Message from the Executive Director Dear Members, As a Home Care worker, you know the satisfaction you get from your work.You play a vital role in helping people stay healthy or assisting them in their recovery from an illness or injury. Often, you develop deep personal relationships with those you care for. But you know that your work can be stressful, both physically and emotionally. At the Home Care Benefit Fund, we understand this, and we are here to help.

In this issue of For Your Benefit, we remind you that you can turn to our Wellness Member Assistance Program when you are feeling stressed or overwhelmed. Our program’s trained social workers can offer support and guidance or a referral to a mental health professional, if necessary.

You will also read about upcoming grief workshops, where you can talk to fellow Home Care members and get the support you need during difficult times, such as when a client passes away.

Taking care of your physical health is just as important, so in this issue you will learn more about our Peer-to-Peer Mentoring Program for Home Care members with Type 2 diabetes. The program pairs Home Care members who have successfully gotten their diabetes under control with those who are still struggling. In addition, our Chief Medical Officer, Dr.Van H. Dunn, weighs in on the importance of staying current with preventive screenings in order to catch conditions or cancers early, before they become life-threatening. And you will learn about a new, non-invasive, highly effective procedure for colon cancer screening that we hope many of you will take advantage of.

Of course, as a Home Care member, you have access to a number of other benefits, such as health fairs, training opportunities, our Citizenship Program and our free Tax Preparation Assistance Program. With tax season here, this program can help ensure that your refund is all that it can be, and if you qualify for the Earned Income Tax Credit (EITC), trained tax preparers will help you file for that, too.

Even If You Are Not Enrolled in 1199SEIU Health Benefits, You Can Take Advantage of Our Free Tax Preparation Assistance Program For years, the 1199SEIU Benefit Fund’s Tax Preparation Assistance Program has been available to help you claim the Earned Income Tax Credit (EITC) and get the most out of your tax return. The program has served thousands of Home Care members, and in 2017, the Fund is again partnering with the NYC Coalition during the city’s annual Tax Season Initiative to help you get your taxes prepared accurately and make sure your refund includes everything you’re entitled to. The NYC Coalition is led by the New York City Department of Consumer Affairs’ Office of Financial Empowerment, and includes Food Bank For New York City and the nonprofit financial education program Ariva.

Remember, even if you do not receive health benefits through the Fund, you are still eligible for free tax preparation assistance. This is one of the valuable benefits that are available to all Home Care members regardless of eligibility status. As always, tax preparation assistance is available to you in multiple languages.

Visit Food Bank For New York City and Ariva Sites for Free Tax Preparation As long as your 2016 household income was below $54,000 (or $30,000 if you are single and have no dependents), our partners at Food Bank For New York City are offering free tax preparation events throughout the five boroughs. This year, Food Bank For New York City will also offer Virtual Volunteer Income Tax Assistance (Virtual VITA) for electronic filing (e-filing) at various locations.You are eligible to use Virtual VITA if your 2016 household income was less than $54,000, whether you have dependents or not.

Here’s how e-filing works: 1. If you will be filing a joint return with your spouse, both of you must be present at the appointment.

You will also need to bring everything on the list on the next page.

2. A site volunteer will verify your identity, review your income- and tax-related documents and help you get started.

3. Your forms and documents will be securely transmitted to Food Bank For New York City’s Tax Preparation Site, where your return will be processed by a certified tax preparer.

4. Your certified tax preparer will call you within 48 hours to go over your tax return and complete the e-filing process.

5. Your tax return will be electronically filed! You must have access to a computer and an email address, because the link to your completed tax return will be sent by email.

The nonprofit financial education program Ariva is also offering free tax assistance at sites in the Bronx and Manhattan. In order to receive assistance at Ariva’s sites, your 2016 household income must be below $54,000 if you have dependent children or below $30,000 if you are single and have no dependents. If you qualify for the EITC, the tax experts onsite will help you file for the credit. Even if you don’t live near one of these sites, be sure to check the “Claim Your EITC” guide on the next page to find out whether you qualify for the credit so you can be sure to apply for it when filing your return.

What You Need to Bring to Have Your Taxes Prepared • Your spouse. If you are filing a joint return, your spouse must be present with you.

• Form 1095-A, if you were enrolled in a qualified health plan through the Health Insurance Marketplace in 2016.

• Photo ID issued by the state or federal government.

For yourself and your spouse if you are filing jointly. For example: driver’s license, state ID, passport, Green Card, etc.

• Form 1095-B and/or 1095-C, if you were enrolled under your employer’s health plan in 2016.*

•

Social Security card (or individual Taxpayer Identification Number letter). For yourself and everyone to be claimed on your tax return, or a letter from the Social Security Administration with the correct Social Security numbers. Copies of past tax returns are NOT acceptable as proof of Social Security numbers. This means that you MUST bring Social Security cards (or copies of the cards) for every dependent you are claiming.

• W-2 forms for all 2016 jobs (last paystub is NOT acceptable).

• Form 1099-B, if you have income from the sale of stocks, bonds or other assets.

• Form 1099-C, if you have a forgiveness of debt.

• Form 1099-G, if you received unemployment insurance in 2016.

• Form 1099-INT, if you received interest from a bank account in 2016.

• Form 1098-E, if you paid student loan interest in 2016 for post-high school education.

• Form 1098, if you paid real estate taxes and/or mortgage interest.

• Form 1098-T, if you paid tuition for post-high school education in 2016.

• Form 1099-DIV, if you received dividends in 2016.

• Form 1099-R, if you receive a pension, or Form SSA 1099, if you collect Social Security.

•

To claim child-care expenses, bring: 2016 total amount paid, child-care agency Tax Employer Identification Number (EIN) or name and Social Security number of child-care provider.

• Sample check with routing and account numbers if you want to use direct deposit.

• Amounts of any other income from your records (jury duty fees, cash earnings, gambling winnings [Form W2-G], etc.).

• A copy of last year’s federal and state tax returns, if available. *You can file your taxes even if you have not yet received your Form 1095-B and/or Form 1095-C. Once you receive these forms, please keep them with your tax records.

Claim Your EITC You can check whether you meet the income requirements for the EITC by using the 2016 tax guidelines listed here: • Income below $47,955 ($53,505 if married, filing a joint return) with three or more dependent children • Income below $44,648 ($50,198 if married, filing a joint return) with two dependent children • Income below $39,296 ($44,846 if married, filing a joint return) with one dependent child • Income below $14,880 ($20,430 if married, filing a joint return) with no dependent children Check your mail for your Fund’s EITC information booklet. Don’t Go It Alone: Get the Care You Need to Support Your Psychological Well-being As a Home Care worker, your job is both rewarding and stressful.

The daily demands of the job not only take a physical toll but a psychological one as well. The close personal bond you form with many of your clients and their families make it especially difficult when they are in pain or pass away. Add long, often inconsistent hours and sometimes extremely physical work, and you know how challenging it can be to manage your own mental well-being and physical health.Your Benefit Fund is here to help.

The daily demands of the job not only take a physical toll but a psychological one as well.

Working alone can leave you feeling isolated and overwhelmed, especially when you are grieving the loss of a client. To help you deal with these difficult emotions, we will be providing grief support groups, where you can talk with other Home Care workers who have experienced the same type of loss. To make it as easy as possible to participate, we will lead these support groups by phone. If you are interested in joining a group session, please call us at (646) 473-6910.

The one-hour groups will be led by our Wellness Member Assistance Program social workers, and you can join the group on one of the dates below by calling (888) 205-5513, and dialing in the conference code 389467.

A Message from Dr. Van H. Dunn, Chief Medical Officer Preventive screenings can help stave off more serious illness—and may save your life.

Since I began my career as a primary care physician at a community hospital, I have counseled patients, friends, family members and anyone else who cared to listen on the value of preventive screenings. Now, as Chief Medical Officer at the Benefit Fund, my primary goal is to help keep you healthy, and preventive screenings are one of the best ways to make that goal a reality.

Cancer screenings are the most effective tool to catch almost every type of cancer before the disease has had a chance to spread to a life-threatening stage, yet far too often, folks “forget” to schedule an appointment or are put off by the inconvenience or discomfort that accompanies some preventive screening procedures. The reason for any cancer screening is to identify, treat or remove precancerous cells, period. And a little discomfort or a few hours of your time is a small price to pay.

Likewise, cholesterol and blood sugar screenings are designed to identify warning signs long before they have a chance to become chronic conditions like heart disease or diabetes.

Preventive screenings are important, just like the immunizations that protect you from the measles, mumps, rubella, malaria and other illnesses, so they should be part of your annual routine to protect your health and see your primary care doctor. If it so happens that you are due for a screening, your doctor can even let you know. And when that happens, by all means, do not put it off: Early detection can mean the difference between treating a condition in its early stages and facing a more serious and potentially life-threatening illness.

“One Call” Radiology Network Offers More Choices MedFocus Radiology Network, the Benefit Fund’s preferred provider for radiology and imaging services, is now part of One Call Care Management. This new partnership will give you access to a broader network of quality radiology providers.

Remember, prior authorization is required for some radiological tests, such as MRI, MRA, PET scans, CAT scans and certain nuclear cardiology tests, so be sure your doctor calls (888) 9101199 to receive approval. To find a One Call facility near you, visit our website at www.1199SEIUBenefits.org.

The Benefit Fund Works with Your Doctors to Improve Your Care As the way people in the U.S. access their healthcare continues to change, your Benefit Fund is working closely with our participating providers to improve your care, most recently through the creation of Accountable Care Organizations (ACOs). As you may know, an ACO is a group or network of doctors who work together to help ensure patients receive the best possible care. In 2014, we entered into our first ACO agreement with Montefiore Medical Center, and in January 2016, we partnered with the Union and Northwell Health (formerly the North Shore-LIJ Health System) to create our second ACO. We will soon launch new partnerships with the Mount Sinai Health System and AdvantageCare Physicians.

What does this mean for you? If you receive most of your healthcare services from Mount Sinai providers, for example, or from any group with whom we partner in the future, you will be automatically enrolled in their 1199SEIU ACO program. We will send you a letter that explains how the program works.You will have a care management team of nurses, pharmacists, social workers and health educators who can work together to create a personalized plan to help you manage a chronic condition, recover from a serious illness, or transition back home or to another setting after a hospital stay. These healthcare providers will be able to access specific information—such as the dates you visited a doctor or hospital, the treatment you received, any tests or screenings you underwent and the prescriptions you received— which can eliminate duplicate medical tests and referrals, and helps ensure you get the care you need when you need it.

Your healthcare benefits will not change, and you can opt out of the program at any time by calling our Member Services Department at (646) 473-9200. All of your information is protected under regulations of the Health Insurance Portability and Accountability Act (HIPAA), and the only people who see it are your healthcare providers.

New Colon Cancer Screening Can Save Your Life No one looks forward to having a colonoscopy, but it has proven to be a lifesaving tool that doctors use to help detect colon cancer, which is highly treatable if caught early. Unfortunately, many 1199ers who should get screened don’t: Our colon cancer screening rate is 44 percent, compared with the national rate of 58 percent. A screening can catch colon cancer early, before it becomes difficult to treat. Treatment for late-stage colon cancer often includes surgery, chemotherapy and radiation—and does not have a high survival rate. It doesn’t have to be that way. If caught early, treatment typically involves removal of polyps or the affected area, followed by regular monitoring. And now, there is a new procedure available—covered by your Benefit Fund—that is convenient, non-invasive and effective in identifying cancer and precancerous cells.

The Cologuard Collection Kit is an easy-to-use, athome colon cancer screening test that requires no special preparation, diet or medication changes.You won’t even have to take time off from work.

So if you’re over age 50 and you’ve been hesitating about getting screened, talk to your doctor today. If you are a good candidate for the test, your doctor will write you a prescription for the test and a Cologuard Collection Kit will be shipped to your home. You collect a stool sample and mail it to a lab in the prepaid, pre-addressed box. The lab will then send results to your doctor within two weeks.

Of course, the Cologuard test is not right for everyone, and is not meant to be a replacement for a traditional colonoscopy if you are at high risk for colon cancer, or have a family history or a personal history of colon cancer, polyps or other diseases such as inflammatory bowel disease, chronic ulcerative colitis or Crohn’s disease that place you at risk for colon cancer.

Please note, if you are considering using the Cologuard test, the Benefit Fund requires that your doctor receive prior authorization before giving you a prescription for the test.To learn more about the Cologuard Collection Kit, visit www.CologuardTest.com.

Your Rights Under the Women’s Health and Cancer Rights Act If you are battling breast cancer, your 1199SEIU Benefit Fund is here to support you every step of the way. Your health benefits give you access to quality oncology care with many of the top cancer treatment centers in the New York metropolitan area, so you won’t have to go outside of your network for care.

Your Benefit Fund also provides coverage for reconstructive surgery, prostheses and other mastectomy-related services.

According to your doctor’s treatment plan, you are covered for: • Reconstruction of the breast on which a mastectomy was performed; • Surgery and reconstruction of the other breast to achieve a symmetrical appearance; • Prostheses; and • Treatment for physical complications associated with all stages of the mastectomy, including lymphedemas.

These benefits are provided under the 1199SEIU National Benefit Fund for Home Care Employees. Coverage is subject to benefit rules, exclusions and limitations that may apply under the Plan, as permitted by law.

Nelsa Alexander As a mentor in our Peer-to-Peer Mentoring Program, Nelsa Alexander has helped 10 other Home Care members with diabetes improve their health—and has maintained positive changes of her own in the process.

In early 2015, the Home Care Benefit Fund partnered with InquisitHealth, a peer mentoring service, to identify members to participate in a new initiative: the Peer-to-Peer Mentoring Program for Home Care members with Type 2 diabetes. The Fund identified Nelsa Alexander as a member who was managing her condition well and reached out to her to see if she’d be interested in becoming a mentor to fellow Home Care members. An 1199SEIU member for 24 years and a Home Health Aide at Rockaway Vendor, Nelsa was eager to join the program. Diagnosed with Type 2 diabetes the year before, Nelsa had struggled at first, but she soon got her condition under control, making healthy changes in her eating habits and increasing her exercise. Inspired, she wanted to share her experience with other members who were not having as much success as she was. (For information on how to become a mentor or a mentee, see the next page.)

Once she signed up, Nelsa received special training through the Training and Employment Funds’ Home Care Industry Education Fund, which included convenient online classes designed to improve her communication skills and help her become an effective mentor. In March 2016, she began mentoring, and found the experience to be more rewarding than she had imagined. Today, she coaches between 8 and 10 co-workers, three of whom share a similar cultural background with her, which has helped when it comes to finding substitutes for traditional—but not so healthy—island foods.

That shared experience is not the only thing that makes the program so successful, said Nelsa. She and

her fellow Home Care members face a common challenge to maintain healthy habits because of their often unpredictable schedules. “We are eating on the run a lot,” she said, “and it can be tempting to stop at a fast-food restaurant or to skip meals, but I tell my mentees that I am also in their shoes, and now, I bring healthy snacks with me, like fruits or almonds, and make a habit of watching my portions.”

Nelsa keeps a list of tips that she can turn to when her mentees are struggling. She’ll suggest things like avoiding shopping for food when they’re hungry, making a list before going to the grocery store, skipping fried foods and taking a “doggy bag” when eating out rather than eating everything on their plate.

But most of all, she said, it’s the relationships that make the biggest difference. “It really makes you feel good to have someone you can call or text if you’re having a hard day,” Nelsa said. “And not only does it make me feel good to be able to help, it keeps me committed to my own program because I have to practice what I preach!”

Here’s How! If you have your Type 2 diabetes under control and are interested in becoming a mentor, we will match you with a fellow Home Care member who needs help managing his or her condition. (Peer mentors have the potential to earn up to $12 per hour.) Or if you’re struggling to get your condition under control, we will match you with a mentor who shares a similar cultural background and speaks the same language. Together, mentors and mentees set goals and discuss ways to achieve them. The program does not replace care from a doctor but provides members with additional support. To become a peer mentor, you will need access to a computer or tablet. As a mentor, you will:

Afrontar el dolor o el fallecimiento de un cliente resulta especialmente difícil debido al estrecho vínculo personal que crea con muchos de los clientes y sus familias. Mensaje del Dr. Van H. Dunn, Director Médico Ejecutivo Los exámenes de detección preventivos pueden ayudar a prevenir enfermedades graves, y pueden salvar su vida.

Desde que comencé mi carrera como médico de atención primaria en un hospital comunitario, he aconsejado a pacientes, amigos, familiares y a cualquiera que quisiese escuchar sobre el valor de los exámenes de detección preventivos. Actualmente, como Director Médico Ejecutivo del Fondo de Beneficios, mi objetivo principal es ayudar a que usted se mantenga saludable, y una de las mejores maneras de que ese objetivo se cumpla es a través de los exámenes de detección preventivos.

Los exámenes de detección de cáncer constituyen la herramienta más eficaz para identificar casi cualquier tipo de cáncer antes de que la enfermedad pueda extenderse hasta alcanzar una fase potencialmente mortal. Sin embargo, sucede muy a menudo que la gente “olvida” programar una consulta o la postergan por la molestia o incomodidad que acompaña a algunos procedimientos de detección preventivos. La razón para realizar cualquier examen de detección de cáncer es identificar, tratar o eliminar células precancerosas. Punto. Unas molestias o unas horas de su tiempo son un precio bastante bajo a pagar por esto.

De la misma manera, los exámenes para determinar los niveles de colesterol y de azúcar en sangre están diseñados para identificar señales de alerta mucho antes de que puedan volverse afecciones crónicas, como una enfermedad cardíaca o diabetes.

Los exámenes de detección preventivos son importantes, al igual que las vacunas lo protegen del sarampión, las paperas, la rubéola, la malaria y otras enfermedades. Por eso, deberían ser parte de su rutina anual para proteger su salud y consultar a su médico de atención primaria. Si debe hacerse un examen de detección, su médico puede decírselo.Y si esto sucede, no lo posponga por nada: la detección temprana puede significar la diferencia entre tratar una enfermedad en su fase temprana y afrontar una afección más grave que puede resultar mortal.

La Red de Radiología “One Call” Ofrece Más Opciones La Red de Radiología de MedFocus, el proveedor preferido del Fondo de Beneficios en el área de radiología y servicios de diagnóstico por imágenes, ahora forma parte de One Call Care Management. Esta nueva asociación le proporcionará acceso a una red más amplia de proveedores de radiología de calidad. Recuerde que se necesita autorización previa para algunos exámenes radiológicos, como resonancia magnética (MRI, por sus siglas en inglés), angiografía por resonancia magnética (MRA, por sus siglas en inglés), tomografía por emisión de positrones (PET, por sus siglas en inglés), tomografía axial computarizada (CAT, por sus siglas en inglés) y ciertas pruebas de cardiología nuclear. El Fondo de Beneficios Trabaja con sus Médicos para Mejorar su Atención Médica La manera en que los estadounidenses acceden a la atención médica continúa cambiando. Estos proveedores de atención de la salud tendrán acceso a información específica, como las fechas en que consultó con un médico o asistió a un hospital, el tratamiento que recibió, cualquier prueba o examen que se le haya realizado y los medicamentos con receta que recibió. De esta manera, se evita repetir pruebas médicas y derivaciones, y ayuda a garantizar que reciba la atención que necesita cuando la necesita.

Sus beneficios de salud no cambiarán, y puede decidir dejar el programa en cualquier momento llamando a nuestro Departamento de Servicios para los Miembros al (646) 473-9200. Los Nuevos Exámenes de Detección de Cáncer de Colon Pueden Salvar Su Vida

Sus Derechos Según la Ley de Derechos sobre la Salud y el Cáncer de la Mujer

Nadie espera con ansias una colonoscopía, pero está demostrado que esta herramienta puede salvar vidas, ya que es utilizada por los médicos para ayudar a detectar cáncer de colon, que puede tratarse si se toma a tiempo.

Lamentablemente, muchos miembros de 1199 que deberían hacerse exámenes de detección no lo hacen: nuestro índice de realización de exámenes de detección de cáncer de colon es de 44 por ciento, en comparación con el índice nacional del 58 por ciento. Un examen de detección puede identificar temprano el cáncer de colon, antes de que sea difícil tratarlo. El tratamiento para el cáncer de colon en etapa avanzada a menudo implica cirugía, quimioterapia y radiación, y su tasa de supervivencia no es muy elevada. Esto no debería ser así.

Si se detecta a tiempo, el tratamiento generalmente consta de la extracción de pólipos en el área afectada, seguida de controles regulares. Actualmente existe un nuevo procedimiento (cubierto por su Fondo de Beneficios) que es conveniente, no invasivo y eficaz en la identificación de células cancerosas y precancerosas. Nelsa Alexander Como mentora de nuestro Programa de Orientación entre Pares, Nelsa Alexander ha ayudado a otros 10 miembros con diabetes de Cuidados en el Hogar a mejorar su salud y ha mantenido sus propios cambios positivos en el proceso.

Nelsa, miembro de 1199SEIU durante 24 años y Asistente de Salud en el Hogar en Rockaway Vendor, se sintió entusiasmada por unirse al programa. Nelsa había recibido el diagnóstico de diabetes tipo 2 el año anterior. Al principio tuvo que luchar, pero pronto logró controlar su afección realizando cambios saludables en sus hábitos alimentarios y haciendo más ejercicio. Una vez inscrita, Nelsa recibió capacitación especial a través del Fondo de Educación de la Industria de Cuidados en el Hogar de los Fondos de Capacitación y Empleo, que incluyó clases en línea convenientes y diseñadas para mejorar sus habilidades de comunicación y ayudarla a convertirse en una mentora eficaz. En marzo de 2016, comenzó con sus tareas de mentora y descubrió que la experiencia era más gratificante de lo que imaginaba. En la actualidad, orienta entre 8 y 10 compañeros de trabajo, tres de los cuales comparten con ella características culturales similares, lo cual ayudó a la hora de buscar sustitutos de alimentos tradicionales, pero no tan saludables, de la isla.

Esa experiencia compartida no es el único motivo del éxito del programa, dijo Nelsa. Ella y sus compañeros miembros de Cuidados en el Hogar enfrentan el desafío común de mantener hábitos saludables debido a sus horarios, en muchos casos, impredecibles. “Muchas veces, comemos apurados”, comenta, “y puede ser tentador parar en un restaurante de comida rápida o saltarse comidas, pero, como les digo a mis alumnos, yo también estoy en sus zapatos y ahora llevo conmigo refrigerios saludables, como frutas o almendras, y tengo el hábito de controlar mis porciones”.

Nelsa tiene una lista de consejos que puede consultar cuando sus alumnos tienen dificultades. Hará sugerencias como evitar comprar alimentos cuando se tiene hambre, hacer una lista antes de ir a la tienda de comestibles, evitar los alimentos fritos y, cuando se sale a comer fuera, pedir que coloquen la comida que sobra en una bolsa en lugar de comer todo lo que haya en el plato. Pero, principalmente, lo más importante son las relaciones. “Uno realmente se siente bien al tener a quien poder llamar o escribir un mensaje en un día difícil”, expresó Nelsa. “Y no solo me hace sentir bien poder ayudar, sino que también mantiene mi compromiso con mi propio programa porque tengo que poner en práctica lo que enseño”.

Si tiene su diabetes tipo 2 bajo control y le interesa convertirse en mentor, lo reuniremos con un compañero miembro de Cuidados en el Hogar que necesite ayuda para controlar su afección. (Los mentores de pares pueden ganar hasta $12 por hora). O bien, si tiene dificultades para controlar su afección, lo reuniremos con un mentor que comparta características culturales similares y hable el mismo idioma. Juntos, los mentores y alumnos fijan metas y analizan las maneras de alcanzarlas. El programa no sustituye la atención de un médico, pero les brinda apoyo adicional a los miembros. Para convertirse en mentor de pares, necesitará tener acceso a una computadora o tableta.

Como mentor, hará lo siguiente: •

Completará correctamente un programa de capacitación por Internet sobre cómo ser un mentor de pares eficaz.

Hablará con el miembro por teléfono dos veces al mes durante seis meses.

Enviará mensajes de seguimiento entre las llamadas, si el miembro puede recibir mensajes de texto.

Summary Annual Report 1199Seiu National Benefit Fund for Home Care Employees This is a summary of the annual report for the 1199SEIU National Benefit Fund for Home Care Employees, EIN: 13-4129368, for the period January 1, 2015, to December 31, 2015. This is a welfare benefit plan offering life insurance, health, dental, vision and prescription drug benefits. The annual report has been filed with the Employee Benefits Security Administration, as required under the Employee Retirement Income Security Act of 1974 (ERISA).

The plan is self-insured and has committed to pay covered claims out of plan assets.

Basic Financial Statement The value of plan assets, after subtracting liabilities of the plan, was $195,080,107 as of December 31, 2015, compared to $96,727,289 as of January 1, 2015. During the plan year, the plan experienced an increase in its net assets of $98,352,818. This change includes unrealized appreciation and depreciation in the value of the plan assets; that is, the difference between the value of the plan’s assets at the end of the year and the value of the assets at the beginning of the year, or the cost of assets acquired during the year. During the plan year, the plan had total income of $294,836,030, including employer contributions of $277,821,781, COBRA payments of $100,622, member premiums of $16,652,791 and other earnings of $260,836.

Plan expenses were $196,483,212. These expenses included $11,600,710 in administrative expenses, $183,633,256 in benefits paid to participants and $1,249,246 in general expenses.
