PRESIDENT’S MESSAGE In December of 1937, a group of general surgeons led by Eslie Asbury, Edgar Gilcreest, Ralph Carothers, and Kellogg Speed collaborated to form a new professional association dedicated to the advancement of clinical and scientific knowledge related to the care of trauma patients. On June 14, 1938, a group of 68 surgeons met in San Francisco and officially founded the “American Association of Traumatic Surgery.” This year, for the first time in 41 years, the AAST returns to San Francisco for the annual meeting, marking the 75th Anniversary of its founding. This is a meeting you do not want to miss.

COME FOR THE CELEBRATION: Several unique exhibits and events will be part of this 75th anniversary including the opening session that will provide special recognition of what I hope will be the largest assembly ever of AAST Past Presidents, a presentation of historical highlights of the AAST by Steve Shackford, a special book published to commemorate the 75 years of the AAST, an “AAST Through the Ages” section including all program books (some will be replicas), hall of presidents, and at the Banquet, a special area to have your photo taken with friends, colleagues, spouses or guests.

COME FOR THE SCIENCE: Listen to original podium, poster, & ‘quickshot’ presentations of the latest studies from work by literally hundreds of investigators: new approaches to the management of specific injuries and critical surgical conditions, transfusion practices, and fresh insights into the biology of shock & inflammation.

Dr. Frank Lewis will give the Fitts Oration this year providing the unique insights of an academic trauma surgeon turned Executive Director of the American Board of Surgery. Drs. Ken Mattox, Rao Ivatury, and Jack McAninch will present ‘Master Surgeon’ lectures reflecting their decades of experience and unique perspective.

P R O G R A M C H A I R M A N ’ S M E S S AG E Dear Members of the AAST, The program committee is excited about the scientific quality of the abstracts selected for presentation during the 72nd Annual Meeting of the AAST in September in San Francisco.

This year, to recognize the efforts of those who did great research but missed the oral podium presentation by a small margin, we will have a new session called “quick shots.”

Twenty abstracts have been selected in this new category. Authors will have three minutes to present a maximum of five slides in a very focused and objective manner. An expert on the subject will ask two very objective questions. Presenters are encouraged to submit a manuscript for possible publication in the Journal of Trauma & Acute Care Surgery, although it is not mandatory.

I hope you are excited, as much as we are, to see the 20 best posters presented orally, as a quick shot, during the annual meeting. In addition, 65 oral presentation, three master surgeons lectures, two panels, Fitts oration and president’s address, 12 lunch sessions and 120 posters will complete the program.

OVERVI EW SCIENTIFIC PROGRAM PRESENTATIONS/PANELS This year 66 oral presentations will be offered over four days. Each presentation will be 20 minutes in length; 10 minutes will be allotted for the presentation and 10 minutes for questions and discussion from the audience.

NEW: Quick Shots. On Friday, September 20th from 9:10 - 11:10 am 20 quick shots will be presented. Each presenter will have three minutes and five slides to give a brief background and a discussant will have 1 minute to ask two questions, with the remaining two minutes for the presenter. Don’t miss this dynamic new session.

SCIENTIFIC POSTERS A total of 120 scientific posters will be available for viewing beginning Wednesday, September 18th at 1:00 pm through Thursday, September 19th at 3:00 pm. Poster categories and professors will be listed in the final program book available on site at the meeting. The Poster Rounds will be held on Wednesday, September 18th from 5:00 – 7:00 pm.

INDUSTRY EXHIBITS At this year’s meeting AAST will host an array of industry leading companies. President-Elect William G. Cioffi will give a short presentation. This is also an opportunity for residents, students and in-training fellows to network with each other. This breakfast requires pre-registration and is a ticketed event. Please check the box either online or on the mail in form if you are planning to attend. There is no cost to attend this breakfast.

International Attendee Breakfast – Friday, September 20th from 6:30 – 7:30 am AAST invites all international attendees to a special breakfast to learn more about the AAST, network with other international attendees and meet the International Relations Committee. online or on the mail in form if you are planning to attend. Ari Leppäniemi, MD, President-Elect, of European Society for Trauma and Emergency Surgery (ESTES) will be presenting. There is no cost to attend this breakfast.

New Fellow Breakfast – Saturday, September 21st from 7:00 – 8:00 am This special breakfast is for new members who have joined AAST within the last two years. AAST encourages all new members at the meeting to attend. This is a chance to meet the new AAST President, as well as, other members of AAST Board of Managers. FITTS & MASTER SU RGEON LECTU R ERS Fitts Lecturer - Frank Lewis, MD Dr. Frank Lewis became executive director of the ABS in 2002. A Maryland native, Dr. Lewis received his medical degree from the University of Maryland and completed his general surgery residency at the University of California–San Francisco.

He then completed a NIH trauma fellowship at San Francisco General Hospital. Prior to the ABS, Dr. Lewis was chair of the department of surgery at Henry Ford Hospital in Detroit and professor of surgery at Case Western Reserve University.

Before going to Detroit, he was professor and vice chair of the department of surgery at UC San Francisco and chief of surgery at San Francisco General Hospital.

Dr. Lewis is a past chair of the ABS and the Residency Review Committee for Surgery of the ACGME. He is also a past president of the American Association for the Surgery of Trauma and the Shock Society. He has also served as first vicepresident of the American College of Surgeons (ACS), as well as chair of the ACS Board of Governors and the ACS Committee on Trauma. Dr. Lewis' clinical interests center on trauma and critical care. He is certified by the ABS in general surgery.

“Pressure, Perfusion and Compartments: Challenges for the Acute Care Surgeon” - Rao R. Ivatury, MD Dr. Rao R. Ivatury is the immediate past Chief of the Division of Trauma, Critical Care and Emergency General Surgery and is now a Professor Emeritus in the Department of Surgery at Virginia Commonwealth University in Richmond. A graduate of one of the oldest Medical Schools in India, he did multiple surgical residencies in India and the U.S. His love for Trauma and Critical Care stems from two decades he had spent in the South Bronx ( the “Fort Apache: The Bronx”) during one of its most violent times. This experience was the foundation for many of the concepts he promulgated in these massively injured patients e.g. “Damage-control” surgery, the need for open abdomen management, the relevance of intra-abdominal hypertension and abdominal compartment syndrome, tissue specific endpoints of resuscitation. He co-edited the only text book on penetrating trauma and the monograph on “Abdominal Compartment Syndrome.” He has nearly 300 publications, 100 abstracts, 30 chapters and three books to his credit. He gave more than 500 invited presentations nationally and internationally.

Dr. Ivatury served the AAST on several committees and was the AAST representative for the Board of Governors of ACS. He was the VicePresident of the AAST and is the current Chair of the ad-hoc committee on Education/MOC.A past President of the Panamerican Trauma Society (PTS), he is the current Executive Director of PTS. He is the current President of the World Society of Abdominal Compartment Syndrome. His passion is teaching and operating.

“The Acute Care Surgeon’s Role in Vascular Trauma” - Kenneth L. Mattox, MD Dr. Kenneth L. Mattox is one of the most recognized surgeons in America. His reputation as an innovator in trauma care is worldwide. He has made original and significant contributions in trauma resuscitation, trauma systems, thoracic trauma, vascular injury, autotransfusion, complex abdominal trauma and multi-system trauma. The “Mattox Maneuver” for abdominal aortic injury was used internationally in years past. His research in preoperative fluid restriction for penetrating trauma shook the foundation of surgical doctrine in this area.

He has been a visiting professor or consultant to more than 800 medical schools, hospitals or health care systems throughout the world.

For the past fifteen years,he has been Program Director of the LasVegasTrauma,Critical Care,andAcute Care Surgery Postgraduate Course, the best attended and most acclaimed trauma conference in the world. In 2008, he added the directorship of another international conference to his “docket,” Medical Disaster Response. In its inaugural year, the course sold out. Doctor Mattox is a member of over 30 professional organizations, and is Past President of the American Association for the Surgery of Trauma, Secretary-Treasurer of the Michael E.DeBakey International Surgical Society,and Past President of the Houston Surgical Society andTexas Surgical Society.He was just named Vice President of the American Surgical Association.

“Genitourinary Trauma - 2013” - Jack McAninch, MD Dr. Jack McAninch received his undergraduate BS degree at the Texas Tech University, an MS degree from the University of Idaho, and his MD degree from the University of Texas Medical Branch where he was an honor graduate and recipient of the Gold Headed Cane Award in recognition as the outstanding graduating senior. He received his general surgery and urology training at Letterman Army Hospital where he was recognized as the outstanding Resident Trainee and received the Robert B. Skelton Award. He is certified by the American Board of Urology and joined the University of California San Francisco (UCSF) faculty in 1977. He currently serves as Chief of Urology at San Francisco General Hospital and Vice Chairman of the Department of Urology.

“Organizing Acute Care Surgery, From National to Hospital Level” - Ari Leppäniemi, MD Ari Leppäniemi is Chief of Emergency Surgery at the Helsinki University Hospital, Finland. He is the Past-President of the Finnish Society of Surgery, International Association for Trauma Surgery and Intensive Care (IATSIC), and the European Association for Trauma and Emergency Surgery (EATES), and the current Vice-President of the European Society for Trauma and Emergency Surgery (ESTES). He is the Editor-in-Chief of the Scandinavian Journal of Surgery, Editor of the European Journal of Trauma and Emergency Surgery, and Associate Editor of the World Journal of Surgery. He has published over 120 original articles and more than 200 reviews and book chapters, mostly on abdominal trauma, acute pancreatitis and abdominal compartment syndrome. His hobbies include fishing, badminton, and jazz.

“A Career in Trauma: So Many Options” William G. Cioffi, MD, AAST President-Elect Dr. Cioffi is Professor and Chairman of the Alpert Medical School of Brown University. He graduated from the University of Vermont College of Medicine in Burlington in 1981 and completed his residency in general surgery at the Medical Center Hospital of Vermont in 1986. Before joining the Brown Medical School in 1994 as Chief of Trauma and Burns, he was the Chief of the Burn Study Branch at the Institute of Surgical Research at Fort Sam Houston, Texas.

Dr. Cioffi is the Surgeon-in-Chief of Rhode Island Hospital and The Miriam Hospital. He serves on various hospital, university and national committees dedicated to improving patient care for critically ill surgical patients. A board certified general surgeon with added qualifications in surgical critical care, Dr. Cioffi’s clinical expertise is in the areas of pancreatic, colorectal and trauma surgery. He has authored or co-authored more than 430 articles, book chapters and abstracts and continues to lecture nationally and internationally in his field.

Dr. Cioffi’s research interests include shock, sepsis, trauma and burns.

Open Access and New Standards in Scientific Publishing

Management of the complex dismounted blast injury: translatable lessons learned from the military’s experience with the management of pelvic, rectal, lower GU, lower extremity and massive soft tissue injuries

Specific subsets of grievously injured casualties wounded by an IED who are dismounted sustain penetrating and blast damage to the perineum/buttock/pelvis. These casualties represent a complex constellation of specific problems which demand prompt decision making by the surgical team regarding priorities. These patients sustain soft tissue injury to the buttock, blast fractures of the pelvis, penetrating if not destructive injury to the anus and rectum, the urethra, scrotum and its contents, sciatic nerve, major junctional blood vessels, intra-abdominal penetration, as well as commonly one or more traumatic amputations.

The focus of this review would be on severe Dismounted Complex Blast Injury (DCBI), which have been defined as combatants who survived high-energy injuries to the bilateral lower extremities (usually proximal transfemoral amputations) and/or upper extremity (usually involving the non-dominant side), in addition to open pelvic injuries, genitourinary, and abdominal trauma. Initial resuscitation and multidisciplinary surgical management appear to be the keys to survival.

and Thomas Scalea, MD Although penetrating thoracic trauma is relatively common in America’s Trauma Centers, life threatening thoracic injuries requiring immediate lifesaving surgical interventions are uncommon and account for approximately 10-15% of the cases.

For these critically injured patients to survive, they require an organized approach including rapid transport to the operating room, often the activation of a massive transfusion protocol along with trauma surgeons that possess the appropriate operative skills to deal with cardiac, pulmonary, thoracic vascular and esophageal injuries. Life threatening thoracic injuries also pose unique challenges for the trauma surgeons with regards to timing and the incisions required to access them.
