28 Applicants are encouraged to request any necessary accommodations as early as possible to allow the University to complete the necessary case-by-case analysis. The accommodation request form asks the applicant’s health care provider to provide information concerning the applicant’s diagnosis, functional limitations, and recommendations regarding accommodation needs. The information provided will be shared only with relevant staff and will be used only as permitted under Section 504 of the Rehabilitation Act of 1973. This information will have no bearing on the applicant’s eligibility for admission.
