Just be HELD Nazareth Home-Clifton and Hosparus Health have recently launched a new nurse practitioner consult program known as HELD: Helping Empower Life’s Decisions. The program’s goal is to improve the care and quality of life for seriously ill senior residents. A HELD consult is designed to explore and establish the resident’s goals for care, improve care coordination and collaboration, provide expert symptom management, assist with advance care planning, and provide patient and family education. The program is offered exclusively at Nazareth Home’s Clifton location, with Hosparus Health nurse practitioners conducting consultations.
