ABOUT THE PROJECT

The Burien Residential Rain Garden Pilot Program works with Burien residents to manage stormwater pollution on private property in low income neighborhoods, as well as increase community knowledge about building rain gardens and their positive effect on the environment through hands-on learning. Support for this project is provided in part by the Puget Sound Stewardship and Mitigation Fund, a grantmaking fund created by the Puget Soundkeeper Alliance and administered by the Rose Foundation for Communities and the Environment.

EnhanceWellness Beginning Monday, September 21st, we will have a highly trained Community Health Worker here to help you begin your journey to better health. Whether someone wants to lose weight, faces health challenges, is grieving, or is coping with depression, EnhanceWellness, a participantcentered motivational intervention with proven results, provides a highly trained community health worker here to work with you. Research has shown that participants in EnhanceWellness can decrease the length of hospital stays, manage their chronic conditions, lower their use of psychoactive drugs, alleviate symptoms of mood disorders, and develop a sense of greater self-efficacy.

EnhanceWellness happens in three steps: Screen, Plan, Action

Begin Your Journey to Better Health

1

AN ENHANCEWELLNESS SCREEN identifies personal strengths and risks. Together, an EnhanceWellness team member and the participant review a detailed health questionnaire.

2

THE ENHANCEWELLNESS ACTION PLAN focuses on areas the participant chooses to work on. This plan can also be shared with the participant’s physician.

3

THE PARTICIPANT THEN MOVES INTO ACTION with the support of community health worker and/or volunteer health mentor, who offer ongoing encouragement, feedback, and monitoring. The team helps with problem solving, health education, and referral to support groups and additional services, including individual and family counseling, if indicated.

EnhanceWellness is a reliable complement to formal healthcare services for older adults. It is backed by independent research and has been shown to yield measurable results in health — an important consideration for service providers, public and private funders, health care partners, and older adults themselves.
