Dear Members, As our branch renewal cycle ends, I would like to thank all members for the support they have shown us by once again choosing to be members of ADAWA. Our branch staff have worked tirelessly over the last two months to make this possible and I will take this opportunity to thank them on behalf of all members for their efforts and the hundreds of support calls they have answered during renewal and throughout the year.

For many members, the advocacy ADAWA provided during COVID lockdowns would have provided an insight into the work that has always occurred on your behalf – which, most recently, has included dealing with issues of vertical integration by private health insurers, and patientmatched medical devices through the TGA regulatory changes. Your individual support, combined with our association, makes this all possible.

This month's magazine includes the first of a three-part feature on cybersecurity, and I cannot stress the importance of taking this seriously in your practice. I’m sure everyone has heard the stories of ransomware infecting a practice, but most will not understand the true impact. Aside from loss incurred by having no computers while you attempt to restore data, all too often we find inadequate backups, and practices are forced to use data from some weeks or months previously. Putting clinical data aside for a moment, that’s a loss of all financial billing records, appointments that occurred and most importantly, the vast majority of upcoming appointments. As previously made appointments can now arrive at any time, practices are stuck in limbo when booking new appointments and often suffer this for many months. This is just one part of the problem and I urge all members to take the time to read this month’s feature.

COVID vaccination rates have made big news in recent weeks and, given our environment, it makes sense to ensure you and anyone in your practice team is vaccinated. Internationally we are now seeing moves towards vaccination requirements to work and it is not impossible to imagine a scenario where this is applied to dentistry in Australia.

A pioneering career:

Dr Peter Gregory Western Australia’s first paediatric dentist WA’s first paediatric dentist recently celebrated 50 years in the dental profession.

He shares his story with Brooke Evans-Butler, reminiscing on a career well spent.

Dr Peter Gregory could not have imagined becoming WA’s first registered specialist paediatric dentist when he graduated from dental school, but after ten years of general practice, he was ready for a change. “After that many years in the same room, I felt I needed new challenges,” he says. “I was still only in my early 30s and I could not see myself doing the same thing for another 30 years.”

Luckily for the children (and parents) of Western Australia, Peter took the plunge when he was offered to study Paediatric Dentistry – completing his MDSc (WA) in Paediatric Dentistry in 1980.

“I was trained here and even though there were no paediatric dentists in WA, I was very fortunate in that I had the support of all of the consultants in the other specialities who were visiting dental specialists at the children’s hospital,” Peter says.

The consultants who helped Peter were something of a dental dream team, comprising prosthodontist Patrick Henry, oral surgeon Ted Adler, orthodontists Bill Brogan and Peter Dillon and periodontists Rob Bower and Albert Tan. “I would spend time with them, in their clinics and in the theatre, and they would also guide me with respect to literature reviews,” he recalls. “They were mentors and I ended up with exceptional training. Further to that, Professor Des Kailis, who was my supervisor, brought in an internationally renowned paediatric dentist from London, Professor Gerry Winter, to

examine me. He did this to ensure the course I did was relevant and appropriately registerable and to also establish the fact that there was a paediatric dentist in Western Australia who had obtained the credibility of an international paediatric dentist.”

At the end of his training, Peter sought international experience, so Princess Margaret Hospital and the Perth Dental Hospital assisted him financially to spend a year at The Northwestern University Dental School and Children’s Memorial Hospital in Chicago, USA, which Patrick Henry was instrumental in setting up, due to his many international connections.

“It was a tertiary hospital and everything within the state of Illinois that was rare would be referred there,” Peter recalls. “I saw some cases there that I would never see anywhere else. The experience was immeasurable.”

The decision to study paediatric dentistry is one that Peter has never regretted. “I always had an affinity to treating children, even when I was in general practice,” he says. “I felt too that I could relate to them. Once, during my period of specialist practice, I was at a familytype function, the evening before a wedding, and a five-year-old tripped and smashed his teeth. The father and I went with the child to my surgery, and I was able to manage, just with the father helping me, to give him some sedation and remove the bits and pieces of teeth. The next day at the wedding the little child came up to me at the church and grabbed my hand.

“The people around me were gobsmacked by the action of this little child, who was traumatised the night before with injections under the nose and the palate. For him to leave his parents, walk over and hold my hand amazed me. So, when I am asked why I decided on paediatrics – I like working with children and I think they like me.”

I can always remember the Christmases when a child got a new bike and an hour later, he had smashed his teeth. I would think, ‘I missed lunch, maybe I’ll get home in time for dinner’ and then another child would do the same thing in the afternoon.

As well as establishing his successful practice, Peter worked for a number of years as the Director of the Dental Department at Princess Margaret Hospital. “Being on the staff of a children’s hospital, we had a lot of on-call work,” he recalls. “I can always remember the Christmases when a child got a new bike and an hour later, he had smashed his teeth.

I would think ‘I missed lunch, maybe I’ll get home in time for dinner’ and then another child would do the same thing in the afternoon. I would take most Christmases at the children’s hospital because I felt other members of the

dental team needed to be with their families, so it was something I did as head of department.”

Having a good rapport with the parents as well as the child patient was also something that was incredibly important as a paediatric dentist.

“We call it the paediatric dental triangle,” he explains.

“There’s the parent, the child, and the paediatric dentist – and the interaction between all of those is complex.

They all have to be in sync, because if any of these three are not in sync, then the whole thing doesn’t work.”

I think the most significant thing was being able to have children come in who were very anxious and being able to treat them, taking them from a negative to a positive attitude towards dentistry.

Despite the challenges, Peter always found the work very rewarding. “I think of all the specialities, paediatric dentistry can be the most rewarding because it’s not just a technical thing,” he explains. “Plus, being associated with a children’s hospital, you see all the dental trauma and emergencies that present, which are usually infection, post-operative haemorrhage and dental trauma. Then, of course, you have the gamut of sick children, and those with developmental problems such as cleft lip and palate. It is such a specialised area, and it gets you out of the 40 hours a week in the one room.”

In addition to the rewarding and always varied work, being able to help a child overcome fear was something Peter most enjoyed.

Over 50 years, Peter has seen many changes – one of the most significant being the ADA’s advocacy for dentists to write prescriptions. “You can imagine how many prescriptions are now written by a dentist,” he says. “Every single one of those had to go to a medical practitioner previously. ADA really pushed for that and it would not have happened if it were not for the ADA.” As well as his work at Princess Margaret Hospital and at his practice (where he saw over 20,000 patients over the years), Peter was also dedicated to teaching at the Dental School at UWA. He also gave his time to various boards and affiliated societies, serving as President of the Australian and New Zealand Society of Paediatric Dentistry, President of the Australasian Academy of Paediatric Dentistry, Examiner and Board Member for the Royal Australasian College of Dental Surgeons and Chairman of the Finance Committee of the International Association of Paediatric Dentistry at different times during his career.

He retired from clinical practice ten years ago.

After more than 50 years in the profession, he’s slowing down, enjoying life and doing things that grandparents do.

“We have a son who lives in Melbourne with two little children, and two daughters and two grandchildren here in Perth. I help my daughter with her two children, and I read, I exercise, and I use the computer. I don’t play golf, but I have lost a lot of weight and I feel very fit. We are a very family-orientated Greek family, so we have our Greek traditions, and I am really enjoying having the extra time.

“I must admit, I didn’t realise just how stressful dentistry was at the time,” he adds. “I was in a stressful area where people are bringing their most prized possession, their child, to you, and often you have to tell them that their child needs a general anaesthetic.”

Overall, he says it has been a great journey. “I guess being the first paediatric dentist – and I will always be the first paediatric dentist in WA – I feel very privileged to have been part of the dental profession, a member of the paediatric dental community and very grateful for the dental education I received at the undergraduate and postgraduate levels from the UWA Dental School.”

Is your practice cybersafe?

There’s no question that Australian businesses are under threat online. Worryingly, health providers are more vulnerable than most to cybersecurity breaches.

According to Accenture, security breaches have increased by 11% since 2018 and 67% since 2014.

Late last year, the Government showed us how seriously it takes cybercrime by launching an official Australian Cyber Security Centre.

The national cybersecurity campaign urges Australians to strengthen their cyber defences and be alert to online threats.

Minister for Defence, Senator Linda Reynolds, launched the ACSC campaign in December 2020, saying: “Cybercriminals are relentless, operating around the clock and around the world, in a bid to steal the data and money from Australian businesses and families.

“Australians are reporting more than one cybercrime every 10 minutes, making it more important than ever that we all remain alert to the threat of cybercrime.”

Dr David Glance, Director of the UWA Centre for Software and Security Practice, says healthcare providers have been subject to increased attacks from cyber criminals.

“However, heath providers are being especially targeted by ransomware attacks because these are designed to make a business inoperable by encrypting all of the information on computers in the practice until a ransom payment is made,” he explains. “Because practices have become increasingly reliant on electronic patient records, it is impossible to treat patients without access to those records.

“Worse still, attackers have been not only encrypting information to make it inaccessible but have been stealing data and threatening to publish it if the ransom is not paid,” he adds. “This is especially concerning when the information is personal information belonging to patients.”

Most valuable to hackers is information that, if released publicly, would cause harm to the business or to its customers or patients to a hacker.

“The average cost of a data breach in healthcare is the highest of any industry, at over $7 million per breach,” says Adam Gordon, ANZ Country Manager at Varonis. “This is due to the highly sensitive nature of the data healthcare organisations collect, such as confidential patient records in the case of dentists and hospitals.

“Attackers know healthcare providers hold large amounts of sensitive patient data, and that they also can’t do their jobs without access to this data. Attackers are using highly targeted campaigns where they steal data, encrypt it, and extort the victim organisation – pay up, or the attackers publish sensitive information,” Adam adds. “Ransomware is also offered as a service – meaning less experienced hackers buy a toolkit and launch an attack that, if not caught, could unleash ransomware across a victim organisation.”

FEATURE ADAWA recently partnered with financial protection company eftsure, as we believe our members will benefit from their services. eftsure’s solution protects business-tobusiness payments by matching beneficiary names with their account number and BSB.

Common mistakes When asked about the common mistakes businesses and healthcare providers make when it comes to keeping sensitive data safe, David says lack of cybersecurity awareness is a problem, as well as implementing safe practices such as keeping systems updated, anti-virus software running, and maintaining regular backups of information.

“Most attacks come via phishing emails – fake emails that try and trick the reader into clicking and executing a malicious program – and so being absolutely scrupulous when interacting with emails is vital,” he adds.

According to Adam, having sensitive files accessible by everyone in the organisation (or more people than required), is another common mistake. “Healthcare organisations need to employ a policy of least privilege, which only gives employees access to the files which are necessary to do their jobs,” he explains. “This will limit the damage hackers can cause if an employee’s account is compromised. It also protects against insider threats, preventing employees from sharing or deleting confidential company data.”

Other mistakes to be aware of include poor password hygiene: “This includes having passwords that are weak and easy to crack, passwords that rarely or never expire, and using the same password on multiple accounts,” he says – as well as leaving ‘ghost users’ enabled. “These are user and service accounts that are inactive but still enabled — which can give hackers an easy way to move through an organisations’ file structures undetected,” he explains. “Hackers often exploit this weakness to steal data or disrupt critical systems.”

We were persuaded of eftsure’s credentials by a case study of a construction and engineering firm that used eftsure’s Know Your Payee solution to prevent a payment of over $1m to a fraudster.

At the beginning of February 2020, the company had been advised of a change of account details for one of their suppliers – a professional company involved in providing services to the construction industry.

The requested change of banking details arrived in an email from the legitimate account of their primary contact at the supplier.

In keeping with the eftsure process, the customer initiated a change request from the eftsure portal requesting the supplier provide their updated details via eftsure. Since the supplier’s email was under the control of the fraudster, the fraudster intercepted the email and completed the onboarding.

This triggered eftsure alerts inside the verification system because the IP address of the fraudster didn’t match the IP address region of the supplier.

As per eftsure’s process, eftsure independently sourced the phone number of the supplier and called the supplier to verify the details, subsequently discovering that fraudsters had been monitoring communication in that compromised email account and using it to attempt to defraud the supplier’s customers.

Once this fraud was exposed, the supplier closed the email account completely and contacted all their other customers to warn them not to accept any changed details.

A respected clinician In this edition of the Western Articulator, we acknowledge the ongoing contributions of oral and maxillofacial surgeon and oral pathologist and CHAT co-founder, Dr David Booth.

“I was unable to study medicine in WA in 1952,” David recalls, when asked what initially drew him to study dentistry. “But if you got a dental degree, you could add a medical degree at an eastern states’ university in three years and work as a dentist to earn money to live on at the same time.”

David went on to become the first oral surgery post-graduate student, before doing a further six months’ study in London and Washington DC to become an OMF pathologist and surgeon.

As well as a well-respected practitioner, David also gave much to the profession in a teaching capacity as a Clinical Associate Professor at the School of Dentistry. When he retired from full-time work, he continued part-time to allow him entry overseas into part-time volunteer practice.

It has been volunteering where David has made another huge impact. It was during a visit to Vietnam that David became aware that basic medical and dental care was not available to many of the Vietnamese people. In 2005, David, along with Dr Gabby Ly, founded CHAT (Christian Health Aid Team, later Children’s Health Aid Team), with the vision of providing dental services to disadvantaged children in Vietnam.

“Dr David Booth is one of the founders of CHAT and he has played an integral role in establishing our organisation since 2005,” says Dr Jennifer Chan, CHAT President. “His vision for providing dental services to the disadvantaged and less-fortunate children has made it possible to help tens of thousands of children in Vietnam. Since 2007, Dr Booth facilitated a program with UWA School of Dentistry to include final-year dental students to have placements on our trips. He also established the David Booth Travel Grant to enable a dental student in financial hardship the opportunity to participate in an overseas placement with us. We are extremely grateful for all the contributions Dr David Booth has made and continues to make within the CHAT community.”

The organisation’s volunteering trips are hugely popular with dental practitioners and dental students – giving participants an amazing opportunity, that also gives back so much to the Vietnamese people.

When asked why volunteering opportunities in dentistry are so important, David says they allow you to set up valuable post-grad experiences before you have obtained the requisite qualification to register with the Dental Board as a specialist.

“If you do the volunteering overseas, say in Vietnam, you contribute to that country’s specialist needs without setting up a full-time specialist practice there,” he adds. “When you go you can take dentist colleagues and dental students with you to give them appropriate experiences.”

An ADAWA member for over 50 years, David has seen many changes, both for the good and the bad of the profession.

“High-speed dentistry came into use while I was a dental student,” he recalls. “But the Dental School did not introduce it into the teaching program immediately because they were concerned that dental students

He adds when he first graduated, they had to use chemical antiseptics and boiling water to sterilise their instruments. “Eventually, small steam sterilisers were introduced into dentistry, then sterilisation in the dental clinic became more reliable.

“All these changes were expensive to buy so setting up a dental practice has become very expensive,” he adds.

“You usually want to arrange a mortgage to buy a house as well at the same time. So many young dentists have to work for someone else or for the government until they have saved up enough money for a mortgage to set up a practice or to buy a partnership.”

As well as the changes and the challenges, there have been many rewards. “You are working to make someone’s smile better or their dentition more reliable to chew on, so the results of your work are always an improvement,” he says. “The good salaries allow you to pay off a mortgage and buy a house, get married, pay children’s school fees as well as live comfortably – if simply – at first. With the increase in dental practices and government dentistry, this gives more opportunities for paid experience as soon as one graduates.”

To recent graduates entering the profession, David advises talking to experienced dentists and ADAWA to progress in dentistry financially.

“The ADA looks after its members well and guards against government rules and financial constraints interfering with its members’ dental practice,” he adds.

My name is Jasmine Li and I was the recipient of the David Booth Travel Grant 2019. I travelled with CHAT to Ho Chi Minh City in November 2019.

I was supporting myself and working two jobs while taking on a full study load at UWA Dental School. Without the travel grant, a Vietnam overseas placement would have been out of reach for me.

Your generosity allowed me to undertake this amazing opportunity. It was truly an eyeopening experience to see how dentistry was practised away from home.

Since graduation, I have developed a special interest in paediatric dentistry, and I believe this is influenced by all the amazing work by CHAT and your donation.

I have been itching to get back to Vietnam.

Unfortunately, I will have to wait a while before I can give back to this amazing cause.

How they compete with the big guys Shahmeer says WA Energy has a unique market offering. “Our competitors are very rigid and operate in one space without deep understanding of how to combine all the faculties,” he says.

“Traditionally, if customers want to save money on their power bills, they try to find another energy retailer or find a solar company.

“However, energy pricing tends to get higher every year and without knowing the ins and outs, the customer doesn’t get cheaper electricity and they get put on difficult contracts with penalty clauses. Energy retailers are heavily compliant focused.

“If they try to install solar, there are countless solar installers but very few genuine solar retailers out there. In the commercial space, it’s even more difficult as energy retailers normally charge a penalty for installing solar. Most solar companies have no experience on the regulatory requirements and customers end up paying more over the long run. Additionally, the solar companies aren’t around long enough to offer the warranties due to the stop and start nature of the game. It’s too volatile and customers get disheartened.”

CASE STUDY Big Brews Liquor

As successful liquor store operators, Big Brews recognised the need to reduce energy consumption to curtail high energy costs. Flavia and Bruno sought advice from WA Energy after receiving recommendations from the LSAWA. Flavia and Bruno’s problem was not only that they had high energy prices and large consumption, they were not happy with the service or solution from the multiple solar companies that had approached them.

Upon request, WA Energy sat down and detailed a plan for Big Brews. It included renegotiating new energy contracts without penalty clauses and lower tariffs. Whilst also including 40kW of Smart Solar with Smart Metering, which gave them the capacity to monitor their loads at any given period.

WA Energy continues to maintain a relationship with Big Brews as the ongoing energy management service means their investment is maintained and advice is given for many years to come.

Having been in both the electricity and solar space, WA Energy is able to offer versatile solutions tailored to the customer. “We analyse energy load data and work out the best solution for a particular business,” he explains. “We look at funding and offer ways to save money without any capital being spent for the customer. As we work on behalf of the customer rather than against them there is the trust and long-term approach to all our solutions. We approach every customer with transparency, honesty, and integrity. We care – and ultimately that works with our unique total energy solution for a business.”

According to Shahmeer, WA Energy can save businesses generally from 40-80% on their total energy, and 5-10% discount on energy rates.

Customer service Sheba Tedla, WA Energy operations manager, says some customers can be apprehensive of purchasing solar products because there are so many options available, but she stresses that at WA Energy, their focus in on helping their customers make the best decision, to get the best-quality product at the best price.

“We are WA owned, so our customers will know each member of our team by name,” she says. “We might be a small team, but our ambition is big – we are a premium brand, offering premium products.

According to Rosemarie Purser, WA Energy customer liaison officer, WA Energy’s point of difference is their focus on customer service.

“All the staff here answer their phones after hours,” she says. “We understand that professionals such as dentists work long hours, so we are always willing to take after-hours calls.

“We want our customers to be happy with our product, not only now, but in the future.”

CASE STUDY High Wycombe Shopping Centre High Wycombe Shopping Centre is a large local shopping centre consisting of Supa IGA, Anytime Fitness and Caring Pharmacy as some of its key retailers.

Originally, Anytime Fitness and Supa IGA agreed to install 100kW and 15kW above the rooftop of their tenancies. Other tenancies joined, taking the shopping centre to a proposed 154kW. For the centre, the Solar PV System will be a much-needed boost to tighten bottom lines by reducing energy costs from 30-60% for the tenancies. The tenancies are all leasing their premises so if they do move or sell the business, the panels can be removed or on sold to the next tenant. The tenants all leased their solar systems to take advantage of the tax and cashflow benefits.

Meet

Dr Josh Graieg

Dr Josh Graieg has become a familiar presenter on the WA Dental CPD circuit this year, with his winning combination of general practice experience and specialist expertise.

Josh with his family

Josh Graieg has had a varied career – working in a mix of private practice, government clinics, university and hospital-based dental clinics since graduating.

When asked why he originally decided to pursue dentistry, Josh says it’s because he enjoys the mix of both art and science. He worked as a general dentist for 15 years before completing his specialist training, a Doctorate in Clinical Dentistry (Prosthodontics) at the University of Sydney.

“After working as a dentist for about seven years, I then decided to get involved with the Royal Australasia College of Dental Surgeons and completed the primary course and examination, and then went on to complete the Fellowship,” Josh recalls. “This involvement with the RACDS really renewed a passion to learn more and improve my dentistry. This led to undertaking as many CPD courses as I could, both locally and internationally, resulting in me wanting to focus on dental reconstruction and oral rehabilitation.

“Having a good grounding in general dental practice has given me a more holistic understanding of dentistry; this is particularly important in prosthodontics, which is largely involved with multidisciplinary patient care.”

Josh will be presenting the Direct Veneers CPD course on September 4. “The veneer course aims to give participants understanding of how to select cases for direct composite resin veneers, contrast some different techniques (like natural layering and injectable composites), and ultimately give practitioners some protocols to plan, place and finish the restorations,” Josh explains.

Josh will also be presenting the Crown Preparation – From Conventional to Bonded course on October 14.

This course will help dental practitioners improve their clinical knowledge on when to perform a traditional crown preparation and when to use a bonded approach. The course will cover: • Ideal conventional crown preparation – reduction, taper, preparation, retentive features and margin design relating to restorative material.

• Bonded crown preparation – introduction to biomimetics and adhesive protocols, preparation design IDS and DME. • Conventional vs bonded - when to prep and when to bond.

• Material selection – monolithic or layered? Highperformance polymers, pressed glass, zirconia, PFM, gold.

Having a good grounding in general dental practice has given me a more holistic understanding of dentistry.

I have three young kids, and I love spending time with the family. Getting outdoors and having fun with the kids… kicking balls, riding bikes, exploring the bush.

When I get a chance, I enjoy playing sport, especially basketball.

Josh has also been involved in ADAWA’s New Practitioner Program, which supports recent graduates.

I like a nice beer; my favourites styles are Saison’s (which are Belgian-style farmhouse ales), or Wild Ales (which are ales brewed with wild yeasts like Brettanomyces).

“I really enjoyed my involvement with the NPP; I love the passion and enthusiasm that many young dentists have,” he says.

As well as working at the Branemark Center, Josh is a prosthodontic consultant with the Craniofacial Unit at Perth Children’s Hospital. “Working at PCH is basically my dream job,” Josh says. “It is a real pleasure to help restore the oral function and aesthetics for children who have many different congenital dental problems. Seeing them smile for the first time is absolutely priceless. Giving a young boy some teeth and then seeing him come back a few months later and seeing his growth shoot up is also really rewarding.”

Josh is also a self-confessed gadget obsessive. “As we all know, working in the mouth is like working in a small, dark cave and we need a lot of different gadgets for various different jobs,” he explains. “I don’t think I could practise dentistry without high magnification loupes and LED light.

“One real gem that I can recommend is the Sonicflex by KaVo, which I find essential for finishing delicate margins for bonded porcelain restorations or minimally invasive direct composite resin restorations. It has fine tips that work via elliptical oscillations, which are safe-sided, where one side is non-cutting so you can get right up close to an adjacent tooth.”

Next on his to-do list? Josh is launching a small multidisciplinary study group, or work group, with recently graduated dentists, and specialists he works with.

If you weren’t in the dentistry field, what would you be doing for a living?

Hopefully running a craft brewery!

What is your favourite book/author?

I enjoy reading fantasy novels. My favourites would be Lord of the Rings by JRR Tolkien, The Belgariad

by David Eddings, or Magician by Raymond E Feist.

Is there anything people might be surprised to learn about you?

My surname “Graieg” is technically an Australian name, because my Great Grandfather was an orphan, as his mother died during childbirth. He later tried to trace his roots, and with some help, actually made the name “Graieg” up! So, if you meet another person with the surname Graieg, it is likely to be a close relative of mine!

ABOUT THE PRESENTER Dr Amanda Phoon Nguyen is an oral medicine specialist in Perth and Darwin, adjunct senior lecturer and PhD candidate at the University of Western Australia. Her clinical and research interests

include oral mucosal disease, orofacial pain, temporomandibular disorders, dental sleep medicine and paediatric oral medicine. She is the Editor and an examiner for the Royal Australasian College of Dental Surgeons, co-chair of Australasian Sleep Association Dental Sleep Medicine Council, and widely publishes for multiple journals. She also represents her specialty at multidisciplinary groups and strongly believes in team collaborations.

She is an executive member of Australian Dental Association (WA), heavily involved with many professional committees, and dedicates her energy to many community volunteer causes and online oral medicine educational/ special interest groups. An international and national speaker, she is passionate about her speciality, regularly lecturing and teaching.

She has a public appointment at the Oral Health Centre of Western Australia.

To help our members achieve that all-important worklife balance, ADAWA is launching its own social club.

Organiser Dr Amanda Phoon Nguyen says the Executive team has always wanted ADA House to be a space for ADAWA members.

“We are very good at using ADA House for events, CPD events and meetings, so it seemed like a natural extension for us to use ADA House for some social events as well,” she says. “It started off with yoga, which has been going on for almost a year now. We recently did a pottery event, which was very well received, so we are hoping to expand on that from around September, with monthly or bi-monthly social events.”

Amanda says the ADAWA team is open to suggestions of the type of social events members would like to attend.

“Yoga is an ongoing activity, but we might also try things like wine tasting, life drawing and acrylic painting classes,” she says. “We will probably redo the pottery class again as that was quite popular, and a few people have suggested cooking classes or foodrelated activities. In the future, we may have some kidfriendly events, or family-friendly events as well. Events will not always be at ADA House, but it’s great to have the space readily available to us.

“If members like the idea of a social club, I hope they will support it, as these activities are always dependant on numbers,” Amanda adds. “Please try to come along.”

All the finer details are yet to be confirmed but interested members should keep an eye on their ADAWA emails and the ADAWA Members’ Facebook Group.

In the meantime, the next event at ADA House will be “Paint & Sip” on Thursday, September 2. The cost is $75.

Casual Conversion

Update and Reminder

In March 2021, the Fair Work Amendment (Supporting Australia’s Jobs and Economic Recovery) Bill 2021 (Bill) passed Parliament. The Bill introduced several changes to casual employment, including providing a statutory mechanism for casual conversion.

Prior to these changes, there was no universal entitlement to casual conversion, rather, employees covered by a modern award (such as the Health Professionals and Support Services Award 2020) (HPSS Award) were able to request casual conversion if they met the eligibility criteria outlined in the award.

Now, the Fair Work Act 2009 (FW Act) requires employers to proactively offer casual conversion to eligible employees. This applies to all employees in the practice, not just those covered by a modern award.

There is a transitional period, which ends on 27 September, to allow employers time to make assessments as to whether they need to offer casual conversion. Practices must therefore determine who is eligible, and make offers of conversion prior to this date.

Who needs to make an offer?

Who is an eligible employee?

The changes apply to national system employers, which are those that fall under the national workplace relations laws. Practices operating as sole traders, unincorporated partnerships, unincorporated trusts and incorporated associations that are not trading or financial corporations in Western Australia are covered by the Western Australia State System instead, and are therefore not covered by the changes implemented in the Bill.

The Bill defines an eligible employee as someone who: • has been employed by the employer for a period of 12 months beginning the day the employment started; and • during at least the last 6 months of that period, the employee has worked a regular pattern of hours on an ongoing basis which, without significant adjustment, the employee could continue to work as a fulltime employee or a parttime employee (as the case may be).

Small businesses, meaning practices with fewer than 15 employees, are also excluded from needing to make an offer.

Employees employed in a small business retain the right to request casual conversion, however the administrative burden of having to actively offer conversion is removed.

If employees meet the above criteria, an offer of casual conversion must be made within 21 days of the employee reaching their 12 month anniversary, unless an exemption applies.

Practices with 15 or more employees are required to make offers of casual conversion to eligible employees.

When can a practice choose not to make an offer?

There are also certain instances in which a practice with more than 15 employees is not required to offer conversion. This includes where a reasonable business ground exists. These include: • the employee’s position will cease to exist in the next 12 months; • the hours of work which the employee is required to perform will be significantly reduced; • there will be a significant change in either or both of the following: • the days on which the employee’s hours of work are required to be performed • the times at which the employee’s hours of work are required to be performed which cannot be accommodated within the days/times the employee is available to work • making an offer of conversion would not comply with a recruitment or selection process required by law.

If a practice elects not to make an offer of casual conversion, it must still notify the employee of its decision in writing.

Making an offer When making an offer of casual conversion, practices must ensure the following criteria are met: • The offer must be in writing; and • Be an offer for the employee to convert to either parttime or full time employment (consistent with their hours of work during the relevant period); and • Be provided to the employee within 21 days after they reach their 12-month anniversary.

Before making the offer in writing, the practice will need to meet with the employee and discuss the specific details of the offer.

Once an offer is made, the employee has 21 days to accept the offer in writing. If the employee does not respond, it is taken that they have rejected the offer.

Next steps Given the transitional period comes to an end on 27 September, practices should be proactive and ensure that, if required, they undertake assessments of casual employees to assess eligibility, and make offers to eligible employees before this date. adawa.com.au

Four things you need to know!

When it comes to considering your options for a loan, we’re conditioned to ask “What’s the interest rate?” Sure, interest rates are important, but there are actually 4 key things you need to know in order to make a good loan decision.

Rates are rates. But, fees come in many guises and can skew your effective rate. From monthly and annual fees to account keeping fees, line fees etc... The truth is, fees can often be one of the most annoying parts of your loan because they are often hidden and sneak up on you. The Government of Western Australia has issued an alert due to the increase in syphilis notifications. The rate of syphilis notifications here has doubled between 2016 and 2020.

Although effective therapy has been available for a long time, the incidence of syphilis has increased worldwide in the past decade. It is endemic in low-income countries and occurs at lower rates in middle-income and high-income countries.

Communities at risk include: • People who experience homelessness • People who use methamphetamine and/or inject drugs • Culturally and linguistically diverse (CaLD) people • People who are 16-35 years’ old • Aboriginal communities in remote areas • Aboriginal people 16-39 years’ old, particularly in regions experiencing an ongoing outbreak of infectious syphilis (Goldfields, Kimberley and Pilbara) • Women of child-bearing age (currently represent 30% of all infectious syphilis notifications in metropolitan Perth) • Men who have sex with men (MSM) in the Perth metropolitan area

Primary Syphilis The mouth is rarely the site of primary syphilis. A chancre, a painless, indurated ulcer at the site of inoculation, develops within 1 to 3 weeks of acquisition. Primary syphilis is usually the consequence of orogenital or oroanal contact with an infectious lesion. Kissing may, very rarely, cause transmission. Primary syphilis of the mouth manifests as a solitary ulcer usually of the lip or, more rarely, the tongue. Chancres may resolve without treatment.

Secondary Syphilis (adapted from Leao 2006)

Clinical Presentation There are three main stages with different clinical presentations and infectivity. Oral lesions may occur at any of the three main stages of the clinical course, including the primary, secondary and tertiary phases.

Oral manifestations of syphilis are multiple and highly variable, and often detected in secondary stage. Only the 8% of patients and the 25% had, respectively, primary and tertiary lesions. Primary syphilis is detected as ulcer in all patients, while secondary syphilis is detected as ulcer in about 50% of cases. Oral presentation may not be synchronous to cutaneous (Leuci 2013).

Because of the heterogeneity of the oral clinical aspects, the differential diagnosis includes a huge group of diseases: traumatic or cancerous or nonspecific inflammatory ulcers, autoimmune (pemphigus/ pemphigoid) or immune-related lesions (lichen planus, erythema multiforme), traumatic (frictional keratosis) or hyperplastic/dysplastic plaques, and other infectious diseases such as tuberculosis, deep fungal, herpes lesions and hairy leucoplakia.

Controversial/rarer presentations include mucosal patches, leukoplakia-like lesions, blistering mucositis, and necrosis of the dorsum of the tongue.

The features of secondary syphilis reflect the hematogenous spread of T. pallidum, and similarly to its other mucocutaneous features, the oral manifestations of secondary syphilis can be more extensive and/or variable than those of the primary disease. Oral lesions arise in at least 30% of patients with secondary syphilis, although very rarely oral ulceration may be the only manifestation of infection.

The 2 principal oral features of secondary syphilis are mucous patches and maculopapular lesions, although nodular lesions may rarely arise.

Macular syphilides: Macular lesions tend to arise on the hard palate and manifest as flat to slightly raised, firm, red lesions.

Papular syphilides: These are rare. They manifest as red, raised, firm round nodules with a grey centre that may ulcerate. The papules usually arise on the anterior dorsal tongue, buccal mucosa or commissures.

Mucous patches: A variety of descriptions of mucous patches have been reported. These may present as oval to crescenteric erosions or shallow ulcers of about 1cm diameter, covered by a grey mucoid exudate and with an erythematous border. Common sites include bilaterally on the mobile surfaces of the mouth. The pharynx, gingivae, tonsils, and very rarely the hard palate can be affected. At the commissures, the mucous patches may appear as split papules.

The mucous patches may coalesce to give rise to, or arise de novo as, serpiginous lesions, sometimes termed snail track ulcers.

Others: macroglossia, painful fissuring and/or papular lesions on the anterior 2/3 dorsum tongue.

ULCERONODULAR DISEASE (LUES MALIGNA) Ulceronodular disease is an explosive generalised form of secondary syphilis characterised by fever, headache, and myalgia, followed by a papulopustular eruption that rapidly transforms into necrotic, sharply demarcated ulcers with hemorrhagic brown crusts, organized in rupioid layers commonly on the face and scalp. The mucosa is involved in about onethird of affected patients. Lues maligna gives rise to crateriform or shallow ulcers on the gingivae, palate or buccal mucosa, with multiple erosions on the hard and soft palates, tongue and lower lip.

NODULAR DISEASE Rarely, secondary syphilis can manifest as nodules alone. This nodular eruption of syphilis has a predilection for the face, mucous membranes, palms of the hands and soles of the feet. Lesions may occur on the vermillion, mimicking squamous cell carcinoma or keratoacanthoma.

LATENT SYPHILIS In early latent syphilis, usually the first 12 months after secondary disease, affected patients are infectious. In late latent syphilis the infectivity falls.

TERTIARY SYPHILIS Clinical disease arises in about one third of patients with untreated secondary syphilis. The oral complications of tertiary syphilis center upon gumma formation, and much more rarely, syphilitic leukoplakia (and controversial risk of oral squamous cell carcinoma) and neurosyphilis.

Gummas tend to arise on the hard palate and tongue, although very rarely they may occur on the soft palate, lower alveolus, and parotid gland. Gumma manifests radiologically as ill-defined radiolucencies that may resemble malignancy. The areas of ulceration eventually heal, although the resultant scarring can, at least on the tongue, cause fissuring.

NEUROSYPHILIS Argyll Robertson pupil, both unilateral and bilateral trigeminal neuropathy and facial nerve palsy.

Potentially, syphilitic osteomyelitis may give rise to trigeminal neuropathy.

CONGENITAL SYPHILIS Early features include diffuse maculopapular rash, periostitis (frontal bossing), and rhinitis. Late features, manifesting at least 24 months after birth, comprise the Hutchinsonian triad of interstitial keratitis of the cornea, sensorineural hearing loss, and dental anomalies.

Diagnosis The diagnosis of syphilis may require a knowledge of the patient’s sexual history, physical examination, and an interpretation of serological and microbiological findings. The diagnosis is often made on clinical and serological grounds without recourse to biopsy.

Histopathology is not always helpful, as there are no specific histopathological features, though some histological features may be indicative. An immunohistochemical reaction using an antitreponemal antibody is highly specific, with specificity ranging from 74% to 94%.

If you suspect your patient has oral lesion due to a syphilis infection, the following steps can be taken: • Swab for polymerase chain reaction (PCR) testing by rotating a DRY swab near the centre of the ulcer for 5 seconds. Apply pressure to express tissue fluid from the wound bed. Send the swab without transport medium to a pathology provider and request a syphilis PCR • Alternatively, refer the patient • Ensure the patient’s GP/primary health care provider (eg Aboriginal health service, Homeless Healthcare) receives a copy of the SYPHILIS PCR test result For serological testing, the non-treponemal tests become positive 1–4 weeks after the appearance of the primary lesion, and 6 weeks after exposure. The most commonly used is the VDRL (Venereal Disease Research Laboratory) test which is the method of choice for follow-up testing during and after treatment.

The treponemal tests are used mainly as confirmatory tests to verify reactivity in non-treponemal tests. The most common is the FTA-ABS test.

Management includes benzathine penicillin G, tetracycline, azithromycin, or doxycycline. However, resistance to azithromycin has emerged rapidly.

ADAWA calls for end to sugarydrink sponsorships in sport The Australian Dental Association of WA has united with WA’s peak public health and medical experts to call for an end to the sponsorship of sport by junk food companies, with one of the largest junk food companies in the world granted official sponsorship of the 2021 Tokyo Olympics in Japan.

Cancer Council WA CEO, Ashley Reid, said the time has come to question unhealthy sponsorship of sport by junk food and sugary drink companies; a call backed by ADAWA. “With Coca-Cola being granted ‘Worldwide Olympic Partner’ of this year’s Olympic Games in Japan, millions of children and families around the world are being exposed to marketing messages pushing these sugarladen products,” Mr Reid said.

ADAWA Chief Executive Officer Dr David Hallett said that tooth decay is the most common chronic disease in Australian adults and children, commonly caused by added sugars. This type of sponsorship of elite and community sports normalises the consumption of sugary drinks.

“Consumption of added sugars is the main contributor to tooth decay in children, young people and adults,” Dr Hallett said.

“Given nearly half of Australian children have tooth decay, it’s clear that we need to do more to raise awareness of how these products are ruining our WA smiles, but also to have an environment that makes healthy choices easier.

“In 2021, with what we know about protecting teeth, we should have eradicated tooth decay.

Instead, not only is tooth decay common, but our members are reporting that these cases of severe decay are far too frequent.”

GOOD NEWS STORIES Art at ADA House Various restrictions were still in place coming out of lockdown, but the first Art at ADA House was still able to go ahead, with appropriate social distancing measures and mask wearing.

July General Meeting It was wonderful to see so many members at July’s General Meeting at ADA House – and what a full evening it was!

The Royal Australasian College of Dental Surgeons unveiled an Honour Board, featuring their WA Regional Committee Chairpersons.

We were honoured to reveal a tribute to former ADAWA CEO, President and Honorary Life Member, Dr Stuart Gairns. The photograph of an evening over Albany Harbour is a scene close to Stuart’s heart, and is now on display in the lecture theatre.

Thank you to oral and maxillofacial surgeon Dr Ken Wan, who presented on Zygomatic implants for the severely atrophic, radiotherapied or resected maxilla.

UWA Dental School Teaching Clinic Renewal The UWA Dental School Teaching Clinic Renewal event was a great success.

Attendees were shown the new dental chairs in the teaching clinic and were introduced to the next new project, upgrading the Clinical Simulation Laboratory (OpTech).

This year, the Mars Wrigley Foundation has awarded approximately $111,000 in grant funding to 10 projects.

Special mention goes to the WA recipients: University of Western Australia: Smiling Signs Auslan Resource This grant will help fund a publicly-available digital resource in Auslan (Australian Sign Language) to improve the standard of care for deaf patients.

University of Western Australia: Healthy Mouths, Brighter Smiles Healthy Mouths, Brighter Smiles will host interactive sessions at schools to improve the oral health outcomes for children from low socio-economic areas.

Shalom House: Shalom House Dental Rehabilitation Program The Dental Rehabilitation Program will deliver free screening, treatment and education to improve the dental health of residents at rehabilitation program, Shalom House.

Healing Smiles Healing Smiles recently held their first meeting for their volunteers for this year. A speaker discussed trauma-informed care in the dental setting, specifically regarding survivors of family and domestic violence. The informative presentation gave many practical tips, most importantly from the patient’s perspective.

ADHF Dental Rescue Day The team from Diamond Smiles Dental in Carramar recently volunteered their time on a Sunday to hold their first Australian Dental Health Foundation Dental Rescue Day. Despite strong winds and heavy rain, they had a successful turn-out, treating eight grateful patients.

In a wonderful show of community spirit, Dandelions WA donated thoughtful kindness kits to send home with each patient, and the local Knit and Natter group personally knitted beanies and mittens for the patients to take home as well.

Homeless Connect Rockingham Dental Health Week was also Homelessness Week, so we were thrilled to be a part of an amazing day at Homeless Connect Rockingham on Tuesday August 3, hosted at Salvation Army Rockingham. The hardworking volunteer team, which included dentists Drs Jonathan Lo, Villas Menon, Sahara Saeedi and Femi Moradi, and dental assistants, Jane and Suzanne, treated 21 grateful patients.

The day was made possible thanks to the combined efforts of Dental Health Services, Australian Dental Health Foundation and ADAWA. A huge thanks goes to sponsors Henry Schein Cares, Colgate and Wrigley’s. Thanks also to Coates Hire Kwinana for providing the hire of the generator. Celebrating the Class of 1990

By Dr Tim Crofts The class of 1990 and friends recently held a reunion at ADA House in Perth. Joining them on this special occasion was honoured guests Winthrop Professor Paul Abbott and Dr Jacky Castro. Also in attendance were Dr Brent Allan and Dr Jane Connor.

Classmates were tasked with sharing a lesson from dentistry, which included: • You can learn more at lunchtime at a CPD event than during the lectures.

• Don’t put away the impression material until you know the impression has worked.

• Treat all patients how you want to be treated. And if they are crazy, demanding and all together irrational, refer them to Dr Delcanho.

• If a patient says they have trouble going numb, believe them!

• Don’t be afraid to fire a bad patient. Life is too short.

Joondalup Golf Club A stunning June winter afternoon was enjoyed by another large contingent of WADA golfers at the outstanding Joondalup Golf Resort. Thirty-one players met to take on the challenging Dunes and Lakes combined layout.

An early start meant the best of the day was enjoyed by all, with most playing in summer attire due to the unseasonably warm weather. Indeed, a few sunburnt faces were noted during the post-round gathering.

We were joined by representatives from our corporate sponsors in Brad Potter from The Healthlinc, Russel Redpath from Dentsply Sirona, Garry Bishop from Health Practice Brokers and David Owen from Swan Valley Dental Laboratory.

Scoring was high, reflecting the perfect conditions.

Seven players managed to at least play to their handicap or better. Third place required a countback of results between three players. Competition for nearest the pin and long drive competitions was similarly fierce.
