Just over a year ago, we welcomed our home care members back into the 1199SEIU Home Care Benefit Fund. We were able to offer eligible members two plans to choose from: Plan A, our Member Choice Home Care Select plan, and Plan B, our Panel Provider plan. Both offer quality health coverage, but with Plan A you have no co-payments for primary care when you select a “medical home,� a health center where your doctors, nurses and other healthcare professionals know you and your medical history and tailor your care to fit your needs. If you are in Plan B, you do have to make co-payments for certain services and prescription coverage – in addition to your weekly premium.

With open enrollment beginning February 1, you have the opportunity to re-evaluate your choice. If you would like to participate in Plan A but your doctor is not a participating provider in one of the Fund’s health centers, you might want to ask if he or she would be interested in becoming one.

In this special edition of For Your Benefit, you will also find information on applying for the Earned Income Tax Credit when you file your tax return.

The Fund has again partnered with the Food Bank For New York City and, for the first time, the Ariva financial education group to offer free tax preparation to members throughout the five boroughs and assist them in filing for the tax credit. Over the years, many home care members have used this program to get thousands of dollars back at tax time, and we encourage you to take advantage of it this year.

Of course, even if you are not enrolled for health benefits, as an eligible 1199SEIU member you have access to a range of other valuable resources.

We have included details on just some of the programs and services your Fund provides, such as your pension benefit, our Citizenship Program to help you prepare for the U.S. citizenship application process and our Member Assistance Program, which can help you find the assistance you need if you’re struggling with personal issues. Please remember that your Benefit Fund is here to serve you.

This is a summary of the annual report for the 1199SEIU National Benefit Fund for Home Care Employees, EIN: 13-4129368, for the period January 1, 2011, to December 31, 2011. This is a welfare benefit plan offering life, health, dental, vision and prescription drug benefits. The annual report has been filed with the Employee Benefits Security Administration, as required under the Employee Retirement Income Security Act of 1974 (ERISA).

Except with respect to the benefits specified under “Insurance Information� below, the plan is selfinsured and has committed to pay covered claims out of plan assets.

Insurance Information During 2011, the plan had a contract with Fidelis Care New York to pay certain life, health, dental, vision and prescription drug claims incurred under the terms of the plan. The total premiums paid for the plan year ending December 31, 2011, were $145,142,476.

Basic Financial Statement The value of plan assets, after subtracting liabilities of the plan, was $15,301,081 as of December 31, 2011, compared to $14,010,192 as of January 1, 2011. During the plan year the plan experienced an increase in its net assets of $1,290,889. This increase includes unrealized appreciation and depreciation in the value of the plan assets: that is, the difference between the value of the plan’s assets at the end of the year and the value of the assets at the beginning of the year or the cost of assets acquired during the year. The plan had total income of $192,074,343, including employer contributions of $144,986,543, NYC Personal Care Providers Rate Adjustment contributions of $46,219,668, COBRA payments of $117,566, and member premiums of $738,977.

Plan expenses were $190,783,454. These expenses included $10,419,333 in administrative expenses, $179,467,295 in benefits paid to participants and beneficiaries, and $132,907 in general expenses.

In response to Hurricane Sandy, the Plan will not terminate benefit coverage regardless of loss or reduction of hours for all Participants during the period from October 29, 2012, through November 30, 2012. Further, Participants who are required to pay, and cannot afford to pay, the weekly co-premium due to a reduction of hours during the period from October 29, 2012, through November 30, 2012, may apply to the Fund to obtain a waiver of co-premium for the weeks during which their hours were reduced.

The Fund believes it is a “grandfathered health plan� under the Patient Protection and Affordable Care Act (the Affordable Care Act). As permitted by the Affordable Care Act, a grandfathered health plan can preserve certain basic health coverage that was already in effect when that law was enacted. Being a grandfathered health plan means that this plan may not include certain consumer protections of the Affordable Care Act that apply to other plans, for example, the requirement for an external review process for claims appeals. However, grandfathered health plans must comply with certain other consumer protections in the Affordable Care Act, for example, the elimination of lifetime limits on benefits.

It’s important to be as sure as you can that your pension, Social Security benefits and other savings will provide you with enough monthly income to meet your expenses. Our Pension Department staff will give you the most up-to-date estimate of what your pension payments will be, and refer you to the agencies that may be providing your other benefits, such as Medicare and Social Security, so that you are fully informed and can feel secure in your planning. Everyone feels troubled or overwhelmed now and then. If you’re dealing with personal issues that could put your health or your job in jeopardy, the Benefit Fund’s Member Assistance Program (MAP) is available to offer you confidential assistance.

Our MAP staff of social workers, counselors and other professionals can help direct you to the best available care for your particular situation. Whether you’re struggling with alcohol or substance abuse; stress, anxiety, depression or other mental health issues; domestic violence or the death of a loved one, they will help you get the support you need. If you didn’t sign up for health coverage through the Fund because you don’t think you can afford it, you may be eligible for premium assistance, and our MAP staff can direct you to the appropriate programs.

Retirement Benefit As an 1199SEIU home care member you are eligible for a defined pension, while fewer and fewer workers are receiving this valuable retirement benefit from their employers. When you retire, you may earn a “defined benefit” from the 1199SEIU home care Employees Pension Fund, which means you may receive a monthly benefit based on the pension credits you’ve earned during your years as a member. In many defined benefit plans, workers contribute to the plan to cover a percentage of the cost, but as an 1199SEIU Home Care worker, you don’t have to make any out-of-pocket contributions to receive a pension when you retire.

Make Sure Your Tax Refund Is All It Can Be For years, the Benefit Fund’s Tax Assistance Program has helped eligible members claim the Earned Income Tax Credit (EITC) and get the most out of their tax return.

In 2012, the Fund is again partnering with New York City’s EITC Coalition to help you prepare your tax return. The EITC Coalition is led by the New York City Department of Consumer Affairs, and includes the city’s Office of Financial Empowerment and the Food Bank For New York City.

Visit the Food Bank For New York City and Ariva Sites for Free Tax Preparation As long as your 2012 household income was below $50,000 (or $18,000 if you are single and have no dependents), our partners at the Food Bank For New York City are offering free tax preparation events throughout the five boroughs, and the nonprofit financial education program Ariva is offering free assistance at two sites in the Bronx, including one in the Northwest Bronx for filers with a 2012 household income below $56,000 (or $25,000 if you are single and have no dependents).

If you qualify for the EITC, the tax experts onsite will help you file for the credit. Even if you don’t live near one of these sites, be sure to check the “Claim Your EITC” box at right to find out whether you qualify for the credit before filing your return.

It’s Open Enrollment Time Once again, if you are not receiving health benefits through the Fund or you would like to change plans, there are two enrollment periods during which eligible members can sign up for health coverage this year. The first is February 1 to February 28 for coverage beginning April 1. The second will be August 1 to August 31, for coverage beginning October 1.

To enroll, you must choose between two plans. In Plan A, our Member Choice Home Care Select plan, you have no co-payments for medical services when you use your pre-selected health center for primary care.You also have no co-payments for covered prescription medications. In Plan B, our Panel Provider plan, you receive the same quality care by using participating providers in the Home Care Benefit Fund network, but you must make co-payments for certain services in addition to your weekly premium contribution.

Get All the Information You Need As you know, the Home Care Benefit Fund offers two healthcare plan options: Plan A and Plan B. If you are receiving health benefits through the Fund, you recently received a mailing from us containing a Summary of Benefits and Coverage (SBC) for the plan in which you are currently enrolled. However, if you would like a copy of the SBC for the other plan, you can view it on our website at www.1199SEIUBenefits.org.

You may also obtain a paper copy of the SBC for either plan free of charge by calling (646) 473-9200.

Keep Your Coverage Even If Your Employment Status Changes It’s especially important to maintain your health coverage if you are out of work on family leave, because of a short- or long-term disability, or in the event of a Workers’ Compensation claim. To maintain your coverage under any of these circumstances, you must continue to pay your weekly premium directly to the Fund every month. If you do not, your coverage will end. Once you return to work, your employer will resume automatically deducting your premium contribution from your paycheck.

If You Are Eligible for Medicaid or Family Health Plus New York State will pay your weekly premium directly to the Fund if you are income-eligible for Medicaid or Family Health Plus (FHP). If you earn more money and your pay exceeds the eligibility requirements for Medicaid or FHP, you must authorize your employer to begin making the weekly deduction from your paycheck. If your eligibility for Medicaid or FHP changes, you will be notified by the State.

Para inscribirse, debe elegir entre dos planes. En el Plan A, nuestro plan Member Choice Home Care Select, no tiene que efectuar copagos por servicios médicos cuando utiliza el centro de salud preseleccionado por usted para sus cuidados primarios.

El primero es del 1 al 28 de febrero para la cobertura que se inicia el 1 de abril. El segundo será del 1 al 31 de agosto para la cobertura que se inicia el 1 de octubre.

Para inscribirse, debe elegir entre dos planes. En el Plan A, nuestro plan Member Choice Home Care Select, no tiene que efectuar copagos por servicios médicos cuando utiliza el centro de salud preseleccionado por usted para sus cuidados primarios.

Under Plan A, our Member Choice Home Care Select plan, you choose one health center for all of your primary care and medical needs. (Your dental coverage is provided through Healthplex.) This will be your “medical home,” a place convenient to your home or worksite where the providers know you and your medical history, and a team of doctors, nurses and other healthcare professionals can tailor your care to best fit your needs.

With Plan A, you have no co-payments for primary care, specialty care, hospital visits or prescription drugs as long as you use your selected health center – you are responsible only for your weekly premium.

It’s Open Enrollment Time Once again, if you are not receiving health benefits through the Fund or you would like to change plans, there are two enrollment periods during which eligible members can sign up for health coverage this year. The first is February 1 to February 28 for coverage beginning April 1. The second will be August 1 to August 31, for coverage beginning October 1.

To enroll, you must choose between two plans. In Plan A, our Member Choice Home Care Select plan, you have no co-payments for medical services when you use your pre-selected health center for primary care.You also have no co-payments for covered prescription medications. In Plan B, our Panel Provider plan, you receive the same quality care by using participating providers in the Home Care Benefit Fund network, but you must make co-payments for certain services in addition to your weekly premium contribution.

Get All the Information You Need As you know, the Home Care Benefit Fund offers two healthcare plan options: Plan A and Plan B. If you are receiving health benefits through the Fund, you recently received a mailing from us containing a Summary of Benefits and Coverage (SBC) for the plan in which you are currently enrolled. However, if you would like a copy of the SBC for the other plan, you can view it on our website at www.1199SEIUBenefits.org.

You may also obtain a paper copy of the SBC for either plan free of charge by calling (646) 473-9200.

Keep Your Coverage Even If Your Employment Status Changes It’s especially important to maintain your health coverage if you are out of work on family leave, because of a short- or long-term disability, or in the event of a Workers’ Compensation claim. To maintain your coverage under any of these circumstances, you must continue to pay your weekly premium directly to the Fund every month. If you do not, your coverage will end. Once you return to work, your employer will resume automatically deducting your premium contribution from your paycheck.

If You Are Eligible for Medicaid or Family Health Plus New York State will pay your weekly premium directly to the Fund if you are income-eligible for Medicaid or Family Health Plus (FHP). If you earn more money and your pay exceeds the eligibility requirements for Medicaid or FHP, you must authorize your employer to begin making the weekly deduction from your paycheck. If your eligibility for Medicaid or FHP changes, you will be notified by the State.

Make Sure Your Tax Refund Is All It Can Be For years, the Benefit Fund’s Tax Assistance Program has helped eligible members claim the Earned Income Tax Credit (EITC) and get the most out of their tax return.

In 2012, the Fund is again partnering with New York City’s EITC Coalition to help you prepare your tax return. The EITC Coalition is led by the New York City Department of Consumer Affairs, and includes the city’s Office of Financial Empowerment and the Food Bank For New York City.

Visit the Food Bank For New York City and Ariva Sites for Free Tax Preparation As long as your 2012 household income was below $50,000 (or $18,000 if you are single and have no dependents), our partners at the Food Bank For New York City are offering free tax preparation events throughout the five boroughs, and the nonprofit financial education program Ariva is offering free assistance at two sites in the Bronx, including one in the Northwest Bronx for filers with a 2012 household income below $56,000 (or $25,000 if you are single and have no dependents).

If you qualify for the EITC, the tax experts onsite will help you file for the credit. Even if you don’t live near one of these sites, be sure to check the “Claim Your EITC” box at right to find out whether you qualify for the credit before filing your return.

Everyone feels troubled or overwhelmed now and then. If you’re dealing with personal issues that could put your health or your job in jeopardy, the Benefit Fund’s Member Assistance Program (MAP) is available to offer you confidential assistance.

Our MAP staff of social workers, counselors and other professionals can help direct you to the best available care for your particular situation. Whether you’re struggling with alcohol or substance abuse; stress, anxiety, depression or other mental health issues; domestic violence or the death of a loved one, they will help you get the support you need. If you didn’t sign up for health coverage through the Fund because you don’t think you can afford it, you may be eligible for premium assistance, and our MAP staff can direct you to the appropriate programs.

Retirement Benefit As an 1199SEIU home care member you are eligible for a defined pension, while fewer and fewer workers are receiving this valuable retirement benefit from their employers. When you retire, you may earn a “defined benefit” from the 1199SEIU home care Employees Pension Fund, which means you may receive a monthly benefit based on the pension credits you’ve earned during your years as a member. In many defined benefit plans, workers contribute to the plan to cover a percentage of the cost, but as an 1199SEIU Home Care worker, you don’t have to make any out-of-pocket contributions to receive a pension when you retire.

Just over a year ago, we welcomed our home care members back into the 1199SEIU Home Care Benefit Fund. We were able to offer eligible members two plans to choose from: Plan A, our Member Choice Home Care Select plan, and Plan B, our Panel Provider plan. Both offer quality health coverage, but with Plan A you have no co-payments for primary care when you select a “medical home,� a health center where your doctors, nurses and other healthcare professionals know you and your medical history and tailor your care to fit your needs. If you are in Plan B, you do have to make co-payments for certain services and prescription coverage – in addition to your weekly premium.

With open enrollment beginning February 1, you have the opportunity to re-evaluate your choice. If you would like to participate in Plan A but your doctor is not a participating provider in one of the Fund’s health centers, you might want to ask if he or she would be interested in becoming one.

In this special edition of For Your Benefit, you will also find information on applying for the Earned Income Tax Credit when you file your tax return.

The Fund has again partnered with the Food Bank For New York City and, for the first time, the Ariva financial education group to offer free tax preparation to members throughout the five boroughs and assist them in filing for the tax credit. Over the years, many home care members have used this program to get thousands of dollars back at tax time, and we encourage you to take advantage of it this year.

Of course, even if you are not enrolled for health benefits, as an eligible 1199SEIU member you have access to a range of other valuable resources.

We have included details on just some of the programs and services your Fund provides, such as your pension benefit, our Citizenship Program to help you prepare for the U.S. citizenship application process and our Member Assistance Program, which can help you find the assistance you need if you’re struggling with personal issues. Please remember that your Benefit Fund is here to serve you.

This is a summary of the annual report for the 1199SEIU National Benefit Fund for Home Care Employees, EIN: 13-4129368, for the period January 1, 2011, to December 31, 2011. This is a welfare benefit plan offering life, health, dental, vision and prescription drug benefits. The annual report has been filed with the Employee Benefits Security Administration, as required under the Employee Retirement Income Security Act of 1974 (ERISA).

Except with respect to the benefits specified under “Insurance Information� below, the plan is selfinsured and has committed to pay covered claims out of plan assets.

Insurance Information During 2011, the plan had a contract with Fidelis Care New York to pay certain life, health, dental, vision and prescription drug claims incurred under the terms of the plan. The total premiums paid for the plan year ending December 31, 2011, were $145,142,476.

Basic Financial Statement The value of plan assets, after subtracting liabilities of the plan, was $15,301,081 as of December 31, 2011, compared to $14,010,192 as of January 1, 2011. During the plan year the plan experienced an increase in its net assets of $1,290,889. This increase includes unrealized appreciation and depreciation in the value of the plan assets: that is, the difference between the value of the plan’s assets at the end of the year and the value of the assets at the beginning of the year or the cost of assets acquired during the year. The plan had total income of $192,074,343, including employer contributions of $144,986,543, NYC Personal Care Providers Rate Adjustment contributions of $46,219,668, COBRA payments of $117,566, and member premiums of $738,977.

Plan expenses were $190,783,454. These expenses included $10,419,333 in administrative expenses, $179,467,295 in benefits paid to participants and beneficiaries, and $132,907 in general expenses.

In response to Hurricane Sandy, the Plan will not terminate benefit coverage regardless of loss or reduction of hours for all Participants during the period from October 29, 2012, through November 30, 2012. Further, Participants who are required to pay, and cannot afford to pay, the weekly co-premium due to a reduction of hours during the period from October 29, 2012, through November 30, 2012, may apply to the Fund to obtain a waiver of co-premium for the weeks during which their hours were reduced.

The Fund believes it is a “grandfathered health plan� under the Patient Protection and Affordable Care Act (the Affordable Care Act). As permitted by the Affordable Care Act, a grandfathered health plan can preserve certain basic health coverage that was already in effect when that law was enacted. Being a grandfathered health plan means that this plan may not include certain consumer protections of the Affordable Care Act that apply to other plans, for example, the requirement for an external review process for claims appeals. However, grandfathered health plans must comply with certain other consumer protections in the Affordable Care Act, for example, the elimination of lifetime limits on benefits.

It’s important to be as sure as you can that your pension, Social Security benefits and other savings will provide you with enough monthly income to meet your expenses. Our Pension Department staff will give you the most up-to-date estimate of what your pension payments will be, and refer you to the agencies that may be providing your other benefits, such as Medicare and Social Security, so that you are fully informed and can feel secure in your planning.
