We received some wonderful feedback on the inaugural issue of Clinical Integration News. By creating a new digital platform for value-based care, our objective is to educate, inform and inspire. In this Spring issue, we focus on new beginnings. Our recently launched co-branded Medicare Advantage plan is the first of its kind in our region. We are also pleased to announce the opening of Holy Cross AgeWell Center, a comprehensive program designed to support our seniors. Lastly, we discuss how care management can serve as an invaluable physician resource, re-energizing the way we handle transitions of care. Meet the New Team Members Jocelyn Fernandez, Administrative Director of Provider Relations Jocelyn Fernandez is an experienced business development and provider relations professional in the healthcare field. Prior to joining Memorial Health Network and Broward Guardian, she served as the Director of Business Development and Physician Relations for Memorial Urgent Care Centers and Memorial Primary Care. She has worked for multiple Tenet South Florida hospitals as part of their business development and physician relations teams. She has a Bachelor’s in Human Services and over 20 years of experience working in the healthcare industry, where she began as a social worker in 1997.

Teshana Gibbs, Director of Business Operations Teshana Gibbs brings her strong healthcare background, robust experience of program management and strategic vision to advance initiatives within the health system. She began her healthcare career at a large academic medical center in New York City, overseeing the creation and roll-out of their first Internal Medicine Residency Program. Yvonne Ortiz, Director of Coding Yvonne Ortiz has been in the healthcare industry for over 16 years. She recently worked with Florida Blue Medicare as a Population Health Manager, handling practice transformation initiatives to improve the performance of key medical practices. Previously, she supported providers with medical coding and quality performance strategies as a Risk Adjustment Coding Consultant. Yvonne has also worked on the provider side in physician practice management. Outside of the medical field, she has served as Regional Human Resource Director, but her passion for coding and practice management has kept her in healthcare.

David Susskind, Administrative Director of Provider Relations David Susskind joined Memorial Health Network and Broward Guardian in January 2022. David is a former New York City Fire Department paramedic who holds a Master’s in Health Policy and Administration and earned his Bachelor’s in Healthcare Administration prior to relocating to South Florida in 2001. David brings extensive experience in progressive leadership roles focused on clinical and non-clinical operations, population health, physician enterprise, practice management, business development and strategy. He is also proficient in provider, client, payor and facility contracting as well as account management.

Medicare Takes a Concierge Approach
In 2021, Atlantic Coast Health Network partnered with Oscar to launch a co-branded Medicare Advantage plan, bringing together our extensive physician network with Oscar’s best-in-class member engagement. The Oscar + Holy Cross Health + Memorial Healthcare System Medicare Advantage plan is the only plan in the market to deliver a highly-personalized concierge experience.
Medicare Advantage, also known as Plan C, is a “bundled” alternative to Original Medicare that provides comprehensive health and drug coverage plus several extra perks. ACHN has taken Medicare Advantage to another level, tailoring to the local population and offering more choices such as dental, vision and transportation—all for one low premium. “What distinguished Oscar from the other companies was the fact that they were very technologicallyoriented,” explained Dr. Charles D. Russo, a cardiology specialist with the Holy Cross Medical Group and part of the ACHN Board of Directors. “Everything can be done from the app on this health plan. There’s a dedicated concierge team, tasked with ensuring that you get the care you need.”
The concierge team includes a nurse who knows the patient’s history, and can coordinate care with in-network providers. That level of high-touch service, coupled with Oscar’s digital platform, engages patients in a way that truly places them at the center of their own care. “With Holy Cross Health and Memorial Healthcare System, we found partners who not only share in our vision to make healthcare accessible and affordable, but have developed their value-based care model in a meaningful way,” said Dennis Weaver, MD, Chief Clinical Officer at Oscar. Holy Cross AgeWell Center Now Open At ACHN, we aim to create initiatives designed to support a healthy lifestyle at every stage in life. For the 65+ population in South Florida, we have been particularly thoughtful with our offerings, always keeping the whole patient in mind. At this time, we are thrilled to announce the opening of the Holy Cross AgeWell Center. This comprehensive, collaborative center is the newest addition to the Holy Cross HealthPlex. Aside from delivering quality healthcare, a team of providers are committed to helping seniors feel their best and live their most active lives. “Holy Cross AgeWell Center is a place where seniors want to come to work on, not only their healthcare, but also their socialization and how they feel inside,” explains Dr. Jennifer Capezzuti, one of the center’s on-site physicians. The Power of Care Management One of the most successful initiatives, at both Holy Cross Health and Memorial Healthcare System, has been the care management program. Many physicians are not aware of the support available from our care management teams. These dedicated nurses and social workers assist with the coordination of care between the PCP, the specialists, the post-acute entities and home health.
Here’s how the care management team relieves physician burden: • Helps patients understand and manage their condition • Reviews discharge list with patient • Explains medication regimen • Assists with making future appointments • Recommends resources such as nutritional counseling • Assesses environmental conditions that may impact outcome • Communicates with physician office We encourage our physician partners to utilize this valuable resource. Care management bridges the gap, improving patient compliance and patient outcomes.
