MESSAGE FROM THE CEO AND BOARD CHAIRMAN The 2014-2015 fiscal year brought accomplishments, challenges, and tremendous growth for our nine core programs. Adopt-A-Family provided services to 2,166 families in Palm Beach County–this is more than a 20% increase from 2013-14. Unfortunately, this increase also clearly demonstrates the needs in our community, which continue to outweigh available resources. As rental costs outpace wages in Palm Beach County, more families find themselves homeless or at risk of homelessness. At Adopt-A-Family, we are committed to doing our part to make a difference in the lives of families that are in these situations.

The most exciting news of the 2014-2015 year relates to our afterschool program, Project Grow.

In the fall of 2014, our Project Grow play space transformed from trampled sod to a multipurpose turf fitted with a brand new basketball court. Outlined in this report, you will read more about this project and the difference it has made in the lives of the 60 children who call Project Grow their home.

As we look to the future, we plan to take ambitious steps to develop, create, and enhance affordable housing solutions for Palm Beach County families. Additionally, we will continue to work tirelessly to ensure that the supportive services and case management we provide as an agency continue to make an impact on the families we proudly serve.

OUR YEAR JULY 2014 Through generous funding support from the Gerstner Family Foundation and the Cathleen McFarlane Foundation, the Housing Stabilization Program doubled its capacity to four full-time case managers working to prevent families from becoming homeless in Palm Beach County.

OCTOBER/NOVEMBER/DECEMBER 2014 Adopt-A-Family completed another successful holiday season spotlighted by the success of our largest fundraising event, the 30th Annual Tree Lighting Celebration. 2014 marked a historic year, in which $400,000 was raised in support of our agency. Additionally, through Santa Shoppe and the Holiday Adopter Program, hundreds of our families were supported by generous donors to ensure they had a special holiday with gifts and treasures.

AUGUST/SEPTEMBER 2014 Project Grow’s playground transformation was completed, creating a state-of-the-art green space and a brand-new basketball court for 60 students in our innovative afterschool program.

JANUARY 2015 Adopt-A-Family honored the Frankels, the driving force behind the Project Grow Playground Transformation.

FEBRUARY 2015 Adopt-A-Family was selected to participate in the Great Charity Challenge for the second consecutive year. $30,000 was raised in support of our nine programs.

JUNE 2015 APRIL 2015 Adopt-A-Family was also awarded a $20,000 grant from BallenIsles Charities Foundation for school supplies to prepare homeless children for the new school year. This grant supported families served through the Senator Phillip D. Lewis Center.

The Senator Philip D. Lewis Center celebrated its third year of operation. Since opening its doors in 2012, hundreds of Palm Beach County families have been provided permanent and emergency housing options to combat episodes of homelessness. AdoptA-Family operates the family component of the Lewis Center.

Adopt-A-Family finished a Community Foundation-funded project to install impact windows for all five buildings of Project SAFE, creating a safer and more energy efficient environment for 40 formerly homeless families.

MARCH 2015 Adopt-A-Family hosted its 3rd annual Ducky Derby at CityPlace in West Palm Beach raising more than $24,000 for Palm Beach County Families.

Adopt-A-Family was named “Community Partner Champion� 2014-15 by the United Way of Palm Beach County, as a featured funded agency.

We were humbled by this honor.

MAY 2015 Adopt-A-Family earned a ninth consecutive 4-star rating from Charity Navigator, placing our charity in the top 1% of those rated nationally for sound fiscal management and commitment to accountability.

Adopt-A-Family hosted the 4th Annual Better Ball invitational that raised $100,000 in support of the agency.

Adopt-A-Family finished in second place during Great Give 2015 for total donations out of more than 400 participating Palm Beach and Martin County nonprofits.

B. SOLUTIONS AND SERVICES PROVIDED

A PLACE CALLED HOME A. A Place Called Home (APCH) is a Permanent Supportive Housing program that serves families experiencing homelessness who have a head-of-household living with a disability.

B. Through intensive case management, referrals, and linkage to relevant community resources, APCH fosters a supportive environment for families while maintaining a core principle of stable and affordable housing.

C. There are currently four residents who are pursuing higher education in APCH. High-Impact Funders: The U.S. Department of Housing and Urban Development (HUD).

BRIDGES TO SUCCESS A. Bridges to Success is a HUD-funded program located in Belle Glade, FL. B. The program is a collaborative with Families First of Palm Beach County to administer this Permanent Supportive Housing program for homeless families.

C. In the past year, 100% of families in the Bridges to Success program either exited to permanent housing or remained in permanent supportive housing.

PROJECT GROW A. Project Grow is an afterschool and out-of-school program which fosters a stable and educational environment for children in kindergarten through fifth grade.

B. Project Grow is customized to meet the unique challenges of 60 formerly homeless and low-income youth in the Lake Worth community. The program builds on the children’s social, emotional, and educational skills to instill the confidence necessary for a healthy childhood. The program incorporates a dynamic STEAM (science, technology, engineering, arts, and math) curriculum that introduces career fields of the future to our students.

C. Project Grow continues to meet and exceed goals set forth by Palm Beach County’s Quality Improvement System (QIS). In May 2015, the program earned a 5th consecutive “Maintenance Level” rating during the Prime Time Afterschool Symposium—a designation only 11 programs of the 120 in Palm Beach County have achieved.

Since November 2013, teachers in Project Grow have administered more than 400

hours of STEAM-

oriented activities. During the 2014-15 school year, 96% of Project Grow students were successfully promoted to their next grade level, exceeding local and national averages.

High-Impact Funders: Battelle Memorial Institute, Early Learning Coalition of Palm Beach County, and The Town of Palm Beach United Way.

HOUSING STABILIZATION PROGRAM A. The Housing Stabilization Program (HSP) is an innovative homeless prevention program that targets families who have fallen behind on rent or mortgage payments due to extenuating circumstances.

B. The program provides short-term financial assistance to families so they can overcome their immediate housing crisis.

Additionally, each family receives case management, budgeting assistance, and access to community resources in order to address life barriers that may prevent them from remaining stably housed after agency intervention.

C. Due to generous support from prominent local and national foundations, HSP has doubled its capacity in the past year. In the most recent year, HSP prevented 181 Palm Beach County families from becoming homeless.

NEIGHBORHOOD STABILIZATION PROGRAM 2 A. In 2009 the Lake Worth Community Redevelopment Agency (LWCRA) partnered with Adopt-A-Family as a sub-grantee for Neighborhood Stabilization Program 2 (NSP2) to rehabilitate, sell, and oversee affordable housing rentals in the community.

B. The program’s initial goal was to halt decline in targeted, low-income neighborhoods with high foreclosure rates and to influence positive community development. Adopt-A-Family provides affordable rent to each family participating in the program at a rate of $575-$725 per month, far below Fair Market Rent in Palm Beach County which is $1,206 for a twobedroom apartment. (Out of Reach, 2015) C. In total, Adopt-A-Family sold 17 homes through NSP2 and currently operates 24 affordable rentals for hardworking income-constrained families in Lake Worth.

PROJECT SAFE A. Project SAFE, Adopt-A-Family’s longest running Permanent Supportive Housing program, provides a safe, consistent, and nurturing environment for 32 families experiencing homelessness.

B. Each resident of Project SAFE receives support from an on-site case manager. This includes providing families with access to financial literacy training, educational opportunities, mental health referrals, life-skills training, and affordable childcare.

C. Project SAFE was the recipient of a unique grant award through the Community Foundation for Palm Beach and Martin Counties. The grant provided funding for the installation of state-of-the-art, impact windows for all five buildings in the program. This upgrade allows for a safer and more energy efficient environment in Project SAFE. High-Impact Funders: Community Foundation for Palm Beach and Martin Counties, HUD, FAA, The United Way of Palm Beach County.

High-Impact Funders: BallenIsles Charities Foundation provided a unique grant in support of 200 formerly homeless students to prepare them for the school year.

IN THE SUMMER OF 2015, – The Lewis Center celebrated its – 3 -YE AR ANNIV E R SA RY.

SERVICE ENRICHED HOUSING A. Service Enriched Housing (SEH) is an innovative housing program for low-income households dedicated to the pursuit of increased financial stability and homeownership.

B. SEH provides safe and affordable housing for 28 families who meet program requirements. Families work collaboratively with the program manager to address credit obstacles, eliminate debt, and learn the necessary steps to become a homeowner.

C. Through intensive credit counseling, budgeting, and achieving financial goals, the program remains a successful model to reach homeownership. Since January 2014 five families have become homeowners. Additionally, current program participants have eliminated $68,000 in debt over the past year—which will directly impact their ability to purchase a home.

WILEY REYNOLDS GARDENS APARTMENTS A. Wiley Reynolds Gardens Apartments is a nine-unit housing program that provides safe and affordable housing to families experiencing homelessness.

SENATOR PHILIP D. LEWIS CENTER A. The Senator Philip D. Lewis Center is the centralized point-of-entry for families and individuals experiencing homelessness in Palm Beach County.

Adopt-A-Family coordinates the family component of the Lewis Center.

B. Through assessment, housing placement, and comprehensive case management, the Lewis Center team provides critical resources to our community’s families who are most in need.

C. Through Rapid Re-Housing, Permanent Supportive Housing, and emergency housing options, the Lewis Center team provided housing solutions to more than 300 families in the most recent fiscal year.

The team also navigated more than 1,800 families providing a link to community-based resources.

B. Social services are provided through a collaborative with Families First of Palm Beach County. Rent is calculated at 30% of each resident’s gross income, which allows families the opportunity to attain self-sufficiency and to pursue life-goals such as an advanced education.

C. In the past year, 89% of the head-of-households in Wiley Reynolds Gardens Apartments were employed full-time.

AAF PROGRAM FACT: In 2009, The Florida Redevelopment Association awarded Wiley Reynolds Gardens Apartments with the prestigious Roy Kenzie Award for Outstanding Affordable Housing Project in the state of Florida.

ADOPT-A-FAMILY ANGELS HONORING TOM & MARI FRANKEL

Adopt-A-Family was proud to honor Tom and Mari Frankel in January 2015 for their dedication to the enhancement of Project Grow.

For nearly 20 years, the Frankels have been visionary advocates for Adopt-A-Family’s afterschool program, Project Grow. In the fall of 2014, their vision was realized as our play space transformed from trampled sod to a beautiful, clean, multi-use green space that included a brand-new basketball court. Our Project Grow students couldn’t be happier with their new playground and an incredible space to call their own.

With the new multi-use space, Adopt-A-Family’s Project Grow has conducted dozens of fun and creative afterschool activities on the new playground. This includes interactive sessions by Kidding Around Yoga, Digital Vibes, Florida Fish and Wildlife, STEAM (science, technology, engineering, arts, and mathematics) outdoor activities, soccer games, and many more. See photos on the next page that show how the Project Grow play space has come to life.

EVENT HIGHLIGHTS

30TH ANNUAL TREE LIGHTING CELEBRATION BREAKS FUNDRAISING RECORD 2014 marked the historic “Pearl Anniversary” of Adopt-A-Family’s Tree Lighting Celebration. The dedicated Tree Lighting Committee, co-chaired by Beth Hennessy and Didi Smith, raised more than $400,000 in support of Adopt-A-Family programs. It was an incredible evening filled with laughter, holiday songs, and warmth.

In all, more than 250 guests were on hand at the esteemed Sailfish Club of Florida for a night of fine dining, fun, and philanthropy.

Tree Lighting featured its well known array of live orchids, holiday décor, themed gift baskets, and multiple auction formats. Featured live auction items included tickets to Saturday Night Live and Jimmy Fallon including airfare, as well as private entertainment for ten with Chef Carrie Hanna. The event also featured charismatic auctioneer Tim Luke from HGTV’s “Cash in the Attic.”

Adopt-A-Family sends gratitude to The Edward L. Hennessy, Jr. & Ruth S. Hennessy Foundation as well as Linda & Ed Edelman for serving as Grand Benefactors. Adopt-A-Family is incredibly grateful for the work of the dedicated Tree Lighting Committee for making this event a historic success.

DUCKY DERBY 2015 On March 8, 2015, over 300 people gathered for the 3rd Annual Ducky Derby at CityPlace. Adopt-A-Family sold nearly 6,000 ducks and raised $24,000 to benefit Palm Beach County families in need. Thanks to Ashleigh Walters from WPTV for emceeing the festivities.

3RD ANNUAL PHYSICIANS TALENT SHOWCASE

Adopt-A-Family’s (AAF) Santa Shoppe and the Holiday Adopter Program are unique holiday events that benefit hundreds of children and their families in AAF programs.

In 2014, our generous community donated more than $40,000 to ensure children in our programs could have a special holiday that they otherwise could not. This season, more than 240 children and their families benefited from Santa Shoppe while Holiday Adopter benefited 139 families with children.

The 3rd Annual Physician’s Talent Showcase raised over $20,000 for Adopt-A-Family’s Project Grow program. Thank you to Kathi Kretzer, President of the Kretzer Piano Music Foundation, the generous sponsors, and the talented physicians who made this evening a success.

SECOND PLACE GREAT GIVE 2015 FINISH Thanks to everyone who participated during this year’s Great Give of Palm Beach and Martin Counties. Due to your support, we exceeded our goal of 150 unique donations, finishing with 181 on and offline donations. This earned us a 2nd place finish for total, unique donations out of 437 participating nonprofits. Including matching dollars and offline donations, Adopt-A-Family raised more than $17,000 for the Great Give. Adopt-A-Family is humbled to achieve this milestone that will ultimately benefit Palm Beach County families in need. Thanks to all who reached out to friends, family, and their support networks to ensure Adopt-A-Family had a strong showing for this unique fundraising event.

A special thank you to the United Way of Palm Beach County, United Way of Martin County, and Community Foundation for Palm Beach and Martin Counties for spearheading this year’s efforts once again. In total, nearly 11,000 donors helped raise over $3 million dollars for local nonprofits.

2015 BETTER BALL INVITATIONAL The 4th Annual Better Ball Invitational marked another successful year for Adopt-A-Family and the dedicated Golf Tournament Committee, grossing more than $100,000 for families in need. This is the 4th consecutive year in which total donation amount increased for the tournament. On Friday, May 1st, 80 golfers and supporters of Adopt-A-Family gathered at the exclusive Pine Tree Golf Club for an incredible day of golf, lunch, and cocktails. Congratulations to Ray and John Lazzara, who won the tournament with a net score of 64.

A FAMILY SUCCESS STORY AND 9TH CONSECUTIVE 4-STAR RATING examples of her peers and made many poor life choices.

It was after the loss of a close relative that Carolyn vowed to turn her life around.

Today, Carolyn has earned her high school diploma and graduated with her Associates Degree in December, 2015. She is also working towards obtaining her Bachelor’s Degree in Human Services from Palm Beach State College and hopes to help others find sobriety.

Carolyn Tirado, a Holistic Therapy Assistant at The Treatment Center, and her sons, Christopher and Phillip, joined the Service Enriched Housing Program in November of 2012. Prior to joining the program,

the family was renting a small one bedroom apartment in Lake Worth and was looking for a better living opportunity. Carolyn had struggled for many years to reach the point where owning a home of her own could be a realistic goal. Carolyn lacked stability when she was younger--her family moved from neighborhood to neighborhood throughout her childhood. At age 11, she then lost her mother to cancer. Without proper parental guidance and role models, she followed the

Receiving 4 out of a possible 4 stars indicates that Adopt-A-Family adheres to good governance and other best practices. Only 1% of charities rated nationally have achieved this distinction. This achievement is a reflection and testament to our incredible supporters, funders, board of directors, staff, and families we serve.

During her time at Adopt-A-Family, Carolyn worked very hard to save for her closing costs and address her credit obstacles so she could purchase her first home.

She has done this despite a recent cancer scare.

On March 30, 2015, Carolyn reached her goal when she purchased a three bedroom/two bathroom home from Habitat for Humanity of Palm Beach County in Jupiter. To obtain an interest free mortgage, the family completed 400 hours of sweat equity that included Carolyn and her sons participating in the construction of the home. The students at Seminole Ridge High School also had a part in Carolyn achieving her dream.

They began the construction of the home at their high school in the winter of 2014. Congrats Carolyn!
