The Value Of Plan Assets, After Subtracting Liabilities Of The Plan, Was $5,012,314 As Of December 31, 2009, Compared To $5,120,748 As Of

January 1, 2009. During The Plan Year The Plan Experienced A Decrease In Its Net Assets Of $108,434. This Decrease Includes Unrealized Appreciation Or Depreciation In The Value Of Plan Assets; That Is, The Difference Between The Value Of The Plan’s Assets At The End Of The Year And The Value Of The Assets At The Beginning Of The Year, Or The Cost Of Assets Acquired During The Year. During The Plan Year, The Plan Had Total Income Of $139,515. This Income Included Earnings From Investments Of $139,515. Plan Expenses Were $247,949.

These Expenses Included $121,293 In Administrative Expenses And $126,656 In Benefits Paid To Participants And Beneficiaries.

SUMMARY ANNUAL REPORT • 1199SEIU/GREATER NEW YORK CHILD CARE FUND This is a summary of the annual report for the 1199SEIU/GREATER NEW YORK CHILD CARE FUND, EIN 75-3086330, Plan Number 501, for the period January 1, 2009, to December 31, 2009. The annual report has been filed with the Department of Labor, as required under the Employee Retirement Income Security Act of 1974 (ERISA).

Basic Financial Statement The value of plan assets, after subtracting liabilities of the plan, was $450,891 as of December 31, 2009, compared to $3,001,156 as of January 1, 2009. During the plan year the plan experienced a change in its net assets of $2,550,265. This change includes unrealized appre-

ciation or depreciation in the value of plan assets; that is, the difference between the value of the plan's assets at the end of the year and the value of the assets at the beginning of the year or the cost of assets acquired during the year. The plan had total income of $54,165, which included employer contributions of $0, employee contributions of $0, gain/loss of $0 from the sale of assets, and earnings from investments of $3,607.

Total plan expenses were $2,604,430. These expenses included $750,907 in administrative expenses and $1,853,523 in benefits paid to participants and beneficiaries, and $0 in other expenses.

NOTICE ABOUT THE EARLY RETIREE REINSURANCE PROGRAM You are a plan participant, or are being offered the opportunity to enroll as a plan participant, in an employment-based health plan that is certified for participation in the Early Retiree Reinsurance Program.

The Early Retiree Reinsurance Program is a Federal program that was established under the Affordable Care Act. Under the Early Retiree Reinsurance Program, the Federal government reimburses a plan sponsor of an employment-based health plan for some of the costs of health care benefits paid on behalf of, or by, early retirees and certain family members of early retirees participating in the employmentbased plan. SUMMARY ANNUAL REPORT • 1199SEIU NATIONAL BENEFIT FUND FOR HEALTH & HUMAN SERVICE EMPLOYEES This is a summary of the annual report for the 1199SEIU National Benefit Fund for Health & Human Service Employees, EIN: 13-1628401, for the period 01/01/09 to 12/31/09. The annual report has been filed with the Employee Benefits Security Administration, as required under the Employee Retirement Income Security Act of 1974 (ERISA).

December 31, 2009, the premiums paid under such “experience-rated” contracts were $7,352,003 and the total of all benefit claims paid under the experience-rated contract during the plan year was $7,010,298.

The annual report has been filed with the Department of Labor, as required under the Employee Retirement Income Security Act of 1974 (ERISA).

Insurance Information The plan has (a) contract(s) with Metropolitan Life Insurance Company to pay the following types of claims incurred under the terms of the plan: The total premiums paid for the plan year ending December 31, 2009, were $8,105.

Basic Financial Statement The value of plan assets, after subtracting liabilities of the plan, was $18,216,062 as of December 31, 2009, compared to $16,566,352 as of January 1, 2009. During the plan year the plan experienced a change in its net assets of $1,649,710. This change includes unrealized appreciation or depreciation in the value of plan assets; that is, the difference between the value of the plan's assets at the end of the year and the value of the assets at the beginning of the year or the cost of assets acquired during the year. The plan had total income of $23,190,972, which included employer contributions of $20,927,623, employee contributions of $756,377, gain/loss of $0 from the sale of assets, and earnings from investments of $1,506,972.

Total plan expenses were $21,541,262. These expenses included $5,509,500 in administrative expenses and $16,031,762 in benefits paid to participants and beneficiaries, and $0 in other expenses.

SUMMARY ANNUAL REPORT • 1199SEIU GREATER NEW YORK BENEFIT FUND This is a summary of the annual report for the 1199SEIU Greater New York Benefit Fund, EIN: 13-6125570, for the period 01/01/09 to 12/31/09. The annual report has been filed with the Employee Benefits Security Administration, as required under the Employee Retirement Income Security Act of 1974 (ERISA).

December 31, 2009, the premiums paid under such “experience-rated” contracts were $873,373 and the total of all benefit claims paid under the experience-rated contract during the plan year was $655,278.

Insurance Information

The value of plan assets, after subtracting liabilities of the plan, was $4,019,346 as of December 31, 2009, compared to $6,620,082 as of January 1, 2009. During the plan year the plan experienced a decrease in its net assets of $2,600,736. The plan had total income of $163,650,707, including employer contributions of $162,164,574, employee COBRA contributions of $394,420, and net earnings from investments of $282,933. Plan expenses were $166,251,443. These include $153,043,540 in benefits paid to participants and beneficiaries and $13,207,903 in general and administrative expenses.

The plan has a contract with Amalgamated Life Insurance Company to pay certain life claims incurred under the terms of the plan. The total premiums paid for the plan year ending December 31, 2009, were $66,165.

Website The next time you visit the Funds' website, there are some great new changes waiting for you. The redesigned site features a simplified menu and a user-friendly Tool Center that will help you find what you're looking for quickly and easily. With the number of members using our website on the rise – almost 85 percent of members report having a home computer, and a growing number say they use the Funds' website to access health benefit information – the new www.1199SEIUBenefits.org puts all your vital resources right at your fingertips.

The site has been redesigned so that fewer clicks are required to get where you want to go, and a drop-down menu from the navigation bar will help you pinpoint the precise location of key information rather than search from page to page. Visitors can use our new Tool Center, which contains the most commonly used resources – like provider and urgent care center directories – and may often serve as “one-stop shopping” for your most frequent needs. The Benefit Finder has also been upgraded so that you can continuously see which benefits you qualify for as you navigate the site, and our search function has been vastly improved, making it simpler to locate specific items.

You'll also notice a new, prominent display of current events, news and announcements, along with a more detailed and accessible calendar, which gives you a complete rundown of what's happening at the Funds and can direct you to registration pages for upcoming programs.

Exciting New Feature for Members The redesigned website also includes a new feature that enables you to create a Personal Information Account, where you can access all of your personal health benefit information as well as request certain services from the Funds. Whether you want to know if a claim has been paid, when your last doctor's appointment was, or what services you are eligible for, or if you need to change your address or order a new ID card, you can accomplish these tasks and more through your Personal Information Account.

This improved self-service capability can save you time on the phone or a trip to our Member Services Department at 330 West 42nd Street.

We plan to expand the Personal Information Account functions to include access to the Pension, Training and Child Care Funds, so watch for more details later this year. But take advantage of the access to your personal health information today!

As of January 1, 2011, dependent coverage extends to young adults up to age 26, regardless of student status.

New Year Brings

New Healthcare Reform Changes As we told you in the last issue of For Your Benefit, the passage of the Patient Protection and Affordable Care Act has brought good news for 1199SEIU members. As of January 1, 2011, dependent coverage extends to young adults up to age 26, regardless of student status. Of course, if your child has access to coverage through an employer or spouse, he or she will not be eligible for enrollment in the Fund’s plan. And if your spouse has employer health coverage, the “birthday rule” will apply to determine whether your dependent child is covered by the Benefit Funds or your spouse’s health plan (the plan of the parent whose birthday falls first in the calendar year is the primary payer). In either case, your child must also enroll in your spouse’s health plan to help coordinate benefits.

In November, we began the process of enrolling new young adult dependents so that their coverage could be in place as of January 1.We also are issuing new 1199SEIU Health Benefits ID cards to these members and their children. If you missed the special enrollment period in November, you can still enroll your eligible young adults.Their coverage will take effect on the first day of the month following 30 days from the day you enroll. As an example, if you enroll your dependent young adult on January 15, his or her coverage will begin March 1.

As of January 1, we have also eliminated the maximum lifetime benefit ($1 million for National Benefit Fund members in the Member Choice program, or $500,000 for GNY members and NBF members not in Member Choice) as well as the $100,000 maximum on newborn coverage and the $100,000 limit on biotech medications.

Most members never reach the lifetime maximum benefits, but if you have reached yours, please contact your Outreach Coordinator or Member Services at (646) 473-9200 to discuss transitioning back into the Funds.We're here to help you!

Every year our Benefit Fund’s Tax Assistance Program helps thousands of members who are eligible for the Earned Income Tax Credit (EITC) claim the money they earned – and deserve.This year, the Benefit Fund is once again partnering with the EITC Coalition led by the New York City Department of Consumer Affairs, including the city’s Office of Financial Empowerment (OFE), H&R Block and the Food Bank for New York City, to give you as many options as possible.

Tax Prep Events As long as your household income was below $50,000 in 2010 (or $18,000 if you’re single and have no dependents), you can have your taxes prepared for free at one of our Tax Assistance locations – even if you earned too much to qualify for the EITC. If you do qualify for the EITC, the program’s tax preparers will help you file for the credit.

Lorraine Calderone For 1199SEIU member Lorraine Calderone, taking advantage of a Benefit Fund worksite health fair helped her take control of her diabetes.

A little more than a year ago, Lorraine Calderone attended a health fair at Mount Sinai Queens Hospital in Long Island City, New York, where she works as an Admitting Registrar, and took part in a free screening for blood sugar levels. “I thought it was great that 1199 had the health fair right here because it allowed me to take advantage of the screening,” she said, “and my A1C level came back very high.”

The A1C test measures a person's average blood glucose level over the previous two to three months, and a high reading like Lorraine’s can indicate diabetes. A week after the health fair Lorraine visited her doctor, who confirmed that she has Type 2 diabetes.Told that she would have to change her diet and closely monitor her blood sugar level if she wanted to stay healthy and maintain an active lifestyle, she was actually a bit relieved to know what was going on.

“I had been suffering symptoms for some time, like always being thirsty and feeling tired,” she said.

“But I passed it off as probably due to old age and wouldn’t have followed up if it wasn’t for the 1199SEIU health fair.” Lorraine, who is 63 and has worked at Mount Sinai for 18 years, knows that her Fund benefits provide full medical coverage and a wide range of doctors, but she said that, like many busy people, she always put off making an appointment.

After her diagnosis, her next stop was an endocrinologist, who provided the necessary kit to test her blood sugar and gave her guidelines for a more nutritious – and safe – diet. She said she feels “very fortunate” not to have to inject insulin as part of her treatment, but she does take a medication that helps control her blood sugar, and has to test her levels twice a day, in the morning and at night.

Since learning she is a diabetic, she’s been committed to a healthier lifestyle that begins with a revamped diet. She eats more fresh fruits and vegetables and fewer processed foods - which are often high in sugar and saturated fat. Lorraine was always a high-energy person, and now her energy is back. Her two grown sons tell her she doesn’t need a gym membership now because she’s “always on the go.” Her children have also helped her stick to her diet, reminding her – whether she cooks for them at home or they go out to eat – which foods are approved and which are not.

“They worry about me,” she said. “But I’ve lost weight, I eat better, and of course I watch my sugar intake very carefully.The whole ordeal has actually been a good thing.”

The newer, stronger, more dangerous flu viruses such as the H1N1 virus make it more important than ever to protect yourself with a flu shot this season. And there are many simple, everyday ways you can try to avoid getting or passing on colds. Here's how: Wash your hands often or use hand sanitizer.

Permita que la Prevención del Resfriado y la Gripe sea un Nuevo Hábito Aún estamos en la temporada del resfriado y la gripe. Como trabajadores de la salud, sabemos la importancia de tomar las precauciones necesarias para evitar la infección. Debido a que los nuevos virus de la gripe, como el H1N1, son más fuertes y peligrosos, es más importante que nunca protegerse con una vacuna contra la gripe esta temporada. Y hay muchas formas simples y que se pueden hacer todos los días para evitar contagiarse o contagiar los resfriados. Algunas de éstas son:

Lorraine Calderone Para la miembro de 1199SEIU Lorraine Calderone, aprovechar una feria de salud en los lugares de trabajo del Fondo de Beneficios le ayudó a controlar su diabetes.

Hace poco más de un año, Lorraine Calderone asistió a la feria de salud en Mount Sinai Queens Hospital en la ciudad de Long Island, New York, donde trabaja como representante de registro, y participó en los exámenes de medición de niveles de glucosa en la sangre. “Pensé que era genial que 1199 realizara la feria de salud justo aquí, porque me permitió aprovechar el examen”, dijo, "y mi nivel de hemoglobina A1C resultó ser muy alto”.

Nuevos Cambios a la Reforma de la Atención de la Salud Como se lo mencionamos en el último número de Para su beneficio, la aprobación de la Ley de Medidas de Protección del Paciente y Cuidado de Bajo Costo conlleva buenas noticias para los miembros de 1199SEIU. A partir del 1 de enero de 2011, la cobertura para dependientes se extiende a adultos jóvenes de hasta 26 años, sin importar su condición de estudiante. Naturalmente, si su hijo tiene acceso a cobertura mediante un empleador o cónyuge, su hijo no será elegible para inscribirse en el plan del Fondo.Y si su cónyuge posee cobertura de salud del empleador, se aplicará la “regla del cumpleaños” para determinar si su hijo dependiente está cubierto por los Fondos de Beneficios o por el plan de salud de su cónyuge (el plan del padre cuyo cumpleaños sea primero en el año calendario será el pagador principal). Cualquiera sea el caso, su hijo también se debe inscribir en el plan de salud de su cónyuge para ayudar a coordinar los beneficios.

To take the pressure off cooking every night, try doubling a casserole recipe or throwing an extra meatloaf in the oven. Before you freeze your leftovers, let the food cool down. If it’s a soup, stew or sauce, occasionally stir to be sure it cools evenly. Portion leftovers into meal-sized containers and label and date them. Place in a single layer in the coldest area of your freezer.

Also: • Leave as little air as possible in the freezer containers and bags.

• Wrap solid foods like meats and baked goods tightly in foil before bagging them.

Snack on low-fat carbohydrates like popcorn, which is a good source of fiber and easy to digest.

Avoid excessive amounts of caffeine and alcohol, which can cause anxiety and possibly lead to depression.

Relax!

Permita que la Prevención del Resfriado y la Gripe sea un Nuevo Hábito Aún estamos en la temporada del resfriado y la gripe. Como trabajadores de la salud, sabemos la importancia de tomar las precauciones necesarias para evitar la infección. Debido a que los nuevos virus de la gripe, como el H1N1, son más fuertes y peligrosos, es más importante que nunca protegerse con una vacuna contra la gripe esta temporada. Y hay muchas formas simples y que se pueden hacer todos los días para evitar contagiarse o contagiar los resfriados. Algunas de éstas son:

Lávese las manos con frecuencia y use desinfectante para manos.

The newer, stronger, more dangerous flu viruses such as the H1N1 virus make it more important than ever to protect yourself with a flu shot this season. And there are many simple, everyday ways you can try to avoid getting or passing on colds. Here's how: Wash your hands often or use hand sanitizer.

The majority of cold and flu viruses are spread by direct contact because germs can live for hours on the skin or on a surface.Washing your hands regularly and cleaning off your work areas and frequently handled items is one of the best ways to prevent germs from spreading.

Cough and sneeze into a tissue or the crook of your elbow.

Germs and viruses can cling to your bare hands and travel to anyone you touch. So instead of coughing or sneezing into your hands, grab a tissue or sneeze into the inside of your elbow, which is less likely to come into contact with someone and spread germs.

A little more than a year ago, Lorraine Calderone attended a health fair at Mount Sinai Queens Hospital in Long Island City, New York, where she works as an Admitting Registrar, and took part in a free screening for blood sugar levels. “I thought it was great that 1199 had the health fair right here because it allowed me to take advantage of the screening,” she said, “and my A1C level came back very high.”

The A1C test measures a person's average blood glucose level over the previous two to three months, and a high reading like Lorraine’s can indicate diabetes. A week after the health fair Lorraine visited her doctor, who confirmed that she has Type 2 diabetes.Told that she would have to change her diet and closely monitor her blood sugar level if she wanted to stay healthy and maintain an active lifestyle, she was actually a bit relieved to know what was going on.

“I had been suffering symptoms for some time, like always being thirsty and feeling tired,” she said.

“But I passed it off as probably due to old age and wouldn’t have followed up if it wasn’t for the 1199SEIU health fair.” Lorraine, who is 63 and has worked at Mount Sinai for 18 years, knows that her Fund benefits provide full medical coverage and a wide range of doctors, but she said that, like many busy people, she always put off making an appointment.

After her diagnosis, her next stop was an endocrinologist, who provided the necessary kit to test her blood sugar and gave her guidelines for a more nutritious – and safe – diet. She said she feels “very fortunate” not to have to inject insulin as part of her treatment, but she does take a medication that helps control her blood sugar, and has to test her levels twice a day, in the morning and at night.

Since learning she is a diabetic, she’s been committed to a healthier lifestyle that begins with a revamped diet. She eats more fresh fruits and vegetables and fewer processed foods - which are often high in sugar and saturated fat. Lorraine was always a high-energy person, and now her energy is back. Her two grown sons tell her she doesn’t need a gym membership now because she’s “always on the go.” Her children have also helped her stick to her diet, reminding her – whether she cooks for them at home or they go out to eat – which foods are approved and which are not.

“They worry about me,” she said. “But I’ve lost weight, I eat better, and of course I watch my sugar intake very carefully.The whole ordeal has actually been a good thing.”

New Year Brings

New Healthcare Reform Changes As we told you in the last issue of For Your Benefit, the passage of the Patient Protection and Affordable Care Act has brought good news for 1199SEIU members. As of January 1, 2011, dependent coverage extends to young adults up to age 26, regardless of student status. Of course, if your child has access to coverage through an employer or spouse, he or she will not be eligible for enrollment in the Fund’s plan. And if your spouse has employer health coverage, the “birthday rule” will apply to determine whether your dependent child is covered by the Benefit Funds or your spouse’s health plan (the plan of the parent whose birthday falls first in the calendar year is the primary payer). In either case, your child must also enroll in your spouse’s health plan to help coordinate benefits.

In November, we began the process of enrolling new young adult dependents so that their coverage could be in place as of January 1.We also are issuing new 1199SEIU Health Benefits ID cards to these members and their children. If you missed the special enrollment period in November, you can still enroll your eligible young adults.Their coverage will take effect on the first day of the month following 30 days from the day you enroll. As an example, if you enroll your dependent young adult on January 15, his or her coverage will begin March 1.

As of January 1, we have also eliminated the maximum lifetime benefit ($1 million for National Benefit Fund members in the Member Choice program, or $500,000 for GNY members and NBF members not in Member Choice) as well as the $100,000 maximum on newborn coverage and the $100,000 limit on biotech medications.

Most members never reach the lifetime maximum benefits, but if you have reached yours, please contact your Outreach Coordinator or Member Services at (646) 473-9200 to discuss transitioning back into the Funds.We're here to help you!

During the plan year the plan experienced a change in its net assets of $1,649,710. This change includes unrealized appreciation or depreciation in the value of plan assets; that is, the difference between the value of the plan's assets at the end of the year and the value of the assets at the beginning of the year or the cost of assets acquired during the year. The plan had total income of $23,190,972, which included employer contributions of $20,927,623, employee contributions of $756,377, gain/loss of $0 from the sale of assets, and earnings from investments of $1,506,972.

Total plan expenses were $21,541,262. These expenses included $5,509,500 in administrative expenses and $16,031,762 in benefits paid to participants and beneficiaries, and $0 in other expenses.

SUMMARY ANNUAL REPORT • 1199SEIU GREATER NEW YORK BENEFIT FUND This is a summary of the annual report for the 1199SEIU Greater New York Benefit Fund, EIN: 13-6125570, for the period 01/01/09 to 12/31/09. The annual report has been filed with the Employee Benefits Security Administration, as required under the Employee Retirement Income Security Act of 1974 (ERISA).

December 31, 2009, the premiums paid under such “experience-rated” contracts were $873,373 and the total of all benefit claims paid under the experience-rated contract during the plan year was $655,278.

Insurance Information

The value of plan assets, after subtracting liabilities of the plan, was $4,019,346 as of December 31, 2009, compared to $6,620,082 as of January 1, 2009. During the plan year the plan experienced a decrease in its net assets of $2,600,736. The plan had total income of $163,650,707, including employer contributions of $162,164,574, employee COBRA contributions of $394,420, and net earnings from investments of $282,933. Plan expenses were $166,251,443. These include $153,043,540 in benefits paid to participants and beneficiaries and $13,207,903 in general and administrative expenses.

The plan has a contract with Amalgamated Life Insurance Company to pay certain life claims incurred under the terms of the plan. The total premiums paid for the plan year ending December 31, 2009, were $66,165.

Because it is a so called “experience-rated” contract, the premium costs are affected by, among other things, the number and size of claims. Of the total insurance premiums paid for the plan year ending

Website The next time you visit the Funds' website, there are some great new changes waiting for you. The redesigned site features a simplified menu and a user-friendly Tool Center that will help you find what you're looking for quickly and easily. With the number of members using our website on the rise – almost 85 percent of members report having a home computer, and a growing number say they use the Funds' website to access health benefit information – the new www.1199SEIUBenefits.org puts all your vital resources right at your fingertips.

The site has been redesigned so that fewer clicks are required to get where you want to go, and a drop-down menu from the navigation bar will help you pinpoint the precise location of key information rather than search from page to page. Visitors can use our new Tool Center, which contains the most commonly used resources – like provider and urgent care center directories – and may often serve as “one-stop shopping” for your most frequent needs. The Benefit Finder has also been upgraded so that you can continuously see which benefits you qualify for as you navigate the site, and our search function has been vastly improved, making it simpler to locate specific items.

You'll also notice a new, prominent display of current events, news and announcements, along with a more detailed and accessible calendar, which gives you a complete rundown of what's happening at the Funds and can direct you to registration pages for upcoming programs.

Exciting New Feature for Members The redesigned website also includes a new feature that enables you to create a Personal Information Account, where you can access all of your personal health benefit information as well as request certain services from the Funds. Whether you want to know if a claim has been paid, when your last doctor's appointment was, or what services you are eligible for, or if you need to change your address or order a new ID card, you can accomplish these tasks and more through your Personal Information Account.

This improved self-service capability can save you time on the phone or a trip to our Member Services Department at 330 West 42nd Street.

We plan to expand the Personal Information Account functions to include access to the Pension, Training and Child Care Funds, so watch for more details later this year. But take advantage of the access to your personal health information today!

NOTICE ABOUT THE EARLY RETIREE REINSURANCE PROGRAM You are a plan participant, or are being offered the opportunity to enroll as a plan participant, in an employment-based health plan that is certified for participation in the Early Retiree Reinsurance Program.

The Early Retiree Reinsurance Program is a Federal program that was established under the Affordable Care Act. Under the Early Retiree Reinsurance Program, the Federal government reimburses a plan sponsor of an employment-based health plan for some of the costs of health care benefits paid on behalf of, or by, early retirees and certain family members of early retirees participating in the employmentbased plan.
