The National Benefit Fund believes it is a “grandfathered health plan” under the Patient Protection and Affordable Care Act (the Affordable Care Act). This is a welfare benefit plan offering educational and training benefits to participating RNs. During the plan year, the plan had total reduction of $21,986, including employer contributions of $-5,191, deferred contributions for overpayments of $-20,826 and interest income of $4,031.

Plan expenses were $1,488,001. These expenses included $111,398 in administrative expenses and $1,376,603 in benefits paid to participants.

And because there is no better way to protect yourself against the flu, we urge you to get your annual flu shot if you have not gotten one already.

Benefits Update & News

New Dental Plus Plan for Early Retirees If you are thinking of early retirement, you may know that the Benefit Funds offered a prescription, vision and hearing plan to National Benefit Fund retirees age 55 or older with at least 10 years of service and to Greater New York Benefit Fund retirees between ages 62 and 65 with at least 25 years of service. As of January 1, 2015, the Benefit Funds are now instead providing a new health benefit plan for our not-yet-Medicareeligible early retirees called the Dental Plus Plan. This new plan includes a dental benefit ($3,000 a year for NBF, $1,200 a year for GNY); a hospital indemnity plan that pays $200 a day, for 10 days, each time you are admitted to a hospital; and a vision benefit.

We created the new Dental Plus Plan, just as we did for our Wage and Eligibility Class III active members, to allow early retirees who want to buy a medical plan on the new Health Insurance Marketplace to receive a subsidy, if eligible, to help pay for the plan. As you probably know, under the Affordable Care Act (ACA),

everyone must have “Minimum Essential Coverage� or face a penalty. Because the Prescription Plan is considered “Minimum Essential Coverage� under the ACA, early retirees would not face a penalty, but if they chose to buy a more complete medical plan on the Health Insurance Marketplace, they would not be able to access a subsidy. In some cases, the tax subsidy can reduce the monthly premium to zero. (Early retirees who think the Prescription Plan might work better for them have a one-time option to switch from the Dental Plus Plan.)

Avoid Out-of-Pocket Costs by Using Participating Providers As an 1199SEIU member, you have access to thousands of healthcare providers within our Benefit Fund’s network. That means you can avoid paying unnecessary out-of-pocket costs when you use participating providers. Doctors who are not in the network can charge anything they want, which can be a lot more than the Fund’s reimbursement rate, and you have to pay the full balance. So going outside of our network to a non-participating provider could result in an expensive – and avoidable – medical bill, so make sure that your doctor, and any provider he or she may refer you to, is part of our network. s

Basic Financial Statement The value of plan assets, after subtracting liabilities of the plan, was $67,746,480 as of December 31, 2013, compared to $41,783,316 as of January 1, 2013. During the plan year, the plan experienced an increase in its net assets of $25,963,164. This increase includes unrealized appreciation and depreciation in the value of plan assets; that is, the difference between the value of the plan’s assets at the end of the year and the value of the assets at the beginning of the year or the cost of assets acquired during the year. 1199SEIU NATIONAL BENEFIT FUND FOR HEALTH AND HUMAN SERVICE EMPLOYEES SUMMARY OF MATERIAL MODIFICATIONS This Summary of Material Modifications describes changes that affect your welfare benefit plan and updates the Summary Plan Description (“SPD�) that was previously distributed to you. You should keep this summary with your current SPD until the booklet is updated to reflect the changes discussed herein. ___________________________________________________________________________ Effective January 1, 2015, the following changes to the 1199SEIU National Benefit Fund for Health and Human Service Employees (“Benefit Fund�) Plan and SPD apply: 1. Sections I.C and IV.A: If you have designated your spouse as your life insurance beneficiary, your divorce will automatically revoke that designation upon notification to the Fund. If you do not change your beneficiary thereafter, your Life Insurance Benefit will be paid as if there is no beneficiary.

2. Sections I.G and VII.B: In the event you comply with the Fund’s terms and acknowledge the Fund’s rights, but you dispute the Fund’s Lien Determination (that a claim for benefits is not covered because it is an expense resulting from an illness or injury caused by the conduct of a third party) you may request an Administrative Review of the Lien Determination by writing to the Liens Department, provided that any proceeds you receive from a settlement, verdict or agreement for compensation from or on behalf of the party responsible for the illness or injury, up to the amount of the lien, are not disbursed for the duration of the appeal. The Fund will notify you in writing of the appeal decision and rationale within 30 days of receipt of the written appeal. If the Administrative Review results in a denial of your appeal, you have the right to request a final Administrative Review by the Chief Benefits Officer or his or her designee in writing no later than 60 days after the receipt of the appeal denial. If your appeal is denied by the Chief Benefits Officer or his or her designee, you have the right to file suit in federal court under the Employee Retirement Income Security Act of 1974 (“ERISA�).

3. Section I.J: For purposes of the subsection regarding continuing your coverage while receiving unemployment insurance, the maximum of six months continuation need not be consecutive, and the number of years you were covered by the Benefit Fund is calculated from when you were last considered a new employee under Section I.B. 4. Section II – preface, II.A, VII.B: A Non-Participating Provider that appeals a benefit denial cannot file a lawsuit on your behalf more than five years from the date of service. Before you receive services from a Non-Participating Provider, you should ask the provider to find out the total Benefit Fund allowance for the planned service by calling (646) 473-7160, and to notify you of what your out-of-pocket expenses will be.

5. Sections VII.A and VII.B: The assignment feature of the Benefit Fund is only for payment of your benefits to providers. No other rights under the plan or ERISA may be assigned or transferred.

6. Section VII.A: If you (or your provider by assignment) receive an overpayment from the Benefit Fund as a result of an improperly billed claim for benefits, the overpaid funds belong to the Benefit Fund, and you agree to hold that money in trust for the Benefit Fund and to reimburse the Benefit Fund within 30 days of receiving the overpayment. In addition, the Plan Administrator may bring a court action to enforce the terms of the Plan or to recover benefit overpayments. ___________________________________________________________________________

For four long days in June 2012, Jeffrey Lee lay in a hospital bed in Brooklyn and thought about how much was at stake if he kept smoking. Just 55 years old at the time, happily married to his wife, Janine, who is an Outreach Coordinator at the Benefit Funds, and proud father to their 16-year-old daughter, Janay, he was terrified at the thought of not being in their lives.Yet there he was, recovering from a heart attack in the cardiac unit of Long Island College Hospital after being rushed to the emergency room. “It was a traumatic experience that put a lot of fear in me,” he said. “And there wasn’t a bit of doubt about why I had a heart attack: it was from smoking cigarettes.”

Jeffrey, a Personal Care Assistant at Mount Sinai/Beth Israel Hospital, had been smoking close to a pack of cigarettes a day for nearly 30 years, but he was now determined to stop. The two months of his strictly monitored recovery after his heart attack helped him avoid most of the triggers that might have led to smoking, he said, since he spent nearly all his time either at home or at a healthcare facility seeing a doctor. But in addition to his heart healing and his health stabilizing enough for him to return to work, two things happened. “I finally realized that every puff had been putting my health in jeopardy, even though I felt fine before my heart attack,” Jeffrey said. “But more important, I realized I could quit – I was living without cigarettes – and it felt good.”

Working in a detoxification and chemical dependency unit at the hospital, Jeffrey saw how hard it was for patients struggling with addiction and said he’s humbled to think his experience might serve as a powerful example. “I tell people that in my mind, I felt like I was health-conscious even though I was smoking – I played basketball every week, I felt like I ate pretty well and had even lost a little weight,” he said. “I blocked out the fact that smoking put me at much higher risk for a heart attack or stroke.”

The experience was a turning point in Jeffrey’s life, and in the last two-and-a-half years he has not only remained smoke-free, he’s become more healthconscious overall. He walks about three times a week, he said, and uses an elliptical machine at home.

A nagging hip injury has limited his time on the basketball court, but he isn’t complaining. “Let’s face it, it’s a young man’s game,” he said with a laugh. “I’m 58 years old and other than wanting to drop a few pounds, I feel great.”

He stressed that it’s been tough to stay away from smoking – or to avoid substituting snacks for cigarettes – but that it has become easier over time. He didn’t use smoking cessation products or medications, but said he thinks anyone trying to quit should use whatever they need to quit, such as nicotine patches, gum or medication.

Jeffrey’s cardiologist told him that in addition to giving up smoking, he had to change his diet in order for his heart to heal and regain strength. So he began grilling and baking his chicken, fish and shrimp dinners rather than frying them; eating fresh fruit and vegetables instead of chips, pretzels or other unhealthy snack foods; and drinking water instead of soda.

Surprisingly, he found that healthy eating reduced his cigarette cravings, boosting his confidence and helping him stay on track to quitting.

Of course, now that his taste buds are back in action, Jeffrey said he needs to watch his portions and avoid impulsive snacking, especially when working his usual 11:00 pm to 7:00 am shift. But it’s a small sacrifice, he said, especially when he thinks about where he was less than three years ago. “I’m fortunate to have health benefits that allow me to stay on top of my condition, and I’ve taken steps to change and to do the right thing,” he said. “There’s no lasting damage to my heart. Now the rest is up to me.”

Stop smoking and start living If you’re a smoker, you probably know all the reasons you should quit – but you probably also know how hard it is to kick the habit for good.

No matter your age or how long you’ve smoked, quitting smoking is worth it. Besides lowering your odds for heart disease, lung cancer, stroke and emphysema, quitting will also protect your loved ones from the harmful effects of secondhand smoke. Giving up smoking could even add years to your life: A recent study in The New England Journal of Medicine found that smokers who quit lived at least six years longer than people who continued to smoke.

It might help to know that if you’re trying to quit, you’re not alone. The good news is that since 2002, there are more former smokers than current smokers in the U.S. And just like the millions who have quit, you can too with the right planning and support. Try these tips and strategies to help you give up smoking once and for all.

Write Down Your Reasons for Quitting Make a list of your reasons to give up smoking – including the names of family members and friends whose company you want to enjoy for years to come.

Set a few personal goals too, such as being able to climb a flight of stairs without being short of breath or really tasting the foods you love again. Add photos of your loved ones and other motivational pictures, and post the list someplace where you’ll see it often.

Set and Announce Your Quit Date It sounds simple, but picking a date to stop smoking completely is important.Your quit date could have special meaning, such as a birthday or an anniversary, to help remind you of your goals. Once you’ve decided, tell friends and family about your plan so they can support you. How? Ask them to keep

your activities together smoke-free or help you find healthful distractions when you crave a cigarette.

Identify and Avoid Triggers Be sure to throw away cigarettes, lighters, ashtrays and all other objects you associate with smoking. Avoiding people and situations that trigger your desire to smoke can be harder, but planning can make a difference.

For instance, carry a pack of gum to chew after meals, during work breaks and at other times when you’re used to smoking.You can also plan other things to do when you get the urge to light up, such as calling a friend, listening to music or going to a smoke-free zone like a store or movie theater.

Get Moving One of the best ways to overcome tobacco cravings – and avoid the weight gain sometimes associated with quitting – is exercise. In fact, a study reported in the Journal of Smoking Cessation showed that smokers who exercised for just 15 minutes a day were 55 percent more likely to quit – and 43 percent less likely to start smoking again – than those who were inactive. So take a walk before work, during your lunch break or after dinner – or better yet – do all three!

Healthy Eating

A Healthy Spin on Comfort Foods During the winter months, and especially around the holidays, we tend to reach for those familiar comfort foods found at family gatherings and holiday celebrations. As delicious as foods like lasagna, fried chicken, meat loaf and mashed potatoes, and rice and beans are, a steady diet of them can add up to extra fat and calories – adding extra pounds and making you feel sluggish. We’ve rounded up some of the best tips for lightening up your favorite comfort meals that will help you keep the comfort but lose the fat and calories.

While the satisfying crunch of crispy fried chicken and onion rings may warm your heart, the extra fat in these foods is far from heart-friendly. For a great-tasting alternative with a fraction of the fat and calories, try oven-“fried” chicken tenders.You can find a recipe for it, along with many other low-fat, great tasting recipes, at the United States Department of Agriculture’s What’s Cooking? website at www.WhatsCooking.fns.usda.gov.

Roasting is another good cooking method, especially for vegetables. With just a touch of olive oil, the natural sweetness in root vegetables like Brussels sprouts, carrots and potatoes comes out. Slow-roasted lean meats, like turkey breast, is another healthy option.

discard the stem and stringy pulp. In a shallow baking dish, place the two halves facedown, cover with foil and bake in a 375-degree oven for about 90 minutes or until tender. Let pumpkin cool, then scoop out flesh and puree or mash it.

Preparation Mash the white beans, onion and water with a fork or blender until smooth. Set aside. In a large pot, add the pumpkin puree, apple juice, cinnamon, black pepper and salt. If using nutmeg, allspice or ginger, add that too. Stir. Add the bean mixture to the pot. Cook over low heat for 15 to 20 minutes, until warmed through.

Jeffrey Lee Después de un grave susto sobre su salud, Jeffrey Lee dejó los cigarrillos para siempre, y hoy está respirando más fácilmente.

Durante cuatro largos días en junio de 2012, Jeffrey Lee estuvo postrado en la cama de un hospital en Brooklyn y pensó sobre cuánto se estaba poniendo en riesgo si seguía fumando. Con solo 55 años de edad en ese momento, felizmente casado con su esposa, Janine, quien es un Coordinador de Ayuda para los Fondos de Beneficios, y orgulloso padre de su hija de 16 años, Janay, estaba aterrorizado de pensar que no sería parte de sus vidas. Por ello comenzó a cocinar a la parrilla y a hornear sus comidas de pollo, pescado y camarones en lugar de freírlas; comenzó a comer fruta fresca y verduras en lugar de papitas fritas, pretzels u otros alimentos de refrigerio no saludables; y a beber agua en lugar de gaseosas.

Sorpresivamente descubrió que comer saludable reducía sus deseos de fumar, elevaba su confianza y le ayudaba a permanecer sin fumar.

Children Ages 9 to 15 Years Each year, the Anne Shore Sleep-Away Camp Program sends more than 800 children of 1199SEIU members to three- and four-week sleep-away camps throughout the Northeast – at minimal cost to their parents.

To apply for sleep-away camp, simply fill out the coupon above and return it to the 1199SEIU Child Care Corporation before January 30, 2015. If your child is eligible, our program staff will contact you to schedule an appointment to attend a mandatory orientation in order to select the camp that best suits your child’s needs and interests. You are required to submit a $10 non-refundable application fee, which must be paid at orientation.

Nuevo Plan Dental Plus para los que Se Jubilan Anticipadamente Si está considerando una jubilación anticipada, es posible que sepa que los Fondos de Beneficios ofrecen un plan de medicamentos con receta, cuidado de la vista y de la audición a los jubilados del Fondo Nacional de Beneficios de 55 años de edad o mayores con por lo menos 10 años de servicios y a los jubilados del Fondo de Beneficios de la zona metropolitana de Nueva York que tienen entre 62 y 65 años de edad y con por lo menos 25 años de servicios. A partir del 1 de enero de 2015, los Fondos de Beneficios están brindando un nuevo plan de beneficios de la salud para los que se jubilan anticipadamente y aún no son elegibles para Medicare, llamado Plan Dental Plus. Este nuevo plan incluye un beneficio dental ($3,000 al año para NBF y $1,200 al año para GNY); un plan de indemnización por hospitalización que paga $200 por día, durante 10 días, cada vez que es ingresado en un hospital; y un beneficio de cuidado de la vista.

Hemos creado el nuevo Plan Dental Plus, de la misma manera que lo hicimos para nuestros miembros activos de la Clase de Salario y Elegibilidad III, para permitir que aquellos que se jubilan anticipadamente y que quieran comprar un plan médico en el nuevo Mercado de Seguros de Salud reciban un subsidio, si son elegibles, para ayudar a pagar por el plan. Como probablemente ya

sabe, bajo la Ley de Cuidado de Bajo Costo (ACA, por sus siglas en inglés) todos deben tener una “Cobertura esencial mínima” o se les aplica una multa. Debido a que el Plan de Medicamentos Recetados es considerado “Cobertura esencial mínima” según ACA, los que se jubilan anticipadamente no deberán pagar una multa, pero si eligen comprar un plan médico más completo en el Mercado de Seguros de Salud, no podrían tener acceso a un subsidio. En algunos casos, el subsidio fiscal puede reducir la prima mensual a cero. (Los que se jubilan anticipadamente que consideran que el Plan de Medicamentos Recetados puede funcionar mejor para ellos tienen una opción de una sola vez de cambiarse del Plan Dental Plus.)

Nuevo Plan Dental Plus para los que Se Jubilan Anticipadamente Si está considerando una jubilación anticipada, es posible que sepa que los Fondos de Beneficios ofrecen un plan de medicamentos con receta, cuidado de la vista y de la audición a los jubilados del Fondo Nacional de Beneficios de 55 años de edad o mayores con por lo menos 10 años de servicios y a los jubilados del Fondo de Beneficios de la zona metropolitana de Nueva York que tienen entre 62 y 65 años de edad y con por lo menos 25 años de servicios. A partir del 1 de enero de 2015, los Fondos de Beneficios están brindando un nuevo plan de beneficios de la salud para los que se jubilan anticipadamente y aún no son elegibles para Medicare, llamado Plan Dental Plus. Este nuevo plan incluye un beneficio dental ($3,000 al año para NBF y $1,200 al año para GNY); un plan de indemnización por hospitalización que paga $200 por día, durante 10 días, cada vez que es ingresado en un hospital; y un beneficio de cuidado de la vista.

Hemos creado el nuevo Plan Dental Plus, de la misma manera que lo hicimos para nuestros miembros activos de la Clase de Salario y Elegibilidad III, para permitir que aquellos que se jubilan anticipadamente y que quieran comprar un plan médico en el nuevo Mercado de Seguros de Salud reciban un subsidio, si son elegibles, para ayudar a pagar por el plan. Como probablemente ya

sabe, bajo la Ley de Cuidado de Bajo Costo (ACA, por sus siglas en inglés) todos deben tener una “Cobertura esencial mínima” o se les aplica una multa. Debido a que el Plan de Medicamentos Recetados es considerado “Cobertura esencial mínima” según ACA, los que se jubilan anticipadamente no deberán pagar una multa, pero si eligen comprar un plan médico más completo en el Mercado de Seguros de Salud, no podrían tener acceso a un subsidio. En algunos casos, el subsidio fiscal puede reducir la prima mensual a cero. (Los que se jubilan anticipadamente que consideran que el Plan de Medicamentos Recetados puede funcionar mejor para ellos tienen una opción de una sola vez de cambiarse del Plan Dental Plus.)

Jeffrey Lee Después de un grave susto sobre su salud, Jeffrey Lee dejó los cigarrillos para siempre, y hoy está respirando más fácilmente.

Durante cuatro largos días en junio de 2012, Jeffrey Lee estuvo postrado en la cama de un hospital en Brooklyn y pensó sobre cuánto se estaba poniendo en riesgo si seguía fumando. Con solo 55 años de edad en ese momento, felizmente casado con su esposa, Janine, quien es un Coordinador de Ayuda para los Fondos de Beneficios, y orgulloso padre de su hija de 16 años, Janay, estaba aterrorizado de pensar que no sería parte de sus vidas. Pero ahí estaba, recuperándose de un ataque al corazón en la unidad cardíaca del Long Island College Hospital después de ser llevado a la sala de emergencias. “Fue una experiencia traumática que me asustó mucho”, dijo. “Y no había la menor duda de por qué sufrí un ataque al corazón: era porque fumaba cigarrillos”.

Jeffrey, un Auxiliar de Atención Personal en el Hospital Mount Sinai/Beth Israel, había estado fumando cerca de un paquete de cigarrillos al día durante casi 30 años, pero ahora estaba decidido a parar. Los dos meses de su recuperación estrictamente supervisada luego de su ataque al corazón le ayudaron a evitar la mayoría de los detonadores que podrían hacerlo fumar, dijo, ya que estuvo la mayoría de su tiempo ya sea en su hogar o en un establecimiento de atención de la salud visitando a un médico. Pero además de curar su corazón y estabilizar su salud en medida suficiente para regresar al trabajo, ocurrieron dos cosas. “Finalmente me di cuenta de que cada bocanada había estado poniendo en peligro mi salud, aunque me sentía bien antes de mi ataque al corazón”, dijo Jeffrey. “Pero lo más importante, me di cuenta de que podía dejarlo, estaba viviendo sin cigarrillos, y me sentía bien”.

La experiencia fue un momento trascendental en la vida de Jeffrey, y en los últimos dos años y medio no solo ha dejado de fumar, sino que se ha vuelto más consciente de su salud en general. Camina cerca de tres veces por semana, dijo, y utiliza una máquina elíptica en su hogar. Una molesta lesión en la cadera ha limitado su tiempo en la cancha de baloncesto, pero no se queja.

“Hay que reconocer que es un juego para jóvenes”, dijo riendo. “Tengo 58 años de edad y aparte de querer bajar unas cuantas libras, me siento bien”.

Recalcó que ha sido difícil seguir sin fumar, o evitar reemplazar refrigerios por los cigarrillos, pero que se ha vuelto más fácil con el transcurrir del tiempo. No utilizó productos ni medicamentos para dejar de fumar, pero dice que toda persona que piense en dejar de fumar debe utilizar lo que necesite para hacerlo, como parches de nicotina, goma de mascar o medicamentos.

El cardiólogo de Jeffrey le dijo que además de dejar de fumar tenía que cambiar su dieta con el fin de que su corazón se cure y recupere sus fuerzas. Por ello comenzó a cocinar a la parrilla y a hornear sus comidas de pollo, pescado y camarones en lugar de freírlas; comenzó a comer fruta fresca y verduras en lugar de papitas fritas, pretzels u otros alimentos de refrigerio no saludables; y a beber agua en lugar de gaseosas. Sorpresivamente descubrió que comer saludable reducía sus deseos de fumar, elevaba su confianza y le ayudaba a permanecer sin fumar.

Por supuesto, ahora que sus papilas gustativas están funcionando nuevamente, necesita cuidar sus porciones y evitar los refrigerios impulsivos, especialmente cuando trabaja en su turno usual de 11:00 pm a 7:00 am. Pero es un pequeño sacrificio, dijo, especialmente cuando recuerda dónde estaba hace menos de tres años. “Soy afortunado de tener beneficios de salud que me permiten estar al tanto de mi estado físico, y he tomado medidas para cambiar y hacer lo correcto”, dijo. “No ha habido un daño permanente a mi corazón. Request the Joseph Tauber Scholarship Application The Joseph Tauber Scholarship Program provides scholarships to children of eligible 1199SEIU members who are attending accredited undergraduate programs. Eligible members are those whose employers contribute to the National Benefit Fund or the Greater New York Child Care Fund. Members must have received Wage Class I benefits for at least one year. Candidates must be high school graduates, postsecondary school students or previous awardees enrolled full time for a minimum of 12 college credits each semester. All candidates must be younger than 23 years of age by September 1, 2015. Previous awardees seeking scholarship support should note that automatic award renewals are not granted; you must reapply each year for consideration.

Pumpkin and White Bean Soup

While the satisfying crunch of crispy fried chicken and onion rings may warm your heart, the extra fat in these foods is far from heart-friendly. For a great-tasting alternative with a fraction of the fat and calories, try oven-“fried” chicken tenders.You can find a recipe for it, along with many other low-fat, great tasting recipes, at the United States Department of Agriculture’s What’s Cooking? website at www.WhatsCooking.fns.usda.gov.

Roasting is another good cooking method, especially for vegetables. With just a touch of olive oil, the natural sweetness in root vegetables like Brussels sprouts, carrots and potatoes comes out. Slow-roasted lean meats, like turkey breast, is another healthy option.

Ingredients 1 15-ounce can of small white beans* 1 small onion (finely chopped) 1 cup water 1 15-ounce can of pumpkin puree** 1 ½ cups apple juice ½ teaspoon cinnamon ½ teaspoon black pepper ¼ teaspoon salt 1/8 teaspoon nutmeg, allspice or ginger (optional)

Rethink Your Plate Portion control is about knowing what you really love to eat – and then eating it in moderation – along with lots of healthy vegetables. To make portion control easier, fill half of your plate with vegetables like salad, sautéed spinach or roasted veggies. Then fill a quarter of your plate with your favorite comfort food (like macaroni and cheese or rice and beans) and the remaining quarter with lean protein (like roasted chicken breast, broiled fish or a turkey burger).

Lose the Guilt The good news is that not all comfort foods are necessarily high in fat and calories. On a cold morning, warm up with a bowl of oatmeal sprinkled with raisins and a touch of brown sugar. Soothe your throat with some homemade chicken noodle soup. A brown rice and guacamole burrito is both a filling meal and a good source of fiber and healthy fats.

So this holiday season, instead of saying no to your favorite comfort foods, give them a healthy makeover!

discard the stem and stringy pulp. In a shallow baking dish, place the two halves facedown, cover with foil and bake in a 375-degree oven for about 90 minutes or until tender. Let pumpkin cool, then scoop out flesh and puree or mash it.

Preparation Mash the white beans, onion and water with a fork or blender until smooth. Set aside. In a large pot, add the pumpkin puree, apple juice, cinnamon, black pepper and salt. If using nutmeg, allspice or ginger, add that too. Stir. Add the bean mixture to the pot. Cook over low heat for 15 to 20 minutes, until warmed through.

A Healthy Spin on Comfort Foods During the winter months, and especially around the holidays, we tend to reach for those familiar comfort foods found at family gatherings and holiday celebrations. As delicious as foods like lasagna, fried chicken, meat loaf and mashed potatoes, and rice and beans are, a steady diet of them can add up to extra fat and calories – adding extra pounds and making you feel sluggish. We’ve rounded up some of the best tips for lightening up your favorite comfort meals that will help you keep the comfort but lose the fat and calories.

Cambios Sencillos Puede eliminar las grasas y las calorías de sus comidas favoritas reconfortantes cambiando por ingredientes magros o sin grasas algunos de los ingredientes con alto contenido en grasas como la mantequilla, u otras fuentes de grasas saturadas.

Stop smoking and start living If you’re a smoker, you probably know all the reasons you should quit – but you probably also know how hard it is to kick the habit for good.

No matter your age or how long you’ve smoked, quitting smoking is worth it. Besides lowering your odds for heart disease, lung cancer, stroke and emphysema, quitting will also protect your loved ones from the harmful effects of secondhand smoke. Giving up smoking could even add years to your life: A recent study in The New England Journal of Medicine found that smokers who quit lived at least six years longer than people who continued to smoke.

It might help to know that if you’re trying to quit, you’re not alone. The good news is that since 2002, there are more former smokers than current smokers in the U.S. And just like the millions who have quit, you can too with the right planning and support. Try these tips and strategies to help you give up smoking once and for all.

Write Down Your Reasons for Quitting Make a list of your reasons to give up smoking – including the names of family members and friends whose company you want to enjoy for years to come.

Set a few personal goals too, such as being able to climb a flight of stairs without being short of breath or really tasting the foods you love again. Add photos of your loved ones and other motivational pictures, and post the list someplace where you’ll see it often.

Set and Announce Your Quit Date It sounds simple, but picking a date to stop smoking completely is important.Your quit date could have special meaning, such as a birthday or an anniversary, to help remind you of your goals. Once you’ve decided, tell friends and family about your plan so they can support you. How? Ask them to keep

your activities together smoke-free or help you find healthful distractions when you crave a cigarette.

Identify and Avoid Triggers Be sure to throw away cigarettes, lighters, ashtrays and all other objects you associate with smoking. Avoiding people and situations that trigger your desire to smoke can be harder, but planning can make a difference.

For instance, carry a pack of gum to chew after meals, during work breaks and at other times when you’re used to smoking.You can also plan other things to do when you get the urge to light up, such as calling a friend, listening to music or going to a smoke-free zone like a store or movie theater.

Get Moving One of the best ways to overcome tobacco cravings – and avoid the weight gain sometimes associated with quitting – is exercise. In fact, a study reported in the Journal of Smoking Cessation showed that smokers who exercised for just 15 minutes a day were 55 percent more likely to quit – and 43 percent less likely to start smoking again – than those who were inactive. So take a walk before work, during your lunch break or after dinner – or better yet – do all three!

For four long days in June 2012, Jeffrey Lee lay in a hospital bed in Brooklyn and thought about how much was at stake if he kept smoking. Just 55 years old at the time, happily married to his wife, Janine, who is an Outreach Coordinator at the Benefit Funds, and proud father to their 16-year-old daughter, Janay, he was terrified at the thought of not being in their lives.Yet there he was, recovering from a heart attack in the cardiac unit of Long Island College Hospital after being rushed to the emergency room. “It was a traumatic experience that put a lot of fear in me,” he said. “And there wasn’t a bit of doubt about why I had a heart attack: it was from smoking cigarettes.”

Jeffrey, a Personal Care Assistant at Mount Sinai/Beth Israel Hospital, had been smoking close to a pack of cigarettes a day for nearly 30 years, but he was now determined to stop. The two months of his strictly monitored recovery after his heart attack helped him avoid most of the triggers that might have led to smoking, he said, since he spent nearly all his time either at home or at a healthcare facility seeing a doctor. But in addition to his heart healing and his health stabilizing enough for him to return to work, two things happened. “I finally realized that every puff had been putting my health in jeopardy, even though I felt fine before my heart attack,” Jeffrey said. “But more important, I realized I could quit – I was living without cigarettes – and it felt good.”

Working in a detoxification and chemical dependency unit at the hospital, Jeffrey saw how hard it was for patients struggling with addiction and said he’s humbled to think his experience might serve as a powerful example. “I tell people that in my mind, I felt like I was health-conscious even though I was smoking – I played basketball every week, I felt like I ate pretty well and had even lost a little weight,” he said. “I blocked out the fact that smoking put me at much higher risk for a heart attack or stroke.”

The experience was a turning point in Jeffrey’s life, and in the last two-and-a-half years he has not only remained smoke-free, he’s become more healthconscious overall. He walks about three times a week, he said, and uses an elliptical machine at home.

A nagging hip injury has limited his time on the basketball court, but he isn’t complaining. “Let’s face it, it’s a young man’s game,” he said with a laugh. “I’m 58 years old and other than wanting to drop a few pounds, I feel great.”

He stressed that it’s been tough to stay away from smoking – or to avoid substituting snacks for cigarettes – but that it has become easier over time. He didn’t use smoking cessation products or medications, but said he thinks anyone trying to quit should use whatever they need to quit, such as nicotine patches, gum or medication.

Jeffrey’s cardiologist told him that in addition to giving up smoking, he had to change his diet in order for his heart to heal and regain strength. So he began grilling and baking his chicken, fish and shrimp dinners rather than frying them; eating fresh fruit and vegetables instead of chips, pretzels or other unhealthy snack foods; and drinking water instead of soda.

Surprisingly, he found that healthy eating reduced his cigarette cravings, boosting his confidence and helping him stay on track to quitting.

Of course, now that his taste buds are back in action, Jeffrey said he needs to watch his portions and avoid impulsive snacking, especially when working his usual 11:00 pm to 7:00 am shift. But it’s a small sacrifice, he said, especially when he thinks about where he was less than three years ago. “I’m fortunate to have health benefits that allow me to stay on top of my condition, and I’ve taken steps to change and to do the right thing,” he said. “There’s no lasting damage to my heart. Now the rest is up to me.”

Sections I.G and VII.B: In the event you comply with the Fund’s terms and acknowledge the Fund’s rights, but you dispute the Fund’s Lien Determination (that a claim for benefits is not covered because it is an expense resulting from an illness or injury caused by the conduct of a third party) you may request an Administrative Review of the Lien Determination by writing to the Liens Department, provided that any proceeds you receive from a settlement, verdict or agreement for compensation from or on behalf of the party responsible for the illness or injury, up to the amount of the lien, are not disbursed for the duration of the appeal. The Fund will notify you in writing of the appeal decision and rationale within 30 days of receipt of the written appeal. If the Administrative Review results in a denial of your appeal, you have the right to request a final Administrative Review by the Chief Benefits Officer or his or her designee in writing no later than 60 days after the receipt of the appeal denial. If your appeal is denied by the Chief Benefits Officer or his or her designee, you have the right to file suit in federal court under the Employee Retirement Income Security Act of 1974 (“ERISA�).

Benefits Update & News

New Dental Plus Plan for Early Retirees If you are thinking of early retirement, you may know that the Benefit Funds offered a prescription, vision and hearing plan to National Benefit Fund retirees age 55 or older with at least 10 years of service and to Greater New York Benefit Fund retirees between ages 62 and 65 with at least 25 years of service. As of January 1, 2015, the Benefit Funds are now instead providing a new health benefit plan for our not-yet-Medicareeligible early retirees called the Dental Plus Plan. This new plan includes a dental benefit ($3,000 a year for NBF, $1,200 a year for GNY); a hospital indemnity plan that pays $200 a day, for 10 days, each time you are admitted to a hospital; and a vision benefit.

We created the new Dental Plus Plan, just as we did for our Wage and Eligibility Class III active members, to allow early retirees who want to buy a medical plan on the new Health Insurance Marketplace to receive a subsidy, if eligible, to help pay for the plan. As you probably know, under the Affordable Care Act (ACA),

everyone must have “Minimum Essential Coverage� or face a penalty. Because the Prescription Plan is considered “Minimum Essential Coverage� under the ACA, early retirees would not face a penalty, but if they chose to buy a more complete medical plan on the Health Insurance Marketplace, they would not be able to access a subsidy. In some cases, the tax subsidy can reduce the monthly premium to zero. (Early retirees who think the Prescription Plan might work better for them have a one-time option to switch from the Dental Plus Plan.)

Avoid Out-of-Pocket Costs by Using Participating Providers As an 1199SEIU member, you have access to thousands of healthcare providers within our Benefit Fund’s network. That means you can avoid paying unnecessary out-of-pocket costs when you use participating providers. Doctors who are not in the network can charge anything they want, which can be a lot more than the Fund’s reimbursement rate, and you have to pay the full balance. So going outside of our network to a non-participating provider could result in an expensive – and avoidable – medical bill, so make sure that your doctor, and any provider he or she may refer you to, is part of our network. s

And because there is no better way to protect yourself against the flu, we urge you to get your annual flu shot if you have not gotten one already.

The National Benefit Fund believes it is a “grandfathered health plan” under the Patient Protection and Affordable Care Act (the Affordable Care Act). As permitted by the Affordable Care Act, a grandfathered health plan can preserve certain basic health coverage that was already in effect when that law was enacted.
