Message from the Executive Director Dear Members, Your Benefit Fund does more than provide you with access to quality healthcare coverage. We also share with you the information you need to stay healthy and informed.

In this issue of For Your Benefit, you will read about a change in the way doctors assess the risk of heart disease—a condition that affects nearly half of our members. In his column, Chief Medical Officer Dr.Van H. Dunn discusses the resources your Benefit Fund offers to help you lower your risk, including our Hypertension Peer-to-Peer Mentoring Program. In addition, you will learn about Dr. Dunn’s new Healthy Living Spotlight videos on diabetes, a common risk factor for heart disease.

We also remind you that it is not too late to get your flu shot. Last year, more than 80,000 people in the U.S. died from complications of the flu. As healthcare workers, you are more susceptible to contracting the virus—and passing it on to your residents—so we encourage you to protect yourself with the vaccination now.

And because stressing over money matters can weaken your immune system and make you more susceptible to colds, the flu and more, we inform you about our free Online Financial Health series. Provided by Mobility Capital Finance (MoCaFi), the fourth part of the series lays out what steps you need to take to prepare for retirement. And because tax season is just around the corner, we remind you to take advantage of the Benefit Fund’s free Tax Preparation Assistance Program, which can also help you file for the Earned Income Tax Credit (EITC), if you are eligible.

New Collective Bargaining Agreement Signed As this issue of For Your Benefit went to press, the collective bargainers reached a new agreement, which protects your health and pension benefits. Look for detailed information from your Union in the coming weeks.

Open Enrollment for Spouses Begins January 1 The open enrollment period for you to sign up your spouse for the Greater New York Benefit Fund health plan runs from January 1 through January 31, for coverage beginning March 1, 2019. To enroll your spouse, you must complete a Spouse Coverage and Payroll Deduction Authorization Form authorizing your employer to deduct the weekly premium from your paycheck. This form must be submitted to your employer before February 1.You can find the enrollment form at www.1199SEIUBenefits.org.

If you’ve already enrolled your spouse, you don’t have to do anything. Of course, you and your dependent children up to age 26 are still eligible for coverage, with no premium, whether or not your spouse is enrolled. Talks Heart Health

Heart disease is the leading cause of death in the U.S. for both men and women—every 40 seconds, someone suffers a heart attack. Closer to home, the prevalence of 1199SEIU members at risk for heart disease is nearing 50 percent, mainly because heart problems are usually associated with other chronic conditions like Type 2 diabetes, hypertension, obesity and high cholesterol, and unhealthy habits like smoking—all risk factors that affect our members at higher-than-average rates (see “Are You at Risk for Heart Disease?” on page 7).

Now, while I hope these facts lead you to examine your own risk factors, they are no reason to get discouraged. Even if you currently have an elevated risk for heart disease, there are a number of ways to lower that risk—and your Benefit Fund is here to help. For instance, our Diabetes Peer-to-Peer Mentoring Program, administered by InquisitHealth, has paired hundreds of 1199SEIU members to help each other better control their Type 2 diabetes. The program has expanded to now include members who need help getting their hypertension, or high blood pressure, under control (see “Get Your High Blood Pressure Under Control with Our Peer-to-Peer Mentoring Program,” below). Our Wellness Member Assistance Program’s team of nurses, nutritionists, health educators, social workers and other healthcare professionals also visit worksites to host health fairs and educational workshops where members can get important screenings for hypertension, diabetes and high cholesterol—and receive information to take action, if necessary.

Get Your High Blood Pressure Under Control with Our Peer-to-Peer Mentoring Program If you’re struggling with hypertension, or high blood pressure, your Benefit Fund can help. Our free and confidential Hypertension Peer-to-Peer Mentoring Program, administered by InquisitHealth, can connect you with a fellow 1199SEIU member who is successfully managing his or her hypertension. In addition to the care you receive from your doctor, the program will offer support from a fellow member who can relate to what you are going through and offer ways to get your condition under control and lower your blood pressure. As part of the program, you’ll receive personalized coaching from your mentor by phone, advice from trained nutritionists to help you make healthy changes to your diet and a free digital blood pressure monitor so you can check your blood pressure at home.You’ll also learn how to use a smartphone app to track your progress and access educational materials.

It’s Not Too Late to Get Your Flu Shot If you haven’t gotten your flu shot and haven’t gotten sick, you’re probably feeling pretty lucky—but that luck may run out. As a healthcare worker, you are at increased risk of catching and transmitting the flu. According to the Centers for Disease Control and Prevention, flu activity peaks between December and February, and it can last as late as May. The good news is that it’s not too late to protect yourself, your family and your residents against the potentially deadly virus. The flu vaccination is the most effective way to prevent the flu, and getting one is quick and easy, as many worksites offer the shot onsite. If your facility doesn’t offer the shot, remember that your Benefit Fund covers it, so you can always visit your primary care provider or local pharmacy.

Are You at Risk for Heart Disease?

Heart disease is the number one cause of death among men and women in the U.S., but many are unaware of the risk factors and lifestyle choices that increase their chances of developing the disease. Now, a recent study found that the method doctors have long used to calculate a person’s risk of developing heart disease may be inaccurate.

The study reported that many people may be unnecessarily taking medication to control risk factors such as high cholesterol and high blood pressure, while others—particularly African-Americans—may not be receiving enough treatment. Whether you think you may be at risk for heart disease or you’re currently being treated for the condition, it’s a good idea to talk to your doctor about where you stand.

Assess Looking at your age, gender, blood pressure and cholesterol levels—and taking into account whether you smoke or have a family history of diabetes—your doctor can determine your risk factor and work with you on a course of treatment, if necessary. African-Americans and Latinos face a higher risk of developing heart disease and are particularly encouraged to undergo a health assessment by their doctor. Also, if you are currently taking medication to lower your blood pressure or manage your cholesterol levels, it is important to be evaluated for your risk of developing heart disease, since several studies have shown that medications can be both under-prescribed and over-prescribed.

Manage If your doctor determines that you have a high risk for heart disease, he or she may prescribe medication to lower your blood pressure or manage your cholesterol levels to prevent a heart attack. If you have a lower risk, you may be encouraged to make lifestyle changes in order to manage or reduce your likelihood of developing heart disease. Losing weight, not smoking (or quitting if you do), exercising, limiting alcohol and eating a heart-healthy diet have all been proven to lower blood pressure and cholesterol, and aid in the prevention of heart issues.

In either case, knowing your risk—and what you can do to lower it—can help you maintain your heart health and avoid any unnecessary treatment.

Heart Disease Risk Factors The major risk factors for heart disease include some that cannot be controlled—such as family history, gender and age. That’s why it’s so important to make changes to manage the ones you can control, such as by exercising, avoiding smoking and eating well. Check this list to see how you measure up: • Increasing age • Male (gender)* • Heredity

(including race)** • Smoking tobacco • High cholesterol • High blood pressure

• Physical inactivity • Obesity/Overweight • Diabetes • Stress • Excessive alcohol drinking • Poor diet and nutrition

* While men are at greater risk of having a heart attack than women—and may have an attack earlier in life—women are more likely to die of a heart attack when they do have one. **African-Americans, Mexican-Americans, Native Americans, native Hawaiians and some Asian-Americans have a higher risk of heart disease than Caucasians do.

Marie Clerjeau With the help of her 1199SEIU benefits, Marie Clerjeau is managing her medical conditions and setting a healthy example for her children.

Before her twins were born in 2005, Marie Clerjeau had never had a weight problem. But after their birth, Marie was diagnosed with a thyroid condition that caused her to gain weight. Although she takes thyroid medication—as well as medication for hypertension—maintaining a healthy weight has been a challenge. A Certified Nursing Assistant at Fulton Commons Care Center on Long Island, Marie also works as a Youth Support Counselor—all while raising her teenage daughter and son. “It can really be overwhelming at times,” she said.

The Benefit Fund helps Marie manage her conditions, while Marie works hard to stay healthy. To manage her conditions, she follows a healthy diet low in sugar and fat, eats lots of fresh fruits and vegetables, and practices portion control. She walks throughout the day, including around the parking lot near work, and uses free weights to build strength. She has also

attended wellness programs at her worksite that focus on stress reduction, and she uses a pedometer to keep track of her steps each day.

Marie shares her healthy habits with her children, encouraging them to limit sugared cereals, cookies and sweetened juices, and to drink more water. “My children motivate me,” Marie said. “I want to set a good example for them and help them stay healthy.”

Marie credits her improving health to the benefits she receives as an 1199SEIU member. “The health benefits help me address my medical conditions,” she said. “And my children have attended camp through the Child Care Fund, so knowing they are well-caredfor reduces my stress.” Marie admits that watching her weight and improving her overall well-being may not always be easy, but she knows it’s worth the effort.

“And 1199SEIU is helping me do it.”

Make Sure Your Tax Refund Is All It Can Be with the Earned Income Tax Credit!

Tax season is almost here, and the Benefit Fund’s free Tax Preparation Assistance Program works to make sure that you are able to prepare your taxes securely and with any income-based credits and deductions you’ve earned. If your 2018 household income was below $55,000 (or $30,000 if you’re single and have no dependents), our partners at the Food Bank For New York City and the nonprofit educational program Ariva are offering free tax prep events throughout the five boroughs this tax season.

Even if you don’t live near a Food Bank For New York City or Ariva site, be sure to check the “Claim Your EITC” guide below to find out whether you qualify for the Earned Income Tax Credit (EITC) before filing your return.

The Food Bank For New York City can also help you find out whether you are eligible for the Supplemental Nutrition Assistance Program (SNAP). The program helps more than two million New Yorkers buy healthy foods like fruit, vegetables, bread, and meat and dairy products, through a You can check whether you meet debit card that can be used like cash. Give Your Brain a Boost at Our Next Wellness Workshop January is a great time to make positive lifestyle changes, and you can start by attending our Wellness Department’s first workshop of 2019. The “Brain Health and Fitness” workshop* will focus on the importance of maintaining a healthy brain—and how regular exercise and good nutrition can help keep your mind sharp. You will learn how to better manage stressors so they don’t dominate your thoughts and distract you from your daily life—including ramping up your exercise routine and eating a healthy diet. Along with “brain games” and other mental exercises, these strategies have been shown to improve brain functioning. You’ll also learn easy-to-cook recipes using ingredients and food combinations proven to support brain power. The day will wrap up with an all-ages exercise class, so remember to wear comfortable clothes and bring the whole family!

REQUEST THE JOSEPH TAUBER SCHOLARSHIP APPLICATION The Joseph Tauber Scholarship Program offers scholarships to children of eligible 1199SEIU members who are attending accredited undergraduate programs. Eligible members are those whose employers contribute to the National Benefit Fund or the Greater New York Child Care Fund. Members must have been receiving Wage Class I or Eligibility Class I benefits for at least one year. Candidates must be high school graduates, post-secondary school students or previous awardees enrolled full time for a minimum of 12 credits each semester. All candidates must be younger than 23 years of age as of September 1, 2018. Previous awardees seeking scholarship support should note that automatic award renewals are not granted; you must reapply each year for consideration.

To request an application for the Joseph Tauber Scholarship Program, provide your information below and submit it before January 31, 2019. Este número de Para Su Beneficio incluye información importante sobre sus derechos en virtud de los planes de beneficios y pensión y de la Ley de Seguridad de los Ingresos de Jubilación del Empleado (ERISA, Employee Retirement Income Security Act), que debe leer y conservar para referencia futura.

Firma del Nuevo Convenio Colectivo de Trabajo Al cierre de este número de Para Su Beneficio, los negociadores colectivos llegaron a un nuevo acuerdo que protege sus beneficios de salud y de pensión. Podrán obtener información detallada de su sindicato en las próximas semanas.

Inscripción Abierta para Cónyuges Desde el 1 de enero El período de inscripción abierta para que usted inscriba a su cónyuge en el plan de salud del Fondo de Beneficios de la Zona Metropolitana de Nueva York se extiende desde el 1 de enero hasta el 31 de enero para la cobertura que comienza el 1 de marzo de 2019. Para inscribir a su cónyuge, debe completar un Formulario de Autorización de Deducción de Nómina y Cobertura de Cónyuge en el que autorice a su empleador a deducir la prima semanal de su salario. Habla de la Salud Cardíaca Las enfermedades cardíacas son la principal causa de muerte de hombres y mujeres en los EE. UU.: cada 40 segundos, una persona tiene un ataque cardíaco. Algo que nos afecta más directamente: la prevalencia de miembros de 1199SEIU que tienen riesgo de sufrir enfermedades cardíacas se acerca al 50 por ciento, principalmente, porque los problemas cardíacos, por lo general, se asocian con otras afecciones crónicas como diabetes tipo 2, hipertensión, obesidad y colesterol alto, y con malos hábitos como fumar. Todos estos son factores de riesgo que afectan a nuestros miembros en tasas que superan la media (consulte “¿Tiene Riesgo de Sufrir Enfermedades Cardíacas?” en la página 21).

Si bien espero que estos datos lo lleven a evaluar sus propios factores de riesgo, no son un motivo para desanimarse. Incluso si tiene actualmente un riesgo elevado de padecer enfermedades cardíacas, hay varias formas de disminuir ese riesgo, y su Fondo de Beneficios lo ayudará. Por ejemplo, nuestro Programa de Orientación entre Pares para la Diabetes, que administra InquisitHealth, colocó en parejas a cientos de miembros de 1199SEIU para que se ayuden entre sí a controlar mejor su diabetes tipo 2. El programa se expandió y ahora incluye a los miembros que necesitan ayuda para controlar su hipertensión, o presión arterial alta (consulte “Controle Su Presión Arterial Alta con Nuestro Programa de Orientación entre Pares”, más abajo). Ahora, un estudio reciente determinó que el método que los médicos han usado durante mucho tiempo para calcular el riesgo que tiene una persona de padecer enfermedades cardíacas puede no ser preciso. El estudio informó que muchas personas pueden estar tomando medicamentos de forma innecesaria para controlar los factores de riesgo como colesterol alto y presión arterial alta, mientras que es posible que otras (en particular, los afroamericanos), no estén recibiendo tratamiento suficiente. ¿Tiene Riesgo de Sufrir Enfermedades Cardíacas?

Evaluar Teniendo en cuenta su edad, género, niveles de presión arterial y colesterol y si fuma o tiene antecedentes familiares de diabetes, su médico puede determinar su factor de riesgo y trabajar con usted en un tratamiento, si es necesario. Los afroamericanos y latinos tienen un mayor riesgo de padecer enfermedades cardíacas; se los anima especialmente a que consulten a su médico para recibir una evaluación de la salud. Además, si en este momento toma medicación para disminuir la presión arterial o para controlar sus niveles de colesterol, es importante que lo evalúen para conocer su riesgo de padecer enfermedades cardíacas, ya que varios estudios han demostrado que los medicamentos pueden recetarse por encima o por debajo del nivel necesario.

Controlar Si el médico determina que tiene un alto riesgo de padecer enfermedades cardíacas, es posible que le recete medicamentos para reducir la presión arterial o controlar sus niveles de colesterol a fin de prevenir un ataque cardíaco. Si tiene un riesgo menor, es posible que se lo anime a que haga cambios en su estilo de vida a fin de controlar o reducir las posibilidades de padecer enfermedades cardíacas. Se ha demostrado que bajar de peso, no fumar (o dejar de fumar), hacer ejercicio, limitar el consumo de alcohol y adoptar una dieta saludable para el corazón disminuyen la presión arterial y el colesterol y ayudan a prevenir problemas cardíacos.

Marie Clerjeau Con la ayuda de los beneficios de 1199SEIU, Marie Clerjeau está tratando sus afecciones médicas y dando el ejemplo de llevar una vida saludable a sus hijos.

Antes de que sus mellizos nacieran en 2005, Marie Clerjeau nunca había tenido problemas de peso. SUMMARY ANNUAL REPORT 1199SEIU Greater New York Benefit Fund This is a summary of the annual report for the 1199SEIU Greater New York Benefit Fund, EIN: 13-6125570, for the period January 1, 2017, to December 31, 2017. This is a welfare benefit plan offering life insurance, health, dental, vision and prescription drug benefits. Insurance Information The plan has a contract with Amalgamated Life Insurance Company to pay certain life insurance claims incurred under the terms of the plan. The total premiums paid for the plan year ending December 31, 2017, were $912,288.

Because it is a so-called “experience-rated” contract, the premium costs are affected by, among other things, the number and size of claims. Of the total insurance premiums paid for the plan year ending December 31, 2017, the premiums paid under such “experience-rated” contracts were $843,592, and the total of all benefit claims paid under the experience-rated contract during the plan year was $407,957.

Basic Financial Statement The value of plan assets, after subtracting liabilities of the plan, was $119,441,956 as of December 31, 2017, compared to $116,507,335 as of January 1, 2017. During the plan year, the plan experienced an increase in its net assets of $2,934,621. This change includes unrealized appreciation and depreciation in the value of plan assets; that is, the difference between the value of the plan’s assets at the end of the year and the value of the assets at the beginning of the year, or the cost of assets acquired during the year. During the plan year, the plan had total income of $197,302,080, including employer contributions of $179,428,785, net appreciation in fair value of investments of $12,107,224, COBRA contributions of $282,892, member spousal contributions of $3,539,834 and other earnings of $1,943,345.

Plan expenses were $194,367,459. These expenses included $16,083,854 in administrative and general expenses, $178,024,428 in benefits paid to participants and beneficiaries, and $259,177 in investment and advisor fees.

Basic Financial Statement The value of plan assets, after subtracting liabilities of the plan, was $1,304,586 as of December 31, 2017, compared to $1,214,844 as of January 1, 2017. During the plan year, the plan experienced an increase in its net assets of $89,742. This change includes unrealized appreciation and depreciation in the value of the plan assets; that is, the difference between the value of the plan’s assets at the end of the year and the value of the assets at the beginning of the year, or the cost of assets acquired during the year. During the plan year, the plan had total income of $3,313,696, which included employer contributions of $3,233,589, program revenue of $70,657 and other earnings of $9,450.

You also have the right to receive from the Fund, on request and at no charge, a statement of the assets and liabilities of the plan and accompanying notes, or a statement of income and expenses of the plan and accompanying notes, or both. If you request a copy of the full annual report from the Fund, these two statements and accompanying notes will be included as part of that report.

Plan expenses were $3,223,954. These expenses included $894,839 in administrative and indirect program expenses, and $2,329,115 in benefits paid to participants and beneficiaries.
