Services Offered

Provides stable housing opportunities for veterans, families, unaccompanied youth, and adults with chronic disabilities.

This program provides transitional housing for males, females and married couples. Rent is a straight daily, weekly, or monthly fee and utilities are included. Residents are required to attend church on Sundays and Wednesdays. It also finds housing for families, offers educational resources, and provides full meals on holidays.

This program provides shelter beds for single women and women with children. Bathing and laundry facilities can also be found here. Temporarily closed for construction –clients being referred to My Sister’s House.

Transitional Housing for men aged 18 and older. Residents pay a $10.00/day fee that provides them with dormitory style beds, clean sheets, toiletries, hot showers and a safe drug free environment.

The Foundation 3 program helps individuals and working families experiencing, or on the brink of, homelessness establish a solid financial foundation by targeting three essential areas: safe housing, stable employment, and education. While enrolled, participants receive rental and utility assistance for 12 months. Weekly case management (includes careerbased education, employment services, etc.) required.

MUST is a place to begin to recover from financial disaster, discouragement or job loss. Between the Emergency Shelter and Supportive Housing, MUST serves as a place of refuge for many in our community.

Transitional supportive housing for homeless men or men facing homelessness through the Hope House. Services are targeted to address the needs of individuals with cooccurring disorders such as posttraumatic stress disorder (PTSD), deaf or hard of hearing, HIV/AIDS, and mental illness. Long Term Housing is also provided here.

This program is for families and individuals who are homeless during extreme temperatures, or during severe weather storms, or who have a chance to gain employment but need an opportunity to eat, rest and groom themselves before starting. We also help families whose housing won’t be ready in time to keep them from living on the streets.

The mission of the program is to help people in economic crisis help themselves. Funds come from a variety of sources with varied forms of eligibility. During this COVID-19 pandemic, additional funds have been made available for families in need.

This program helps folks with rent/ mortgage and utility payments as well as with Rapid Rehousing. Families and folks must demonstrate that they are able to become stable with this assistance. They are open TuesdayFriday from 10am to 5pm. PLEASE CALL FIRST.

Helpful Terms and Definitions

• Safe Haven – A project that offers supportive housing that (1) serves hard to reach homeless persons with severe mental illness who came from the streets and have been unwilling or unable to participate in supportive services; (2) provides 24-hour residence for eligible persons for an unspecified period; (3) has an overnight capacity limited to 25 or fewer persons; and (4) provides low demand services and referrals for the residents. (HUD Definition)

• Transitional Housing – is that which assists people transition from homelessness to greater independence and self-sufficiency. Typically, a person/family who is homeless will move into transitional housing as part of a comprehensive program with selfsufficiency as the end goal. Transitional housing usually offers supportive services like job training/assistance, GED preparation/tutoring, childcare services and/or referrals, budget/financial management workshops, etc. In most cases, persons/families pay a modest fee or percentage of their income for rent. Short-term housing may be 60-90 days while long-term housing may allow for residency up to 2 years.

• Literally Homeless – (1) Individual or family who lacks a fixed, regular, and adequate nighttime residence, meaning: (i) Has a primary nighttime residence that is a public or private place not meant for human habitation; (ii) Is living in a publicly or privately operated shelter designated to provide temporary living arrangements (including congregate shelters, transitional housing, and hotels and motels paid for by charitable organizations or by federal, state and local government programs); or (iii) Is exiting an institution where (s)he has resided for 90 days or less and who resided in an emergency shelter or place not meant for human habitation immediately before entering that institution.

• Chronically Homeless - a homeless individual with a disability who lives either in a place not meant for human habitation, a safe haven, or in an emergency shelter, or in an institutional care facility if the individual has been living in the facility for fewer than 90 days and had been living in a place not meant for human habitation, a safe haven, or in an emergency shelter immediately before entering the institutional care facility. In order to meet the ‘‘chronically homeless’’ definition, the individual also must have been living as described above continuously for at least 12 months, or on at least four separate occasions in the last 3 years, where the combined occasions total a length of time of at least 12 months. Each period separating the occasions must include at least 7 nights of living in a situation other than a place not meant for human habitation, in an emergency shelter, or in a safe haven.

• Imminent Risk of Homelessness - Individual or family who will imminently lose their primary nighttime residence, provided that: (i) Residence will be lost within 14 days of

the date of application for homeless assistance; (ii) No subsequent residence has been identified; and (iii) The individual or family lacks the resources or support networks needed to obtain other permanent housing.

• Homeless under other Federal statutes - Unaccompanied youth under 25 years of age, or families with Category 3 children and youth, who do not otherwise qualify as homeless under this definition, but who: (i) Are defined as homeless under the other listed federal statutes; (ii) Have not had a lease, ownership interest, or occupancy agreement in permanent housing during the 60 days prior to the homeless assistance application; (iii) Have experienced persistent instability as measured by two moves or more during in the preceding 60 days; and (iv) Can be expected to continue in such status for an extended period of time due to special needs or barriers.

• Fleeing/Attempting to Flee DV- Any individual or family who: (i) Is fleeing, or is attempting to flee, domestic violence; (ii) Has no other residence; and (iii) Lacks the resources or support networks to obtain other permanent housing.

Every year, the US Department of Housing and Urban Development releases maximum household income limits to determine program eligibility.

The Clayton County Adolescent Health and Youth Development (AHYD) Program was designed to enhance the skills and improve the health status of adolescents through opportunities and programs developed in collaboration with families, communities, schools and other public and private organizations throughout Clayton County. Programs and services target youth ages 10 to 19. The "Indicators of Youth Risk Behavior" are used to identify communities that have the greatest need for risk prevention and reduction.

AHYD works with various community partners to sponsor programs that reinforce positive attitudes and healthy behaviors, as well as activities aimed at decreasing youth violence, substance abuse, poor school performance, and early sexual activity. These AHYD programs provide a comprehensive and holistic approach with assisting our youth in the following ways:

Healthier Generations Project (HGP)

Healthier Generations Project (HGP) ofers care coordination to eligible pregnant women, new mothers, and children under the age of two. The specifc components of HGP include (1) community and health promotion, (2) care coordination (including home visitation), (3) parenting education, (4) interconception care, (5) newborn care and support, and (6) comprehensive screenings, referrals, and linkage to other public health and community services. HGP also ofers 24/7 DAD, a program which focuses on parenting and support services for fathers of children enrolled in HGP. So that, fathers can beneft and families are strengthened.

M.O.M.S. (Making Our Mothers Successful) & PAT (Parents as Teachers) Programs

MOMS/PAT is a FREE home visiting program ofered by the Clayton County Board of Health to decrease the infant mortality rate in Clayton County. The program fuses the PAT’s evidenced-based curriculum with the Resource Mothers concept to ensure that participants have healthy pregnancies and healthy babies. The program participants receive education, linkage to services, support, and social interaction in a group setting.

The mission of the Center for the Visually Impaired (CVI) is to empower people impacted by vision loss to live with independence and dignity. CVI provides services to people of all ages with all degrees of vision loss. The BEGIN program provides early intervention and family support for children from birth to age 5 and their families. The STARS (Social, Therapeutic, Academic and Recreational Services) program provides after school, summer, weekend and mentoring opportunities for school age youth. Clients of the Maxwell Low Vision Clinic, available to individuals starting at age 6, receive a low-vision evaluation and training on assistive devices and aids such as magnifers and large print items to help with daily activities. The New View Adult Rehabilitation Program serves working-age adults by providing vision rehabilitation services, including classes in orientation and mobility, activities of daily living, assistive technology and job readiness and job placement assistance. The New View also works with older adults in the community by providing training in the home and classes at satellite locations such as senior centers. Support groups are available to help with adjustment to vision loss. Volunteer opportunities are available for individuals or groups interested in helping CVI fulfll its mission.
