Dear Members, The holidays can be such a wonderful time to celebrate with our friends and family, but we all know they can also make it hard to focus on our health and well-being.

The important thing to remember is not to be so hard on yourself if you skip a workout or have that extra dessert. The New Year offers the perfect opportunity to get back on track, and your Benefit Fund is here to help.

If you would like a copy of the Fund’s Notice of HIPAA Privacy Practices, please visit www.1199SEIUBenefits.org, or request one in writing by writing to the 1199SEIU Benefit Fund Privacy Officer at 330 West 42nd St, 9th Floor, NY, NY 10036.

You are a plan participant, or are being offered the opportunity to enroll as a plan participant, in an employment-based health plan that is certified for participation in the Early Retiree Reinsurance Program. The Early Retiree Reinsurance Program is a Federal program that was established under the Affordable Care Act. Under the Early Retiree Reinsurance Program, the Federal government reimburses a plan sponsor of an employment-based health plan for some of the costs of healthcare benefits paid on behalf of, or by, early retirees and certain family members of early retirees participating in the employment-based plan. By law, the program expires on January 1, 2014.

Under the Early Retiree Reinsurance Program, your plan sponsor may choose to use any reimbursements it receives from this program to reduce or offset increases in plan participants’ premium contributions, co-payments, deductibles, co-insurance or other out-of-pocket costs.

In this issue of your member magazine, For Your Benefit, you will find practical tips on keeping yourself healthy through the winter, from fitting in nutritious meals and exercise to getting more sleep and leaning on your support network. We also remind you that it is still not too late to get your annual flu shot. In addition, you will read an inspiring story about how one of your fellow members used the workplace support of her Benefit Fund, Union and employer to lose 90 pounds and help ensure a healthier future for herself and her daughter.

If the plan sponsor chooses to use the Early Retiree Reinsurance Program reimbursements in this way, you, as a plan participant, may experience changes that may be advantageous to you, in your health plan coverage terms and conditions, for so long as the reimbursements under this program are available and this plan sponsor chooses to use the reimbursements for this purpose. A plan sponsor may also use the Early Retiree Reinsurance Program reimbursements to reduce or offset increases in its own costs for maintaining your health benefits coverage, which may increase the likelihood that it will continue to offer health benefits coverage to its retirees and employees and their families.

The National Benefit Fund believes it is a “grandfathered health plan” under the Patient Protection and Affordable Care Act (the Affordable Care Act). As permitted by the Affordable Care Act, a grandfathered health plan can preserve certain basic health coverage that was already in effect when that law was enacted. Being a grandfathered health plan means that this plan may not include certain consumer protections of the Affordable Care Act that apply to other plans, for example, the requirement for an external review process for claims appeals. However, grandfathered health plans must comply with certain other consumer protections in the Affordable Care Act, for example, the elimination of lifetime limits on benefits. Questions regarding which protections apply and which protections do not apply to a grandfathered health plan and what might cause a plan to change from grandfathered health plan status can be directed to the plan administrator at (646) 473-9200.

We have also included information about the valuable benefits your Fund provides, including our tax preparation assistance program, which helps thousands of members get more money back in their tax refunds every year, and upcoming wellness workshops. Finally, to make sure we can keep providing the benefits that you and your family depend on, it is important that we have your most up-to-date information on file. If you are a National Benefit Fund member and you received a letter requesting documentation to verify the eligibility of your dependents, please make sure you respond as soon as possible. Doing so will not only help ensure that your dependents continue to receive the healthcare they need, but it will protect our limited benefit dollars as well.

Inpatient rehabilitation and detoxification must be medically necessary, but the 30-day annual limit on inpatient rehabilitation and the 7-day annual limit on inpatient detoxification are hereby eliminated.

Have You Updated Your

Eligible Dependents’ Information?

If your spouse and/or children are covered by the National Benefit Fund, you may have received a letter in the fall requesting documentation to update their eligibility information from HMS Employer Solutions, who is conducting our Dependent Eligibility Verification Program. If you don’t send your information to HMS by the deadline, your dependents’ coverage will be terminated.

Completing the process is simple and easy: Just make copies of all your required documents, fill out any required Coordination of Benefit forms and submit everything to HMS online, by fax or by mail. If you believe your eligible dependents’ coverage was mistakenly terminated and you would like to reinstate their coverage, you must contact the Benefit Fund at (646) 473-9200 to find out what you need to do.

This Tax Year, Make Sure You Claim All That You’re Entitled to with the

Earned Income Tax Credit Take Advantage of Our Free Tax Prep Assistance Program and Make Sure Your Refund Is All It Can Be The 1199SEIU National Benefit Fund’s Tax Assistance Program is available to help you claim the Earned Income Tax Credit (EITC) and get the most out of your tax return.

The program has served thousands of members through the years, and in 2014, the Fund is again partnering with New York City’s EITC Coalition to help ensure your tax return is prepared accurately and your refund includes everything you’re entitled to. The EITC Coalition is led by the New York City Department of Consumer Affairs, and includes the City’s Office of Financial Empowerment and the Food Bank For New York City.

Section VI. F IF YOU RETIRE BETWEEN AGES 55 THROUGH 64 WITH AT LEAST 10 YEARS OF PENSION FUND CREDITED SERVICE

9OU ARE ELIGIBLE FOR THE BENElTS DESCRIBED IN THIS SECTION WHEN YOU RETIRE AT OR AFTER AGE WITH at least 10 Years of Pension Fund Credited Service.

If your spouse is covered by Medicare, he or she is covered for the same benefits, except for Life Insurance.

If your spouse is not covered by Medicare, he or she is covered only for prescription drugs, vision care and burial benefits. When your spouse reaches age 65 or is covered by Medicare, he or she will receive the same benefits you receive, except for Life Insurance.

If you are eligible for Medicare and live outside of New York City, Nassau County or designated counties in Florida, you will only be able to receive Retiree Health Benefits from the Benefit Fund if you are enrolled in the Fund’s Medicare Part D Prescription Drug Program, an Employer Group Waiver Program (EGWP) sponsored by the Fund and its Pharmacy Benefit Manager (Express Scripts). When your spouse becomes eligible for Medicare, he or she must enroll in this Prescription Drug program in order to receive benefits through the Benefit Fund.

This Benefit Fund Medicare Part D Prescription Program will provide you with a full prescription benefit similar to the benefit described in Section II.L of this Summary Plan Description. The Benefit Fund will supplement this Program by continuing to provide you with the Retiree Health Benefits described in this section.

In certain situations, the Fund may grant a waiver of this requirement if you are already enrolled in another Medicare Part D Plan. Please note that you and your spouse will be responsible for the full cost of your Medicare Part D premium, if any.

If you choose to disenroll from this Program for any reason, you and your spouse will no longer be eligible to receive the Retiree Health Benefits described in this section of this SPD from the Benefit Fund.

ELIGIBILITY To receive the benefits described in this section from the Benefit Fund, you must either (I) be covered by the Benefit Fund as an active employee right before you retire, or (II) retire from covered service within one (1) year of the date (a) your Employer was no longer obligated to make contributions on your behalf; or (b) your Employer was terminated as a Contributing Employer to the Benefit Fund; or (III) be receiving Benefit Fund Disability or an award of permanent partial disability or permanent total disability from Workers’ Compensation from a disabling condition or event that commenced or occurred while you were actively employed by a Contributing Employer to the Benefit Fund.

Your retiree health benefits through the Benefit Fund can’t exceed the coverage you had just before you retired. For example, if you did not have prescription benefits just before you retired, you are not covered for prescription benefits after you retire.

ELIGIBILITY To receive the benefits described in this section from the Benefit Fund, you must either (I) be covered by the Benefit Fund as an active employee right before you retire; or (II) retire from covered service within one (1) year of the date (a) your Employer was no longer obligated to make contributions on your behalf; or (b) your Employer was terminated as a Contributing Employer to the Benefit Fund; or (III) be receiving Benefit Fund Disability or an award of permanent partial disability or permanent total disability from Workers’ Compensation from a disabling condition or event that commenced or occurred while you were actively employed by a Contributing Employer to the Benefit Fund. …

As long as your 2013 household income was below $52,000 (or $20,000 if you are single and have no dependents), our partners at the Food Bank For New York City are offering free tax preparation events throughout the five boroughs.

The nonprofit financial education program Ariva is also offering free assistance, at sites in the Bronx. At Ariva’s site in the South Bronx, your 2013 household income must be below $52,000 if you have dependents or below $18,000 if you are single and have no dependents.

At Ariva’s site in the Northwest Bronx, your 2013 household income must be below $56,000 if you have dependents or below $25,000 if you are single and have no dependents. If you qualify for the EITC, the tax experts onsite will help you file for the credit. Even if you don’t live near one of these sites, be sure to check the “Claim Your EITC” guide at the top of the page to find out whether you qualify for the credit before filing your return.

For information about the Food Bank For New York City’s sites, call 311. For information about Ariva’s site in the South Bronx, call (718) 292-2983. The site will be open January 16 to April 15. For information about Ariva’s site in the Northwest Bronx – by appointment only – call (718) 933-3101. The site will be open January 29 to April 9.

Section VI. D IF YOU RETIRE AT AGE 65 OR OLDER AND LIVE OUTSIDE NEW YORK CITY, NASSAU COUNTY OR DESIGNATED COUNTIES IN FLORIDA

Pg. 138 MEDICARE PART D PREMIUM - INCOME RELATED ADJUSTMENT As a result of their enrollment in the Fund’s Medicare Prescription Employer Group Waiver Program (EGWP), some members whose incomes are above certain levels established by Medicare may incur an additional Medicare premium a reduction in their monthly Social Security benefit, which is known as an Income Related Medicare Adjustment Amount (IRMAA).

Section VIII. A YOUR ERISA RIGHTS …

CONTINUE GROUP HEALTH COVERAGE s )F YOU LOSE HEALTH COVERAGE FOR YOURSELF SPOUSE OR DEPENDENTS UNDER THE 0LAN AS A RESULT OF A qualifying event, you or your dependents may have to pay for continued coverage. Review this Summary Plan Description and the documents governing the Plan on the rules governing your COBRA continuation coverage rights. s You will be provided a Certificate of Creditable Coverage, free of charge, from the Plan when you lose coverage under the Plan,when you become entitled to elect COBRA continuation coverage, when your COBRA continuation coverage ceases, if you request it before losing coverage, or if you request it up to 24 months after losing coverage.

Without evidence of creditable Coverage, you may be subject to a preexisting condition exclusion for 12 months (18 months for late enrollees) after your enrollment date in your coverage.

Pg. 43 YOUR HIPAA RIGHTS When your Benefit Fund coverage ends, a federal law – the Health Insurance Portability and Accountability Act (HIPAA) – protects you if your new health plan excludes pre-existing conditions.

When your Benefit Fund coverage ends, under HIPAA you and/or your Dependents are entitled by law to, and will be provided with, a Certificate of Creditable Coverage. Certificates of Creditable Coverage indicate the period of time you and/or your Dependents were covered under the Fund (including COBRA coverage), as well as certain additional information required by law. The Certificate of Creditable Coverage may be necessary if you and/or your Dependents become eligible for coverage under another group health plan, or if you buy a health insurance policy within 63 days after your coverage under this Benefit Fund ends (including COBRA coverage).

The Certificate of Creditable Coverage is necessary because it may reduce any exclusion for preexisting coverage periods that may apply to you and/or your Dependents under the new group health plan or health insurance policy.

The Certificate of Creditable Coverage will be provided to you shortly after this Benefit Fund knows, or has reason to know, that coverage (including COBRA coverage) has ended. The Certificate of Creditable Coverage will also be provided once the Benefit Fund office receives a written request, provided that the request is received within two (2) years after the later of the date your coverage under the Benefit Fund ended or the date your COBRA coverage ended.

Accordingly, the Benefit Fund will provide you with Certificates of Creditable Coverage showing when you were covered by the Benefit Fund: on your request, within 24 months after your Benefit Fund coverage ceases; when you are entitled to elect COBRA (see Section I.K); when your coverage terminates, even if you are not entitled to COBRA (see Section I.K), and when your COBRA coverage ceases.

You should retain these Certificates of Creditable Coverage as proof of prior coverage for your new health plan. For further information, call the Member Services Department of the Benefit Fund at (646) 473-9200.

Section VI.A RETIREE HEALTH BENEFITS Pg. 127 Retiree Health Benefits are available for you and your eligible spouse only.

Dependent children are not covered for these benefits regardless of their age. ‌ Pg. 128 For example: If you did not have prescription coverage right before you retired, you are not covered for prescription benefits after you retire. or if your spouse was not covered while you were a working member, you cannot enroll your spouse after you retire. ‌

Section I.K YOUR COBRA RIGHTS Informing You of Your Rights 0G In considering whether to elect continuation coverage, you should take into account that a failure to continue your group health coverage will affect your future rights under federal law.

0G First, you may lose the right to avoid having pre-existing condition exclusions applied to you by other group health plans if you have more than a 63-day gap in health coverage, and election of continuation coverage may help you not have such a gap. Second, you may lose the guaranteed right to purchase individual health insurance policies that do not impose such pre-existing condition exclusions if you do not get continuation coverage for the maximum time available to you.

Section VI. C IF YOU RETIRE AT AGE 65 OR OLDER AND LIVE IN NEW YORK CITY, NASSAU COUNTY OR DESIGNATED COUNTIES IN FLORIDA

Five Great Reasons to Get Your Flu Shot Even though January and February are peak flu season, it’s still not too late to protect yourself, your family and your patients or residents from the potentially serious virus. If you still haven’t gotten your shot, here’s why you should:

1. Getting a flu shot is the single best way to protect yourself against the virus, according to health experts, so getting vaccinated now can still help you stay healthy through the winter.

2. As a healthcare worker, you have a higher chance of getting the flu since you work with many people who are already sick. You’re also at greater risk if you’re pregnant, age 50 and older or have a chronic medical condition such as asthma, diabetes or heart disease.

3. A flu shot cannot make you sick. This is one of the biggest myths surrounding the flu shot. The flu viruses in the vaccine are inactivated (killed), so they can’t cause infection, and the vaccine is tested for safety.

4. Healthcare workers are required by New York State law to get a flu shot or wear a mask when they’re near patients or residents. Getting the flu shot protects you and your patients or residents – and it’s a lot more convenient than wearing a mask.

5. Getting your flu shot is convenient – and covered! Flu vaccinations are available through most healthcare employers and worksites and at local Rite Aid pharmacies. If your employer isn’t offering the flu shot, remember that your Benefit Fund covers the vaccination, so just make an appointment with your healthcare provider.

Synobia Santiago Breaking a pattern of poor nutrition and fitness habits helped one young mother lose 90 pounds and changed her and her daughter’s lives.

It was a little more than two years ago when Synobia Santiago hit what felt like her lowest point. She was the proud mother of a beautiful little girl and was excited about their future, but Synobia’s weight was already taking a toll on what they could do together. Heavy for most of her life, she tired easily and struggled to find the energy to play with then 2-year-old Giselle. Enough is enough, Synobia remembered thinking to herself one afternoon; things have got to change. “I thought about the life that I wanted for her and I felt like I had to lead by example,� she said. “I didn’t want her to grow up overweight like I did.�

Only 21 years old at the time, Synobia considered herself lucky to have avoided any health problems related to her weight. But she also knew that it was only a matter of time before her luck ran out. Working in the housekeeping department at Mount Sinai Medical Center, Synobia said the culture of health created at her worksite and the encouragement of her Benefit Fund Outreach Coordinator and her Union organizers and delegates were constant reminders of the importance of a healthy lifestyle. That support, combined with her hopes for Giselle and memories of her own childhood, pushed her into action. “There were so many things I wanted to do that I missed out on because of my weight, like joining the volleyball team or going to school dances,� she said. “I want my daughter to live a healthy, full life, with no regrets due to obesity.�

Synobia had tried pills to suppress her appetite and fad diets in the past and said they not only had intolerable side effects – such as making her heart race – but they actually increased her cravings for unhealthy foods, so she was well aware that crash diets or other quick fixes were not the answer.

Instead, she set out to make changes she could sustain: specifically, better nutrition through a wellbalanced diet combined with an exercise program.

At first, she felt self-conscious about working out at a gym or the park, but she invested in an inexpensive

treadmill and began exercising at home. Before long, Synobia had gained the confidence to join a local gym and also started walking in Highland Park, close to where she lives in Brooklyn. Eventually, she worked her way up to a routine that includes jogging at a moderate pace. “Running has been a great release for me,� she said. “The feel of the wind blowing through my hair makes me feel like I’m leaving all my worries right there on the pavement.�

While exercising regularly remains a key to her health, Synobia has also eliminated nearly all the added sugars and carbohydrates from her diet. As a rule, she and Giselle now eat lean meats, vegetables and brown rice for their main meals – always making sure to stick with small portions, she said. For snacks, their favorites include bananas with a little peanut butter and plain popcorn, without butter or salt.

The results have been amazing, Synobia said, as she’s dropped 90 pounds and now focuses on maintaining her weight loss and toning her muscles. She added that she hopes her story can help inspire others to make positive changes in their life, saying she now has no regrets. Perhaps most important, she said, is that she now has plenty of energy for all the activities a 4-year-old can come up with. “Giselle is everything to me,� Synobia said. “We have so much fun together, but I know from experience it wouldn’t be this way if I didn’t take care of my health.�

The Benefit Fund will only pay benefits up to the amount the Benefit Fund would have paid if your spouse had been covered by his or her employer’s plan. You and your spouse will have to pay any charges not paid by the 1199SEIU National Benefit Fund and your spouse’s employer’s plan.

Your stepchildren, foster children and grandchildren are not covered by the Benefit Fund.

Children of your same-sex partner cannot be covered by the Benefit Fund, unless you are their legally recognized parent or they are legally adopted by or placed for adoption with you.

Pg. 36 WHEN CHILDREN ARE COVERED BY BOTH PARENTS If you and your spouse both have dependent coverage, benefits for your children are coordinated as follows: s The primary payers are the children’s employer-sponsored coverage through their employment and/or through their spouses, if any. s The secondary payer is the plan of the parent whose parent whose birthday is earliest in the year; and s 4HE OTHER PARENT S PLAN IS THE next payer. ‌

AFTER YOUR CHILD REACHES AGE 19 Your child’s coverage may be continued from your child’s 19th birthday up to his or her 26th birthday if: s You file a properly completed Young Adult Enrollment Form with the Benefit Fund when your child turns age 19 and each year after that until your child’s 26th birthday; and

WHEN YOU ARE COVERED BY MEDICARE ‌ If you prefer, you may elect to end your coverage under the Benefit Fund and elect to have Medicare as your only insurance. However, if you elect this option, the Benefit Fund may not provide any benefits that supplement those provided under Medicare.

They’re not eligible to enroll in another employer-sponsored health plan (excluding parent coverage). ‌

Section I. G WHEN OTHERS ARE RESPONSIBLE FOR YOUR ILLNESS OR INJURY ‌

QUALIFIED MEDICAL CHILD SUPPORT ORDER The Benefit Fund will comply with the terms of any Qualified Medical Child Support Order (QMCSO) as the term is defined in the Employee Retirement Income Security Act (ERISA) of 1974, as amended.

A QMCSO may require the Benefit Fund to make coverage available to your child even though the child is not, for income tax purposes or Fund purposes, your legal dependent, because of separation or divorce.

In order to be a qualified order, the medical child support order must: s Be issued by a court or authorized state agency; s Clearly specify the alternate recipient; s Reasonably describe the type of coverage to be provided to suchalternate recipient; s Clearly state the period to which such order applies; and s Indicate the name and last known address of the member who is required to provide the coverage and the name and mailing address of each child covered by the order.

The Plan Administrator will determine the qualified status of a medical child support order in accordance with the Benefit Fund’s written procedures.

Pg.39 If you fail or refuse to sign a lien or to comply with these terms, the Plan Administrator may suspend your eligibility for benefits, until the Benefit Fund is fully repaid.

Section I.F WHEN YOU AND YOUR SPOUSE OR CHILD ARE COVERED BY DIFFERENT PLANS

Winter-Proof Your Health!

Let’s face it: Staying healthy during the winter can be a challenge.

More time spent indoors, fewer daylight hours and the post-holiday blues can make sticking to your healthy eating and exercise routines tough. But that doesn’t mean it’s impossible. Here are our top ten winter survival tips: 1. Get a flu shot. Because it can’t be said enough: The flu shot is the best way to protect yourself against the virus. (And while you’re at it, don’t forget to wash your hands frequently to the tune of “Happy Birthday� twice!)

2. Head outside whenever you can.

For 10 to 20 percent of people in the U.S., the shorter days of winter can bring on the “winter blues,� or Seasonal Affective Disorder (SAD).

Making sure you get as much natural light as you can throughout the day can help.

3. Get plenty of sleep. Lack of quality sleep can leave you vulnerable to colds and flu. Avoid heavy meals, caffeine and alcohol close to bedtime; go to bed the same time every night; and make your bedroom sleep-friendly (no electronics).

4. Spend time with family and friends.

Connecting with family and friends is important all year round but especially during the winter months, when we tend to hibernate. Studies have shown that people with a support network have stronger immunity than those without a support system.

5. Don’t skip your workouts. Exercise does it all: It can help fight seasonal depression, help you maintain a healthy weight and bolster your immune system.

10. Get help sticking to your New Year’s resolutions. Enlist a friend with similar goals to stay motivated throughout the coming year.

The Centers for Disease Control and Prevention recommends strength training two days per week – in addition to cardio activity. Resistance training can help reduce the amount of muscle we lose with age, prevent low-back pain and improve balance and easy movement as we tackle our daily activities. Weight Loss Comes When You

Eat Fish, Live Longer You can add years to your life, just by eating fish. A recent study of nearly 2,700 people by the Harvard School of Public Health found that those who had high levels of omega-3 fatty acids in their blood from eating fish lived, on average, 2.2 years longer than those with the lowest levels. Fish eaters also had a 27 percent lower risk of dying from any cause and a 35 percent lower risk of developing heart disease.

The American Heart Association recommends eating two 3.5-ounce servings of fatty fish, like salmon, herring, anchovies or trout, each week.

Candidates must be high school graduates, postsecondary school students or previous awardees enrolled full time for a minimum of 12 college credits each semester. All candidates must be younger than 23 years of age by the start of the fall 2014 semester. Previous awardees seeking scholarship support should note that automatic award renewals are not granted; you must reapply each year for consideration.

2014 Anne Shore Sleep-Away Camp Program for

Children Ages 9 to 15 Years Each year, the Anne Shore Sleep-Away Camp Program sends more than 800 children of 1199SEIU members to three- and four-week sleep-away camps throughout the Northeast – at minimal cost to their parents.

La mayor parte del tiempo la pasamos en interiores, hay menos horas de luz y la depresión posterior a las fiestas puede hacer que seguir su rutina de alimentación saludable y ejercicios sea más difícil. Pero eso no significa que sea imposible. A continuación le presentamos nuestros diez consejos principales para sobrevivir el invierno:

7. Tome suficientes frutas y verduras ricas en vitaminas. Los productos alimenticios frescos como manzanas, uvas, pepinos, ciruelas y calabacines tienen mucha vitamina C y antioxidantes que pueden fortalecer su sistema inmunitario, y también son una excelente forma de mantenerse hidratado. Puede hacer de las sopas sustanciosas su plato principal, utilizando verduras de invierno como por ejemplo, zapallos, zanahorias, remolachas, brócoli y pastinacas.

4. Pase tiempo con la familia y amigos.

Conectarse con la familia y amigos es importante durante todo el año pero especialmente durante los meses del invierno, cuando tenemos la tendencia de hibernar. Los estudios han demostrado que las personas con una red de apoyo tienen una inmunidad más fuerte que aquellos que no cuentan con ella.

3. Duerma bastante. La falta de un sueño de calidad le puede hacer vulnerable a los resfriados y la gripe. Evite comidas pesadas, cafeína y el alcohol cerca de la hora de acostarse, vaya a dormir a la misma hora cada noche y haga que su dormitorio sea adecuado para dormir (sin dispositivos electrónicos).

¿Ya ha Actualizado la

Información de Sus Dependientes Elegibles?

Si su cónyuge o sus hijos están cubiertos por el Fondo Nacional de Beneficios, es posible que haya recibido una carta en el otoño en la que se le solicita documentación para actualizar su información de elegibilidad procedente de HMS Employer Solutions, quien está llevando a cabo nuestro Programa de Verificación de la Elegibilidad de los Dependientes. Si no envía su información a HMS antes de la fecha límite, se dará por terminada la cobertura de sus dependientes.

Perfil del Miembro

Synobia Santiago Romper un patrón de nutrición deficiente y malos hábitos de acondicionamiento físico ayudó a una joven madre a perder 90 libras y cambió su vida y la de su hija.

Fue hace poco más de dos años cuando Synobia Santiago alcanzó lo que sintió que fue su punto más bajo. Era una orgullosa madre de una linda niña y estaba muy entusiasmada sobre su futuro, pero el peso de Synobia ya estaba afectando lo que podían hacer juntas.

Con sobrepeso durante la mayor parte de su vida, se cansaba fácilmente y luchaba por encontrar la energía para jugar con Giselle que entonces solo tenía 2 años de edad. Synobia recordaba haber pensado para sí misma una tarde: ya basta, las cosas tienen que cambiar. “Pensé sobre la vida que quería para ella y sentí que tenía que enseñar con el ejemplo”, dijo. “No quiero que crezca con sobrepeso como lo hice yo”.

Al trabajar en el departamento de limpieza del Mount Sinai Medical Center, Synobia dijo que la cultura de salud creada en su centro de trabajo y el apoyo de su Coordinador de Ayuda del Fondo de Beneficios y de sus organizadores y delegados del sindicato fueron recordatorios constantes de la importancia de un estilo de vida saludable. Ese apoyo, combinado con sus esperanzas para Giselle y los recuerdos de su propia infancia, la motivaron a tomar medidas. “Había tantas cosas que quería hacer y que no hice debido a mi peso, como unirme al equipo de voleibol o ir a los bailes de la escuela”, dijo ella.

“Quiero que mi hija viva una vida saludable y plena, sin arrepentimientos debido a la obesidad”.

Synobia había intentado en el pasado con píldoras para reprimir el apetito y las dietas de moda y dijo que no solo tenían efectos secundarios intolerables, tales como hacer que su ritmo cardíaco se acelerara, sino que en realidad aumentaban sus ansias por consumir alimentos no saludables. Por ello sabía muy bien que las dietas repentinas u otras soluciones rápidas no eran la solución.

En lugar de ello, se decidió a hacer cambios que pudiera mantener: Específicamente, una mejor nutrición a través de una dieta bien balanceada en combinación con un programa de ejercicios. Al comienzo, se sentía avergonzada como para ir a hacer ejercicios en un gimnasio o en el parque, pero invirtió en una banda caminadora barata y comenzó a hacer ejercicios en su

Cinco Grandes Motivos para Ponerse la Vacuna contra la Gripe Incluso aunque en enero y febrero son la temporada alta de la gripe, todavía no es tarde para protegerse a sí mismo, a su familia y a sus pacientes o residentes de un virus potencialmente grave. Synobia Santiago Romper un patrón de nutrición deficiente y malos hábitos de acondicionamiento físico ayudó a una joven madre a perder 90 libras y cambió su vida y la de su hija.

Fue hace poco más de dos años cuando Synobia Santiago alcanzó lo que sintió que fue su punto más bajo. Era una orgullosa madre de una linda niña y estaba muy entusiasmada sobre su futuro, pero el peso de Synobia ya estaba afectando lo que podían hacer juntas.

Con sobrepeso durante la mayor parte de su vida, se cansaba fácilmente y luchaba por encontrar la energía para jugar con Giselle que entonces solo tenía 2 años de edad. Synobia recordaba haber pensado para sí misma una tarde: ya basta, las cosas tienen que cambiar. “Pensé sobre la vida que quería para ella y sentí que tenía que enseñar con el ejemplo”, dijo. “No quiero que crezca con sobrepeso como lo hice yo”.

Ese apoyo, combinado con sus esperanzas para Giselle y los recuerdos de su propia infancia, la motivaron a tomar medidas. “Había tantas cosas que quería hacer y que no hice debido a mi peso, como unirme al equipo de voleibol o ir a los bailes de la escuela”, dijo ella.

“Quiero que mi hija viva una vida saludable y plena, sin arrepentimientos debido a la obesidad”.

Synobia había intentado en el pasado con píldoras para reprimir el apetito y las dietas de moda y dijo que no solo tenían efectos secundarios intolerables, tales como hacer que su ritmo cardíaco se acelerara, sino que en realidad aumentaban sus ansias por consumir alimentos no saludables. Por ello sabía muy bien que las dietas repentinas u otras soluciones rápidas no eran la solución.

Información de Sus Dependientes Elegibles?

Si su cónyuge o sus hijos están cubiertos por el Fondo Nacional de Beneficios, es posible que haya recibido una carta en el otoño en la que se le solicita documentación para actualizar su información de elegibilidad procedente de HMS Employer Solutions, quien está llevando a cabo nuestro Programa de Verificación de la Elegibilidad de los Dependientes. Si no envía su información a HMS antes de la fecha límite, se dará por terminada la cobertura de sus dependientes.

Hay que admitirlo: mantenerse saludable durante el invierno puede ser un desafío.

La mayor parte del tiempo la pasamos en interiores, hay menos horas de luz y la depresión posterior a las fiestas puede hacer que seguir su rutina de alimentación saludable y ejercicios sea más difícil. Pero eso no significa que sea imposible. A continuación le presentamos nuestros diez consejos principales para sobrevivir el invierno:

Eat Fish, Live Longer You can add years to your life, just by eating fish. A recent study of nearly 2,700 people by the Harvard School of Public Health found that those who had high levels of omega-3 fatty acids in their blood from eating fish lived, on average, 2.2 years longer than those with the lowest levels. Fish eaters also had a 27 percent lower risk of dying from any cause and a 35 percent lower risk of developing heart disease.

The American Heart Association recommends eating two 3.5-ounce servings of fatty fish, like salmon, herring, anchovies or trout, each week.

Let’s face it: Staying healthy during the winter can be a challenge.

More time spent indoors, fewer daylight hours and the post-holiday blues can make sticking to your healthy eating and exercise routines tough. But that doesn’t mean it’s impossible. Here are our top ten winter survival tips: 1. Get a flu shot. Because it can’t be said enough: The flu shot is the best way to protect yourself against the virus. (And while you’re at it, don’t forget to wash your hands frequently to the tune of “Happy Birthday� twice!)

2. Head outside whenever you can.

For 10 to 20 percent of people in the U.S., the shorter days of winter can bring on the “winter blues,� or Seasonal Affective Disorder (SAD).

Making sure you get as much natural light as you can throughout the day can help.

3. Get plenty of sleep. Lack of quality sleep can leave you vulnerable to colds and flu. Avoid heavy meals, caffeine and alcohol close to bedtime; go to bed the same time every night; and make your bedroom sleep-friendly (no electronics).

4. Spend time with family and friends.

Connecting with family and friends is important all year round but especially during the winter months, when we tend to hibernate. Studies have shown that people with a support network have stronger immunity than those without a support system.

She was the proud mother of a beautiful little girl and was excited about their future, but Synobia’s weight was already taking a toll on what they could do together. Heavy for most of her life, she tired easily and struggled to find the energy to play with then 2-year-old Giselle. Enough is enough, Synobia remembered thinking to herself one afternoon; things have got to change. “I thought about the life that I wanted for her and I felt like I had to lead by example,� she said. “I didn’t want her to grow up overweight like I did.�

Only 21 years old at the time, Synobia considered herself lucky to have avoided any health problems related to her weight. But she also knew that it was only a matter of time before her luck ran out. Working in the housekeeping department at Mount Sinai Medical Center, Synobia said the culture of health created at her worksite and the encouragement of her Benefit Fund Outreach Coordinator and her Union organizers and delegates were constant reminders of the importance of a healthy lifestyle. That support, combined with her hopes for Giselle and memories of her own childhood, pushed her into action. “There were so many things I wanted to do that I missed out on because of my weight, like joining the volleyball team or going to school dances,� she said. “I want my daughter to live a healthy, full life, with no regrets due to obesity.�

Synobia had tried pills to suppress her appetite and fad diets in the past and said they not only had intolerable side effects – such as making her heart race – but they actually increased her cravings for unhealthy foods, so she was well aware that crash diets or other quick fixes were not the answer.

Instead, she set out to make changes she could sustain: specifically, better nutrition through a wellbalanced diet combined with an exercise program.

At first, she felt self-conscious about working out at a gym or the park, but she invested in an inexpensive

treadmill and began exercising at home. Before long, Synobia had gained the confidence to join a local gym and also started walking in Highland Park, close to where she lives in Brooklyn. Eventually, she worked her way up to a routine that includes jogging at a moderate pace. “Running has been a great release for me,� she said. “The feel of the wind blowing through my hair makes me feel like I’m leaving all my worries right there on the pavement.�

While exercising regularly remains a key to her health, Synobia has also eliminated nearly all the added sugars and carbohydrates from her diet. As a rule, she and Giselle now eat lean meats, vegetables and brown rice for their main meals – always making sure to stick with small portions, she said. For snacks, their favorites include bananas with a little peanut butter and plain popcorn, without butter or salt.

The results have been amazing, Synobia said, as she’s dropped 90 pounds and now focuses on maintaining her weight loss and toning her muscles. She added that she hopes her story can help inspire others to make positive changes in their life, saying she now has no regrets. Perhaps most important, she said, is that she now has plenty of energy for all the activities a 4-year-old can come up with. “Giselle is everything to me,� Synobia said. “We have so much fun together, but I know from experience it wouldn’t be this way if I didn’t take care of my health.�

The Benefit Fund will only pay benefits up to the amount the Benefit Fund would have paid if your spouse had been covered by his or her employer’s plan. You and your spouse will have to pay any charges not paid by the 1199SEIU National Benefit Fund and your spouse’s employer’s plan.

Your stepchildren, foster children and grandchildren are not covered by the Benefit Fund.

Children of your same-sex partner cannot be covered by the Benefit Fund, unless you are their legally recognized parent or they are legally adopted by or placed for adoption with you.

Pg. 36 WHEN CHILDREN ARE COVERED BY BOTH PARENTS If you and your spouse both have dependent coverage, benefits for your children are coordinated as follows: s The primary payers are the children’s employer-sponsored coverage through their employment and/or through their spouses, if any. s The secondary payer is the plan of the parent whose parent whose birthday is earliest in the year; and s 4HE OTHER PARENT S PLAN IS THE next payer. ‌

AFTER YOUR CHILD REACHES AGE 19 Your child’s coverage may be continued from your child’s 19th birthday up to his or her 26th birthday if: s You file a properly completed Young Adult Enrollment Form with the Benefit Fund when your child turns age 19 and each year after that until your child’s 26th birthday; and

WHEN YOU ARE COVERED BY MEDICARE ‌ If you prefer, you may elect to end your coverage under the Benefit Fund and elect to have Medicare as your only insurance. However, if you elect this option, the Benefit Fund may not provide any benefits that supplement those provided under Medicare.

They’re not eligible to enroll in another employer-sponsored health plan (excluding parent coverage). ‌

Section I. G WHEN OTHERS ARE RESPONSIBLE FOR YOUR ILLNESS OR INJURY ‌

QUALIFIED MEDICAL CHILD SUPPORT ORDER The Benefit Fund will comply with the terms of any Qualified Medical Child Support Order (QMCSO) as the term is defined in the Employee Retirement Income Security Act (ERISA) of 1974, as amended.

A QMCSO may require the Benefit Fund to make coverage available to your child even though the child is not, for income tax purposes or Fund purposes, your legal dependent, because of separation or divorce.

In order to be a qualified order, the medical child support order must: s Be issued by a court or authorized state agency; s Clearly specify the alternate recipient; s Reasonably describe the type of coverage to be provided to suchalternate recipient; s Clearly state the period to which such order applies; and s Indicate the name and last known address of the member who is required to provide the coverage and the name and mailing address of each child covered by the order.

The Plan Administrator will determine the qualified status of a medical child support order in accordance with the Benefit Fund’s written procedures.

Pg.39 If you fail or refuse to sign a lien or to comply with these terms, the Plan Administrator may suspend your eligibility for benefits, until the Benefit Fund is fully repaid.

Section I.F WHEN YOU AND YOUR SPOUSE OR CHILD ARE COVERED BY DIFFERENT PLANS

Section I. I Pg. 42 WHEN YOUR BENEFITS STOP If you are no longer employed by a Contributing Employer, stop working or your employer is not obligated to make payments to the Benefit Fund on your behalf: All benefits end 30 days after the last day for which your employer is required to make contributions to the Benefit Fund* on your behalf unless your benefits are continued as described in Sections I.D and I.J or Section VI. However, your benefits end on the last day of employment if you or your spouse are eligible for Medicare, in which case, coverage for – Retiree Health Benefits medical (doctor visits, laboratory and radiology), surgical and hospital benefits will stop immediately * This may include contributions for severance or other wages paid to you, such as vacation, etc. ‌

When your spouse or child is covered by another plan, or benefit coverage is available through your spouse’s employer, the Benefit Fund will coordinate payment of your benefits with that Plan.

For your care: s 4HE "ENElT &UND IS THE primary payer. It makes the first payment on your claim. s 9OUR SPOUSE S PLAN IS YOUR secondary payer. It may cover any remaining balance, according to the terms of that plan.

For your spouse’s care: s 9OUR SPOUSE S PLAN IS THE primary payer. The Benefit Fund is your spouse’s secondary payer.

For your child’s care: s When your child is covered by another employer-sponsored plan (excluding parent coverage), your child’s plan is the primary payer.

Pg. 43 YOUR HIPAA RIGHTS When your Benefit Fund coverage ends, a federal law – the Health Insurance Portability and Accountability Act (HIPAA) – protects you if your new health plan excludes pre-existing conditions.

When your Benefit Fund coverage ends, under HIPAA you and/or your Dependents are entitled by law to, and will be provided with, a Certificate of Creditable Coverage. Certificates of Creditable Coverage indicate the period of time you and/or your Dependents were covered under the Fund (including COBRA coverage), as well as certain additional information required by law. The Certificate of Creditable Coverage may be necessary if you and/or your Dependents become eligible for coverage under another group health plan, or if you buy a health insurance policy within 63 days after your coverage under this Benefit Fund ends (including COBRA coverage).

The Certificate of Creditable Coverage is necessary because it may reduce any exclusion for preexisting coverage periods that may apply to you and/or your Dependents under the new group health plan or health insurance policy.

The Certificate of Creditable Coverage will be provided to you shortly after this Benefit Fund knows, or has reason to know, that coverage (including COBRA coverage) has ended. The Certificate of Creditable Coverage will also be provided once the Benefit Fund office receives a written request, provided that the request is received within two (2) years after the later of the date your coverage under the Benefit Fund ended or the date your COBRA coverage ended.

Accordingly, the Benefit Fund will provide you with Certificates of Creditable Coverage showing when you were covered by the Benefit Fund: on your request, within 24 months after your Benefit Fund coverage ceases; when you are entitled to elect COBRA (see Section I.K); when your coverage terminates, even if you are not entitled to COBRA (see Section I.K), and when your COBRA coverage ceases.

You should retain these Certificates of Creditable Coverage as proof of prior coverage for your new health plan. For further information, call the Member Services Department of the Benefit Fund at (646) 473-9200.

Section VI.A RETIREE HEALTH BENEFITS Pg. 127 Retiree Health Benefits are available for you and your eligible spouse only.

Dependent children are not covered for these benefits regardless of their age. ‌ Pg. 128 For example: If you did not have prescription coverage right before you retired, you are not covered for prescription benefits after you retire. or if your spouse was not covered while you were a working member, you cannot enroll your spouse after you retire. ‌

Section I.K YOUR COBRA RIGHTS Informing You of Your Rights 0G In considering whether to elect continuation coverage, you should take into account that a failure to continue your group health coverage will affect your future rights under federal law.

0G First, you may lose the right to avoid having pre-existing condition exclusions applied to you by other group health plans if you have more than a 63-day gap in health coverage, and election of continuation coverage may help you not have such a gap. Second, you may lose the guaranteed right to purchase individual health insurance policies that do not impose such pre-existing condition exclusions if you do not get continuation coverage for the maximum time available to you.

Five Great Reasons to Get Your Flu Shot Even though January and February are peak flu season, it’s still not too late to protect yourself, your family and your patients or residents from the potentially serious virus. If you still haven’t gotten your shot, here’s why you should:

1. Getting a flu shot is the single best way to protect yourself against the virus, according to health experts, so getting vaccinated now can still help you stay healthy through the winter.

2. As a healthcare worker, you have a higher chance of getting the flu since you work with many people who are already sick. You’re also at greater risk if you’re pregnant, age 50 and older or have a chronic medical condition such as asthma, diabetes or heart disease.

3. A flu shot cannot make you sick. This is one of the biggest myths surrounding the flu shot. The flu viruses in the vaccine are inactivated (killed), so they can’t cause infection, and the vaccine is tested for safety.

4. Healthcare workers are required by New York State law to get a flu shot or wear a mask when they’re near patients or residents. Getting the flu shot protects you and your patients or residents – and it’s a lot more convenient than wearing a mask.

5. Getting your flu shot is convenient – and covered! Flu vaccinations are available through most healthcare employers and worksites and at local Rite Aid pharmacies. If your employer isn’t offering the flu shot, remember that your Benefit Fund covers the vaccination, so just make an appointment with your healthcare provider.

If your spouse is covered by Medicare, he or she is covered for the same benefits, except for Life Insurance.

If your spouse is not covered by Medicare, he or she is covered only for prescription drugs, vision care and burial benefits. When your spouse reaches age 65 or is covered by Medicare, he or she will receive the same benefits you receive, except for Life Insurance.

If you are eligible for Medicare and live outside of New York City, Nassau County or designated counties in Florida, you will only be able to receive Retiree Health Benefits from the Benefit Fund if you are enrolled in the Fund’s Medicare Part D Prescription Drug Program, an Employer Group Waiver Program (EGWP) sponsored by the Fund and its Pharmacy Benefit Manager (Express Scripts). When your spouse becomes eligible for Medicare, he or she must enroll in this Prescription Drug program in order to receive benefits through the Benefit Fund.

This Benefit Fund Medicare Part D Prescription Program will provide you with a full prescription benefit similar to the benefit described in Section II.L of this Summary Plan Description. The Benefit Fund will supplement this Program by continuing to provide you with the Retiree Health Benefits described in this section.

In certain situations, the Fund may grant a waiver of this requirement if you are already enrolled in another Medicare Part D Plan. Please note that you and your spouse will be responsible for the full cost of your Medicare Part D premium, if any.

If you choose to disenroll from this Program for any reason, you and your spouse will no longer be eligible to receive the Retiree Health Benefits described in this section of this SPD from the Benefit Fund.

As long as your 2013 household income was below $52,000 (or $20,000 if you are single and have no dependents), our partners at the Food Bank For New York City are offering free tax preparation events throughout the five boroughs.

The nonprofit financial education program Ariva is also offering free assistance, at sites in the Bronx. At Ariva’s site in the South Bronx, your 2013 household income must be below $52,000 if you have dependents or below $18,000 if you are single and have no dependents.

At Ariva’s site in the Northwest Bronx, your 2013 household income must be below $56,000 if you have dependents or below $25,000 if you are single and have no dependents. Have You Updated Your

Eligible Dependents’ Information?

If your spouse and/or children are covered by the National Benefit Fund, you may have received a letter in the fall requesting documentation to update their eligibility information from HMS Employer Solutions, who is conducting our Dependent Eligibility Verification Program. If you don’t send your information to HMS by the deadline, your dependents’ coverage will be terminated.

Completing the process is simple and easy: Just make copies of all your required documents, fill out any required Coordination of Benefit forms and submit everything to HMS online, by fax or by mail. If you believe your eligible dependents’ coverage was mistakenly terminated and you would like to reinstate their coverage, you must contact the Benefit Fund at (646) 473-9200 to find out what you need to do.

Dear Members, The holidays can be such a wonderful time to celebrate with our friends and family, but we all know they can also make it hard to focus on our health and well-being.

The important thing to remember is not to be so hard on yourself if you skip a workout or have that extra dessert. The New Year offers the perfect opportunity to get back on track, and your Benefit Fund is here to help.

You are a plan participant, or are being offered the opportunity to enroll as a plan participant, in an employment-based health plan that is certified for participation in the Early Retiree Reinsurance Program. The Early Retiree Reinsurance Program is a Federal program that was established under the Affordable Care Act. Under the Early Retiree Reinsurance Program, the Federal government reimburses a plan sponsor of an employment-based health plan for some of the costs of healthcare benefits paid on behalf of, or by, early retirees and certain family members of early retirees participating in the employment-based plan. By law, the program expires on January 1, 2014.

Under the Early Retiree Reinsurance Program, your plan sponsor may choose to use any reimbursements it receives from this program to reduce or offset increases in plan participants’ premium contributions, co-payments, deductibles, co-insurance or other out-of-pocket costs.

The New Year offers the perfect opportunity to get back on track, and your Benefit Fund is here to help.
