his time of year, we celebrate our patients with Patient Appreciation Days. This has become an annual celebration in our office. It’s an opportunity to show gratitude and give back to our patients. I encourage you to take out a paper and pen (old technology I know) and at the top write THINGS I AM GRATEFUL FOR. This is a great exercise to do individually or with your family and friends. You can write down as many things you are grateful for and then review it everyday as a reminder or you can do a 30-day Challenge and write one new thing on the list each day that you are grateful for. Either way, when you focus on the positive and good in your life watch how it opens you up to more of it. he famous sixteenth century Japanese swordsman Tsukahara Bokuden is said to have had a student who was famous for being incredibly skilled, faster than anyone else, and almost unbeatable in combat. One day this disciple was walking down the street and passed near a very unsettled horse, who, feeling his presence, bucked and tried to kick him. However, the young man was so quick to react that he avoided the kick in one swift move. Everyone who saw this was amazed and cheered for him. The disciple was very proud of himself, going to his master later that day to relate the story. To his surprise, Bokuden was not happy, and expelled the student from his school. Bokuden’s sons asked their father why he had expelled such a great protege. He replied: “A man who cannot foresee danger ahead is a lost cause, no matter how skilled. I thought he was a man of better judgment but I was mistaken.” From this story we see that Bokuden valued good judgment much higher than great skill, because great skill is actually a dangerous thing in the hands of a careless person. I am reminded of this when I’m driving down the highway and someone goes by me at 20 or 30 miles per hour faster than the speed limit weaving in and out of traffic. Their poor judgment is no match for their (questionable) driving skills, and many times with terrible consequences. This is the main reason why a Martial Artist must first endeavor to perfect oneself as a person before attempting to perfect their technique. To be in control of one’s actions and emotions is a higher achievement than to overcome an opponent. Our Karate students learn many strikes, kicks, blocks, takedowns, etc. But they know what matters most, as one of my students put it quite clearly just a few days ago as she was leaving class. I asked this ten year old: “Miss Susan, before you go, what can you improve on in the week ahead?” She turned to me and said: “Think before I act.” I stopped for a second to reflect and said: “I could not have said it better. You go and do just that.” It was such a simple statement, but of very profound meaning. What if we carefully consider what we are about to say or do, even when we are under pressure? Even after we have been hurt or insulted, to respond in a truly thoughtful way. To consider the repercussions of our actions before taking them and do what is best, not just what feels good at the time. When we do that, this is the greatest victory one can achieve. A victory over self.

When you have an agent who is a broker helping you (like me), you’re able to compare the different insurance companies and see what company might have a good fit for you. Brokers don’t have a specific interest in only one insurance carrier so we are unbiased. If you happen to get a telesales call, you’ll end up speaking to a sales person, likely in another state, who will talk fast and only go over the perks of the plan. He/She may not cover your medication costs or whether or not your physicians are in the network. The person might also not realize you get a regular injection/infusion that is covered by your Medicare Supplement without a copay but if you switch to the Medicare Advantage Plan you’ll end up with a 20% coinsurance payment with each injection. The odds of winning the lottery are slim to none, but that doesn’t deter millions of Americans from spending, on average, about $70 billion a year on lottery tickets— approximately $230 per capita, including children.1 In 2022, players across the United States surpassed that average, spending more than $107.8 billion on lotteries, according to the North American Association of State and Provincial Lotteries (NASPL). You may not win, but you can dream. The first step you should take after you buy your ticket is to sign the back. This will help prevent someone else from claiming to be the winner if you lose your ticket (and they find it) or it’s stolen—and then declared a winner. If you’re lucky and win a lottery jackpot, take these precautions: • Consider making a tax-deductible gift to your library. • Choose anonymity if your state allows it. If your name makes it into the news, you’re likely to be slammed with financial requests from charities, relatives, friends, potential scammers and more. • Carefully handpick an attorney, certified public accountant, and investment advisor to work with you (and each other) to help you make legal and financial decisions that support your goals and work in your best interest. • Before you cash in your ticket, consult with your accountant or tax professional to help you decide whether to take the prize money all at once, or stretch it out over 20-30 years in annuity payments. Getting together for Thanksgiving and Christmas can be very exciting and energizing and a time for relationships to be strengthened. At the same time, such gatherings can be stressful and anxiety inducing, especially for people with hearing loss. While the holiday season can be a joyous time full of fun, food and laughter, those with hearing loss often struggle with the noise and commotion that comes from family gatherings, parties, and noisy dinners. Often, individuals with hearing loss avoid such occasions and stay away from important social activities. Two of the reasons for avoiding socializing include becoming tired and avoiding embarrassment and conflict. Hearing loss is much more complicated that most people realize, since the final stage of hearing occurs in the brain, not the ears. The act of hearing requires energy and mental power. When an individual has hearing loss, more energy is required to achieve understanding than when a person has “normal hearing.” Background noise increases the difficulty of hearing and increases how much energy a person uses to understand speech. In quiet environments, straining to hear causes mental and physical tiredness. Struggling to hear in noise causes even greater mental and physical fatigue. Hearing in noise can be significantly improved with hearing aids. Wearing hearing aids has been proven to decrease “listening fatigue” in both quiet and noisy environments. Hearing aids increase the intensity of sound energy in the specific area a person needs help, decreasing the amount of mental energy needed to “figure out” what is being said. However, the longer a person waits to get hearing aids, the less effective the ability to hear in noise becomes due to the changes that occur in the brain over time. Many people find ways to fake that they are hearing well, when in fact they are struggling. These tactics require significant energy. Mishearing can be embarrassing,
frustrating and lead to conflict. Teasing and pointing out mistakes in hearing can be mentally and emotionally detrimental. Recently, a patient frustratedly shared how contentious interacting with family members had become. When the patient misheard, family members would ask sarcastically, “Do you have your hearing aids in?” Then when they were told “yes,” they continued with “Well do you have batteries in them?” or “Are they turned on?” Comments like these are generally not helpful and cause tension and resentment, although in this case, it did lead to the patient seeking more help by getting better hearing aids. Being patient and understanding can make social activities more pleasant for those with hearing loss. we enter into the fall and winter seasons, kids are back in school, and illness is again on the rise. This time of year, there tends to be a lot of respiratory viruses, or colds, going around. Respiratory viruses commonly cause cough, congestion, runny nose, fevers, sore throat, fatigue, and body aches. There are several things you can do to help decrease risk of spreading these colds: • Frequent hand washing • Covering coughs and sneezes • Staying home when feeling sick • Avoid touching your face, mouth and nose • Minimize time around others who are sick • Make sure to clean toys and commonly touched surfaces like counters regularly • Receive recommended vaccines including annual flu and COVID boosters Despite best efforts colds tend to sneak in at some point or another, thankfully there are many things that can help alleviate uncomfortable symptoms at home. Some supportive measures include: • Getting plenty of rest • Drinking clear fluids to stay hydrated • Humidifier or steamy showers • Warm tea and honey • Suctioning the nose of young children and babies Sometimes supportive measures are not enough and seeking medical care is necessary. There are two options for the flu vaccine: the inactivated vaccine, given as a shot, or the live-attenuated vaccine, given as a nasal spray. These are considered equally effective. The nasal spray is approved for children over age 2.

In
August 2023, the average 30-year mortgage interest rate hit its highest level since 2000. At 7.48%, they were nearly triple where they were during the pandemic when they reached a low of 2.65% in January 2021. Meanwhile, median home prices rose for 10 consecutive quarters beginning in fall 2020, only declining for two straight quarters to $416,100, which is still 26.4% higher than the quarter just before the pandemic. The National Association of REALTORS® reported that existing home sales volume in July 2023 slipped 16.6% from the previous year, even as home prices rose to $406,700 from $399K a year ago. While many markets are still robust, others have slowed considerably. How can sellers bring more homebuyers to the table? As a concession, the seller can offer to pay discount points to the homebuyer’s lender to help the homebuyer get a lower mortgage interest rate. Called a seller-paid mortgage rate buydown, these discount points are typically 1% of the homebuyer’s loan amount and every discount point paid reduces the mortgage interest rate by 0.25 percentage points. If the mortgage amount is $400K, a discount point would be $4,000, and the monthly principal and interest payment at 7.5% would be $2,797. A sellerpaid discount point would lower the interest rate to 7.25% and a monthly payment to $2,729. Over the life of the loan, the borrower would save $24,535.
