PURPOSE OF MEMBERSHIP DIRECTORY The Arizona Alliance for Community Health Centers’ (AACHC) Membership Directory is published as a guide to the community. It is intended to serve as a reference for state, local, public and private health and human service agencies, as well as students and potential medical providers searching practice opportunities in Arizona. Due to the possibility of expansion of sites and services, and changes in personnel in the future, please refer to our website (www.aachc.org) for the most current information.

ARIZONA COMMUNITY HEALTH CENTERS AT A GLANCE The Arizona Alliance for Community Health Centers (AACHC) has served as the Primary Care Association for the state of Arizona since 1985. AACHC has consistently strived to address the needs of Arizona’s Community Health Centers (CHCs), as well as the diverse populations that they serve. Arizona’s CHCs are local, nonprofit, healthcare providers serving people of all income levels in medically underserved communities. Arizona’s CHC network has evolved into a healthcare delivery system of 21 Health Center Program Grantees, two Federally Qualified Health Center LookAlikes, and many other healthcare organizations. Community Health Centers are located in 14 of the 15 counties in Arizona, and our CHCs and Look-Alikes serve over 565,000 patients and provide over 2,000,000 annual visits. The number of patients and patient visits continues to grow. Nationwide, health centers serve as the primary health care home for almost 23 million people a year. Arizona’s health centers provide high-quality, affordable primary care and preventative services and often provide onsite dental, pharmaceutical, and behavioral health services. Health centers’ costs of care rank among the lowest, saving taxpayers millions of dollars. CHCs help remove common barriers to healthcare access because they: • Are located in high-need areas, identified by the federal and state government • Provide enabling services that helps ensure access to quality, comprehensive healthcare is available, including transportation, translation, case management, health education, and chronic disease management • Are open to all residents regardless of insurance and income status • Tailor services to fit the special needs of their local communities and provide services based on the recommendations of local residents CHCs focus on improving the health of individual patients, as well as improving the health status of the entire community. The medical home model, which is already widely used in health centers today, has been identified as the future of healthcare delivery.

Chiricahua Community Health Centers, Inc. Health Center Program Grantee/Federally Qualified Health Center Chief Executive Officer: Jonathan Melk, MD www.cchci.org Chiricahua Community Health Centers, Inc. (CCHCI) is a diverse not-for-profit health center program grantee/federally qualified health center providing a variety of services. It has been developed on a multi-dimensional model that includes medical, dental, and behavioral health carefully integrated with an approach that address every aspect of life (physical, emotional, social, occupational, intellectual, and environmental) for the uninsured, underinsured, veterans, migrant and seasonal farmworkers, and all residents of Cochise County. Currently operating from seven fixed sites and three mobile units, a full range of primary and preventive health care services are provided for the entire family. CCHCI is the largest primary care provider in Cochise County and the only true sliding-fee-scale provider.

Circle the City Health Center Program Grantee/Federally Qualified Health Center Chief Executive Officer: Brandon Clark www.circlethecity.org Circle the City, a 501(c)(3) nonprofit, nondenominational public charity, exists to create and deliver innovative healthcare solutions that compassionately address the needs of men, women and children facing homelessness. In 2012, Circle the City launched Arizona’s first medical respite center, a 50-bed facility where adults experiencing homelessness could rest and heal following periods of illness and injury. In 2015, Circle the City opened The Parsons Family Health Center, an integrated community health center also serving the homeless of Maricopa County. Circle the City operates a robust street outreach program utilizing a 2-exam room mobile clinic. Together, the family health center, medical respite center, and the mobile outreach clinic create a seamless continuum of homeless-specialty care in Maricopa County.

Community Health Center of Yavapai Health Center Program Grantee/Federally Qualified Health Center Director: Annette Olson www.chcy.info The Community Health Center of Yavapai (CHCY) has been serving all of Yavapai County since 2001. Since its inception as the Prescott Free Clinic, Inc. in 1993, it has stayed true to its mission to contribute to the well-being of the people of Yavapai County by offering integrated primary health services including adult medical, pediatrics, reproductive health, dental and care coordination. With three full-time locations in Prescott, Prescott Valley, and Cottonwood, CHCY is the medical home for the uninsured, underserved and underinsured in Yavapai County.

Copper Queen Community Hospital Rural Health Clinic Chief Executive Officer: James J. Dickson www.cqch.org Copper Queen Medical Associates (CQMA) has three clinics that service southeast Cochise County, Bisbee, Palominas and Douglas. CQMA Rural Health Clinics partner with the Copper Queen Community Hospital in Bisbee to provide primary care, diagnostic services including lab and diagnostic imaging, physical therapy, home health, and telemedicine. In June 2004, the Douglas and Bisbee Clinics were established as part of the largest hospital-based Rural Health Clinic network in Arizona. The Palominas Clinic was added in October of 2007 in the growing Palominas-Hereford area. All three clinics provide specialty-supported primary medical care to over 30,000 patients in the US/Mexico border region.

Horizon Health and Wellness Health Center Program Grantee/Federally Qualified Health Center Chief Executive Officer: Norman E. Mudd www.hhwaz.org Horizon Health and Wellness has developed an innovative practice model that embodies our philosophy of whole person care while promoting wellness. This approach to health care addresses the whole person by integrating behavioral health with primary health care while promoting wellness and prevention. In this model, our medical and behavioral health staff can work with patients in the same exam rooms to ensure integrated care. Both are involved in assessments, brief interventions and followup with patients. Where appropriate, patients may also be referred for wellness activities (e.g. smoking cessation, nutritional education, physical activities, etc.), as well as traditional psychotherapy, and psychiatric services. Horizon Health and Wellness is excited to implement this innovative practice model at a time when health care is undergoing transformation nationally. It is our hope that our integrated model will improve outcomes for people and reduce health care costs for society.

La Paz Regional Hospital Hospital with four Rural Health Clinics Chief Executive Officer: Victoria Clark www.lapazhospital.org La Paz Regional Hospital and Rural Health Clinics is a non-profit organization located in La Paz, County, AZ, and offers comprehensive primary and secondary healthcare services at the hospital and its clinics in Bouse, Parker, Quartzsite, and Salome. Wesley Community Center Health Center Program Grantee/Federally Qualified Health Center Chief Executive Officer: Betty Mathis www.wesleycenterphx.org Since 1950, Wesley Community Center, located in south central “downtown� Phoenix, AZ, has provided programs, services, and activities for primarily Hispanic families through its family/ youth services, community development, adult education, and healthcare. Wesley Health Center, opened in 2003 for uninsured patients, became an FQHC in 2009 through a grant from the American Recovery and Reinvestment Act (ARRA) and HRSA. In 2011, primary healthcare services and preventive/health management classes for everyone were provided to over 8,000 unduplicated patients. Additionally, through partnerships with 35 other nonprofit agencies/organizations, community center services supported an additional 7,600 individuals through: after-school/summer programs; fitness, nutrition, sewing, and gardening classes/opportunities; and emergency food assistance.

Southwest Center for HIV/AIDS Chief Executive Officer: Cindy Quenneville www.swhiv.org Founded 25 years ago, Southwest Center specializes in HIV prevention, HIV care, and services for Lesbian, Gay, Bisexual, Transgender and non-binary gender individuals. Services are offered in English and Spanish and most are open to anyone who needs them. Services open to the public include 1) primary and naturopathic medical care; 2) individual, couple and group counseling; 3) nutrition assessments and counseling; 4) a Vitamin and Herb Shop offering high-quality vitamins and supplements; and 5) HIV and other sexually transmitted disease testing. Hepatitis C testing is available to high-risk populations. In addition, a number of services are specifically available for HIV-positive individuals. These include care coordination, case management, psychosocial support, and complementary and alternative medical (CAM) care such as acupuncture, reiki, cranial sacral, and guided meditation. HIV education and outreach is offered at organizations, community events and local schools. We welcome referrals. HIV-specific programs and testing are provided free of charge. Medical care, counseling and nutrition services are available on a sliding fee scale and private insurance is accepted for some services. We also accept Federal Ryan White coverage, AHCCCS/MMIC, and Medicare. We are co-located with MIHS’ McDowell Healthcare Center and Avella Pharmacy, on light rail in Central Phoenix.

The Health Center is a 638 tribally owned and operated clinic. As an ambulatory clinic, it provides health care services to all eligible Native Americans.
