Home Care members can now enroll in coverage at any time during the year rather than during specified open enrollment periods. This means that even if you do not have the required number of hours to be eligible for health coverage, you should still complete the enrollment forms, because once you do become eligible, we can activate your benefits as soon as possible.

Remember, whether you receive health coverage through the Home Care Benefit Fund or not, you can take advantage of such resources as health fairs, our 24-Hour Health Helpline, training opportunities, our Citizenship Program and, of course, our Tax Preparation Assistance Program. Trained tax preparers can help ensure that your tax return is accurate and complete, and can help you apply for the Earned Income Tax Credit (EITC) if you’re eligible.

You will learn about an important change in the enrollment process for health coverage as well. Home Care members can now enroll in coverage at any time during the year rather than during specified open enrollment periods. Great news! You can now sign up for health benefits for yourself or for yourself and your children through the Fund anytime. If you haven’t signed up yet, you no longer have to worry about missing the deadline. Even if you haven’t worked the required qualifying 100 hours per month for two months in a row to be eligible for health coverage, you should still complete and submit your Enrollment and Payroll Deduction Authorization Forms. That way, when you do work the required number of qualifying hours, we can easily activate your health coverage.

Even If You Are Not Enrolled in 1199SEIU Health Benefits, You Can Take Advantage of Our Free Tax Preparation Assistance Program For years, the 1199SEIU Benefit Fund’s Tax Preparation Assistance Program has been available to help you claim the Earned Income Tax Credit (EITC) and get the most out of your tax return. The program has served thousands of Home Care members, and in 2016, the Fund is again partnering with the NYC Coalition during the City’s annual Tax Season Initiative to help you get your taxes prepared accurately and make sure your refund includes everything you’re entitled to. The NYC Coalition is led by the New York City Department of Consumer Affairs’ Office of Financial Empowerment, and includes the Food Bank For New York City and the nonprofit financial education program Ariva.

Remember, even if you do not receive health benefits through the Fund, you are still eligible for free tax preparation assistance. This is one of the valuable benefits that are available to all Home Care members, regardless of your eligibility status. As always, tax preparation assistance is available to you in multiple languages.

Visit the Food Bank For New York City and Ariva Sites for Free Tax Preparation The New York City Department of Consumer Affairs’ Office of Financial Empowerment coordinates the City’s annual Tax Season Initiative, which includes the NYC Coalition, a coalition comprised of the Food Bank For New York City, Ariva and many other free tax preparation providers.

As long as your 2015 household income was below $54,000 (or $30,000 if you are single and have no dependents), our partners at the Food Bank For New York City are offering free tax preparation events at locations throughout the five boroughs of New York City. The Food Bank For New York City will also offer the Virtual Volunteer Income Tax Assistance (Virtual VITA) program for convenient electronic filing (e-filing) at various locations. You are eligible to use Virtual VITA if your 2015 household income was less than $54,000, whether you have dependents or not.

If you are interested in this type of online filing, you can visit www.FoodBankNYC.org/tax-locations to find a site near you.

Here’s how it works: 1. If you will be filing a joint return with your spouse, both of you must be present.You will need to bring everything on the list on page 5.

2. A site volunteer will verify your identity, review your income and tax-related documents and help you get started.

3. Your forms and documents will be securely transmitted to the Food Bank For New York City’s Tax Preparation Site, where your return will be processed by a certified tax preparer.

4. You can expect a call from the tax preparer in just 48 hours to go over your tax return and complete the e-filing process.You must have an email address because the link to your completed tax return will be sent by email.

The nonprofit financial education program Ariva is also offering free tax assistance at sites throughout New York City. In order to receive assistance at Ariva’s sites, your 2015 household income must be below $53,000 if you have dependent children or below $30,000 if you are single and have no dependents. If you qualify for the EITC, the tax experts onsite will help you file for the credit. Even if you don’t live near one of these sites, be sure to check the “Claim Your Earned Income Tax Credit (EITC)” guide on page 5 to find out whether you qualify for the credit before filing your return.

What You Need to Bring to Have Your Taxes Prepared •

Your spouse. If you are filing a joint return, your spouse must be present with you.

•

Form 1098-T, if you paid tuition for post-high school education in 2015.

•

Photo ID issued by the state or federal government. For yourself and your spouse if you are filing jointly. For example: driver’s license, IDNYC card, state ID, passport, Green Card, etc.

•

Form 1099-DIV, if you received dividends in 2015.

•

Form 1099-R, if you receive a pension, or Form SSA-1099, if you collect Social Security.

•

Form 1095-A, if you were enrolled in a qualified health plan through the Health Insurance Marketplace in 2015.

•

Form 1095-B and/or 1095-C, if you were enrolled under your employer’s health plan in 2015.*

•

To claim child-care expenses, bring: 2015 total amount paid, child-care agency Tax Employer Identification Number (EIN) or name and Social Security number of child-care provider.

•

Social Security card (or individual Taxpayer Identification Number letter). For yourself and everyone to be claimed on your tax return, or a letter from the Social Security Administration with the correct Social Security numbers. Copies of past tax returns are NOT acceptable as proof of Social Security numbers. This means that you MUST bring Social Security cards (or copies of the cards) for every dependent you are claiming.

•

W-2 forms for all 2015 jobs (last paystub is NOT acceptable).

•

Sample check with routing and account numbers if you want to use direct deposit.

•

Form 1099-B, if you have income from the sale of stocks, bonds or other assets.

•

•

Form 1099-C, if you have a forgiveness of debt.

Amounts of any other income from your records (jury duty fees, cash earnings, gambling winnings [Form W2-G], etc.).

•

Form 1099-G, if you received unemployment insurance in 2015.

•

A copy of last year’s federal and state tax returns, if available.

•

Form 1099-INT, if you received interest from a bank account in 2015.

•

Form 1098-E, if you paid student loan interest in 2015 for post-high school education.

*You can file your taxes even if you have not received your Form 1095-B and/or Form 1095-C. Once you receive these forms, please keep them with your tax records.

•

Form 1098, if you paid real estate taxes and/or mortgage interest.

Claim Your EITC You can check whether you meet the income requirements for the EITC by using the 2015 tax guidelines listed here: •

Income below $47,747 ($53,267, if married, filing a joint return) with three or more dependent children

•

Income below $44,454 ($49,974, if married, filing a joint return) with two dependent children

•

Income below $39,131 ($44,651, if married, filing a joint return) with one dependent child

•

Income below $14,820 ($20,330, if married, filing a joint return) with no dependent children

Being a Home Care Worker Can Be Very Stressful— You Don’t Have to Go It Alone There’s no doubt that providing home health care can be demanding. In fact, home care workers are frequently exposed to a wide range of potentially serious safety and health hazards, according to the Centers for Disease Control and Prevention, the Occupational Safety and Health Review Commission and the National Association for Home Care & Hospice. And, as any home care worker can tell you, the one hazard that’s almost impossible to avoid is stress.

It sounds hard to believe, but many people are still embarrassed to admit that they struggle with mental health issues, which can cause them to avoid getting the care they need. The National Alliance on Mental Illness reports that just 41 percent of adults in the U.S. with a mental health condition received mental health services in the past year, with African-Americans and Hispanic-Americans seeking help at about one-half the rate of Caucasians.

Fortunately, as a member of the 1199SEIU Home Care Benefit Fund, you have resources available to you that many workers don’t have. Our Wellness and

Member Assistance Program (MAP) Department is staffed with social workers and therapists who will work with you to find counseling or referrals to programs that can help set you on the path to a stronger sense of well-being and a less stressful lifestyle. Even if you do not receive health coverage through the Fund, you can take advantage of Wellness/MAP resources.

The bottom line is that you don’t have to go it alone. English as a Second Language (ESL) Course for Home Care Members Great news! Whether or not you receive health coverage from the Home Care Benefit Fund, as an 1199SEIU Home Care member, you may be eligible to take part in our English as a Second Language (ESL) program. The ESL program for Home Care members holds classes Monday through Friday nights from 5:30 pm to 8:30 pm, and on Saturday during the day from 10:00 am to 2:00 pm. eCaring Program Keeps Members on the Cutting Edge of Healthcare Technology Your Home Care Industry Education Fund is partnering with eCaring, Inc., to introduce a new technology initiative called the “eCaring CareTracker” tool. If you are a Home Care member who is assigned to a client with an illness that requires real-time monitoring and reporting, you may be chosen to participate in a pilot program in which you will be trained to use a tablet with the eCaring tool to communicate healthcare information electronically. Fifteen Home Care peer trainers will train as many as 300 Home Care members to use the tablets and the CareTracker tool. As many as six employers will have members participating in the project, and you may be one of them. If the pilot is successful and clients receive better care as a result of the use of the tool, the eCaring service may become available to more clients and even more Home Care members will be trained. Qué Necesita Llevar para Preparar Su Declaración de Impuestos • Su cónyuge. Si presenta una declaración conjunta, su cónyuge debe estar presente.

• Formulario 1099-DIV, si recibió dividendos en 2015.

• Formulario 1099-R, si recibe una pensión, o Formulario SSA-1099, si recibe pagos del Seguro Social.

•

Una identificación con foto emitida por el gobierno estatal o federal. Suya y de su cónyuge si presentan declaración conjunta. Por ejemplo: licencia de conducir, tarjeta IDNYC, identificación estatal, pasaporte, tarjeta de residencia, etc.

•

Tarjeta del Seguro Social (o carta con el Número de Identificación de Contribuyente individual).

Suya y de todos los que se incluirán en su declaración de impuestos, o una carta de la Administración del Seguro Social con los números correctos del Seguro Social. Las copias de declaraciones de impuestos anteriores NO se aceptan como prueba de los números del Seguro Social. Para reclamar gastos de cuidado infantil, lleve: cantidad total pagada en 2015, número de identificación tributario (Employer Identification Number o EIN, por sus siglas en inglés) como empleador de la agencia de cuidado infantil o nombre y número del Seguro Social del proveedor de cuidado infantil.

• Cheque de muestra con número de identificación del cheque y la cuenta si desea usar el depósito directo.

De hecho, los trabajadores de cuidados en el hogar suelen estar expuestos a una amplia variedad de posibles peligros de salud y seguridad, según los Centros para el Control y la Prevención de Enfermedades, la Comisión de Revisión de Seguridad y Salud Ocupacional y la Asociación Nacional de Cuidados en el Hogar y Cuidados Paliativos.Y, como puede decirle cualquier trabajador de cuidados en el hogar, el peligro que es casi imposible de evitar, es el estrés.

Los Programas eCaring Mantienen a los Miembros al Día sobre lo Último en Tecnología Médica Su Fondo de Educación de la Industria de Cuidados en el Hogar se está asociando con eCaring, Inc. para presentar una nueva iniciativa tecnológica conocida como la herramienta “eCaring CareTracker”. Si es un miembro de Cuidados en el Hogar que tiene asignado un cliente con una enfermedad que exige monitoreo e informes en tiempo real, es posible que le elijan para participar en un programa piloto en el que le capacitarán para usar una tableta con la herramienta eCaring para comunicar electrónicamente la información médica. Un total de 15 capacitadores de Cuidados en el Hogar capacitarán a unos 300 miembros de Cuidados en el Hogar para usar las tabletas y la herramienta CareTracker. Hasta seis empleadores tendrán miembros participando en el proyecto, y usted puede ser uno de ellos. Si el programa piloto resulta un éxito y los clientes reciben una mejor atención como consecuencia de usar la herramienta, el servicio eCaring podrá estar disponible para más clientes y se capacitará a más miembros de Cuidados en el Hogar. Summary Annual Report 1199SEIU National Benefit Fund for Home Care Employees This is a summary of the annual report for the 1199SEIU National Benefit Fund for Home Care Employees, EIN:13-4129368, for the period January 1, 2014, to December 31, 2014. This is a welfare benefit plan offering life insurance, health, dental, vision and prescription drug benefits. The annual report has been filed with the Employee Benefits Security Administration, as required under the Employee Retirement Income Security Act of 1974 (ERISA).

The plan is self-insured and has committed to pay covered claims out of plan assets.

Basic Financial Statement The value of plan assets, after subtracting liabilities of the plan, was $96,727,289 as of December 31, 2014, compared to $27,846,907 as of January 1, 2014.

During the plan year, the plan experienced an increase in its net assets of $68,880,382. This change includes unrealized appreciation and depreciation in the value of the plan assets; that is, the difference between the value of the plan’s assets at the end of the year and the value of the assets at the beginning of the year or the cost of assets acquired during the year. The plan had total income of $198,882,150, including employer contributions of $191,576,917, COBRA payments of $83,704, member premiums of $7,207,639 and other earnings of $13,890.

Plan expenses were $170,981,745. These expenses included $9,272,423 in administrative expenses, $24,375 in interest expenses, $160,679,182 in benefits paid to participants and $1,005,765 in general expenses.
