Fur-ever Companion Memorial Plaques

Whether your dog was your companion, best friend, secret keeper, buddy, comfort, or anything in between, we have a unique opportunity to remember him or her with a commemorative memorial plaque at one of his or her favorite places to play - the Hoffman Park Dog Park. Each plaque will become part of the rainbow that will be affixed to the fence inside the park.

Become Part of our Next Installation

To become part of the next installation this summer, complete the Dog Park Memorial Plaque Form found online. Forms are also available at carypark.com and at the Community Center. To be a part of the next installation, forms are due by May 30 and will be installed in June.
