editorial perspective Information As Good As Gold

R. David Bradberry, DMD

By the time you read this another winter will be in the books and the 2014 session of the state legislature will be over. This was a relatively quiet year with the GDA monitoring about 150 bills (out of hundreds introduced) that could have potentially impacted dentists as health care professionals, employers, and small business owners. The GDA also worked to protect the state funding the Board of Dentistry needs to operate and protect the public adequately. Thanks to careful monitoring and consistent lobbying by the GDA, the Board received even more money than originally allotted by the governor. Last year the GDA proposed and fought to get its signature bill completed and signed into law. This is the bill that moved the Georgia Board of Dentistry from under the supervision of the Secretary of State’s office to become administratively attached to the Department of Community Health alongside the medical and pharmacy boards. This will and already has provided for a better, more efficient operation of the dental board. They have an executive director and staff members who can now dedicate more time to the operation of the dental board which makes them better able to uphold their mission— protecting the public and regulating and enforcing the standards of practice. They will continue to respond in a timely way to requests, oversee licensees, and keep a sharp eye out for the non-licensed practice of dentistry. Many of you are aware (but some are not) of how your association’s advocacy team is structured to recognize bills that are introduced and evaluate them for any potential positives or negatives to our practices. Here GDA staff members help a lot by reading the bills and highlighting areas of concern before forwarding the bills to your volunteer leadership. That’s where you come in. We as GDA members need to attend LAW Days during the session and get to know our legislators outside of the session so that we

can keep them informed and let them know we are willing to listen and help. Every legislator I have spoken to personally values your input. They are more than willing to get to know all of you. After all, you are part of their electoral base, otherwise known as constituents. They really get pumped for LAW Days and want to hear your concerns. As you know (I hope), LAW Days are broken up into attendance by district. All of the districts work hard and do a good job getting people to travel to the Capitol in Atlanta, but more attendees are always a good thing. If you haven’t gone to a LAW Day, you should. Yes, the days are on Wednesday mornings, but if you want to protect your patients and the profession it is a very good use of your time. Once you get to know your legislator, then consider being their Contact Dentist. This is simply a means to designate dentists to serve as direct avenues of information for legislators when different bills come up. It’s a two-way street—you have someone you can call and they have someone they can call. Again I signal the same message as I have before. Get involved, go to a district meeting and bring a friend, stay on top of the legislative information that is sent out in Action and in GDA emails, and be ready to help out. Many hands make the work a simpler task. The legislature is adjourned for 2014 but the work does not stop. Information is as good as gold. Next year is already shaping up to be a challenging year and everyone will be needed on the line helping out. Please be one of them; you’ll be glad you did.

Kudos to the GDA dentists who are stepping up to assist the South Georgia Medical Center’s Pearlman Cancer Center (PCC) with providing dental care to cancer patients in need. The American Society for Radiation Oncology selected PCC last fall as one of two Georgia recipients of a Survivor Circle grant. PCC received $8,000 to provide direct support for cancer patients. According to a story done by WCTV in Valdosta, the PCC physicians and staff elected to use the grant to help patients undergoing treatment for head and neck cancers, as well as cancer patients in need of dental care. The decision was made to provide funding of up to $500 per patient for dental services, particularly services required in order for a patient to prepare for cancer treatment. Several dentists in the Southwestern District are a part of the PCC dental care support program. These dentists include Dr. Fernando Alvarado, Dr. Bo Broadfoot, Dr. Brad Bynum, Dr. Eric Castor, Dr. Nelson Clements, Dr. Sherry Colvin, Dr. Timothy Grantham, Dr. Matthew Carter,

Dr. Heather Colson Hardy, Dr. Brett Hester, Dr. Jason Holloway, Dr. Mili Hunt, Dr. Joseph Kirbo, Dr. Jason Lairsey, Dr. Edwin Lamb, Dr. Ashley Moorman, Dr. Bill Moorman, Dr. Ronald Reagin, Dr. John Roan, Dr. Ron Shiver, Dr. Joel Stites, Dr. Scott Summers, Dr. Shane Wood, and Dr. Barclay Woodward. WCTV noted that the first patient received care through the grant program in March with the help of dentists from Advanced Dental Care—Drs. Ashley Moorman, Barclay Woodward, and Bill Moorman.

FREE SCREENING GDA Program Several hundred GDA member dentists participate in the GDA School Entry Free Oral Evaluation Program. This is a statewide service for children entering Georgia public schools for the first time. The State of Georgia requires students to undergo basic ear, eye, nutrition, and dental examinations regardless of the grade being entered. Preregistration varies by school district and this program is valid throughout the year. A list of GDA members participating in the School Entry Free

The dentists and staff of Advanced Dental Care in Valdosta recently assisted a cancer patient in need of dental care as part of a grant program administered by the Pearlman Cancer Center. The center received $8,000 in funding to provide support for cancer patients and staff elected to focus on the dental needs of patients.

Oral Evaluation Program is distributed annually to all Georgia public school superintendents, county public health departments, Georgia newspapers, and Georgia television and radio stations. The list is also posted on the GDA web site www.gadental.org under the GDA Cares tab. Do you want to be on the GDA list and put your name in front of Georgians statewide? Email your name to Phyllis Willich at the GDA office at willich@gadental.org. FYI: The nutrition screening mentioned above became a new requirement in 2013. According to the Georgia Department of Health, the original Form 3300, Certificate of Ear, Eye and Dental Examinations, was revised to add the nutrition information and to make it clear and easy to understand for health providers, parents, and schools. Parents or guardians of children being admitted for the first time to a public school in Georgia must present a new Form 3300 to school authorities. Although parents are responsible for providing the necessary State examination

form, a supply of forms for in-dental office use can be obtained from the local county health department or board of education. Dentists may obtain the revised Form 3300 on the GDA web site under the GDA Cares tab.

A Look at the GDA’s Future with Executive Director Frank Capaldo The Georgia Dental Association welcomed Frank Capaldo as Executive Director on March 3. The director search committee and Board of Trustees all agreed that Frank’s years of leadership experience combined with the capabilities of GDA dentists and executive staff will produce positive results for the GDA. “Frank is an outstanding leader who will ensure that Georgia Dental Association dentists continue to enjoy a high caliber of membership service,” said Dr. Marshall Mann, GDA president. “We are excited about combining Frank’s proven track record for success with the capabilities and enthusiasm of our GDA members, leadership, and executive office staff.” During Frank’s leadership of the Independent Bankers Association of New York State and the National Franchisee Association, his programs and initiatives resulted in significant gains in membership, member engagement, and non-dues revenue. Frank spoke with GDA Action shortly after his first day on the job and talked about his desire to continue to grow the GDA and provide dentists with services they can only obtain through their professional association.

GDA Action: What drew you to apply for the executive director position of a dental association when your most immediate background has been providing leadership for banking and franchise associations? Frank Capaldo: I believe it is a perfect professional fit—the GDA brings out the entrepreneurial spirit I have to grow organizations. The members in the associations I have managed have all been small businesses and/or primarily ones that were family owned and/or established by families. For example, 95% of franchisees in the U.S. are small businesses. The banks I served were the 97% of small community banks owned by

10

GDA ACTION APRIL 2014

families, many in the fourth and fifth generation. The building material industry is the same—mostly family-owned lumber and supply companies. Like the small business dentist, franchisees, community bankers, and building material business owners all face the same issues, from government regulation to changing business models and practices. One of our jobs at the GDA is to develop and provide small business solutions for dentists using the clout and cost-benefit of a united association.

GDA Action: As part of the interview process, you talked at length with the GDA leadership and studied the association’s history and current operations. What would you describe as the Association’s strengths? Frank Capaldo: From what I’ve observed and have reviewed so far, the GDA has a strong and involved volunteer-led Board of Trustees and House of Delegates, and many committed members. Moreover, we have an experienced executive office staff committed to service and the mission of the Association. I am continuing to learn by asking questions, listening, and assessing the strengths of the Association and seeing where our challenges lie. Your Board of Trustees has in hand a process plan I call the “The First 90-120 on the Job.” This document talks about the need for me to listen to all the leadership parties, from committee chairs to district officers, and establish communication channels. The GDA staff and I need to talk about existing initiatives and resources. At the end of this process, your Board will receive a report that discusses key recommendations. While I have ideas and strategies that have worked in the past, we need to know what will work at the GDA. We must have leadership and staff involvement and approval so there is a clear shared vision going forward.

New Executive Director Frank Capaldo’s years of leadership experience combined with the capabilities of GDA dentists and executive staff will produce positive results for the Association.

GDA Action: What is your philosophy about how a professional association should operate and how it should reach out to and serve its members? Frank Capaldo: Service, Service, Service, and member driven! Different generations and age groups have different priorities. To serve them all, an association must consistently deliver value on different levels that is relevant to diverse needs. For

GDAction 0414.qxd

4/14/14

4:23 PM

Page 11

example, members of all age groups may wish to have their profession and livelihood protected from legislative and regulatory intrusion, such as dilution of the role of the dentist. However, while a recent graduate may be consumed with concern over their student loans, an established practitioner is probably more worried about finding ways to lower the cost of running the practice. Our job is to have a solution that helps each group and one that they can only obtain through GDA membership.

GDA Action: You successfully undertook the task of increasing membership in each of the last three associations you have managed. Leaders within organized dentistry have noted that while overall membership numbers are growing, market share is decreasing. What are some keys you can share about

motivating professionals to see the value of membership? Frank Capaldo: While I have ideas and strategies, we have to wait and see what our leadership and staff assessment process brings over the next 120 days. My experience tells me that there are a number of factors that contribute to slowing membership growth. They include among others, a lack of communication or ineffective communication vehicles, not consistently appealing to different ages and types of professionals with services that fit their unique needs, and a lack of leadership engagement. Individuals may perceive that an association is not relevant, or that services are of poor quality, overpriced, or can be obtained elsewhere. An individual may have multiple personal or community commitments they believe prevent them from joining. The “keys, as you refer to them, are different depending on the age and stage of practice for a dentist. I have been

successful in finding out what different groups of individuals need versus what they want, and then creating and re-inventing value propositions to attract and retain membership. Some values cross all lines. However, one group may believe that a certain value attributable to the association is a given, while others may believe you have to earn it. For example, when I entered the practice of law and joined the BAR association, I believed that government relations was a given and would happen whether I paid dues or not. As my involvement representing associations grew over the years, I realized I must be involved and protect not just my interests, but those of my profession. Truly, there is no free ride in life! In addition to the specific strategies that the GDA must develop to motivate current members to renew membership

GDA Action: There are dentists who view membership as unnecessary because their practices benefit from the efforts of those dentists who do join. For instance, members and non-members benefit from state laws passed largely due to GDA advocacy efforts. What would you say to individuals who have resisted repeated calls to join?

CAPALDO Continued from page 11

and attract potential members, I do believe there is one universal service we must offer, besides value. Staff, Association leadership, and I must constantly engage and communicate with members and potential members, and I mean engage and communicate in a personal touch member driven way. Email and all of the electronic communication channels we have are super and I utilize all of them. But I have found there is nothing better than the personal call and face-to-face meeting. I will do (and I am doing) a lot of that!

Frank Capaldo: I do have a lot of experience in this area as I discussed above. Membership is a two-way street. In order to obtain value and benefit from your association, you must explore what the association offers. Just like with a new car or perhaps a new dental chair, you have to test drive it! You must not only experience involvement in the process of government both in the legislative and regulatory processes that impact you, but also in the governance of the association. Moreover, if you want more from your association you have to be part of it so you can improve it and make it better. My grandfather said he did not like committees, but if the committee was making decisions about his life and business, he certainly wanted to have a say! I would tell potential members that their profession is only as strong as its weakest link, which is the non-member. Together we have power and can accomplish anything.

GDA Action: The GDA’s most valued membership service has for several years been legislative advocacy. You began your association career participating in advocacy activities. Can you share your philosophy on the value of advocacy and your plans for how the Association will continue advocacy activities? Frank Capaldo: Advocacy has always been a keynote of associations I have managed, primarily because of member focus on legislative and regulatory activities. My philosophy and practice is that Government Relations is a primary, necessary priority for the GDA and will remain so as long as the membership sees threats to, and opportunities for, their

profession. However, advocacy means more to me for the GDA than having a strong presence in the legislature. We must advocate for dentistry in the media and with the public. If one leg is missing, the advocacy platform of the GDA is off balance. My philosophy on the importance of advocacy is not only that it is of primary value to the GDA, but invaluable to the GDA when the other two legs of media and public/consumer advocacy are added. I have many ideas and experiences to bring to the table in this area and plan to share a number of recommendations and with member involvement develop a plan for approval by the GDA leadership.

GDA Action: You have a number of great ideas! Now tell us a little about you personally. Frank Capaldo: I have five children— four daughters, one son—and two granddaughters. My wife Lisa is a CFO and together we love to golf, fly fish, and ski. I enjoy chess and cycling to sustain mind and body. Lisa loves rescuing dogs and cats and we currently have three of each kind. My mom has lived in Georgia for more than 20 years. She retired as an executive from INA / CIGNA after 35 years. Lisa and I spent over a decade living in Atlanta when I worked with the franchisee association. Our two youngest children call Georgia home, as do Lisa, my mother, and I. I earned bachelor’s degrees in Business and Sociology from Susquehanna University, a Masters in Statistical Analysis from Rutgers University, and my legal degree from the University Of Dayton School Of Law. I enjoy volunteering and I have held positions with numerous not-for-profit and corporation boards. I truly look forward to continuing my volunteer efforts especially with Special Olympics and the Atlanta Mission.

GDA Annual Meeting Clinical Program Preview

How to Deliver Comprehensive Dental Care Despite the Challenges Facing the Profession The GDA Annual Meeting this July in Amelia Island, Florida, will feature a wide range of continuing education. There will be clinical courses on Saturday on topics ranging from full-mouth rehabilitation with implants in a fixed hybrid immediate bridge to soft tissue grafting to cone beam computed tomography in endodontics and more. Thursday, Dr. Gary Glassman will bring attendees a hands-on event on improving endodontic technique. Dentists and staff members will have a chance to laugh and learn during Friday’s course on improving communications with Jody Urquhart. There will be plenty of valuable information to take away. Attendees will also have a unique chance on Friday to hear from two dentists who can help them thrive in a challenging dental practice environment. Not bad for a trip to the beach. The GDA is pleased to host “The FACE of Comprehensive Clinical Dental Care in 2024: Is There Room to SMILE?” conducted by Roger Kiesling, DDS, and David Preble, DDS. Dr. Kiesling is a Helena, Montana, based general dentist and the current Eleventh District Trustee of the American Dental Association. Dr. Preble is Vice President of the Practice Institute of the American Dental Association. The institute provides input on programs, products, and services that help ADA members better operate their practices and promote the interests of the dental profession on health care finance, health outcomes, and quality, and informatics and standards. “Knowledge is power,” said GDA Meeting Clinical Chairman Dr. Thomas McDonald. “Member dentists who are aware of the potential for marketplace, governmental, and industry changes can better position their practices for the future. Dentistry has never been a profession to sit back and allow others to dictate our practice model

14

GDA ACTION APRIL 2014

or interfere with our unique patient relationship. This is a must-see program for GDA members who do not want to get caught with their ‘heads in the sand.’ ” The course will discuss the challenges facing dentists who want to deliver optimal comprehensive clinical dental care. There is a perception by some patients that such care is unattainable. Dental benefits are disappearing and many patients believe without insurance they cannot obtain care. Prevention of dental disease has reduced the need for complex care in younger adults. The list goes on. But this course is positioned to help dentists understand the challenges, identify the need for care that does exist, and make such care desired by dental patients. “I scheduled this course because as I have listened to our dental leaders and observed the national political process over the past five years, it is apparent that the profession of dentistry will be changing,” said Dr. McDonald. “I feel that our members need to get the most accurate and current information. Also, our younger members need to be reminded of what a great profession we have and that the challenges in our future can be best faced together. The collegial nature of the dental association will facilitate the best approach to protect our doctor-patient relationship. Our predecessors fought for these ideals in the 1970s while the medical profession handed itself over to the insurance industry and we cannot give them up now.” The ADA Practice Institute was created to help dentists navigate this fast-changing health care environment. Dr. Preble is sure to discuss a recent first-of-its-kind analysis by the ADA’s Health Policy Resource Center that identified now as the time for the profession to define its destiny. Dental spending per capita has flattened. Utilization of dental services is down for adults. Patients are becoming health care consumers and actively shopping

for low priced plans. There is also a new emphasis on “value”— a new emphasis to measure and report quality of care and to balance quality and cost. “Clinical education helps improve clinical skills and practice management education helps improve business skills,” said Dr. Kiesling. “Bridging the gap between the two is difficult. Family Meetings: Leaving Your Loved Ones More Than Just Money Imagine an organization worth tens of millions of dollars where the co-CEOs refuse to share financial information and guidance with the employees who would eventually take over management. Most would not expect the business to continue its success once the CEOs step down as inevitably they must. Should we expect anything different if that organization is a family? Succession planning (for a business or a family) increases the opportunities for multigenerational success. So often in the world of finance we focus on the earning, investment, saving, and spending of money. UBS clients are now expressing a far greater interest in learning more about expanding the succession planning process to include how best to prepare heirs and

pass down values. Their definitions of “success” and “inheritance” are expanding to include individual happiness and family cohesiveness.

Why Have Family Meetings? A recent study revealed that high taxes and poor investment advice were not the reasons for intergenerational dissipation of wealth. The study found that 60 percent of the time it was a trust and communication breakdown among family members that plays the biggest role.1 Some families have achieved successful succession planning with an informal, unstructured process. For most, however, a more formal process is preferable (or even

necessary). Family meetings can provide a safe, structured context for discussing these issues with descendents. “Family meeting” is a commonly used phrase that defies formal definition. “Family” may include one or more generations and may or may not include in-laws and domestic partners. “Meeting” can be in person or through an electronic gathering. Although definition is difficult, the phrase “family meeting” can be best described by the expectations and goals of each family member. Therefore, to succeed, disparate expectations and goals must be understood, unified, and addressed.

Page 24

FAMILY MEETINGS Continued from page 23 The underlying purpose of family meetings is communication. Reasons and goals for the communication include:

money. Though peacefully resolved, unrelated issues can sidetrack a meeting and cause discomfort. Experience shows that, just as there are some frequently cited reasons for having family meetings, there are some common (mostly unspoken) reasons to avoid one.

1) The head(s) of the family meet to develop and formalize the values and goals they wish to convey.

• “I don’t want my children to know about my financial wealth. It will ruin them.”

2) A large family meeting is called to communicate and refine those values and goals. The family then develops a governance model incorporating those values and goals to organize and manage ongoing meetings and future decision-making.

• “I’ve sacrificed and worked hard for my wealth and I want to enjoy it. I don’t want to want to be judged and told by my children to ‘practice what I preach.’”

3) Ongoing meetings (possibly including spouses and young family members) are held to educate and put in action the values and goals.

• Disseminating information and making shared decisions

• “I have trouble balancing my desire for discretion and privacy with the need to teach my heirs about money.”

• Modeling family leadership behaviors to younger family members

• “I don’t want to raise issues between my children and my new spouse.”

This general structure greatly increases the chance of developing financially savvy heirs with a ready source of information and community. It helps to build strong family ties and create a sense of support and system of accountability. All families govern themselves and reach decisions; the key difference for families whose wealth lasts for generations is that they do it by design. Regular, well-planned, and well-run family meetings incorporating clear values, historical connectivity, and non-family advisors and educators help open multi-generational communication channels. They also encourage familial and financial accountability among successors. At its essence, the family meeting is a learning opportunity for every generation attending. Participants learn not simply by listening, but also by watching the activities and behaviors of family leadership through organizing, running, and appreciating the benefits of the family meeting.

• Transmitting purpose and values • Providing education regarding financial matters • Strengthening family bonds • Establishing accountability for actions of family members • Sharing news, concerns, opportunities, and challenges

• “It’s too much work.” As with all new endeavors, there can be a fear of the unknown and the issues that may arise. It is important for families to determine what goals they wish to achieve and keep the focus there.

Why Not Have Family Meetings? In one initial family meeting, Mother, Father, and advisors gathered to discuss various topics and develop a plan for future, broader meetings. The meeting began well but a tension soon grew between the two spouses. The totally unexpected issue that arose dealt with values pertaining to spending / saving money. Specifically, for years, Mother wished to remodel the kitchen in their home and Father refused to spend the

Some are unable to get past these obstacles and for them, family meetings are not a genuine option. Those that choose to proceed understand that the possible benefits of such meetings justify the imagined risks.

The Family Meeting Process Once the determination has been made that the positive potential from a family meeting outweighs the negative, meeting planning can proceed. There is no “typical” process for family meetings. Each family’s meetings are as unique and complex as the families themselves. Experience shows, however, that the greatest success is achieved by following this general format:

1) Vic Preisser and Roy Williams, Preparing Heirs: Five Steps to a Successful Transition of Family Wealth and Values (2003).
