SPRING IS IN THE AIR Spring is almost upon us and what better time is there to introduce fresh concepts and strategies into your practice as an investment in your future growth and development? Hot on the heels of ABDO's recently-launched Strategic Plan for 2022/26, comes a new Sustainability Policy template for members.

Conceived as part of ABDO's innovative #ThinkingGreen campaign, the template is designed to help members demonstrate how their practice is meeting its ethical and environmental responsibilities. The step-by-step template can be found in the SEE Hub, which hosts a whole range of other useful #ThinkingGreen resources.

Whilst visiting the site, why not dip into the latest management and leadership training opportunities? In April, members will have the chance to attend two new workshops: one on change management and the other on conflict management. Both workshops will be held at the ABDO National Resource Centre, and details and booking access can be found in the Events section.

Another new resource is a Focus on Dry Eye section in the ABDO Clinical Hub, bringing together up-to-date research and current thinking in this area. The Clinical Hub also hosts information on myopia management, optical coherence tomography, eye diseases, extended services and much more.

From the Holmes front Business support resources f you are already in business or thinking about starting a business in the optical sector, one of ABDO’s membership benefits is the Business Support Hub (BSH). Personally, I have found this a great resource when I have been stumped with an issue.

The BSH contains a wealth of information and advice – ranging from business growth and managing cash flow to helping launch new services to give your practice those all-important unique selling points. ABDO has also researched and worked with partner organisations to look at cost savings on utilities and telecoms, for instance, and member discounts are offered on fees for a range of professional services.

In addition, we are looking at ways to support you in developing your business, leadership and management skills. We have joined forces with the Institute of Leadership and Management, a professional membership body Make the most of ABDO's offering webinars, podcasts, business resources networking opportunities, worksheets and training to enhance your knowledge. One of my friends did this course during lockdown, and she told me it kept her sane and connected to optics when she was furloughed.

There are useful practical tips and a development programme that can be tailored to your own needs. Importantly, the management and leadership courses available lead to Chartered Management Institute qualifications.

Sometimes it can feel isolating working in practice. ABDO has a mentor matching service within the BSH that offers a peer-to-peer e-mentoring platform, matching professionals with various experience. This is a great way to improve your self-confidence and an opportunity to focus and reflect on your own practice.

Working on your business and not in your business can be valuable time off the practice floor. On the website there are many resources already in place that you can work with straight away.

Guidance on payroll, human resources, training your team and various management tools are all easily accessible.

Once your business is up and running, there are links to find out more about marketing strategies, communicating using social media, advertising and PR. Within each section, there are a few short tips for quick wins and longer articles to enable you to learn about each topic in more depth.

The BSH is designed to cover all aspects of your business and you can take from it what you need.

Vision for the future ABDO's newly-launched Strategic Plan will guide the Associaton's work over the next five years on behalf of members.

ABDO president, Jo Holmes, said: “This plan means we can look forward with confidence and with a positive vision of what we want to achieve for our members.

“We look forward to working on the actions within the plan over the next five years – for the benefit of dispensing opticians and contact lens opticians and, ultimately, for the benefit of the public across all four nations of the UK. Thank you to everyone who has contributed to the plan’s development,” added Jo. Alistair Bridge, ABDO head of strategy, who led the development of the Strategic Plan, said: “Developing the plan has involved looking closely at the factors that are likely to shape the future, and considering the implications for our members and for ABDO as an organisation.”

The plan also outlines an updated purpose along with a new vision and new values for ABDO. An Annual Plan will be published each year showing how the organisation is working towards its vision, whilst responding to any new developments.”

GOS fee rise in Scotland From 1 April 2022, the fees paid by the Scottish government to those carrying out examinations under the General Ophthalmic Services (GOS) contract in Scotland will increase by three per cent – backdated to include all GOS examinations undertaken from 1 April 2021.

The announcement follows ongoing talks with the Scottish government, and marks the first major review of the Scottish GOS contract, which was introduced in 2006 and brought free eyecare to Scotland.

The Scottish government and Optometry Scotland (OS) are now aiming to put in place a formal process for annual remuneration reviews.

David Quigley, OS chair, said: “These are significant developments, which acknowledge the excellent service provided by community eyecare colleagues across Scotland.

“We have campaigned for an increase and implementation of a sustainable funding model to safeguard and enhance the future of community optometry, and thanks to a strong working relationship and ongoing constructive dialogue with the Scottish government we are pleased to see this come to fruition.”

Medal award for the real Deal

Solid solution for myopia management

Sue Deal has been presented with the ABDO Medal of Excellence for her exceptional service to the Association over many years.

After qualifying as a dispensing optician in 1980, Sue soon went on to become an ABDO practical examiner.

She began lecturing and examining in Malaysia in 2001, and six years later became a senior tutor for ABDO College.

Sue served for many years as a member of the Academic Committee, and over the past 15 years has continued to be a theory marker and moderator for ABDO College. Over the past 18 months, she has acted as an external moderator for the dispensing theory examinations.

Sue was presented with her medal by ABDO president, Jo Holmes, who said: "Sue first became a member of ABDO in January 1978, helping to organise meetings for members in her area. She has been a long-standing examiner and course tutor, has examined and taught all over the world, and is a fantastic moderator for the PQE theory exams.

She has also been writing Optical Assistant Corner for Dispensing Optics Online since January 2021.

"Sue is always available to help at short notice and goes beyond whatever we ask of her," added Jo. Commenting on her award, Sue said: "I am delighted and honoured to receive this award from ABDO. I have worked for the Association in many different capacities, but I have never really thought of it as 'work'. I have thoroughly enjoyed every minute, and have made many good friends along the way. I would like to thank ABDO, not only for the award, but for the wonderful opportunities in the past, present and the future."

Essilor is keen for practitioners to harness the practical support it is offering to introduce Stellest myopia correction and control lenses in practice.

While Stellest lenses correct myopia, they have also been shown to slow down its progression. Clinical trial results demonstrate that after two years, children wearing Stellest lenses for at least 12 hours a day saw their myopia progression reduced by 67 per cent on average (0.99D) Stellest lenses have been shown compared to children wearing single to slow down myopia progression vision lenses.

Designed with Highly Aspherical Lenslet Target (HALT) technology, a constellation of 1,021 invisible aspherical contiguous lenslets distributed over 11 rings on the surface of the lens have been designed to create, for the first time, a volume of unfocused light in front, and following the shape, of the retina. This signal is said to help slow down the eye elongation and therefore myopia progression.

The lenses are specially designed for children and suitable for a variety of frames.

Training as well as point-of-sale is available – and the lenses are part of Essilor’s 2022 multi-pair lens offer running throughout the year.

Essilor managing director, Tim Precious, said: “Stellest lenses provide a solid solution for the eyecare practitioner to recommend to children if the practitioner feels their myopia needs to be managed. It will set the standard for myopia management."

New deal with Alpine Esports

Driving sim-racers to victory Shamir and Alpine have extended their strategic partnership into the gaming world.

Shamir first partnered with the Alpine F1 Team in May 2021, to enhance the visual quality and safety of race team members. Now, the company is providing optical assistance for Alpine Esports’ sim-racing and gaming roster as its Optical Performance Partner.

The Esports squad train for up to eight hours per day on simulators to prepare for online events. To aid their training, Shamir will provide ophthalmic assessments and supply eyewear with enhanced lens technology and increased blue light protection.

Alpine Esports and Shamir will also collaborate to develop new technologies to improve the visual quality of simracing. This R&D programme will be based around the relation between the brain and the eye to lower response timings, ensure less visual fatigue and improve cognitive abilities.

Yagen Moshe, Shamir CEO, said: "After partnering with the Alpine F1 Team, becoming the Optical Performance Partner of Alpine Esports represents for us another step in our mission to provide improved visual experience in all areas of activity. Off-setting plastic waste CooperVision has introduced a new initiative to ensure its one-day contact lens brands are 'plastic neutral'.

Having teamed up with the social enterprise, Plastic Bank, for every box of one-day lenses now distributed in the UK and Ireland, CooperVision will purchase credits to fund the collection, processing and reuse of general plastic waste equal to the weight of the plastic used in the lenses, blister and outer carton packaging.

Through Plastic Bank, ocean-bound plastic materials are reprocessed for reintroduction into the global supply chain. Collectors receive a premium for the materials they collect, which helps them provide basic family necessities.

Doug Bairner (pictured above), country manager UK and Ireland at CooperVision, said: “Plastic plays a critical role in the hygienic delivery and sterile protection of our contact lenses, and how that plastic is managed is important to us. The commitment to making all of our one-day lenses in the UK and Ireland plastic neutral is an innovative addition in CooperVision’s holistic approach to sustainability.”

On-trend colour edit Colour trends for 2022 have been incorporated into the latest style edit for the Cocoa Mint ophthalmic collection from Eyespace.

New Line Art Aria

Positive Impact (PI) has appointed three new directors as part of a restructure following its expansion as an optical distributor in recent years.

Professional services lead, Phil Thompson, is now professional services director, and business development manager, Tony Whyatt, has become commercial director. Finance manager, Chris Amor, has been appointed as finance director.

PI's founders have also changed role. Managing diirector, Maxine Green, is now sales and operations director, with Nick Atkins moving into the role of managing director.

PRODUCT SPOTLIGHT - SUNGLASSES

A new dawn is breaking Next month sees the launch of Neubau Eyewear's special edition Transition sunglass models – Xenon and Neon.

Channeling the first rays of a new dawn through their futuristic designs, mirrored lenses and high-contrast neon-coloured accents, the styles are crafted using sustainable technologies and beta-titanium from selected suppliers in Japan.

Each style is 3D-printed using the brand's own 100 per cent plant-based material, Natural3D. Obtained from non-genetically modified castor oil beans, this bio-based polymer is processed with a no-waste method. A linear titanium element adds a colourful spark.

The campaign for Transition was created by South Africa-based photographer, Nadia von Scotti. Her images, capturing different scenes of the South African landscape, were taken at night and at dawn to capture the moment of transition.

The two models will be available in black, black/lime, black/purple and black/white.

he increasing prevalence of diabetes mellitus amongst the global population is becoming a major health epidemic1, with an estimated 578 million diabetic adults globally by 20302. The rise of diabetes has resulted in further economic strain on public health services, with Type 2 diabetes alone making up for nine per cent of the UK National Health Service budget, at a cost of more than £8.8bn per year3.

Concurrent with the demographical and epidemiological changes that have occurred over the last decade4, diabetes mellitus has become a significant threat to public health and is one of the most prominent causes of vision loss across the world. Notably, diabetes is associated with potentially sight-threatening ocular pathologies such as diabetic retinopathy (Figure 1 - page 17) and diabetic macula oedema5 (Figure 2 - page 17).

The World Health Organisation emphasises that diabetic retinopathy is the fifth leading cause of visual impairment and the fourth leading cause of blindness in the world6. In the UK alone, it is estimated that 1.5 million people have diabetic retinopathy7. Research also indicates that those with diabetes are more likely to develop other eye conditions such as cataract and glaucoma8.

The early detection and monitoring of eyecare amongst diabetic patients is, therefore, vital in the prevention of ocular complications, yet this is largely dependent on the current level of knowledge that the public have regarding diabetic eyecare. Additionally, as the forefront of primary eyecare providers in the UK, ophthalmic practices and eyecare professionals solicit a duty of care in augmenting the level of understanding and awareness that diabetic patients have regarding the effects that diabetes can have on their ocular health.

PUBLIC AWARENESS AND UNDERSTANDING A cross-sectional study was carried out to examine the existing level of public awareness of diabetic eyecare in order to gain a better understanding about the level of knowledge and awareness that diabetic patients have in relation to their diabetes and its effect on ocular health.

The research also aimed to highlight how involved local ophthalmic practices were in educating their diabetic patients, and their efficacy in raising public awareness.

The aim of this article is to inform optical professionals of the study results, with a view to enabling relevant improvements to be made to the current approach to diabetic eyecare employed by ophthalmic practices in the UK.

This cross-sectional study employed a pragmatic, mixed methods approach to research and involved the collection and analysis of both quantitative and qualitative data via self-administrated written questionnaires.

Diabetic participants were recruited over two independent ophthalmic practices in one region in the North West of the UK over a period of two months, from December 2019 to February 2020. A total sample size of 84 patients was achieved, of which 54.76 per cent (n =46) were male and 45.24 per cent (n=38) were female (Figures 3, 4 and 5).

There were six main findings from the data collected in the study, which can be categorised into two areas: 1) What level of understanding and awareness do patients with diabetes have on how diabetes can affect their eye health; and 2) What is the patient perception of the primary eyecare and optical professionals in their patient care.

FINDING ONE Finding one relates to the correlation between how long a patient has had diabetes, and their thoughts as to whether diabetes has affected the health of their eyes.

Participants who had diabetes for more than five years were found to increasingly share the view that diabetes did have an

effect on ocular health, when compared to those who had diabetes for five years and under (Figure 6). This finding reflects the results of similar studies by Ullah et al 9, Foreman et al 10 and Gupta et al 11, which also found a relationship between longer duration of diabetes and better levels of patient knowledge.

This association may be justified by suggesting that a longer time period of having diabetes may be accompanied with increased levels of experience due to a generally greater number of eye-related appointments. As a result, these patients may have received advice from their healthcare professionals regarding diabetic eyecare a greater number of times. This implies that those who have had diabetes for longer may have a statistically higher chance of being aware of the fact that diabetes can have an effect on eye health.

CPD Equally however, similar studies by Fenwick et al 12 and Achigbu et al 13 found no statistical significance between the duration of diabetes and patient knowledge. Therefore, in order to confirm the validity and reliability of this finding, it is recommended that a cross-national, longitudinal study be conducted to more accurately determine whether the duration of diabetes has a significant association with the level of knowledge that diabetic patients have regarding their eyecare.

Finding two covers the responses given by participants when asked to list eye conditions they thought were related to diabetes.

Blurry vision, cataract and dry eye were listed as the most prevalent responses, whilst diabetic retinopathy and blindness were cited the least (Figure 7). These results are in accord with the findings from similar studies by Hartnett et al 14, Achigbu et al 13 and Srinivasan et al 15, which also affirmed a lack of knowledge regarding eye pathologies related to diabetes, notably diabetic retinopathy and blindness.

This finding has highlighted the need for increased efforts to be made in augmenting patient awareness about diabetes related eye conditions. This may be achieved through the implementation of educational programmes, which should be made easily accessible to the general public.

Although the UK has implemented campaigns such as Diabetes Week and National Eye Health Week, it could be suggested that the development of online educational programmes on diabetic eyecare may be more effective in engaging with a wider demographic at a faster rate.

This is due to the recent rise in digital technology and online learning across both the younger and older generations16.

The importance of providing educational programmes for the public on diabetic eyecare extends for both diabetic and non-diabetic patients, as studies have shown that those who are educated on diabetes are more likely to provide guidance to family members and friends who are diabetic9.

Thus, evaluating the current methods utilised in educating the public on diabetic eyecare and considering a more digital and online approach, may be the way forward in increasing public levels of knowledge

and encouraging patients to actively engage in their eyecare. Indeed, educating the public is a fundamental preventative step in decreasing the number of complications related to diabetes17.

FINDING THREE Twenty-five per cent of participants did not know which type of diabetes they had (Figure 8). This finding further emphasises the importance of educating the public on their diabetes and supports the results from a similar study by Huang et al 18.

The latter study found that 13.2 per cent of participants were unaware of their diabetes status, and 83.4 per cent of participants with diabetic retinopathy were unaware of having the eye disease.

FINDING FOUR The majority of participants were more knowledgeable regarding the regularity of their diabetic screening recall, compared to their eye examination recall. (Figures 9 and 10).

In line with guidance from the College of Optometrists19, patients with diabetes who are part of a diabetic retinopathy monitoring scheme have a recommended minimum re-examination interval of two years; those who are not part of a diabetic retinopathy monitoring scheme have a recommended recall of every one year.

A comparitively reduced awarenesss of eye examination recall demonstrates the need for ophthalmic practices to examine and evaluate the efficacy of the current protocols employed by primary eyecare professionals in informing patients regarding their eye examination recall, and to consider applying additional strategies to ensure that diabetic patients are aware of their recall intervals. Better patient awareness of when they are due for their eye test, and the reason it is important as a patient with diabetes, may increase the attendance of eye examinations, and thereby enable the continued monitoring of eye health in diabetic patients20.

One of the recommendations would be through the development of continued professional development courses for primary eyecare professionals on how to more effectively communicate with diabetic patients, to make them more aware of their eye examination recalls by emphasising on the reasons why regular monitoring of eye health is of such vital importance to prevent ocular complications.

CPD Another recommendation would be for optical practices to implement patient-focused booking systems, in order to improve patient awareness of their eye examination recall and to encourage better levels of attendance.

Patient-focused booking is a method of organising medical appointments that is currently being utilised in hospitals for consultant outpatient appointments in Wales, Scotland and Northern Ireland21. It involves advising patients to book in their appointments at a maximum of four to six weeks before their recall interval, and has proven to reduce the number of nonattendees22 as patients are able to book an appointment closer to their due date, thereby increasing patient memorability of when their appointment is due23.

Thus, implementing patient-focused booking in ophthalmic practices may aid in improving patient memorability of their eye examination recall, and ensure that diabetic patients are seen at the correct interval. As articulated by Khohomela and Maluleke24, eye examinations at specific regular intervals are a preventative measure for ocular complications and, as such, patient awareness of their eye examination recall is profoundly essential in ensuring the regular monitoring of eye health.

HOW CAN EYECARE PROFESSIONALS PLAY A PART? FINDING FIVE Another important finding attained from this study, were the responses given by participants when asked which sources provided them with the most information about their diabetes and eyecare. The most cited source was their GP or doctor’s surgery (41.6 per cent). Participants had received more information from online sources compared to their opticians.

(Figures 11, 12 and Table 1).

Additionally, participants were asked what more they thought their opticians could do to ensure that they had better levels of awareness regarding diabetic eyecare. For this qualitative question, the majority of participants stated that receiving written advice via leaflets and booklets would aid in increasing their ability to retain the information given by their optician. These responses relate to the results from a similar study by Hartnett et al 14, in which clinicians

concluded that more informative educational methods using written and visual materials were required to improve patient knowledge on the effects that diabetes can have on ocular health.

This finding indicates the need for ophthalmic practices to consider providing patients with more written resources in order to increase patient knowledge of diabetic eyecare. This may include increased emphasis on providing all diabetic patients with additional materials after their eye examinations, such as information leaflets available from the College of Optometrists or handouts published by the National Eye Institute.

It may also be suggested that the development of information brochures on diabetic eyecare in a wider range of languages may enable more people to be educated.

FINDING SIX The final question in the research questionnaire sought to determine patient perceptions of their own level of knowledge regarding diabetic eyecare, using the Likert scale (Table 2). Contrary to expectations, only 14.29 per cent considered their level of knowledge to be below average, indicating that patients believe they have adequate knowledge regarding diabetic eyecare. However, their responses to other questions within the questionnaire do not reflect this.

This finding demonstrates the significant improvements that may be required to bridge the gap between many patients' low current knowledge on diabetic eyecare and the information that is actually available for them. This further reinforces the need to better educate the public about diabetes and its effects on eye health.

CONCLUSION AND RECOMMENDATIONS The empirical findings in this study enhance our understanding of the level of knowledge that diabetic patients have in relation to their eyecare. They also provides a greater insight into the perceived role that dispensing opticians and optometrists in ophthalmic practices have in bridging the gap between patients and their awareness.

Whilst this study may not confirm the responses as a representative of the national diabetic population due to the relatively small sample size of 84 participants, it does partially substantiate in enabling common themes to be identified and showcases some interesting statistical findings. The findings of this study suggest a general lack of coherent knowledge of diabetic eyecare amongst participants, and provides motivation for a cross-national study to be conducted, involving a greater sample size to achieve more valid and reliable results.

Notwithstanding its limitations, this study makes several recommendations for practical applications on how the implications from the findings may be implemented, in order to improve patient knowledge about diabetic eyecare in the UK. These include the development of

online educational courses for the public, and additional training for primary eyecare professionals.

This study also introduces the idea of implementing patient-focused booking systems in ophthalmic practices to improve patient memorability of their eye examination recalls, and recommends for more written resources to be deployed by ophthalmic practices to encourage patients to actively engage in their eyecare.

The research concludes that ophthalmic practices may need to enhance and develop a patient-centred approach to primary diabetic eyecare, in order to improve patient levels of knowledge and enable better management and prevention of ocular complications relating to diabetes.

IN PRACTICE – DOING THE DAY JOB ANTONIA CHITTY

Excellence in dispensing ometimes we focus on all the extra skills that dispensing opticians (DOs) have. From management to low vision to minor eye conditions services, a DO's role is a diverse one. But it is also important to think about the key role of a DO – and that is to dispense lenses and spectacles. In this article, we speak to two DOs about doing the day job.

Simon Tester is a DO at Iconic Opticians, Plymouth. Reflecting on the start of his career, Simon says: “To be honest, I fell into optics. My mum worked in an opticians when I was 16, so I got some weekend work and started making glasses in the lab. It's now 20 years on and I have been qualified as a DO for 14 of those.”

Simon enjoys the role, he says, because: “For me, every day is different. Every patient has a different set of needs and it is a fresh challenge to make sure those needs are met.”

Whatever the challenge, Simon believes that frame selection is a key skill for the DO. He advises: “Before rushing in, ensure you are listening and using a lot of open questions. Really understand the patient’s hobbies, interests and environments. See each person as an individual. Do plenty of fact finding to build a better picture.

The tools and the technology help – but first get into the patient’s mindset.”

To help the patient have a great experience and really understand their options, Simon likes to use visual tools.

"I can show examples of varifocals, lens thickness, coatings or light reactive lenses. The patient gets an insight and is more able to be part of the decisionmaking process, and make an informed decision. There are some great online tools, but I also like to have examples to hand in practice. The tool that helps the patient gain a bigger understanding is the best one in my eyes.”

Looking at continuing professional development, Simon says: “For me, myopia control lenses, both contact lenses and spectacle lenses, are an exciting new development, which can help to tackle a big problem. I’m trying to learn as much as possible. Twenty years ago, we never would have thought we could slow the progress of myopia – and now we can improve patients’ day-to-day lives.”

"We looked at different ideas, including contact lenses, but decided on an inverse bifocal. He brought his instrument in so we could measure up.

We chose a reasonably deep frame with adjustable pads so we could tweak once they arrived. He found it really successful. It was a tough one, and all credit to the glazing company.”

FRAME SELECTION IS A KEY SKILL FOR THE DO

NEVER A DULL MOMENT Simon works over a couple of locations.

“In the city centre, we have a mix of patients – from young to retired," he explains. "In our Plymstock location, we see more retired people, and run post cataract and glaucoma clinics. With everyone living longer, you see more of these conditions. A lot of the older population are savvier with electronics.

They used to have bifocals but now have a whole new range they need to see with a single solution. Meeting those different demands can be a challenge, but can be met with success.”

Of some of the patients he has seen, Simon reflects: “We had a gentleman who played the trombone in the Royal Marines. It was two weeks before Armed Forces Day, and he was due to be on parade. He needed to see the music on his instrument above eye level and drop

During the pandemic, Simon found himself seeing more younger patients as the local hospital didn’t have a paediatric department and the other local practice was closed. “I remember a three-year-old with Down's syndrome who didn’t like a hat, let alone glasses," he recalls. "It took time, patience and some lovely frames from Erin’s World to get him to wear his +3.00DS prescription. He was very happy in his glasses in the end.”

Summing up with advice for other DOs, Simon says: “There are times you will be frustrated, but there is a network of other DOs to talk to and rely on, plus support from ABDO. I’m looking forward to more face-to-face interaction as the pandemic settles. No matter who you work for, we all help each other out.”

He continues: “The greatest part of this job is that it evolves. There is never a boring day. We are in a time when people

with age-related macular degeneration can have a chip in their retina and there are 3D-printed prostheses. So much research is going into the world of optics.

DOs may feel forgotten in this, but we help with the outcome for the patient that makes such a difference to their dayto-day life. It is a wonderful career, full of lifelong learning.”

other days I was up until 10pm waiting for deliveries but the whole experience has been humbling. Now at Allegro we cover several hospital trusts plus several private sector hospitals and have a team dispensing this range. The clinics see 25 to 30 patents a day. And then you go back to the practice to do everything else. It is full on.”

DISPENSING ON THE FRONT LINE

Kim Walker works at Allegro Optical and was named Dispensing Optician of the Year 2021 in the Optician Awards after an exceptional year in which her job transformed due to the pandemic.

Kim says: “Just over a year ago, I was contacted by Mike Hatch from Bollé Safety. They had developed specialist sealed unit safety specs to protect frontline workers from Covid, and Mike asked if I would be interested in going into hospitals and dispensing them. I wanted to support frontline hospital workers. I had been a patient in the first Covid wave and had seen the struggles they went through with fogging.

“Bollé offered two frames, with skull grip, a fixed bridge and a very good adjustable strap, which can go in the washing machine – a key benefit,” Kim continues. “They are very comfortable, available as single vision and varifocal and they can be disinfected with wipes. For me, the anti-fog coating was a gamechanger.”

Kim went onto the frontline quickly after that with Bollé and started dispensing more than 40 patients a day with prescription safety goggles.

She says: “Some days I got snowed in,

Looking back on how she got into optics, she says: “My sister was born with spina bifida, which gave me the motivation to be a physiotherapist. I was studying anatomy and physiology aged 17 and got a part-time job in Sparrow Opticians.

"At that time, the owner Mr Hampshire was very influential in optics locally. He owned three practices and worked in the hospital. Mrs Hampshire contacted me and said that I had a flair and passion for optics. Around that time, the physiotherapy course I was aiming for was cancelled due to lack of applicants. I had people who believed in me, and I applied for dispensing at Bradford. I’m still in contact with the Hampshire family, and they are so proud of me right now.”

Kim really enjoyed her training. She says: “I love biology and maths, and I wanted to help people. What better job to do? I loved it then and I still love it now. It has challenges, but I feel great pride in it.

With my recent work, I have had NHS consultants in tears thanking me for enabling them to finally see properly to do their jobs. And when you put a pair of

glasses on a baby and they can see for the first time, they don’t hide their shock and happiness.”

Kim has also had her fair share of more challenging dispenses. She explains: “I have done a monocle for near use for a sergeant major who had had laser surgery. I had to glaze it myself. And Mr Hampshire himself would only wear trifocals. To dispense a trifocal is quite interesting.”

On low vison, Kim says: “There is always something new to learn. I’m also interested in paediatrics. Two months old is the youngest child I have seen. I love seeing them grow. There was one little boy who would always break his glasses, so I saw him regularly and he would bring in the pink ones from his pocket money sweets. He was such a lovely little boy; that’s 20 years ago now, but you remember things like that.”

For any DO who has found working through the pandemic a struggle, Kim has this to say: “I try to start every day afresh. The day before could have been horrific; it is challenging working with stressed people in the NHS. But we will get through this. If you work hard, work well as a team, you will make it through.

"Everyone needs their sight and our support, and I try to help as many people as I can on a day-to-day basis. It is a short life and you have to do something that you love. I have loved every minute of my career so far," Kim adds.

I had a dream...

A bout to return to optics after a four-year absence, I wasn't surprised to be rudely awoken at 4.50am by a poignant dream. So much was swirling around in my head – mainly due to fear, anxiety, Covid and the unknown related to how the industry would have changed. Little did I realise that the dream was trying to tell me something about my recent encounters.

There were no questions relating to me, my feelings or suitability for the job.

My reaction to this erupted from a position of anxiety and nervousness, progressing to sheer anger and annoyance. In my dream, I demanded to have the interview I deserved – insisting that the process be a fairer, more

SUCCESSFUL INTERVIEWS SHOULD BE A TWO-WAY STREET

As I chartered the recruitment process for my return to optics, I had been taken aback to find myself being treated as a 'newcomer' to optics – rather than an experienced dispensing optician with knowledge and experience dating back to 1987. This had made me feel out of touch and out of date with advancing technology – and astutely aware of how I was being assessed as a potential employee.

Whilst out of the profession, I worked in many unskilled roles whilst caring part-time for my mother. One of my bosses was particularly unapproachable and only interested in the business and himself. He had zero regard for me, my ideas, concerns or feelings. The working relationship broke down after eight weeks, and it was totally unfulfilling on all sides.

In my dream, this ex-boss sat facing me – firing off questions about my

structured two-way interaction. To my surprise, after initial resistance and anger, my ex-boss began to see the sense in having an open, clear and equal dialogue. He realised that, in the longrun, this was needed for a positive and productive working relationship.

Of course, this was only a dream– but it helped me recognise that my interviews in the real world were actually very one-sided. I was not being allowed to express myself nor truly understand whether or not the potential employer and I would be a good fit. Something needed to change in order to make the recruitment process much less of a lottery.

A PERSONAL APPROACH In my opinion, any recruitment and/or interview process should have a clear set of structured and relevant questions; ideally reciprocated and open. This is not

just to ascertain one's academic skills or practical knowledge, but to delve more into each party's personality. In any relationship, it is necessary to learn about interests, motivation, fears, likes and dislikes, expectations and so on.

A developing working relationship is no different – because a sense of ease, familiarity and openness allows learning to become easier in the long-term. We become more capable by feeling we can learn in a positive environment, leading to greater job satisfaction and productivity.

This thinking, and my recent recruitment process experiences, led me to devising a sample set of questions for any first interview process: 1. If not this job, what other job would you like to apply for?

2. Three years ago, what were you doing and how have you changed?

3. What gives you the most satisfaction in your work nowadays?

4. What do you find most difficult in the workplace?

5. What aspect of your work do you feel you do best?

6. Outside of work, what gives you the most pleasure and satisfaction?

7. What are your aspirations for the future?

8. Who do you admire most – and dislike most – in the world and why?

9. What has been your greatest achievement and worst disappointment?

10. If you had one wish, what would it be?

The above questions allow barriers to be relaxed and a lot of very useful information to be gained in terms of character and personality traits. Both parties can be open and equal, and already a bigger picture can emerge. It's a structured way to build a solid personal foundation prior to deciding whether compatibility exists, before moving on to more specific questions about job suitability.

When I was 21 and sitting in the managing director's office in Perth, Western Australia, I felt particularly relaxed and comfortable applying for a dispensing optician position. In my eyes, Mr S was the perfect boss who, from the outset, made me feel comfortable and willing to put the effort in and do my best for him. It was simple. All he did was put me on his level by insisting I called him by his first name, take my tie off so I was more relaxed, and always be conscious that the work I was about to do was important – yet secondary to life, my family and my health.

Since then, I have worked for hundreds of managers and directors – and only one other individual features in such a truly positive light. When I had financial and personal worries, I expressed my concerns to this manager and subsequently received a visit at home by the company director.

Not only did he remember my wife's and daughter's names, but he came along with his accountant to discuss my salary, which I had expressed concern about. Having listened to my personal circumstances, he not only decided to significantly raise my salary but told me a very personal story about his wife and how stress had affected their life. He then gently offered me advice about how I could best manage mine.

It was all about him taking an interest in me, valuing me and making me feel special. The salary increase was great, but after being made to feel so valued I'd probably have worked for him without the increment.

DEVELOPING STRONG BONDS It's not rocket science, but 30 years on I am still experiencing interviewers like the boss in my dream who are on the completely wrong path. It should be all about being comfortable, having a sense of belonging and feeling valued. The development of any working relationship should be structural, regular and continuous.

In the same way we adhere to continuing professional development, I believe we should also have compulsory development of working relationships.

The sample interview questions mentioned earlier are just a few examples, which can be expanded and modified once the working relationship has been established over time.

It's equally important to develop strong, open and reciprocated bonds with our patients, allowing them to get

to know us better without the sort of boundaries we see with the old schoolmaster/pupil scenario. Does your practice encourage regular interaction with patients via structured questionnaires – giving patients the freedom to enquire, suggest and gain information so they feel more valued and involved in their eyecare? This would surely help them feel part of the 'practice family' with the ability to interact freely and frequently.

Having recently been introduced to a patient payment plan in an independent practice, I thought this was a great start.

It encouraged patients to frequent the practice more often – yielding an improved clinical experience as well as a better bottom line. But it's only one part of the relationship building process.

Once again, by enabling structured and regular two-way communication, the practice and patient could significantly benefit. The latest developments, promotions, new lens and frame technology and general updates concerning staff, for example, would allow greater involvement. Patients would be encouraged to provide feedback on matters concerning them; how they could be better serviced, educated or informed about their overall eye health well-being.

As a suggestion, I would like to see practices devise a routine questionnaire to send to all patients approximately every six months – if they don't already.

This would provide 10 questions requiring feedback, and a comments box. Typical questions might include: 1. Do you feel we provide a professional, yet comfortable, environment for you – and is there anything we could do better?

2. Do you think we are value for money?

3. Do we have a good frame selection?

4. Was the eye examination thorough and well explained?

5. Were you given good information and was everything well explained?

6. Were the staff friendly and helpful?

7. Would you recommend us to others?

8. Were we punctual and on time ,and were you made to feel valued?

9. Was the practice clean and tidy, and well presented?

10. Is there anything else you would like to comment on or ask about?

Regularly asking questions such as these will help the practice – and its staff – develop a closer relationship with

patients and respond to feedback. Many practices assume their patients are satisfied with the service they receive, but we cannot be 100 per cent sure of this unless we ask. Keeping detailed records will also help to continually improve standards of patient satisfaction. This process should be ongoing and viewed by all in practice as a 'must' rather than a 'maybe'.

DO YOU REGULARLY CANVASS PATIENTS FOR FEEDBACK?

CONCLUSION So, concluding my recent findings on my return to optics after a considerable absence – and thanks to my rather vivid dream – I'd like to highlight my personal recommendations. These are: 1. Look at the way you recruit staff. Try to visualise being in the prospective employee's position. Have a structured process in place, and provide this regularly throughout the working relationship.

2. Always endeavour to be on the same wavelength and at the same level as the employee. Generate openness, friendliness and ease.

3. Develop and improve patient/practice interaction through regular structured questionnaires, and encourage your patients to provide regular feedback so that you can offer them the best service possible.

I look forward to implementing these myself where possible, as I continue my journey into a very different world of optics to the one I once knew.

Homeward bound E xactly a year ago I wrote about my experiences working for NHS Test and Trace as a clinical case worker (Dispensing Optics March 2021).

As a self - employed contact lens optician (CLO), my practice work had been put on hold due to the pandemic – and this new role kept me busy working from home in the meantime.

Test and Trace in this format came to an end in May 2021 and so it was back to the day job. In 2009, my optometrist wife Denice and I set up a domiciliary eyecare business, which we ran in conjunction with our practice work. We made the decision to concentrate our efforts on this, rather than go back into practice. So as soon as the rules permitted, we dusted off our equipment and hit the road.

Home visits had been off and on for some time and we had built up quite a waiting list. Many people had missed their routine examinations and were eager for us to visit as soon as possible. Their vision had deteriorated, spectacles had been broken and hospital appointments missed; there was a lot of catching up to do.

Patients were comfortable with us coming into their homes; the personal protective equipment, hand washing and equipment disinfection seemed to provide reassurance. Contact time was kept to a minimum by carrying out history and symptoms the day before, over the phone.

Care homes were understandably very strict to begin with, and would conduct their own lateral flow test on us and we would wait in the car until the result came through. More recently, most are happy to admit us if we produce our vaccination certificates and our own lateral flow test result.

DREAM DISPENSING SITUATION It can’t be overstated how much housebound patients need good vision and appropriate spectacles. Most watch the TV a lot, many read, do puzzles or have hobbies such as knitting or sewing. Increasingly they use computers, either PCs or tablets, to occupy their time or to keep in touch with friends and relatives.

Being with a patient in their own environment is a dispensing optician's dream: you can see that patient at their computer desk, in front of their TV, or reading in their favourite armchair. Working distances can be assessed accurately, seg heights or fitting cross positions measured to suit their posture, and lighting assessed and advised upon.

A recent example of this was a lady who struggled to use her computer with her existing bifocals. We got her to sit at her computer desk and it transpired that she was

unable to pull her chair in close enough to the screen due to her restricted mobility – meaning she sat further away than normal. A pair of single vision intermediates solved the problem.

Dry eyes are a very common condition in housebound patients and something we are able to offer advice on.

Their homes are invariably very warm, they often sit close to the fire, and will spend hours either watching TV or reading with the inevitable reduction in blink rate. A full dry eye regime is not always possible for these patients, but good advice and the recommendation of unpreserved eye drops helps to alleviate their symptoms.

We have recently embraced eGOS, which I was sceptical about initially – doubting that we’d get sufficient signal strength to operate the system in many of our locations. Much to my surprise, it’s worked really well.

Only once or twice have I had to wander outside desperately seeking 4G. It now means all our home visits are pre-authorised, and we are made aware if another practitioner has already visited recently, which had been a problem in the past.

HOUSEBOUND PATIENTS DEPEND ON THEIR VISION AND SPECTACLES

Domiciliary eyecare is not for the faint hearted – and it is physically demanding. We carry five cases of equipment and frames with us, which on one occasion we had to drag up five flights of stairs in a tower block when the lift was out of order. You need to think outside the box when setting up for the examination; the patient may be bed bound for example, or you may need to balance the laptop test chart on a book case or kitchen worktop as very often the patient can’t be moved.

Most domiciliary patients live alone, and before Covid we would often be offered a cup of tea. The patient would welcome the opportunity for a chat as many of them are lonely. It is very challenging work, but also very rewarding when you can improve the patient’s quality of life by satisfying their visual needs.

CRAFT-LOVING DOS ispensing optician Debbie Gregory is practice manager for Kelly Eyecare in Ringwood, Hampshire. For as long as she can remember, Debbie has been fascinated with – and passionate about – colour.

"I studied art all through school and college, taking inspiration for my projects mainly from nature," says Debbie. "I then began crocheting several years ago. This was a new outlet for my creativity and I found it mindful and relaxing too.”

Debbie loves creating new fabrics with yarn. She uses yarn from blankets as well as other media such as wet felting, embroidery, punch needle and rag rugging.

She's currently focusing on up-cycling products.

“I can often be found in charity shops looking for interesting fabrics and yarn to add to my ever-increasing stash," says

DRY EYE DISEASE The optical assistant (OA) is often the first point of contact for a patient who has come to the practice with a problem with their eyes. Although it is not the role of an OA to diagnose, it can be helpful to have some knowledge of the more common eye conditions encountered in practice.

It is important for OAs to have an understanding of different eye conditions, to help ensure the patient receives the best care possible from the appropriate eyecare professional.

The Chief Executive will have a high public profile, will represent the Association on the national stage and be expected to have the political and diplomatic skills to work effectively with other organisations within the sector.
