Dear ADAWA family, It’s that time of year again: membership renewal time.

While some members pay their membership fees annually, and others pay it over a period of months, the value of your membership fees goes a long way beyond just renewal. Your membership has aided in reducing the effect of this pandemic on our practices and our community; it has assisted in providing PPE resources and making sure our ADAWA team answers your questions in a timely manner via email, phone calls, social media and messaging. This is just one example of how your ADAWA family stands beside you to offer reassurance, advice and help in times of uncertainty.

We have managed to keep the premiums for our members to a minimum increase of CPI this year, as we understand that the uncertainty of COVID-19, with short-notice patient cancellations and staff shortages, continue to make an impact on a day-to-day basis.

Your membership has aided in reducing the effect of this pandemic on our practices and our community; it has assisted in providing PPE resources and making sure our ADAWA team answers your questions in a timely manner via email, phone calls, social media and messaging.

Nevertheless, to continue to provide the high-quality service that we do and to have the resources that you expect from your ADAWA team, it's important that each and every member renews their membership as soon as they can. I have said this before, and I reiterate it again, we are a community, and we are only strong when we work together.

There are many external forces affecting our community: we can advocate on behalf of our community and be taken seriously only when we have one voice. A perfect example of this was the advocacy seen in our most recent federal election.

Our association at a federal level has proposed a senior’s dental benefit scheme similar to the child dental benefit scheme for the Government to support. Every single State branch advocated on the same lines, and it was heartening to see that the ADA voice was heard by the parties during the election. This was just one of the examples of how a united team can have a powerful voice to make change.

On another topic, it is with a heavy heart I write about the retirement announcement of our much-loved and valued CEO, Dr David Hallett. Dr Hallett has been the CEO and the heart of our association for the last eight years.

With such big shoes to fill, it is important that we have a fair and transparent approach, which is broad scoping in the search to find our new CEO, who will take our association forward to the next level and into the future.

Dr Hallett has been a true value to our membership, always available with a smiling face and full of passion to help our members.

There would only be very few members who either haven’t been on the receiving end of his timely advice or who have been helped with a sticky situation.

For me personally, it's going to be a hard hit, as Dr Hallett has been my friend, mentor and colleague on the ADAWA council for over a decade. Working with Dr Hallett in my current role as the ADAWA President has shown me how much work and effort goes in at an operational level to keep ADAWA moving along smoothly. This is not only the management of staff, but also the negotiations and communications with external stakeholders. These efforts are what most of us members can easily take for granted.

The services of an executive search firm, Gerald Daniels, have been engaged by our council to help in the CEO search. An exhaustive search process is currently being undertaken to provide ADAWA council with a shortlist of candidates. As we progress through to the next stages in this search process, we shall continue to keep you updated.

ADAWA is a strong and dedicated association with highly valued members. We should never forget that it is our membership which gives ADAWA the strength to continue to be a successful organisation.

We have now started a search for the new ADAWA CEO: we know that there is no way that we will find a clone of Dr Hallett, as he is irreplaceable, but all efforts will be made to find the best person to lead our association.

No one wants to think about the ‘what ifs’ but if you are a partner in a successful business, the ‘what ifs’ can’t be ignored. Awards AND accolades It was a full house for ADAWA’s May general meeting, at which the ADA Federal President and CEO presented awards to very-well-deserving members.

ADA Federal President Mark Hutton and CEO Damian Mitsch flew into WA for the May general meeting, after an enforced absence of two years. Both were delighted to finally present well-deserved awards to ADAWA members, while meeting Federal Councillor Dr Janina Christoforou inperson for the first time.

Other esteemed guests at the general meeting included Dr David Booth (who was presented with an ADAWA Distinguished Service Award) and Rae Booth, who accompanied him.

We were also fortunate to hear from Dr Nandika Manchanda, specialist paediatric dentist, who spoke about hypomineralised enamel in the paediatric patient.

DR DAVID HALLETT ADAWA CEO Dr David Hallett was awarded the ADA Distinguished Service Award. “Receiving the ADA Distinguished Service Award was a great honour, and it was very flattering to receive recognition from the Federal Branch,” David says. “An award like this would not have been possible without the support of the ADAWA office staff and present and past executive and council members.”

DR TERENCE (TERRY) PITSIKAS AM Terry received the ADA Life Membership for his outstanding contribution to the profession. Terry says he doesn’t volunteer with any expectation of reward or award, but he was very honoured. “It really has been a labour of love,” he says.

WINTHROP PROFESSOR PAUL ABBOTT AO Paul says it was a great surprise to receive the letters telling him that he had been given the awards – firstly from the Federal ADA in 2021, and recently from the WA Branch.

“I feel incredibly honoured to have been considered worthy of these awards as I would not have considered my contributions to have been enough to justify them,” he says. “I have enjoyed the opportunities, and the ability, to contribute to the association over many years and I thank the Federal Councils and the Branch Councils for their faith in me when asking me to be part of the various activities and committees on which I have served.

DR KANG KIM Former ADAWA President Dr Kang Kim was honoured to receive the ADA Outstanding Young Dentist Award.

“There are so many young dentists who are striving to make a broader contribution to dentistry,” he says. “We're blessed with commendable displays of altruism, innovative thinking and hard work which bring distinction to our profession in WA and around Australia. Among the countless outstanding young dentists, I don't think of my contribution as more significant than those of my colleagues, but I am very honoured to have my contributions recognised.”

Kang was also a recipient of the ADAWA Distinguished Service Award. “I received this award alongside a gentleman who has given a lifetime of service to dentistry and to helping those less fortunate,” he says.

“The next recipient was one of the most distinguished academics and clinicians of his generation. The final recipient was someone who, as laid back as he seems, put his own life on hold to lead our profession during one of the most difficult external challenges we have ever faced. Needless to say, it was a humbling experience. I am truly grateful for this honour.”

DR AMANDA PHOON NGUYEN Amanda says she was honoured to be an award recipient alongside colleagues she holds in high regard.

“As Dr Terry Pitsikas said when accepting his award, we do not volunteer with any expectation of reward or award. I have had occasions where I asked myself why I do it, with the heartache, late nights, and frustration. It’s natural when you care about something,” she says. “To put yourself out there is also to draw criticism, and that is part of the role. But for me, the experience has also been invaluable and character building, and working together with good teams to add value to what we do and make a difference that is hopefully long lasting has been such a highlight and makes it all worth it.

“I strongly encourage anyone who wants to contribute to the profession to do so,” she adds. “You are needed!

ADAWA has many incredible members that put in huge hours and I look forward to seeing these acknowledgements continue across the board.”

It’s not every day you see a Jedi at ADA House! When Michael Lyons was notified that he was eligible to receive his 40-year Continuous Membership Pin, he was not able to attend the general meeting (in March 2022). However, when he was told he could attend the following General Meeting and noticed that the date was May 4, he decided to accept his pin in Jedi get-up, complete with lightsabre. “We all need to have a bit of fun,” he says.

Sponsorship and support A-Dec's territory manager, Joe Minervini, is wellknown in West Australian dental circles. However, his commitment to the dental community goes far beyond the provision of quality dental products, as Brooke Evans-Butler discovers.

For the past 30 years, Joe Minervini has been quietly servicing the West Australian dental community.

He began his career working in customer service assistance for Ivoclar in 1989, and over the next 13 years moved into a sales position, selling equipment. In 2002, he joined West Coast Dental as their equipment specialist selling A-dec, and then, in 2008, joined A-dec (a family-owned company committed to giving back to the community) as the territory manager for WA.

ADHF SPONSORSHIP Late last year, A-dec joined the Australian Dental Health Foundation as a supporting sponsor. The sponsorship provides a generous $40,000 per year over three years to help ADHF deliver their dental programs to assist disadvantaged Australians. Joe says as a global company, A-dec has always encouraged public service.

ST PAT’S ORAL HEALTH CLINIC Joe has been a supporter of St Pat’s since its inception – helping the St Pat’s team with ideas and designs for their oral health clinic. “We thought a lot about the dentists who were all going to be volunteers and decided that if these dentists were going to give their time to help the homeless, the equipment they were going to use had to be easy to use, seamless and not break down,” he says.

“At the time, St Pat’s was given some donated equipment, but we know that a lot of the time dentists donate equipment because they are upgrading, so the equipment was old and tattered. We didn’t want the equipment to break down during a volunteer’s shift, in case they didn’t come back, so one of the important things I put forward for St Pat’s was to get equipment that was new and reliable.”

Anyone who has volunteered at St Pat’s will know how well-equipped the clinic is, and Joe helped to not only supply a high-quality chair but was behind the scenes sourcing the x-ray (Acteon X-Mind Unity), the digital processor (Acteon PSPIX2) the steriliser (W&H Lisa FA) and the handpieces (W&H Synea).

We thought a lot about the dentists who were all going to be volunteers and decided that if these dentists were going to give their time to help the homeless, the equipment they were going to use had to be easy to use, seamless and not break down.

“We always want to give back to the community and this sponsorship agreement for the next three years is our way of helping the community become stronger, healthier and more successful,” he says.

“To do so, I connected with other manufacturers and suppliers around the country and asked them if they wanted to help by donating new equipment or offer heavily subsided prices to get the clinic together.”

Joe is also on-hand to ensure the equipment at St Pat’s stays in good condition, as the first point of call if there are any issues.

EDUCATING DENTAL STUDENTS

UPDATING PRACTICES

As well as giving his advice to dental practices, Joe is invited to OHCWA to teach dental students about the setting out of sterilisation rooms, infection control and dental equipment.

Being part of the dental community’s volunteering efforts is rewarding, but it is also satisfying to work with practices to upgrade their spaces – helping to ensure dentists have the choice of quality and reliable of equipment that suits them best. “I really enjoy when someone comes to you looking for equipment and you come back a couple of months later and an old practice or empty room is all brand new and a functional space,” he says. “I love being able to tell people how we have turned a run-down clinic into something amazing and how much more the dentists and assistants have enjoyed working just by updating their equipment. I like being involved in those stories.”

“Michelle Lewis looks after the sterilisation processing at OHCWA and a few years ago, asked me to come in and give a speech on understanding the basics of what an autoclave does and some of the guidelines around sterilisation,” he says.

“I also do a handpiece maintenance program for North Metropolitan TAFE dental assistant students. It gives an introduction to handpieces and the dos and don’ts of looking after them.

“It is important for students to have an understanding of the equipment they are using because if the handpiece drills are not looked after or maintained properly, they will fail and break and it always happens in the middle of a procedure, which is frustrating for the patient and upsetting for the dentist.”

Changing lives Dr Chae Park

In their different ways, Drs Chae Park and Wendy Tran have been selflessly making a huge difference to people in need, both in Australia and across the world.

DR CHAE PARK Prosthodontist Dr Chae Park was first exposed to dentistry volunteerism when she was an undergrad at the University of Otago in New Zealand, taking part in an outreach program to assist Māori children. “My parents migrated in 1995, so I found it an amazing turnaround from being an immigrant to having the opportunity to give back to the society where we were very welcomed,” Chae recalls.

This desire to help others was again sparked when Chae attended a Women in Dentistry event and heard the attendees talking about giving back to domestic violence survivors.

“Domestic violence could happen to any of us, so I really liked the idea of helping these women,” Chae says.

“I never had experience with domestic violence, but I do know of a few acquaintances who have, and it was very difficult for them to get out of that situation. The women who come through Healing Smiles must be ready to move on, so to ask for help is very courageous.”

As a prosthodontist, Chae mainly provides restorative consultations to Healing Smiles patients. “The majority of my patients have had all of their teeth removed, so we provide some kind of prosthesis, whether it will be a full or partial denture,” she says. “Many of them come into the clinic very scared and some are sceptical of why we want to treat them for free. But they are all so grateful at the end of the day, and I always get a lot of tears and big smiles at the end of treatments.”

There have been many satisfying moments as a volunteer, and Chae says every patient has her own story.

“I had a lady who went through extreme emotional domestic violence. She is very smart but never had the courage to pursue her dream of going to university,” Chae says. “She told me she didn’t deserve this and that – and it was almost like being a psychologist, encouraging her that she was worthy and that part of moving forward was to dream bigger than you think you can.

Earlier this year she was telling me she is getting distinctions in her courses. Stories like hers motivate me and keeps me grounded.”

To other specialists thinking of volunteering, Chae says it only takes a bit of time, patience and a desire to give back – and you receive so much in return.

“Through volunteering I get to meet lots of different people,” she says. “I am very thankful for what I have, and it is empowering that I can help others; it gives me joy.

“If you have ever thought about volunteering, just give it a go. We are all very busy, juggling families, kids and work, but it doesn’t take a lot of time and it gives so much back.”

DR WENDY TRAN It was seeing her sister volunteer that first motivated Wendy to look for volunteering opportunities. “My sister is an engineer and needed to volunteer for extra credentials on her CV to apply for jobs,” Wendy says.

“That was how her industry works, but for me who was doing health and science at the time, I just thought volunteering was a good idea.”

Wendy started volunteering at university with the guild, doing things like helping out with UWA Open Day – but it was during her second year of Dental School that she was introduced to CHAT (Children’s Health Aid Team), where she saw first-hand the difference that volunteering could make.

“CHAT goes to Vietnam, and as my parents are Vietnamese, I thought I could do the trip and also arrange a holiday. My parents were encouraging, so I signed up for a trip at the end of my second year.

“It was an eye-opening experience,” she says. “You don’t see dental caries like that in children in Australia, even at OHCWA. It was hard work, you were sweating, it was humid, there was no nice dental unit. But there was always something you would have not seen or done at the time, especially as a second- year student.

“It was also amazing how the kids in these orphanages were aware that they were quite fortunate to have people come to help,” she adds. “They understood the value of getting this dental treatment. It was very rewarding, and I did the trip again with two friends in my third year. It was a great experience.” Back in Perth, Wendy continued helping CHAT with their advertising and marketing, although that has been stalled due to COVID. “It has been a different kind of experience, reminiscent of OHCWA in terms of the kind of cases I would see there.

It has been nice because since I graduated, I have been in private practice and most patients I see have beautiful white teeth, so coming back to these dental cases where you have to plan out a full mouth rehab has been rewarding and the ladies have been really grateful.”

Wendy has been seeing two ladies consistently since volunteering with Healing Smiles.

“I have been doing almost a complete clearance and slowly doing quadrant extractions on one of the ladies,” she says. “The first time I saw her she was really withdrawn and was dealing with being in pain constantly. I told her to try to stop smoking because I had done multiple extractions and there was a risk of dry socket and she said that she needed to smoke. The next time I saw her she had quit smoking. She is also less withdrawn; she hops in the chair and is ready to go.

“Seeing her demeanour change from being so withdrawn to quite motivated has been satisfying. To be able to get to the vision I have for her when she will be stable with potentially a new smile will be great.”

Wendy said volunteering is helping to keep her on her toes, so she doesn’t fall into the stagnant lifestyle of ‘work, home and social life’ and recommends volunteering to other recent graduates. “It has given me exposure to cases I might not have otherwise been able to see. As cheesy as it sounds, volunteering helps fills a hole in your heart.

“I got into volunteering thinking I am not doing much, or I am just one of many other volunteers and my contribution is very minor compared to someone with much more experience. So, I am always surprised when I see the impact that can be made for a patient.” adawa.com.au

Born out of a need for better and more accurate images of dentistry, Dental Stock Photos was founded

by Dr Roslyn Franklin. She’s now partnered with ADAWA to provide great dental images at a reduced price to our members.

If you’ve ever looked for a stock photo for your practice website or a presentation only to be confronted with cheesy and inaccurate dental images, then you’re going to be impressed with Dental Stock Photos.

Dentist, infection prevention control consultant, and Dental Stock Photos director, Dr Roslyn Franklin, says the idea was born out of frustration.

“In mid-2020 I was asked by Ansell Healthcare to do a webinar presentation on infection control challenges when reopening dental practices,” she recalls. “They asked me to make the webinar suitable for a global audience and said they would purchase any imagery for me.

I storyboard all the shots. I try to find a shot similar to what I want to do and put it in my storyboard so I can say: ‘This is what I want the angle to be and the type of shot that I want.’

“I could not find a single stock image that was suitable.

Every single photo I rejected for a reason. It was either, ‘No – we don’t sit that way’ or ‘No – we don’t hold instruments that way,’ or ‘No, we don’t wear PPE that way.’ I decided it was time for me to do something about it.”

Roslyn approached a photographer friend, Tanita Seton, and suggested they take some accurate dental photos to sell to Shutterstock, but Tanita encouraged Roslyn to start her own website instead.

After getting a quote for a website, Roslyn commissioned Tanita to take 1,000 images to get the catalogue up and running. The Dental Stock Photos website was launched in November 2020.

Anyone who knows Roslyn will know how meticulous she is when it comes to infection control, so she painstakingly takes the time to ensure that every shot is accurate. adawa.com.au

“We’ll then also do variations, so in one shot a dentist might be wearing blue PEE, with another shot they might swap their surgical mask for an N95, or we get the patient to change to a differentcoloured top to give extra options.”

Fast-forward to 2022, and more and more people in the dental industry are discovering the benefit of having access to accurate dental images, with users including ADAWA (for our website relaunch and magazine), Bite Magazine, Ansell and the ADA Federal Branch (who recently finished a one-year subscription and have used the images on their website, in EDMs and for publications, including the ADA Guidelines for Infection Prevention and Control). The images of dental procedures are also widely used for presentations, and for marketing for social media and practice websites.

Roslyn has also branched into custom shoots and was recently commissioned by Ansell to show two new gloves being used ‘in action’ in the dental practice.

There are now more than 3,000 photos in the Dental Stock Photos catalogue, with new photos being added every Tuesday. The range is extensive, with collections including children’s dental, dental aerosols, dental equipment, flatlays, dental x-rays, treatments, oral hygiene, PPE and so much more.

The launch of Dental Stock Photos was initially slow, as Roslyn discovered the website wasn’t user-friendly, so she spent the next year refining the website and getting the word out.

CONTRIBUTING PHOTOGRAPHERS Roslyn’s next focus is to get more contributing photographers on board, as she would like to include before and after shots of dental procedures of implants, Invisalign, orthodontics and veneers, for example. She would also like more marketing photos of anything related to teeth, oral health and oral hygiene. “A lot of dentists do their own photography, so I would love to get those images on the website,” she says. “A dentist who would like to contribute photographs would need to get patient permission and I will provide all the model release forms. There is a screening process the photos have to go through, so not every photo would go on the website, but I would love to get the word out. The photographer’s bio will go on the website and include a link to their practice, and they have a chance to earn a commission from the photo, so it’s a win-win for everyone.”

I take my own gloves, masks and gowns because I don’t want to use the practice’s resources up,” she says.

A meeting of minds Boasting every Doctor of Dental Medicine and BDSc graduate as a member, the UWA Dental Alumni Society offers many wonderful opportunities for reunions, fundraising and support.

UWA Dental Alumni Society President Dr Gwen Chin is understandably excited about the society’s upcoming plans. Together with volunteer committee members Drs Meheransh Chopra, Raphaela Varajao, Amit Gurbuxani, Janina Christoforou and EeMay Cheung, the society is planning a number of initiatives and events for 2022, including reunions, fundraising and support for students.

“We are not a rich society so fundraising is the forefront of our agenda, but the alumni also tries to help in small ways when we can.”

Gwen says the alumni aims to help students navigate the transition to graduates, so they try to form networks to reinforce the bonds between the alumni and students. In collaboration with ADAWA, the alumni is planning an event for final year students to connect the students with different aspects of dentistry, including country dentists, government dentists, private practitioners, specialists and volunteerism groups. “I refer to it as ‘the speed dating event’, because it will be an informal eventing when students are split into a group of about six to eight people and they meet with a group,” Gwen says. “After about 20 minutes, the group of students moves to the next group of dentists, so they get the opportunity to connect with everyone.”

FUNDRAISING

There are also plans for other events to connect students and alumni throughout the year.

Reunions are always a highlight of an alumni society, and the UWA Dental Alumni Society is no exception. Gwen says the alumni’s first event for 2022 (a reunion for final years from 1997 to 2001) in February was a great success. “We had maximum capacity, which was 40 people because of COVID-19 restrictions, and the venue was lovely, with food and drinks in abundance,” she says.

As well as an opportunity to connect, the reunions also have an educational component.

Fundraising for the UWA Dental School is a key focus of the alumni society, and Gwen says the current goal is raising funds for the Clinical Skills Simulation Lab. “Most of the phantom heads at Optech are old and are not a good functional apparatus to train with. For us to begin training in treating patients, this is where the foundations are laid,” she says. “We want to focus on raising funds for this purpose to give dental students a good foundation to start their journey into the profession.”

As well as fundraising through events, Gwen says members of the alumni have been generous when it

In 2021, he completed his Master of Health Leadership and Management from UNSW. He has spoken at a number of forums including the ADA Dental Congress and AICD Next Gen Directors event. Dr Kim is currently working as a healthcare consultant at EY.

Dr Michael Frazis

Prior to his upcoming course at ADA House, we caught up with Dr Michael Frazis.

Dr Michael Frazis graduated from Adelaide University in 2012 and says he spent the first five years of practise honing his basic restorative skills, before focusing on complex restorative and surgical skills and undertaking implant training.

• Fixed vs removable replacement options

For several years, he has been taking part in hands-on courses and teaching basic restorative skills, and has been an educator and trainer for RIPEGlobal since their inception in 2020.

• Cementation and maintenance

For Michael, being able to educate others has been an important and rewarding part of his work. “It all stems from being shy when I was a teenager,” he says. “I knew when I got into dentistry that it involves a lot of interaction with the general public, and I needed to increase my own confidence.

“I started teaching first aid to improve my confidence and ended up falling in love with the process of seeing students grow and develop their skills. It is very rewarding. A lot of people in my family are teachers or educators so it too was a natural progression for me. I started teaching dentistry with Dr Lincoln Harris back in 2016 and haven’t looked back ever since.”

Michael will be coming to Perth to present his course, Non-Implant Replacement Options for Single Missing Teeth, in July.

The course aims to add tools to the dentist’s treatment options so they can best manage their patient’s needs no matter the circumstances or complicating factors, and will discuss dentures, cantilever bridges, Maryland bridges and everything in-between. Learning outcomes include:

“The non-implant replacement options course that I created with the help of Dr Sravan Chunduru (prosthodontist) is designed to fill a gap in people’s dental education,” Michael explains. “Most courses focus on how to keep a tooth in the mouth or how to replace it with an implant. For a lot of patients this is not only impractical but also contraindicated due to cultural, budget and medical reasons.

“I will be outlining the various options available to replace a missing tooth without implants. Rotational path of insert dentures, sectional bridges, cantilever and resin bonded bridges will all be discussed along with more traditional options.”

Michael hopes the course opens attendees’ eyes to a few more treatment planning options that they can use for complex patients, as well as helping to delay the need for implants in young patients. He says it is important for dentists to have other options for missing teeth aside from implants.

Patient, resilient, loyal.

If you weren’t a dentist, what would you be doing for a living? Anything involving my hands, taking things apart and putting them back together. I have told my wife if I had to stop doing clinical dentistry, I would become a lab tech.

“Dentists treat a whole mix of people from all ages and with a range of medical conditions,” he says. “Having only one treatment pathway for all these patients is illogical. The treatment options for a 14-year-old patient to replace congenitally missing laterals are not the same as for a 60-year-old patient missing their lower incisors with a large soft and hard tissue defect.”

Michael is developing his Non-Implant Replacement Options course into a cloud-based course so it can reach more people around the world, as he believes patients are missing out on great alternative treatment options.

What is your favourite book? Sapiens: A Brief History of Humankind by Yuval Noah Harare. I love books about the human condition and what makes us who we are.

What do you enjoy doing in your spare time?

Spending time with my wife and children, exploring new locations and eating. Lots of eating.

If there anything people might be surprised to learn about you? I bake bread every week, which I’ve done for the last four years. I will eat this with my breakfast, which is the same breakfast I’ve had every day for the last four years.

ABOUT THE PRESENTER

Although implants are one of many tools available to a dentist for the replacement of missing teeth, other fixed and removable alternatives shouldn’t be overlooked.

Dr Michael Frazis graduated from the University of Adelaide in 2012 and has worked in a mix of rural and metro practices. He enjoys all aspects of dentistry, with a particular interest in cosmetic work, complex surgery and nervous patients. He spent the first few years of his career focusing on patient communication and expanding his basic skill set. Dr Frazis is a frequent contributor to the online dental forum RIPE and is an educator and content creator for Ripe Global. He has been involved with teaching dentists of all ages in a variety of hands-on courses and lectures around Australia.

It is very easy to find a lecture or course on implants or how to save seemingly unrestorable teeth, but it can be difficult to find out what to do when the patient can’t have an implant but still wants to replace the missing tooth.

There are many techniques used to restore missing teeth that have been lost to time. As more and more dentists do their own implants, the old techniques used before implants were widespread have been lost in dusty textbooks.

Feeling thestrain

The COVID-19 pandemic has brought its fair share of stress to the dental profession over the last two years. Throughout the pandemic, clinical psychologist Alex Hof has presented webinars and workshops to ADAWA members, advising on coping with the stresses and strains of COVID-19.

As Western Australia’s rate of community infection continues to rise, Alex is again on hand to offer advice and support to dental practitioners in the State. She notes that dentists are under particular financial pressure, as dentistry does not lend itself easily to telehealth.

“One of the fundamental principles in psychology is that all of us find uncertainty challenging, even those of us that thrive on change,” Alex says. “The reality is, we all have our limits.

“When you have uncertainty, when you feel like you have very little control over key aspects of your life, then your level of psychological distress is likely to escalate, not because you are weak but because you are human.”

TAKING CONTROL In uncertain situations, Alex suggests accomplishing small tasks.

“If you’re good at playing the card game bridge, join an online group and play. If you’re good at making a particular meal, then make it.

“Try to think about little projects around the house that you can do and get a sense of achievement from getting things done. It might be weeding the rose garden, cleaning the pantry, painting the spare room.

Take pride in what you have achieved, lean back, have a cup of tea and think ‘I did that’. It really helps get back that sense of being in control.

“Every time you turn on the radio or open a newspaper there is doom and gloom over COVID and this can get to you,” she adds. “So being consciously aware of those things you can do and can control is important.”

GO BACK TO BASICS “Try to get a bit of exercise in, even if it is just walking the dog,” she says. “Get out into nature once a week.

Catch up with friends that make you laugh.”

DON’T JUDGE YOUR EMOTIONS Be aware that individual stress signs can be physical, behavioural, cognitive or emotional. “It’s important not to judge your emotions and think you should be coping better. Some people may look like calm swans – but beneath the water you have no idea how much their legs are thrashing around,” Alex says.

“It’s also important to recognise that a problem shared is a problem halved; it sounds simplistic, but it’s true.

Talking with a friend, finding those people who are good listeners and having a chat can make such a difference.”

IF YOU’RE CONCERNED ABOUT A COLLEAGUE It’s always best to say something rather than nothing.

“A useful strategy is to comment on the difference you’ve noticed in them,” she says. “Don't jump to a conclusion but say something like, ‘Are you OK? You've been a bit quiet all morning,’ or ‘Are you OK? It’s not like you to snap at the reception staff.’

“Reflect to them what you have observed. There can be all sorts of reasons for a change in a person's behaviour. If they say everything is OK but your gut feeling is that it’s not, then you can say, ‘OK but I'm here if you need anything,’ “And if, in the following days, the person's behaviour continues to concern you, you can ask again, but gently, and again tell them what you have observed,” she adds. “And then just be available.”

IMPACT ON THE DENTAL COMMUNITY Chair of ADAWA Infection Control Committee, Dr Peter Duke, says the professional and personal lives of dentists have been significantly reshaped during this rapidly changing and challenging time.

“Working conditions, financial pressures as well as altered interactions with patients and colleagues caused by the pandemic have directly affected the mental health of many dentists and their team members,” he says.

“Due to the chronic duration and widespread repercussions of the COVID-19 pandemic, COVID-19 stress and burnout has put additional pressure on everyone in the community, including the dental community.

“As dental professionals we recognise the importance of mental health and wellbeing for our patients. However, we may not always recognise or prioritise our own mental health needs.

“Research has shown that even in pre COVID-19 times, occupational stress was common amongst the dental profession and was a key driver of burnout and mental ill health. These problems can affect individuals at any stage of their career. There has traditionally been a reluctance amongst the dental profession to speak about personal problems, including mental ill health, and seek support. This can be driven by concerns about confidentiality, embarrassment, and the impact of health disclosures for professional registration.”

4. TRY NOT TO JUDGE YOUR EMOTIONS.

Executive director of the Samaritans, Zrinka Highfield, believes that dentists – just like other members of the community – should prioritise self-care, building resilience and seeking help when you are struggling.

There is no right or wrong in emotions – it’s ok to feel what you are feeling in this moment. Aim to move toward accepting the presence of your emotions.

1. BE KIND TO YOURSELF.

5. AVOID UNHEALTHY HABITS.

Practice self-compassion. In life there are things we can’t change but many things we can change. Beating yourself up over things, especially those you can’t change, will just make you feel worse. It is not your fault you are feeling this way.

2. BE MINDFUL. Being present in the moment can help you become aware and manage overwhelming thoughts and feelings.

3. DO SOMETHING YOU ENJOY. Have some “me” time. It can be as simple as listening to your favourite music, going for a walk or seeing a movie. We live in a world that seems to value long working hours and everything else that comes with it.

We hardly have time to enjoy life, connect with friends/ family and do things that bring us joy. Put aside some time every week to do something you enjoy. Rebalance the scales in favour of your own wellbeing.

Try not to rely on smoking, drinking and caffeine to help you cope. Men are more prone to this behaviour than women. It’s only a temporary crutch that can make you feel worse long-term and won’t solve your problem.

6. REMEMBER THAT YOU ARE NEVER ALONE. You don’t have to suffer in silence. Talking to a friend/ family member can help you feel listened to and supported. By acknowledging your feelings out loud can help. Maybe you would rather talk to someone anonymously and in that case, you can reach out to a helpline.

7. SEEK HELP. Sometimes you may need that extra bit of help and that is ok. Your GP can be a great source of help.

BOUND ANGLE POSE

What the pose does: Gently stretches the inner groins, hamstrings and the back spine.

What the pose does: The bound angle pose stretches the inner thighs, groins and knees.

How to do it: Lie down with your back and head resting on the mat. Tuck your chin in towards your chest. Bend your knees and have your feet flat on the mat, then lift your left leg and place the left ankle over your right knee. Then thread your hands through to hold on to your right knee or shin. Keep both feet lightly flexed to protect the knee. Repeat on the other side.

How to do it: Sitting on your bottom, make sure that both sit bones are evenly placed on the mat. Bring your feet in together in a diamond shape. Hands can either be placed gently over your knees or you can clasp your feet. Avoid this pose if you have a groin/knee injury.

CAT - COW POSE

ARM BIND

Cat pose: Stretches the back torso and neck.

What the pose does: Stretches shoulder joints.

How to do cat pose: On the exhale, tuck the tail bone, lowering the head and chin towards the chest, navel draws up towards the spine.

How to do it: Sit cross-legged on your bottom. Reaching your left hand up to the sky, bend your elbow and place your left palm on the centre of your back. Then internally rotate your right arm so the back of your palm is against your back, reaching your fingertips if possible.

Repeat on the other side.

Cow pose: Stretches the front torso and neck.

How to do cow pose: Place your hands shoulder width and knees hip width apart firmly on the mat. On the inhale, lower your belly towards the mat while lifting your eye gaze and tail bone up.

Modification: Either sit on a block or bolster or place them under the outer thighs for support.

Modification: Hold on to a strap instead of your hands.

With a neck injury, keep the head in line with the torso.

SEATED POSE

What the pose does: The puppy pose is a chest opener and releases tension in the back.

What the pose does: Lifts the spine and opens the chest. An opportunity to reconnect with breath.

How to do it: Kneeling on the mat, with hips over your knees, place your chest down onto the mat with your hands reaching forward.

How to do it: Sit in a comfortable seated position, placing your hands face up or down on your knees.

Modification: Sit on a bolster and widen the knees.

Modification: Place a block or bolster under your chest or forehead.

Eventually, a successful practice will reach the point where it’s time to expand and grow. That may involve moving to bigger or more upmarket premises so you can see more patients, offer additional services, raise your fees, attract a different clientele or hire more staff.

Financing the growth of your practice is a common stumbling block. One strategy you may have heard about to achieve this is borrowing through your SMSF. This may be a tax-effective way to purchase your own clinic or rooms, or invest in your practice.

BUYING PROPERTY WITH YOUR SMSF

THE CONSIDERATIONS

If you operate from a physical location, it may make sense to buy your practice through your SMSF and lease the property back to your business - especially, if you plan to stay in those premises for many years to come.

SMSFs take some effort to set up and manage. You’ll be responsible for complying with super and tax laws and managing any investments you make. You need to factor in whether you have the time and skills to do this alongside your other commitments. Alternatively, you may need someone to help you with this.

Owning your own premises is an investment in yourself and your business. It also gives you much greater control over an important business asset and offers protection. Once you own the property, you can’t be forced out by a landlord who decides to sell, or increases the rent to a level you can’t afford.

Given many dentists stay in the same premises for a long time, buying the property via your SMSF could be a savvy long-term investment. The rental payments you make on the property go back into your SMSF. If these match the loan repayments, you’re essentially creating a savings plan and hopefully a good level of capital growth in the years to come.

THE BENEFITS Investing in commercial property with your SMSF may have significant business advantages, such as: • Potential tax advantages: Any rental income and capital gain on the sale of the property will only be taxed at the maximum tax rate of 15 per cent • It may be easier to manage: Property may be simpler to manage than other more volatile investments • It may free up other capital: Using your SMSF to buy your rooms may leave you with other practice capital to invest in your business • It may reduce your personal financial risk: Superannuation assets may be secure from creditors if you run into financial trouble, but expert advice on this is essential.

Determining if commercial property ownership makes sense in relation to your business goals is key, as is assessing your purchasing and borrowing capacity. It’s important to factor in all the additional costs that apply when buying property such as valuations, conveyancing fees and stamp duty.

There may also be the risk of putting all your SMSF funds into commercial property instead of diversifying with other investments. It might be worth looking into other ways to use your SMSF as well – such as investing in your company or business (known as inhouse assets) or investing in a managed fund.

IN CONCLUSION Purchasing commercial property or other assets using your SMSF requires careful consideration, planning and expert advice. There can be significant upsides too, however you’ll want to consider your own personal circumstances and financial position to determine if the benefits outweigh the risks for you. Using your super to finance business assets or grow your practice may make sense now, but you want to ensure that in doing so, it will also leave you in a comfortable position come retirement.

Why interest rates aren’t everything!

Remember the other crucial components of your loan.

In the rapidly changing interest rate environment that we are currently living in, we encourage you to dig a little bit deeper into what you really want from your loan and what is most important to you over a longer term.

Some of the most common things we hear when we speak to our clients about their loans are, “What’s the interest rate?” and, “I’m just chasing the lowest rate.”

At the moment, interest rates are changing from one day to the next, and it doesn’t look like that’s going to be slowing down any time soon! So, if you are just after the lowest interest rate, it’s likely that the lender you choose now may not be the cheapest in a few months’ time.

Sure, the rate might look cheap, but if you’re paying an annual or quarterly fee, then what is the real cost to you?

For example, if you know you are planning to sell your property within the next year, opting for a 2-year fixed interest rate could be costly to you, even if the rate on offer is the lowest option at the time, due to potential break costs and early repayment fees.

THE SERVICE OFFERED BY A LENDER There’s nothing worse than sitting on hold to a call centre for hours to ask a question about your loan. Or waiting for days on end to receive an updated loan statement. So maybe having your own dedicated relationship manager to look after all of this for you could be worth its weight in gold!

THE STRUCTURE OF YOUR LOAN Just because the interest looks cheap, doesn’t mean it’s necessarily the most cost-effective option. As easy as one-two-three: reflecting on provider numbers

The announcement that dental hygienists, dental therapists and oral health therapists would be able to apply for a provider number and claim directly for services provided under the Child Dental Benefits Schedule (CDBS) seems like it happened a long time ago, but is now just around the corner, with a planned commencement date of 1 July 2022.

Many of us interface with dental hygienists, dental therapists and oral health therapists in our daily practice, whether as practice owners or associates, and e. Even if we don’t directly, it’sis important to have an understanding of this change,and this is. This also presents a good opportunity to reflect on use of provider numbers in general terms.

Provider numbers are, in essence, a site-specific fee-claiming mechanism. In relation to the upcoming changes, the CDBS services that can be claimed will be dependent on what registration is held by the individual hygienist, dental therapist and oral health therapists with the Australian Health Practitioner Regulation Agency.

From 20 June 2022, hygienists, dental therapists and oral health therapists will be able to apply to Services Australia for a provider number. As the commencement date approaches, further information will be provided to the professions on the changes and the Services Australia process to apply for a provider number.

Information on how to apply for a Medicare provider number is available in resources on the Services Australia website1. There are also training resources on compliance and auditing requirements under the Child Dental Benefits Schedule.

It is important to note that applying for an individual provider number is optional. If the hygienist/dental therapist/oral health therapist is currently billing under a dentist’s provider number for the CDBS, they can continue with these arrangements after 1 July 2022.

The Department of Veterans Affairs has informed that there is no immediate plan to provide dental hygienists, dental therapists and oral health therapists direct access to the DVA Dental Program via the new provider numbers.

On the face of it, something so relatively simple should not be contentious, and yet we see an alarming number of cases relating to the inappropriate use of a provider number. . With the advent of this change we wanted to highlight some of the risks relating to use/misuse of provider numbers in Dental dental practisece, in the hope that we can raise awareness of the potential issues involved.

CASE STUDY Dr W was a recent graduate and looking for a permanent practice role. He had given a lot of consideration about the type of practice he wanted to work in and was seeking a group practice with plenty of other practitioners as he was concerned about professional isolation. Dr W was keen to be surrounded by likeminded colleagues to enable him to develop and grow. The practice did not have a principal dentist, but rather a ‘flat hierarchy’ of practitioners and a non-dentist practice owner.

On assessment, Dr W formed the view that the contract seemed fair, and the practice seemed reasonably well equipped. The other dentists were all welcoming and said that they had a good flow of patients, particularly new patient examinations, so made a good income.

He took the job. For the first few weeks, he kept a tight eye on his billings, and indeed, he had a steady flow of patients and a good income. With the passage of time, Dr W became more relaxed at the practice, as he was happy there, and had formed good relationships with both colleagues and patients alike.

He was surprised to receive a letter from the Health Fund, stating that he was an outlier and requesting validation for the number of codes charged per patient, and the number of five surface fillings. The five-surface

filling comment came as a surprise to Dr W who could not recall having placed many five surface fillings.

The practice owner reassured Dr W that they would manage this on his behalf, stating it was likely that the front office staff had entered the codes on the wrong provider number – a common administrative mistake, and that they would look into this and respond. Dr W did not seek advice from Dental Protection as he believed the matter to be ‘routine’ and in hand.

The second Health Fund letter followed quickly, with similar allegations to the first. Dr W again contacted the practice owner to alert them to the issue and seek some assistance. However, this time the practice owner did not respond in a helpful way, and Dr W felt quite threatened by their reaction. At this point, Dr W contacted Dental Protection, who advised him to look a little deeper and see what patients’ invoices cited. To Dr W’s horror, the HICAPS transactions did not reflect his clinical notes, with two surface fillings being invoiced as five surface fillings, patients being charged for teeth adjustments that Dr W had not undertaken, and most alarming, a recent crown not being visible in the invoice stream, having been replaced with an invoice for eight x five surface fillings instead.

When Dr W challenged the front office staff, they were nonplussed, calling him naïve, and advising him that the only way to treat patients fairly was to maximise their Health Fund rebates. They confirmed that they were working under the direction of the practice owner, and that their ‘fair billing’ policy attracted many patients.

Dr W was alarmed by these statements, and even more so when he realised that because the billings had gone through on his provider number, he was solely responsible for the repayment of all monies inappropriately claimed, back to the Health Fund.

These billings we in the tens of thousands of dollars.

Dental Protection assisted Dr W in responding to the Health Fund and Dr W subsequently moved on from the practice.

LEARNING POINTS This case is sadly not unique, with many practitioners finding codes are put through on their provider number without their knowledge or altered without their knowledge.

As the ‘owner of the provider number’ you are solely responsible for the codes billed. This means that regardless of the percentage you may have been paid for any treatment, if the money is clawed back by the third party who made the payment, 100% is clawed back from you, and you alone.

You are responsible for the accuracy and validity of all codes put through under your provider number. Make sure you regularly check your HICAPs receipts to ensure that the coding is correct.

For CDBS claims you can also check through PRODA. Take the time to close off your provider number when you leave a practice to ensure that no inappropriate billing is put through on your provider number after you have left.

Provider numbers can be suspended or withdrawn if there is a continued pattern of inappropriate billing.

Vale Lawrie Bartlett 1938 - 2022

Lawrie was educated at Scarborough Primary School, then Perth Modern School, and was awarded a State Bursary to study dentistry. He graduated from the Faculty of Dental Science with a BDSc in 1960.

On graduation, he was employed by the Perth Dental Hospital, before marrying Gay and taking a school dental caravan to Port Hedland to practise in the town and outlying areas.

In 1965 Lawrie, Gay and their two children returned to Perth, where he did locum work for a short time, prior to building his own practice in Wembley Downs, where he worked until his retirement in 1998.

Lawrie was especially interested in children’s dentistry, establishing a thriving practice in Bournemouth Crescent, and was active in the then Children’s Dentistry Society. He was also invited to lecture to the student dental nurses.

Lawrie was a great sportsman and was invited to join East Perth Football club, but instead played University football. He was vice captain of the Green Team and earned a trip to Hobart. He was awarded a Full Blue for his efforts in 1958.

Later he played with The Wembley Amateurs and enjoyed coaching the junior teams there.

Whilst at university, a group of us played social squash and continued after graduation till age caught up and golf became our sport.

Lawrie played regularly as a member of Cottesloe Golf Club from 1987 to 2015 and also at Wembley with the Wednesday Whackers from 2010 to 2019.

Fishing was a life-long passion with Lawrie, and we spent many happy times fishing at Rottnest, The Abrolhos, Dirk Hartog Island and The Mackerel Isles.

In the latter years we fished, dived for abalone and crabbed locally on a regular basis. Lawrie’s great skill in fishing meant that we always went home with a good catch of quality fish.

Lawrie’s passing leaves behind many friends as well as his partner Gay, children Vernon, Yasmin and Nicole, and four devoted grandchildren.

Key Things for Practice Owners to Keep in Mind In running a practice, practice owners must consider more than just the day-to-day operations. Looking at overarching issues such as the culture of the practice, individual management styles, and the practice’s future are also important. Having a good understanding of these can help your practice to be more successful as you develop strategies to deal with issues and move towards where you want the practice to go.

PRACTICE CULTURE Ensuring the practice has a positive work culture is a key consideration for practice owners.

Practices are obligated to provide a safe working environment for all workers; however, many forget that this obligation extends to the psychosocial environment. Workplaces that have issues including toxic working relationships, unreasonable expectations, and the inability to raise grievances can give rise to mental health hazards and claims of bullying and harassment.

While having clear policies for raising grievances, codes of conduct and behaviours expectations are the more formal and essential ways of targeting these, incentives can also be key in creating good practice culture. Workplaces that provide additional flexibility to staff, encourage, and foster good relationships through teambuilding exercises, and recognise staff for their achievement will find it easier to retain good staff and talent.

MANAGEMENT STYLE An essential part of being a practice owner is undertaking management action in the workplace.

Regular assessment of management style and its efficacy and perception in the practice is important for practice owners and managers. Whilst you may be the kind of leader who likes to make decisions by themselves, you then may need to consider how you will consider staff input so that it is clear that their concerns are still being heard. When undertaking management action, practice owners may consider whether they prefer to take informal steps to address conduct and performance issues before undertaking a formal process? Crafting clear policies and procedures, as well an environment that is supported by your management style is important in having a successful practice.

Importantly, any management action must be reasonable in the circumstances. Workplace bullying occurs when an individual or group behaves unreasonably towards a worker and that behaviour creates a risk to health and safety. Reasonable management action conducted reasonably is not workplace bullying. If a practice owner manages employees in an unreasonable manner, they are at risk of a workplace bullying claim. For example, unreasonable management action may occur in performance or disciplinary processes where no procedural fairness is afforded and where the allegations are baseless. Practices should ensure that they engage in regular training with management staff as to how to appropriately manage employees.

GOALS FOR THE PRACTICE Another consideration is what the path going ahead for the practice may look like. If you are looking to expand, you may consider whether to provide additional training and development for your employees to provide them with additional skills and the ability to take on further responsibility. It may also be that you are looking to hire new staff. This requires a consideration of your current recruitment strategies and what you are looking for in a candidate.

If the practice has identified the need to build a more positive working culture, practice owners can take actions such as creating clearer policies around workplace expectations, conducting surveys, and holding meetings to get staff feedback in order to determine what steps are necessary to make the practice the best place it can be for its workers.

WADA GOLF

Hartfield Country Club A COVID-depleted field met at Hartfield Country Club in Forrestfield for the third fixture of the WADA Golf season. This event was sponsored by Insight Dental Ceramics, and we were pleased to be joined by Insight owners and ceramists Andres Vivanco and Angus Wilshaw for the day.

The early onset of the winter rains provided challenging conditions for the 17 players who braved the damp and squally conditions. Scoring reflected the tough conditions, with no one able to play to their handicap.

Once again, the winner was determined by a countback, with Ian McCarrey again missing out to eventual winner Richard Williams, both scoring 32 stableford points. Stu Phillips was a close third with 31 stableford points.

The winner’s prize was presented by Andres and Angus, who also took the opportunity to address the group on the comprehensive laboratory services provided by Insight as a locally owned and operated business. The accompanying photograph shows Andres and Angus presenting Richard Williams with the winner’s prize. This was the first time Insight Dental Ceramics have sponsored a WADA Golf event and our thanks are extended to Andres and Angus for their generous support.
