With all that is going on at Lafayette Recreation we thought it would be appropriate to update you on some interesting information that you might not be aware of, for instance: Did you know that in October 2013, Lafayette’s Departments of Recreation, Senior Services & Facility Management along with Parks, Open Space & Golf became one of only 4 agencies in Colorado and 118 agencies nationwide to receive accreditation through the Commission for Accreditation of Parks and Recreation Agencies. This thorough review and evaluation process will help ensure that Lafayette continues to provide the highest quality programs, services and facilities to our community. Did you know, that according to the U.S. Dept. of Health & Human Services most health benefits occur with at least 150 minutes (2 hours and 30 minutes) a week of moderate intensity physical activity? That’s 30 minutes a day, 5 days a week; and BBRC has a wide variety of activity options to assist you in meeting that goal. Whether it’s basketball, pickleball, racquetball or handball; or treadmills, stepmills or LES MILLS (ie. Body Pump, Body Combat) we’ve got you covered. Did you know, that Lafayette’s Oatmeal Festival has been a success for over 15 years, and that the new Lafayette Quest Event just finished its 2nd year and has raised thousands of dollars to combat childhood obesity in our community through the Recreation Center’s scholarship fund? Did you know, that the newly remodeled, ADA accessible indoor hot tub, steam room and sauna project just completed has added significant space and comfort to each of these amenities? To comply with current ADA accessibility standards, we have transformed our indoor hot tub, steam room and sauna into a luxurious spa. Be sure to stop in for a soak and much deserved restoration after your next workout. Did you know that at any given time the Recreation Center employs around 150 people from all walks of life in our community, including: Front Desk, Child Care, Preschool, Afterschool, Aquatics, Athletics, Fitness and Maintenance staffs; not to mention the myriad of exceptional contractors that partner with the City to provide services? Facility Amenities Recreation Center Amenities

Aquatics Area: A four-pool complex including a leisure/therapy pool with a variety of water features, a lazy river/tube ride, a 25-yard, 6-lane pool and a 150 foot waterslide with landing pool. Fitness Atrium: Features an extensive free weight system, Nautilus selectorized machines, stationary bicycles, Stairmasters, rowing machines, treadmills and Precor ellipticals. Gymnasium: A large hardwood floor, available for basketball and volleyball. Racquetball Courts: Two full-size courts; one has a competition glass wall. Wallyball option available on both courts. Running Track: 16 laps = 1 mile; cushioned oval. Dance/Fitness Room: Designed for fitness and dance with mirrors, no-shock hardwood floor, and a great sound system. Preschool Rooms: Rooms specifically designed for our licensed preschool programs.

Code of Conduct The City of Lafayette Recreation & Facility Management Department seeks to create a welcoming and wholesome environment for all facility users and program/activity participants. To help ensure a safe and satisfactory experience for all, the City expects that respectful, courteous and considerate behavior will be exhibited at all times by those visiting our facilities or participating in programs and/or activities. A violation of the Code of Conduct may include, but is not limited to: disorderly conduct, the use of offensive, threatening or abusive language or gestures, theft, public intoxication, use of drugs, sexual misconduct, harassment, failure to cooperate with Department personnel, possession of any type of weapon, non-compliance with Department rules and regulations, any unlawful activity, and any other behavior deemed to be inappropriate. The City reserves the right to deny admission and/or take appropriate disciplinary actions against any individual violating this Code of Conduct.

Indoor Hot Tub and Spa

Multi-Use Room: Includes a small kitchen and is used for classes, meetings and special events. Wet Area: A co-ed dry sauna, wet steam, indoor whirlpool and outdoor adult only hot tub where individuals can relax after a workout. Child Care: Available for children ages 6 months to 7 years of age. Parents must be using the facilities. See page 10 for hours of operation & costs. Family Locker Rooms: Two cabanas are available to assist families with small children who are unable to be alone or in opposite gender locker room; as well as guests needing help due to special circumstance, ie. disability or injury.

Facility Guidelines

RECREATION Center POLICIES & PROCEDURES • All facility guests using a daily fee area must check in at the front reception desk. Guests found to be using the facility without paying will be asked to leave and may be suspended from future use. • There is no smoking, chewing tobacco, marijuana, alcoholic beverages or gum allowed in the Center. Food and drink are permitted only in designated areas (lobby, multiuse rooms, outdoor patio). Sport drinks and water past the front desk must be in a spill-proof bottle. • A photo I.D. is required for all equipment checkout, including locker padlocks and towel rentals. Acceptable identification includes a valid Drivers License or School photo ID. If you do not have one of these, you may purchase a Recreation Center ID for $5.00. • The Recreation Center is NOT responsible for lost or stolen articles. It is strongly recommended that you do not bring any valuables into the Center. Please turn in any found articles to the lost & found at the front desk. Lost & found items may be identified and then retrieved from the front desk. • The Center lockers operate on a padlock system. With a photo ID, any recreation center guest may check out a lock from the front desk. A lost lock or key is $8.00; if both are lost the fee is $16.00. Patrons may bring their own lock to secure their belongings. All padlocks remaining on lockers at the end of each business day will be cut off.

• Racquetball reservations may be made two days in advance. Failure to show for a racquetball reservation without a two hour notice may result in a suspension of court reservation privileges. Racquets are available for checkout (with a photo ID) during your reserved time on the court only. Adult annual pass holders may reserve 7 days in advance. • Appropriate apparel must be worn in accordance with the area of the facility being used. Shoes and shirts are required at all times. Only non-marking shoes are allowed on the racquetball courts or in the gymnasium. Proper swim attire (bathing suits) must be worn in all pools (No street clothes will be allowed in the pools). Shirts and non-marking shoes are required in the weight room and gymnasium. • Changes in the recreation schedule may occur without notice. The Adult Swim times may be cancelled, or the hours changed, during holidays and school in-service days. Call the Recreation Center at 303-665-0469 or the information line at 303-665-0468 if you have any questions. • A courtesy phone is located in the vestibule and outside the family locker rooms for personal calls. Please be considerate of other guests when using mobile phones in the facility.

• Cameras/camera phones are not allowed in the locker room areas. Please stow these items in a bag or carrying case. • In the best interest of all our patrons, children 4 years of age and older are required to use the gender-appropriate locker room. • Family Locker Rooms are available, and are intended for use by adults with small children who are unable to be alone and/or be in the opposite gender locker room.

AGE GUIDELINES All areas: For your child’s safety, all children under ten (10) years of age must be supervised by a guest 16 years or older. Pool areas: Children 5 years of age or younger who will be swimming must display the appropriate wristband and be accompanied by a paying guest 16 years or older who is in the water within arms reach of the child(ren).

Fitness Areas: Weight and fitness equipment is available to anyone age 15 years and older. Those between the ages of 15-17 must complete a weight room/fitness equipment orientation with one of our weight room attendants in order to use these areas. This brief class, along with a weight room ID card, is a service offered by the Recreation Center free of charge. Participants will learn proper use and treatment of various machines and weight room etiquette. Sign up at the front desk. - Youth 12-14 using the Expresso Bikes must be supervised by an adult present in the fitness atrium. - Parent/Youth Workouts are permitted anytime, with the exception of Monday – Thursday 5-7:30pm (our peak time). Parent and child must complete a Weight Room Orientation, children must be 12 years old or older and working out directly with their parent (see pg. 32 or 54 for more details).

Volleyball Requires 1/2 gym rental plus $20 setup fee. Minimum 2 day advance reservation

Running track: available to anyone age 12 years and older. Infants are permitted in a personal baby carrier/harness only.

Is your large group interested in using the pool? Call the Aquatics Department and book your group. Group rate is for any combination of ages for 10 or more and must be reserved a week in advance. For more information, please contact Jeff Richards at 303-665-0469.

The recreation staff will enforce all rules and safety policies for the benefit and enjoyment of all. Our Center Staff is always available to help you. If you have any problems, please feel free to contact any member of the staff.

Americans with Disabilities Assistance Individuals with disabilities are invited to contact the Recreation Department to discuss any special accommodations or adaptations which are necessary for participation in our programs and services. Gymnasium/Child Care Equipment Checkout A variety of equipment available for facility guests to check out from the front desk. Users need only present a valid ID card which will be held until the equipment is returned. Some of the equipment available includes: - Basketballs - Volleyballs - Weight belts - Exercise balls

There is no charge for the equipment unless it is not returned. Lost equipment will be charged the full replacement cost.

Child Care: 6 Months - 7 Years Space is limited to a first-come, first-served basis. We provide a positive, fun environment for children. This service is available to participants remaining inside the Bob L. Burger Recreation Center. If you have a child with special needs, please call two days in advance so we can make accommodations. Child Care is not available on holidays. (Schedule subject to change - check at the front desk.) Swim Lessons The purpose of the American Red Cross Swimming and Water Safety program is to teach individuals of different ages and abilities how to swim and be safe in, on and around the water. In a logical progression, the program covers the knowledge and skills needed for aquatic skill development. As participants develop these skills, they will become better swimmers. Our program features American Red Cross certified swim instructors who give personalized attention to each student. The preschool (Dolphin) classes strive for a 4:1 student to instructor ratio and a 5:1 ratio for our Level 1-6 Program.

Swim Lessons Lifeguard Training Pre-Course Always wanted to be a lifeguard but wasn’t sure you had what it takes? Then we have the class for you! You’ll be able to practice all the skills necessary to get ready for the lifeguard training course including the 300 yard swim of breaststroke or freestyle, treading water for 2 minutes without your hands, and getting a 10 pound brick from the bottom of the 10 foot with a certified instructor. The class is twice a week and is offered two weeks before each lifeguard training course. Water Safety Instructor Class 16 + YEARS Pre-Requisites: 16 yrs old on or before the last day of class. Baseball, Softpitch, & Softball 7-11 yr old Softpitch is designed as a co-ed introductory league for those progressing from T-ball into pitched ball for the first time using a pitching machine. The ball will travel slower to allow for more success. A great way to introduce your child into the sport! Practice will be held once or twice per week in the evenings. The season is based on a seven game schedule with every effort to make up cancellations. Age determined as of July 31. The Baseball/Softball program is an instructional program for boys and girls 9-11 years old who are progressing from softpitch, as well as players not ready for live pitch baseball. Age determined as of July 31. This division uses a pitching machine to help kids make the transition to baseball and softball. Using a pitching machine, the emphasis will be placed on participation and fun while focusing on skill development. Practices will be once or twice per week in the evenings. The season is based on a seven-game schedule with every effort to make up cancellations. Location: Local parks and schools. REGISTRATION DEADLINE: Wednesday, May 28. Since classes are progressive, your child must have completed the previous level and mastered all skills taught or obtain permission from the gymnastics instructor to move to the next level. Early childhood preschool program For young children, meaningful and long-lasting learning requires active thinking and experimenting to find out how things work. This is best accomplished through purposeful play facilitated by highly intentional teaching practices. The Goals of our Curriculum The most important goal of our early childhood curriculum is to help children become enthusiastic learners and form friendships. This means encouraging children to be active and creative explorers who are not afraid to try out their ideas and to think their own thoughts. Our goal is to help children become independent, self-confident, inquisitive learners. We’re teaching them how to learn, not just in preschool, but also all through their lives. We’re allowing them to learn at their own pace and in the ways that are best for them. We’re giving them good habits and attitudes, particularly a positive sense of themselves, which will make a difference throughout their lives. The areas of development are:

Social: To help children feel comfortable in school, trust their new environment, make friends, and feel they are a part of the group. Emotional: To help children experience pride and self- confidence, develop independence and self-control, and have a positive attitude toward life. Cognitive: To help children become confident learners by letting them try out their own ideas and experience success, and by helping them acquire learning skills such as the ability to solve problems, ask questions, and use words to describe their ideas, observations, and feelings. Burger Recreation Center After School Enrichment Program seeks to enhance our community and enrich the lives of children in the City of Lafayette by providing a safe, supportive and structured environment that supports overall health and well-being. The B.A.S.E. program offers quality educational, recreational and cultural programs that promote the social, physical, intellectual, and emotional development of children and youth. Drop-In Fitness Classes HATHA YOGA FOR STRENGTH AND ENERGY A class filled with powerful movement to increase strength, flexibility and improve your overall well being. Challenge your preconceived notions of what your abilities are and overcome barriers to allow yourself to find both inner and outer strength. This class is appropriate for practitioners with some experience. Step This challenging class offers a fun and varied cardiovascular workout that can be modified to suit the fitness level of each individual participant. Parent/Youth Workout Times Parents and youth 10 years and older are now welcome to work out together at the Bob L. Burger Recreation Center. This includes use of weight room, cardiovascular equipment and track. Work out anytime, except Monday-Thursday, 5-7:30pm, our peak time. • Child must be 10 years or older. • Parent and child must be working out together. Not just in the facility together. This means that the parent is supervising their child’s workout. • Parent and child must come in together to do a weight room orientation with a weight room supervisor, Monday-Thursday 5-6:30pm, before beginning workouts on their own. Orientation is free of charge, daily admission fee required for parent and youth. By appointment only. All appointments may be booked at the Front Desk. Whether you are just starting to exercise or are a veteran exerciser, assessing your current level of fitness will provide important information for your health. For those just starting out, a Fitness Assessment will provide a baseline measurement, which can be referred back to in order to measure progress. A Fitness assessment also examines Cardiovascular Risk Factors, which is especially important for those just starting aerobic exercise. For the veteran exerciser a Fitness Assessment will allow you to fine tune your fitness routine, and make changes where appropriate. day trips and excursions Meet new people; explore the Denver Metro area and events. We’ve even added special Cultural Arts events for those who enjoy the Denver cultural scene. Sign up early! Our trips tend to fill fast! Please note that each trip will specify if it is a van or bus trip. Trips that are van trips are senior friendly but not handicapped accessible. Bus trips are handicapped accessible. Trip Etiquette •Please arrive on time: arrive 15 minutes before scheduled departure. Please register in advance. Please note: we are unable to take drop-in sign-ups at the bus pick-up point on the day of trip departure. •Please be considerate: of those with limitations who may require the front seat or a window seat. Respect your trip escorts: they have been trained by staff to lead your trip and are volunteering their time. •Please be flexible: sometimes things just don’t go as planned, traffic jams, weather, or disappointing food. We encourage you to make the best of the situation. We plan to! Trip Departure Information Leaving on a Trip: All trips leave from the parking lot of the Senior Center at 103 S. Iowa Ave. Please plan to meet the bus or van there.

While on a Trip, please depart at the scheduled time: plan to board the bus 15 minutes before the scheduled departure time. Due to scheduling, the bus needs to leave on time. If you return more than 15 minutes late, you will miss the bus! You will then be responsible for paying for your own transportation back to Lafayette. Waitlist Information Lafayette Senior Services may partner with other cities for some of the trips. This means that seats are limited to 13 per city. After the registration deadline, if a partner trip is full, a waitlist is established. The waitlist is filled on a first come, first served basis, city by city. For trips we do on our own, when the trip is full a waitlist is established. The waitlist is filled on a first come, first serve basis. CULTURAL ARTS TICKETED EVENTS Take advantage of our special pricing for trips to some of Denver’s finest performances. For the holiday season, we’ve put together some events that you won’t want to miss. What better way to celebrate than seeing a performance of the Nutcracker or A Christmas Carol. If you’re a music lover, you can attend the musical production of Beatles hit’s or the St. Patrick’s Day concert with the Colorado Symphony. The Nutcracker

The holidays are not complete until you join us for The Nutcracker, featuring unforgettable characters, dazzling costumes, classic choreography and Tchaikovsky’s extraordinary arrangement performed live by the Colorado Ballet Orchestra. Senior Advisory Board The Senior Advisory Board meets the 2nd Wednesday of every month from 3:30-5:30 pm at the Lafayette Senior Center. The Board provides support, expertise, guidance and direction with regard to the issues that are important to seniors in the City of Lafayette. Room Rental Fees Rental of the Senior Center includes the use of the main room and a conference room. An additional fee is charged for kitchen use. Fee includes your setup and clean-up time. Adult Resources ONGOING Activities MORNING COFFEE AT THE LAFAYETTE SENIOR CENTER In the morning, we have the coffee ready and we often have something to enjoy it with! On Wednesdays, we provide samples of Panera Bread treats and a coffee bar with some extras! While you’re here check out our Drop-In programs and see what your friends are doing. Fun things are going on Monday-Friday!

The Lafayette Senior Center has a regulation size billiard table that is available on a first-come, first-served basis. We provide the cues and balls; you provide the expertise and the fun! There is no charge. No need for a reservation, just stop by.

Free bread courtesy of Panera Bread will be available every Wednesday morning at 10:30am Bread is handed out on a first-come, first-served basis. The amount per person is determined by the amount received from Panera Bread. Amount may vary from week to week. We ask that each person limit their selection to two bagged items of their choice.

If you’re looking for something to do the Lafayette Senior Center has a variety of table games, including chess and checkers available. We have puzzles, comfortable places to sit and a great atmosphere. We also have table tennis. Most items are available during business hours. Due to programs being offered in the Center, table tennis may not always be available. Check with us by calling 303-665-9052 ext.0 if you want to use these items. WiFi The Senior Center is wired. Bring your laptop and connect here. The Senior Center is a great place to relax and use your own computer. Computer lab The Lafayette Senior Center has a nice computer room for your use. We offer computers, and the ability to print. When you need help, our staff is nearby. You can also find computer classes offered in our lab in the Adult section of the Guide. There is no charge for the use of the computer lab. We do ask that you provide your own printer paper. Day

CRAFT GROUPS

Our Drop-In Program Is A Wonderful Resource For Social Activity! We have lots of fun things going on every Monday through Friday when the Senior Center is open. In addition, there are Drop-In Programs that meet regularly at Josephine Commons. All you have to do is drop by and join in! Everyone is welcome! No pre-registration is required for our Drop-In Programs regardless of the location!

KNIT One Pearl Two Instructor Mary Banecks will teach participants how to knit hats using a loom. Making a loom hat is simple and no experience is required. You will be able to make hats for the whole family so come and have some fun. Everything is provided to learn. We donated over 400 hats to the needy last year. Yarn donations are always needed! DROP-IN PROGRAM SPACE We encourage adults to develop their own drop-in activities. There are a number of organized groups in different locations within the City. If you want to start a drop-in activity group and you need a place to meet, call our Recreation Specialist at 303-665-9052 ext. 5. We’ll try to work out space for you at the Senior Center if your group is activity centered, of interest to adults and open to all who want to join. Join our group of dedicated volunteers who sew small quilts (lap size) that are donated to nursing facilities and other agencies. Can’t make this time? Sew at home! Support Services Community support services The information that follows is designed to help you with options that promote personal well-being and quality of life! Reduced Fees for Adults 55+ Please call our Resource Specialist at 303-665-9052 ext. 3 to determine if you qualify and to complete an application. The Loan Closet We offer a variety of basic medical equipment items for rent on to City of Lafayette residents for short term use. Equipment may only be rented to a consumer or family care-giver. Call us at 303-665-9052 ext. 0 for more information or to make a Loan Closet Reservation. Reserved equipment will be available for pick-up on Tuesdays and Fridays from 1:00-3:30pm at Lafayette Senior Services, 103 S. Iowa Avenue. Each item requires a $5.00 per month fee (unless very low income) for all items except Wheelchairs, Escort chairs and the Ramp which are $10 a month (unless very low income). Equipment is limited to the supply on hand and is available on a first come, first serve basis. Equipment may be rented for up to two months per item. Via is a door through door transportation service enabling the rider to schedule medical appointments, shopping, and other destinations in and around Boulder County. Parks Information Park Regulations

(see page 87 for a full listing of Park Amentities) • All parks open at 6 a.m. and close at 10 p.m. daily. • Glass containers of any kind are not permitted in any park or recreation facility. • Consumption of alcoholic beverages in any park or recreation facility is prohibited, unless the person or party acquires a special use permit. Alcohol permits are only available when renting Park Shelters. • Alcohol beverages may only be consumed by members of the group holding the shelter permit and may not be resold to the general public. • Motor vehicles are not permitted outside the designated parking area of any park or recreation facility. • Littering is prohibited in any park or recreation facility. Please use trash receptacles provided. • Dogs must be leashed at all times while in any park or recreation facility. • Horses are not permitted in parks or on trails unless otherwise designated. • Fires permitted in designated areas only where grills are provided. • Persons or parties possessing a valid permit have priority for using the sheltered picnic areas only. Permits must be available for inspection at all times. • It is unlawful for any person(s) other than City personnel to solicit, sell or offer any article, product, or service in any park without prior approval, permit or contract from the City. • Firearms are not permitted in any park or any recreational facility. • Noise Ordinance - Any and all use of sound amplifying equipment requires a Special Events and/or Sound Amplification Permit available through the City Clerk’s Office at City Hall, 1290 South Public Rd., Lafayette, Colorado. • Special approval must be received for items such as dunk tanks, hot air balloons, any kind of carnival setup, carnival games or activities, use of tents, catering services, awnings or rented shelters, etc. A written request for these must be submitted to the Recreation/ Facility Management Department at the time reservation is made and paid for. Park reservation Information

The City of Lafayette Recreation Department coordinates and schedules the use of all park areas, facilities and fields. Prior approval, permit and/or contract with corresponding fee and/or insurance coverage may be required. Anyone desiring to reserve a park, shelter or facility for parties, gatherings, games/practices, event, etc. may do so by calling or coming to the Bob L. Burger Recreation Center and acquiring the appropriate approval. Fees for such usage varies by location, size, length and purpose of the event.

1) The Recreation Department reserves the right to cancel or combine any class due to an insufficient number of participants. You will be notified prior to the first class if it has been cancelled and you will receive a full refund, credit or transfer to another class. 2) Late fees: Any youth sports registration not received by the registration deadline, will be assessed a $10 late fee. Payment will be taken (check or credit card) for youth league wait list registrations, but will not be processed until the child is placed on a team.

We strive for positive experiences and excellence in everything we do. If you must cancel from a class, you must request the cancellation seven days prior to the start of the class in order to receive a 75% refund. Day Camp deposits and Preschool/B.A.S.E. registration fees are non-refundable. • All cancellations initiated by the participant must be approved by the program supervisor, and will be assessed a 25% cancellation fee. • Class transfers are subject to a $5.00 transfer fee.

Registration Reduced Rates

The City of Lafayette provides a program for eligible participants, which will reduce fees for youth by 50% for activities, sponsored by the Recreation Department, as well as reduce the daily admission fee by 50%. This reduced rate program is available to all Lafayette Residents meeting one of the following guidelines: *Temporary Assistance for Needy Families (TANF) *Supplemental Social Security Income (S.S.I. or S.S.D.) *W.I.C. Recipients *Section 8 or Public Housing *Medicaid Recipients *Child Health Plan Plus (CHP+) *Food Stamps *Free Lunch Program Please Note: Proof will need to be provided at time of application. All applicants 18 years and older MUST provide: 1) ID (one of these): Colorado Driver’s License Colorado Identification Card U.S. Military Card Native American Tribal document AND 2) Must sign the Lawful Presence Affidavit (included inthe Reduced Rate Application) If you meet one of the above standards, you need to completely fill out the reduced rate application form. Attach a copy of the documentation for the category you qualify for and either mail it in or bring it to the Recreation Center for processing. Applications will not be processed without required documentation, copy of identification, or a signed Lawful Presence Affidavit. Please allow five business days to process. You will be notified by mail of your acceptance in this program.

Reduced Rates the City of Lafayette are not eligible for the reduced rate. These classes are identified with an asterisk (*). If you have questions regarding this please contact the Recreation Center at 303-665-0469. 5. No single child can receive more than $100 in fee reductions per calendar year. Remaining funds are not transferable to any other child. 6. Families that are approved for reduced rates for programs will be notified through the mail. This letter will explain the procedure for using this program. 7. The reduced rate money allotted to each child for the year or for the remaining portion of the year will be prorated based on the date the application is received by the Recreation and Facility Management Department. Please refer to the prorated schedule listed below: Winter/Spring - $100- Applications must be received by 4/30/14. Summer - $75 - Applications must be received by 6/30/14. Fall - $50 - Applications must be received by 9/30/14.

FACILITY USE:

The City of Lafayette offers a reduced rate plan for those Lafayette residents who apply for assistance and meet Social Services criteria (same as listed above for program fee assistance). Eligible citizens (adults and youth) will be able to use the Recreation Center for half of the daily admission fee. The applications are available at the Recreation Center. These applications may be turned in throughout the year. The reduced rate is good through the calendar year. Applicants must apply each year for this program.

Please note: Medicaid cards will no longer be accepted as proof of Medicaid. If you are a Medicaid recipient, please include documentation from Medicaid stating the eligibility dates for this program.

PROGRAM USE:

1. Once the application has been approved, the youth listed on the application will be entitled to one-half (50%) reduction in the resident fee for a selected class or program. Fees must be paid at the time of registration. Please Note: If you register and pay for a class before your reduced rate is approved, refunds or household credits will not be given. 2. This eligibility is good for one calendar year - January through December. There is no need to re-apply for reduced rates each time a brochure is published 3.
