This is what we are in the job to do – provide the best possible service to our patients.

When asked about the decision to specialise, Samy says he found periodontics fascinating. “I have always enjoyed the surgical side of dentistry and wanted to help people keep their teeth,” he says.

He has been in Western Australia for six years now, and calls it home. “This is where my life is – my friends and my work,” he says. “I wanted to become a periodontist, and at the time, I was living in New Zealand. There are six or seven universities that offer the course and each of them only take one or two students a year, so I applied everywhere in Australia and New Zealand, and WA accepted me first.

Samy says contributing to a dentist’s knowledge is very rewarding. “Knowledge is enabling, and it makes people better dentists so they can provide better service to their patients. This is what we are in the job to do – provide the best possible service to our patients.”

He also finds his practice work satisfying. “Mostly I enjoy the improvement you can see in someone’s quality of life after having treatment,” he says. “I love seeing the progress of a patient and being able to build good relationships with them over the course of treatment.”

Dr Samy Francis Which three words best describe you? Honest, hard-working and aspiring to learn.

What do you enjoy doing in your spare time?

I like cooking and I like running. I started running around a-year-and-a-half ago because I wanted to be a bit fitter. I could then barely run 1km, but then bit by bit I started to run 2km, 3km, 5km – then signed up for a 10km run and that went well, so I signed up for a half marathon, which is 21km. The next step was a marathon (42km) and I did one and it went well. Since then, I have been running regularly; every week I will run about 50-60km, and I have more marathons to run during the year.

What is something you have done that’s a bit different?

I recently took part in the ShitBox Rally. It is a car rally that you do in a shitbox (or a car that is under about $1,000) and we drove on mainly unsealed roads from Hay, NSW, to Adelaide to raise money for the Cancer Council. I did the rally with Dr Greg Celine (paediatric dentist) in a 1999 Daihatsu Charade and our team raised between $7000-$8000. The whole rally contributed $2.1 million to support cancer research. It was definitely an experience to remember.

Dr Glen Liddelow completed a BDSc and a MDSc from The University of Western Australia and a Doctorate in Clinical Dentistry (Prosthodontics) from the University of Sydney. He is a Board-certified prosthodontist, member of the Academy of Australian and New Zealand Prosthodontists, and Fellow of the Royal Australasian College of Dental Surgeons. Glen is Angeles (UCLA) Global Institute for Dental Education (gIDE) program.

He is a consultant prosthodontist at Perth Children’s Hospital and Royal Perth Hospital. Glen has presented over 300 papers and lectures both nationally and internationally and is a member of numerous university and industry boards concerned with dental education. ABOUT THE PRESENTER

Due to many reasons, many of our patients break their teeth, leaving us with very little tooth structure available for restoration afterwards.

Crown lengthening of posterior teeth is a procedure that aims at exposing enough tooth structure by removing some of the gingival and bony support of the tooth, allowing us to restore the teeth successfully. But, what are the factors that you should consider prior to deciding that this tooth is a candidate for crown lengthening? Also, in the dental implant era, is there still room for crown lengthening procedures for posterior teeth? This presentation will answer these questions and discuss further considerations and limitations of crown lengthening procedure at posterior teeth.

Dr Samy Francis works as a periodontist in private practice in Perth. Samy grew up in Egypt and in 2008, he completed a Bachelor of Dental Science withDistinction.

Since graduating he has worked in both private practice and the public sector in Egypt, New Zealand, Central Queensland, Canberra, regional NewSouth Wales, and regional and metropolitan Western Australia. In 2018, DrFrancis completed a threeyear postgraduate Doctor of Clinical Dentistry(Periodontics) degree through the University of Western Australia.

He worked in private practice in Perth, Bunbury, Darwin, andAdelaide until he decided to settle in Perth full-time. Samy is currently the treasurer of the Western Australian branch of the Australian Society ofPeriodontology (ASP WA). He is also a member of Faculty of Dental Surgeons,Royal College of Surgeons (Edinburgh), and the Royal Australasian College ofDental Surgeons in periodontics.

Dr Armand Putra completed his undergraduate degree in 2010 (University of Queensland) and graduate degree in 2016 (University of Washington). After his Prosthodontic training, Dr Putra completed a one-year implant Fellowship training at Loma Linda University.

He was awarded Certification by the American Board ofProsthodontics and Nationals Board for Certification in Dental Laboratory Technology in 2017. He is currently an Affiliate Professor at the University ofWashington (graduate prosthodontics), a member of American Academy of Fixed Prosthodontics, a fellow of the American College of Prosthodontics, and a Diplomate of the American Board of Prosthodontics.

In 2018, He has joined the team at Perth Prosthodontics as an associate, where he treats all aspects of prosthodontics with special interest in aesthetic and complex inter-disciplinary cases.

Have you ever given advice on a case in this manner, to then find later that the advice was flawed, when other facts come to light? Dr Annalene Weston, Dentolegal Consultant at Dental Protection, considers the risks to you, the patient, and your colleague of ‘corridor consults’.

A collective mind could be considered greater than the sum of its parts, and naturally, it is appropriate to reach out to colleagues and talk with them if you are unsure of a diagnosis or a treatment plan. Practitioners enact this formally by referring a patient to a specialist or seeking a specialist desktop review of their diagnostic records.

In itself, this carries little risk to any of the involved parties and is of great benefit to the referring practitioner and the patient. It is, however, important to be mindful of the fact that the specialist colleague can only make an assessment based on the quality of the information they are provided. Incomplete or poor records will impact on their ability to review the case fully or give meaningful advice.

It can be difficult for the requesting practitioner to know exactly how much information is needed, and, when we are unsure of a case, to know what diagnostic information we are missing. There are also times where brevity is a necessity, perhaps due to time constraints with the patient, or of our colleague. These brief interactions can carry with them significant risk if the information is not all available.

THE PATIENT The person most at risk in any corridor consultation is the patient. Incomplete or incorrect information can lead to an incomplete or incorrect diagnosis, causing the treatment to commence along the wrong path, which is often not identified until the patient suffers with a poor outcome. This recognition often occurs when the patient has pain, or their treatment has failed, and may be picked up by an independent third party. Regretfully, many practitioners in this third-party role have been known to be highly critical of the treating practitioner, leading to complaints to the regulator and legal claims.

THE PRACTITIONER While it may seem like a sensible step to take, seeking informal advice from colleagues can carry with it significant risk. Ultimately, you are responsible for the care you provide, regardless of ‘who told you to’ so advice outside of formal consultations/formally requested desktop reviews can carry with it significant risk. Should the patient complain, the complaint will be to you or about you. If the matter goes in front of the regulator, they will be assessing the steps you took and the treatment you provided, regardless of who directed you to do so. The manner in which you approached the colleague can also be prejudicial.

THE COLLEAGUE GIVING ADVICE While you do not have the same risks as your colleague, as ultimately, the treatment provider must provide the care that they see fit, corridor consults are not without risk for the provider of the advice. Be mindful of incomplete information, perhaps positioned to bias your thinking or solicit a preferred response from you, that may differ from that you would give if you had the whole picture. Potential reputational damage is a high risk for those providing informal information, perhaps even in a public arena, if your advice is publicly shown to be inaccurate/incorrect.

WHERE DOES SOCIAL MEDIA FIT INTO ALL OF THIS? Informal ‘Facebook style’ calls for diagnosis and treatment plans from colleagues en-masse are frowned upon by the regulator as being a poorly thought-out approach for advice when provided by a practitioner to support their thought process around treatment.

This particularly holds true if the ‘full picture’ was not provided in this request for advice.

There are also further risks to creating or responding to the ‘Hello brains trust, what do you think about…’ type of post. Largely because you cannot control and predict the comments made. Should a colleague (or two) engage in some unprofessional discussion on the thread, and you are the poster, then it is prudent to remove the thread immediately. If, however, you have provided comment or advice, and then the thread takes an unprofessional turn, it is prudent to contact one of the page administrators to have to post managed or removed. This advice is given as images of these social media ‘bust ups’ often make their way in front of the regulator, and you would not want your name associated with this level of unprofessional behaviour.

BRINGING IT ALL TOGETHER Seeking advice from a colleague is both prudent and sensible and can lead to professional and personal growth as well as a potentially better outcome for the patient. The critical issue is to ensure that this approach is made more formally with the requisite and necessary information available to the colleague to ensure the best quality advice and the best outcome for the patient.

We must always conduct ourselves professionally when interacting with colleagues, including on social media and be aware of the over arching and underpinning guidance around this.

Mid-Career Considerations for Dentists There are various career considerations at the midpoint of one’s career, however, perhaps the most important is ensuring that one is getting what they want out of their career.

For dentists, a mid-career assessment likely includes considering work-life balance, and, the potential for opening up your own practice, having established your skills and professional identity in the early stages of your career.

WORK-LIFE BALANCE Having a good balance between personal life and work life is undeniably important, not only for your enjoyment of life in general, but also for productivity at work. Many dentists will have spent a great deal of time in their early career prioritising work in order to develop their professional identity, however this may leave some feeling burnt out and in need of better work-life balance.

There are various strategies which can be employed to ensure that work life balance is achieved.

First, have a positive support system in place. Being comfortable opening up and sharing concerns with close friends and family and even colleagues can alleviate the impact of stressors. Having a positive relationship with colleagues or others in the profession can also help one to feel more supported in the workplace, and thus, less stressed.

Secondly, prioritising self-care is a great way to ensure that work-life balance is achieved. Self-care looks different for everyone, however, may include taking a break, exercising, mindfulness and meditation practices and having uninterrupted time for family and friends.

Figuring out what restores you and makes you happy can improve productivity at work.

Setting goals can also be a great way to improve worklife balance. Being intentional about managing your time, doing the things that you enjoy and how you plan on achieving them can help with the journey.

OPENING A PRACTICE Whilst many mid-career-stage dentists may intend to wind down and have more balance, others may be hungry for more, and seek to open up their own practice once they feel established as a practitioner. Naturally, there are many considerations to opening a practice, including financial and time constraints. In addition, practitioners will also need to consider whether they are restrained from opening a practice within a particular time frame or area pursuant to their contract of employment or contractor agreement.

Common post-contractual trade restraint clauses may include restricting a dentist from opening a practice or working in another practice within a specified area, restricting a dentist from opening a practice for a period of time after the employment or engagement ends, and expressly prohibiting the solicitation of patients and staff.

To have any effect, such a clause must be reasonable in the circumstances, and be reasonably necessary to protect the interests of the original practice.

Before making any decision to open a practice, practitioners should review their contract of employment or contractor agreements for restraint of trade clauses and consider whether they have the desire and capacity to own a practice. Practitioners should seek specific advice from financial and legal advisors and professionals as to the legal obligations of opening a practice of their own and any associated risks.

Pete, a 17-year-old male, was referred with a complaint of several weeks’ history of recurrent right facial swelling over the right angle of the mandible. The non-painful swelling lasts for about a week before resolving. Pete denies any fever, malaise, or other systemic symptoms. He does note that his right third molar (48) is erupting.

His medical history is unremarkable, and he did not have any known allergies. He has never smoked and does not consume alcohol.

CT of the mandible does not demonstrate any pericoronal or periapical pathology associated with the third molar, but a diffuse non-specific soft tissue swelling is reported in the buccal sulcus and mucosa adjacent to the right mandibular molars.

Extra-oral examination demonstrates a mild soft swelling over the right angle of the mandible. There is no redness nor heat. Intraoral examination demonstrates deep linear ulcers along the right buccal sulcus and gingival tissues of the right mandibular first and second molars. The right third molar is partially erupted, and the operculum does not appear to be inflamed.

Dr Lalima Tiwari | Oral Medicine Specialist A PEBBLY PROBLEM What I love about oral medicine is that it links what is often an isolated field of dentistry to medicine. It provides an avenue of health care where even innocuous oral lesions can lead to a significant medical diagnosis for a patient. As dentists, we are in the perfect position to aid in these diagnoses, through detection of oral lesions and disorders. The below case describes a rare genetic disorder which was diagnosed from an incidental finding in a routine dental examination.

HISTORY A 56-year-old male was referred by his general dentist regarding “white bumps” affecting the oral mucosa, that were incidentally noted at a routine dental examination. The patient was asymptomatic and unaware of these lesions. His medical history was significant for hyperlipidaemia and hypertension that was managed with 10 mg ezetimibe and 5 mg perindopril arginine, respectively.

CLINICAL FINDINGS Examination findings revealed longitudinal ridging and white stripes of the fingernails (Figure 1). No regional lymphadenopathy was palpable extra-orally.

Temporomandibular joint and muscles of mastication were within normal limits. Normal mouth opening and range of motion was noted. Intraorally, multiple white-pink papules, 2-3mm in greatest diameter, in a cobblestone pattern were noted to affect the maxillary hard palate, bilaterally. Similarly, generalised white-pink papules, in a cobblestone pattern were noted to affect the labial attached gingiva on the maxilla and mandible, including the mandibular right edentulous alveolar ridge (Figure 2). Pin-point multiple white papules were noted on the bilateral buccal mucosa (Figure 3).

SELLING OR BUYING A

THE CORRECT DIAGNOSES IS: Orofacial granulomatosis Orofacial granulomatosis (OFG) is a rare chronic inflammatory and granulomatous condition.

OFG may be idiopathic, or may present in conjunction with other granulomatous diseases including Crohn’s disease or sarcoidosis. OFG is most common in young adults, but may present at any age, and there is no sex predilection.

Clinically, OFG presents as a recurrent, usually painless soft orofacial swelling. Most commonly, the lip is affected however not always, as was the case with this patient. Eventually the swelling becomes persistent and indurated and often cosmetically undesirable. Other orofacial manifestations include ulceration, gingival involvement, angular chelitis and vertical fissures in established lesions.

Biopsy of an appropriate location and depth will demonstrate non-necrotising granulomatous inflammation on haematoxylin & eosin staining, and the pathologist must exclude infectious causes (e.g mycobacterial or fungal infections) or foreign-body reactions.

Systemic granulomatous diseases must be excluded by investigations such as blood tests and radiology as appropriate. As between 10 - 40% of patients with OFG have associated Crohn’s Disease, referral to a gastroenterologist is prudent even in the absence of gastrointestinal symptoms.

The management of OFG is often difficult, with variable and unpredictable clinical outcomes. Treatment may include dietary restrictions, corticosteroid therapy or other immunomodulatory drugs.

With prompt diagnosis and treatment, persistent orofacial swelling may be prevented. Follow-up is imperative, as recurrence is common.

Clinical Q+A in Oral Medicine THE CORRECT DIAGNOSES IS: Hailey Hailey Disease

Darier Disease

Differential diagnosis for this presentation includes Hailey Hailey Disease and Darier Disease. An incisional biopsy was undertaken from the right hard palate.

Histopathology results demonstrated suprabasal clefting with some evidence of acantholysis. Scattered corps ronds were seen. The superficial epithelium showed hyperkeratosis and parakeratosis. The underlying stroma showed a sparse inflammatory infiltrate, consisting predominantly of lymphocytes.

These features were diagnostic of Oral Darier Disease. The patient was subsequently referred to a dermatologist to exclude cutaneous manifestations.

Hyperkeratotic plaques were found on his trunk and the patient was treated with 30 mg acitretin.

Darier Disease, also known as keratosis follicularis, is a rare autosomal dominant genodermatosis. It is caused by the mutation of ATP2A2 gene, a gene that encodes a calcium pump in the endoplasmic reticulum. The condition has a high penetrance but variable expressivity. As such, every person that inherits this genotype will have varying manifestations in severity and features. Subsequently, recalling a family history of this condition can become difficult and it is important not to exclude Darier Disease based solely on a negative family history of the disease.

Clinically, the condition can affect the skin, nails and mucous membranes.

Skin manifestations are characterised by hyperkeratotic papules and plaques, often in seborrheic areas such as the forehead, central chest, back and scalp margins. The plaques can appear papillomatous or verrucous and can manifest as painful fissures. Nail findings include red and white lines oriented longitudinally over the nail. In 50% of patients with Darier disease, the oral mucosa is involved. Commonly, asymptomatic multiple hyperkeratotic papules are noted in a cobblestone pattern in the oral cavity. Occasionally, gingival hyperplasia can be seen.

Clinical and histopathologic correlation is required to diagnose Darier Disease. Management includes avoiding exacerbating triggers such as heat, occlusive clothing and friction. Use of topical and oral retinoids, corticosteroids and antibiotic agents are used to treat symptomatic cutaneous lesions. Asymptomatic oral Darier Disease does not warrant any specific treatment. Individuals with Darier Disease are susceptible to bacterial and viral infections including human papillomavirus and herpes simplex virus. Genetic counselling should be provided to the patient given the pattern of autosomal dominant inheritance.

Jan and John Owen report on the Kimberley Dental Team’s most recent (very busy and very enjoyable) trip to the region.

Our first team arrived through Broome and travelled across to Halls Creek, where we spent the week.

Navigating COVID protocols was somewhat of a challenge, but everything went very well and we were able to see and treat 160 people at the Halls Creek hospital dental clinic, Yuri Yungi Aboriginal Service dental clinic and at the communities of Ringer Soak and Yiyili.

We also performed very well at the Kimberley Hotel quiz night and enjoyed a sausage sizzle BBQ at Caroline Pool.

The outgoing and incoming teams had a changeover in Kununurra while John and I prepped for our trip to Warmun Community. The new team met us in Warmun on Sunday afternoon, and we had a busy two days working in the community before returning to Halls Creek for the rest of the week. The week two team provided a service for 174 people in Warmun, Frog Hollow and Halls Creek. Our final team came in through Broome where they met the outgoing team, handed over vehicles and drove across on Sunday.

This final week was spent in the Kutjungka region visiting the communities of Billiluna, Balgo and Mulan where we saw and treated 161 people before returning to Broome via an overnight at Fitzroy Crossing that included a celebratory dinner.

Statistics exceeded expectations of the numbers of people treated in May 2022. The teams provided examinations, oral health education and dental services for 495 people. Additionally, these services included 154 extractions, 219 restorations, 26 scale and cleans, 45 topical fluoride treatments and 584 fissure seals.

Thank you to the Australian Dental Association (WA Branch), for their ongoing support of KDT in general and especially for awarding Grants for finalyear dental students in 2022. These Remote Area Student Scholarships greatly assist with their remote placement costs. KDT are now preparing for a return to the region on July 22 for three weeks.

The dental community came together for a spectacular event to raise money for a very worthy cause.

Get dentists out of their PPE and they are a glamourous bunch, as was seen at the inaugural Dental Charity Ball, held at the State Theatre of WA in June.

Dr Amanda Phoon Nguyen, who co-organised the event with Dr Sean Archibald and Dr Jenny Ball, says she’d been thinking about running a charity gala ball fundraiser for some time.

“Frankly, it was something that several of my friends and colleagues had asked me to organise over the years, and I thought why not give it a go,” she says.

“It seemed like a perfect blend – supporting charity and fundraising, having fun and also showcasing some of the amazing musical talent we have in the dental industry.”

Amanda says she usually runs one or two fundraising events per year, and this time, the timing coincided with the launch of Uniting Smiles (a charity dedicated to looking after the oral health needs of vulnerable West Australians).

“It’s a charity I am involved in and a big supporter of,” she explains. “While I have previously fundraised for Uniting Smiles, it was a relatively small amount and I felt that we could do more. I approached Jenny and Sean as we work well together, and I knew that they had relevant areas of expertise. It worked perfectly and they were a dream to work with.”

Generous sponsors came on board, including main sponsor SEARCHBOX, and silver sponsors Credabl and Henry Schein Australia, whose support meant the event could be a reality.

The event was open to everyone in the dental community, and 180 attended, including Guest of Honour Libby Mettam MLA (Shadow Minister for Health, Mental Health and Disability Services; Prevention of Family and Domestic Violence).

THE ENTERTAINMENT DJ Kesh entertained the crowd, with several dentists on the list of performers for the evening, including Dr Luke Chow and Tiffany Chow from ‘Kestrel N Kites’ (no one has ever made flossing sound so good!), Dr Ali Wallace and the band ‘Dental as Anything’, and Drs Sophia Hawes and Simon Tee. Dr Dean Martin was also on the list to perform, but unfortunately was unable to attend.

PRIZES Dr Graham and Wendy Carmichael were instrumental in organising the raffle and silent auction prizes, and Amanda says the generosity of prize donators and sponsors was overwhelming.

Dr David Hallett was invited to draw the raffle prize (with a total value of over $2,500), which included a white-gold and diamond necklace from Rohan Jewellers and a Saltair Gracetown two-night getaway.

WADA GOLF

Joondalup Country Club Nearest-the-pin prizes for the day, sponsored by our corporate partners, were won by the following: Under threatening skies, 22 committed WADA golfers took to the Joondalup Country Club Quarry and Lakes combined course, for our traditional early winter fixture.

Fortunately, the showers held off and the group was rewarded for their commitment with a magnificently presented course. Some treacherous pin placements, and a strong north-westerly breeze made for challenging conditions.

The event was sponsored by Dentaurum Australia, a first-time WADA Golf sponsor. Based in Sydney, Dentaurum is the oldest dental supply company in the country and has a proud history of product and service excellence, supplying a wide range of orthodontic, prosthetic and hygiene consumables. Sadly, we were not able to be joined by any Dentaurum representatives on the day. Our sincere thanks are extended to Dentaurum Managing Director, Kevin Ferrari, and his team for their generous support of this event. We look forward to a long association with Dentaurum for future WADA Golf events.

Our winner for the day on a fine score of 38 points was Peter Shipway. Keenan Inderjeeth returned to form with a steady 36 stableford points. Garry Bishop from Health Practice Brokers was a close third on a countback from Richard Williams, both with 35 points.
