Our skin is talking to us. As aestheticians, we have experience translating what our skin says, but our clients cannot decipher it and consequently misinterpret their skin needs.

Professional guidance helps client understand their skin, identify their triggers, and be mindful of critical ingredients to help achieve healthy glowing skin. After all, our skin is the billboard of what is happening inside. With over two decades as an aesthetician, with my personal skin struggles and experience developing a skincare brand, I felt it was my responsibility to inaugurate a program with the principles to aid clients in listening to their skin because they spend most of their time with it.

This approach is holistic, scientific, and involves collaboration with experts. In three steps, it covers the key elements of maintaining healthy skin:

• Ingredients

• Mindset

1. Triggers and symptoms

Climate change introduced by global warming is no secret. While the world’s scientists concentrate on reducing carbon footprint, the impact of climate change on skin health must be borne by us. With the change in environment, external triggers have intensified. The ever-increasing global population also demands accelerated food production, often using growth hormones and raising internal triggers. Internal and external triggers constantly change, so no one regimen fits all. Triggers set off or provoke a skin reaction, ultimately leading to skin concerns.

Now more than ever, there is a greater need to educate our clients on identifying possible triggers. Clients must be able to uncover the source of their triggers by keeping a log of their environment, diet, and activities to pinpoint their triggers. Hyperpigmentation, which is a common symptom triggered by external factors, is becoming increasingly prominent and difficult to treat as the climate becomes more aggressive. Internal triggers are also on a rise, such as diabetes, poor diet, poor exercise, stress, and hormonal imbalance. These internal triggers present external symptoms such as acne, hyperpigmentation, and skin inflammation. If left untreated and unprotected topically, the appearance will intensify when exposed to the external environ-

ment and lead to further damage to the skin. Clients must understand their internal triggers.

2. Ingredients

Increasing evidence suggests a shift towards natural products and collagen preservation for long-term skin health. Look for formulations with ingredients that support each other to attain enhanced results. For example, when using a collagen precursor, use ingredients that strengthen and support collagen production. Promote tailor-made skin regimens according to your client's triggers, environmental exposure, and internal factors. I have been practicing “skin cycling” for 20 years before it was termed and found much success with my clients. It has proven to be practical and mindful of our skin barrier. Regimen changes should be encouraged seasonally to match the environmental changes. We diligently follow wardrobe changes seasonally but keep the same skin program annually.

3. Mindset

Ensure your treatment space provides a supportive and safe place to discuss skin problems. A skincare routine provides stability in one’s day, and research suggests that it benefits mental health. There are numerous ways to practice self-care, but focus -

ing on improving skin health pays off for your emotional health. Set realistic expectations and think long-term. Ageing cannot be stopped. Ageing signs are influenced by gravity, facial movements, sleep, daily routine, and genetics, but we can help slow down signs.

Let us impart the knowledge to our clients so they can learn how to listen and treat their skin outside our aesthetic room. Set goals for improved skin quality, develop a treatment plan, and remind them that skincare is an investment.

After all, we must wear our skin for the rest of our lives, so let's wear it well.

The aesthetics industry continues to evolve with new treatment devices and ingredients introduced regularly; however, chemical peel innovation has stalled. Traditional chemical peels utilize high percentages of caustic acids at a low pH to exfoliate and initiate the wound healing response. While effective, the inflammatory approach can lead to undesired side effects, social downtime, and premature cellular senescence. Next-generation chemical peels leverage delivery systems to effectively carry acids into the skin without irritation, discomfort, risks, or downtime.

A Vicious Cycle of Inflammation and Damage

The aging process and visible damage accompanying the major skin concerns we treat–acne, discoloration, and impaired barrier–are all linked to cellular senescence. Senescent cells are cells that no longer function properly but have not been recycled or destroyed by our immune system. Not alive but not dead either, they are often referred to as zombie cells. The connection between senescence and inflammation lies in senescence-associated secretory

phenotype (SASP). SASP promotes inflammation and leads to the production of more senescent cells. To put it simply, inflammation leads to senescence, and senescence leads to SASP, which leads to more inflammation and senescence–a vicious cycle. Unfortunately, the latest science and research on aging indicates the inflammation associated with high percentages and low pH of acids in traditional chemical peels leads to more senescent cells.

Finding the Sweet Spot

Alpha hydroxy acids (AHA), such as mandelic, lactic, and glycolic acids, provide gentle exfoliation. Recent data suggests that when used in lower concentrations with a pH between 3-3.5, AHAs may reduce senescent cells and provide anti-inflammatory effects. AHA, however, can’t naturally penetrate the skin’s lipid bilayer, so the depth of penetration has long been dictated by percentage, pH, and time on tissue. Newer delivery systems have changed this way of formulating and administering peels. Transcutol is an example of an advanced delivery system that effectively carries these acids into the skin without irritation, discomfort, risks, and downtime. The addition of ingredients like tranexamic acid (TXA), glycerin, and

hexylresorcinol effectively targets specific concerns such as discoloration, acne, and impaired barrier.

Combining clinically proven acids and targeted ingredients with advanced delivery systems allows for optimal outcomes without the traditional hassles. While inflammation once was our only option for creating true change in the skin, science now offers ways to work with the skin rather than against it. Noninflammatory, nodowntime chemical peels provide a safe alternative to treat more clients more often for unparalleled results and profitability for you and your practice.

Breaking the Cycle

Removing the inflammatory component of chemical peeling allows clinicians to safely treat more clients more frequently than before. It is also pos -

sible to combine these peels with other modalities in new ways, including physical exfoliation, light therapy, microcurrent, nano-needling, and facials–use of steam, massage, and extractions–and more. The long list of things clients can’t do post-peel is also eliminated. For example, working out immediately post-treatment is not a problem. By using next-generation science, these peels offer a solution for all skin types, concerns, Fitzpatricks, and lifestyles, ensuring faster results, improved experiences, and increased client retention.

Dr. Jennifer Linder is a board-certified dermatologist and founder and Chief Medical Officer of Linder Health, a professional skin health company. She is also a fellowship-trained skin cancer surgeon using the Mohs micrographic technique. Previously, Dr. Linder owned PCA SKIN, where she served as Chief Scientific Officer, guiding all product development and clinical trials for the company.

A graduate of Washington University Medical School, Dr. Linder completed her internship at the University of Pennsylvania, followed by a dermatology residency at Thomas Jefferson University. Our first year esty, Sofia Montero Rorrer, shares how she’s sharpening her skillset.

Hi Sofia!

Sofia, what are you working on currently?

Besides focusing on my schoolwork, I’ve been learning mostly about acne since that is what I want to specialize in.

What is the most valuable lesson you've learned so far?

The most valuable lesson I’ve learned so far is how to communicate with clients. For example, how to properly conduct a skin consultation and conversation to make them feel more comfortable. My top priority is ensuring my clients feel relaxed during treatment and that I’m focusing on what they want done - within reason, of course!

Have any of your techniques been improved through practice?

Yes, some of my techniques have been improved through practice!

Those techniques include extractions during facials and makeup application. I’ve worked on practicing my extractions with my fingers instead of using tools because some people prefer not having tools used. The more I have practiced makeup, I’ve gotten to create a better base and expand my eyeshadow looks.

What product is your "skin savior"if you could only choose one?

My skin savior, my holy grail through and through, is the Mandelic Acid from The Ordinary. I have extremely sensitive skin and rosacea, so acid treatments usually cause major irritation for me. However, Mandelic acid is light enough that it still removes my impurities while keeping my skin healthy and glowing.

THE MOST VALUABLE LESSON I’VE LEARNED SO FAR IS HOW TO COMMUNICATE WITH CLIENTS

What advice would you give to those considering attending an esthetics school or program?

I think my biggest piece of advice is to make sure that the program offers a large amount of practical hours so you become more comfortable working. I also feel it is the best way to truly understand what you learn in your lectures.

Also, make sure the price is reasonable and within your budget. I’m a firm believer in not taking out loans unless it is absolutely necessary.

What is your business called and where are you located?

Bella Reina Spa in the All American City - Delray Beach, Florida.

Can you give a little information about your background as an esthetician?

I've been an esthetician for almost 30 years, also known as a Skincare Coach, and was highlighted in The New York Times. I look at the root cause of the skin issue and focus on inside to outside. I absolutely adore empowering women to glow, as it gives them confidence and strength in this often chaotic world.

The real back story is, I was securitizing mortgage backed securities and wanted something that was calming and relaxing the – the complete opposite. At the time, I had terrible midthirties acneic skin and didn't know why. So I went to school and I fell in love with making the skin glow.

What beauty philosophy do you stand by?

True glowing skin comes from within. It’s not just about what we see on the outside, it is also about nourishing the skin and the body from the inside. One of my founding beliefs is that you can combine the power of nature with cutting-edge science to create skincare that supports the body’s ability to repair down to the cellular level.

As estheticians, I feel that we have a responsibility to include hydration, stress management, lifestyle habits, and nutrition as part of our total approach to each client's inner and outer glow. Continuing education is key to grasping a total body approach.

What is your favorite treatment?

I adore our Bella Royale Facial Treatment. In 90 minutes, we can have a client looking truly amazing. It includes a cool peel, hyaluronic microchanneling, collagen threads, ultrasound, peptides, a Blue Day/ Night Mask, and an amazing twopiece Peptide Mask with Super Hero Ingredients.

What brands do you use and why?  Skin Diva Labs, EpiStep, Dr. Grandel Skincare, Environ Skincare, and SkinScript RX.

Each brand has a special place in our skin care "beauty arsenal" for being able to give clients the best.

What is your secret to beautiful skin?

By far the use of differing modalities: microneedling, microchanneling, microcurrent, ultrasound, sonophoresis, etc., combined with the most powerful cellular changing quality ingredients. And of course beauty from within.

What are your favorite tools to use in the treatment room and why?

Absolutely in love with the Epi-Step

(we have two). This powerhouse machine looks like it would do nothing, but it channels hyaluronic into the skin (without penetrating) and plumps up the skin. We then add ultrasound to push it in further, but it's a workhorse for glowing beauty. We also love our multi-use tools, one that does microcurrent and ultrasound, HydraFacial and Oxygen echo.

What is your favorite part about working for your company?

Giving women hope and confidence for those who thought their skin could never change. Plus, how wonderful is it to go to "work" every day, when it's to help others glow from within.
