FUNDING Primary funding streams include state or federal Title XIX waiver services. For individuals with intellectual and developmental disabilities, these can include Consolidated, Person/Family Directed Support, Autism and OBRA waivers. For individuals, birth through 21, with special healthcare needs and require a personal care attendant, funds are available through medical assistance.

PROVIDING FAMILY SUPPORTS FOR CHILDREN AND ADULTS WITH DISABILITIES.

• Personal Care Attendants Provides support to individuals birth through age 21 with special healthcare needs for daily living skills. This support must be prescribed as medically necessary from the individual’s physician and approved by Medical Assistance’s (MA) Managed Care Organization (MCO) or directly through the state MA program.

ACHIEVA HOME CARE PROVIDES IN-HOME AND COMMUNITY SUPPORT FOR PEOPLE OF ANY AGE WITH INTELLECTUAL AND DEVELOPMENTAL DISABILITIES (I/DD) • Home and Community Habilitation Provides ongoing assistance to individuals for many everyday tasks. Caregivers assist the individual with improving and growing independently while considering their preferences and interests in order to live life to the fullest. Habilitation supports can also be provided to small groups of two or three.

• Respite - Hourly Provides relief to families caring for a loved one with special needs. Supports can be provided in the individual’s home or in the community. This time can be used for families to take care of themselves, rest and relax and get a break from the day to day challenges that come from caring for someone with special needs.

• Companion or Family Aides Provides the individual with supervision for their health and safety. The caregiver is able to provide minimal assistance with daily living activities. This is used primarily when non-medical or non-habilitiatve care is needed.

Family aides may also provide supervision to other individuals in the household such as siblings or elderly family members. Family aides are referred and approved by the individual’s Administrative Entity (AE).

• Homemaker/Chore Provides assistance to an individual with special needs or the family where the individual resides with household tasks that may prove to be to difficult to perform. An individual can qualify for this support only when other household members, landlord or agency are unable to provide the task.

• School, Medical Aides or Childcare Provides specialized care of an individual with intellectual and developmental disabilities (I/DD) as requested by school districts, medical facilities, community organizations, etc. to assist as temporary aides when extra support is needed by these community organizations.

ACHIEVA Home Care is regulated by the Department of Human Services, licensed by the Department of Health and is a member of the Pennsylvania Homecare Association. ACHIEVA caregivers are required to meet state reqirements and have personal or professional experience in working with individuals with disabilities.

All candidates must secure satisfactory clearances and trainings needed for an individual’s plan of care.
