OF OUR ONGOING CAMPAIGN DEDICATED TO MAKING STRIDES AGAINST BREAST CANCER, I BEGAN TO REFLECT ON WHAT IT ALL MEANS TO ME AS I FINISH MY 4TH YEAR AS A SURVIVOR. Once breast cancer attacks your body, even after you have been declared cancer-free, awareness and vigilance never go away, nor should they. Sometimes I think it’s just hiding out and waiting to attack me again when I least expect it. Last year, my regularly scheduled checkups showed what my doctors called an “anomaly.” Not a recurrence, but a possible precursor. Fortunately, it was a false alarm. A great deal of emotion and uncertainty went into that episode. Looking at it in retrospect, was the “false alarm” worth it? Darn right it was! The early warning anomaly could have just as easily been life-saving. I am grateful for all of the advances in detection, even if I have to go through a scare. Screenings and, when necessary, treatment to defeat this killer have improved tremendously over the past 50 years. Women who were victimized and died then are being saved today and are thus able to live fulfilling lives. However, we are still not

I just did some research and while I am far from being a statistician, it looks like the death rate from new cases in the U.S. has decreased by about 30% since 1992 and 89% of newly diagnosed cases now survive 5 years. All of this is certainly progress. However, there were 266,000 new cases diagnosed in 2018. That’s 15% of all new cancer cases. What can be done to lower this? Lifestyle

Something to think about... Have you ever considered buying an investment property? Many homeowners are sitting on a record amount of equity– more than double the level in 2011. If you are part of that group you may want to consider buying a second home or an investment property outside of California, where prices are lower. You will still earn the same amount of equity on your principal residence if you refinance and cash out, plus you’ll build equity on a second home. A long term or vacation rental or an Airbnb are all good options. Either way, someone else will be paying your mortgage while you’re gaining equity and possibly making a monthly profit.

Q. WHAT ADVICE OR REASSURANCE DO YOU GIVE A WOMAN WHO RECEIVES A BREAST CANCER DIAGNOSIS? A. It is very important for women to know that breast cancer is a very common diagnosis, and most women survive their breast cancer. But the No. 1 cause of death in breast-cancer survivors is actually heart disease, not recurrence. Therefore, we need to take care of the cancer, but we also need to take care of the patient. That means maintaining a healthy lifestyle with good nutrition and exercise, and managing risks like diabetes, high blood pressure and cholesterol.

Q. HOW IS TREATMENT FOR BREAST CANCER CHANGING? A. We used to define cancers according to the part of the body where they originated, but as we gained a better understanding of cancer biology, it became clear that no cancer is just one disease. Our only tools were surgery, chemotherapy and radiation, but today we try to discover and target the mechanisms driving the origin and development of a cancer. In breast cancer, this leads us to three general categories: hormone receptor-positive cancers, HER2/neu-positive cancers and triple-negative cancers. Understanding the drivers of each disease gives us the opportunity to develop personalized therapies targeting those mechanisms. Researchers around the world are working hard to develop new strategies to fight breast cancer through immunotherapy, the discovery of new gene targets, the development of “smart bombs” that attack cancer cells and minimize treatment side effects, and other technologies. UCLA physician-researchers and others are currently exploring several frontiers. One research area that looks promising is the identification of androgen receptors, which have been found on all types of breast cancers—even a sizable number of triple-negative cancers, which have been difficult to target. Studies are now underway to determine the potential clinical benefit of targeting these androgen hormone receptors, and there have been some promising results. Beyond this basic level of personalization, we sometimes do genetic profiling of a tumor

Q. IN THE PAST YEAR, THERE HAS BEEN DISCUSSION ABOUT OVERTESTING AND OVER-TREATMENT OF EARLY-STAGE BREAST CANCER. HOW ARE TREATMENT DECISIONS MADE? A. National testing and treatment guidelines are formulated by analyzing statistics on lives saved, costs and other factors like negative impact from tests or treatment, with the goal of ensuring that patients receive the care they need but are not receiving unnecessary tests or treatments with no proven value. For example, there is no proven role for what we call tumor marker blood tests or whole body scans for follow-up evaluation of early stage breast cancer. As a woman and a doctor, I understand a patient’s desire that we provide all appropriate treatment and testing, and we discuss options together along with the pros and cons of such interventions. Keep in mind that any test can produce false-positive and false-negative results. Tumor marker tests, which are appropriate in some cases, are chemical tests done on a blood sample, and we’re realizing that medications and even supplements can influence the results. For example, recent reports showed that biotin, a common supplement that many women take, can affect some tumor marker test results. This doesn’t mean it changes the cancer, but it can change the test results. Therefore, whenever a test comes back abnormal, we need to take a breath and recheck it. We do not jump to conclusions just because one test is a little bit off, and we only make treatment decisions if we do several tests confirming that there is a real problem.

WHEN IT COMES TO BREAST RECONSTRUCTION AFTER SURGERY FROM BREAST CANCER, THE BIGGEST MYTH IS THAT THERE ARE FEW OPTIONS AVAILABLE. “What I want to make clear is that there are many options for women,” says Dr. James Watson, a UCLA academic plastic surgeon and a clinician with his own cosmetic clinic who has 23 years of clinical experience in cosmetic surgery. “I usually give women eight to 10 options—not all work for everyone, but we can find three or four options for every patient who comes in who is diagnosed with breast cancer,” he says. The majority of breast cancer today can be treated with a lumpectomy and radiation, “and we don’t recommend any reconstruction surgery. The majority of the time they do fine and don’t need any reconstruction.” However, a few patients end up with breast asymmetry, “in other words, the breast that had cancer ends up being smaller than the other side. So the good news is there are several things that can be done.” After a patient like this waits about two years, “we can either do a lift for the opposite breast; we can do a reduction on the opposite breast; or in some cases, we can do fat grafting to that breast that has the lumpectomy to take care of the asymmetry,” explains Dr. Watson, further adding that as many as 80% of patients today who are diagnosed with breast cancer can get by with a lumpectomy and don’t need a mastectomy. The fat grafting technique involves liposuction with a syringe from other parts of the body. “We wash the fat and inject the fat into the breast that had the lumpectomy to take care of a deformity from that,” says Dr. Watson, who has been the recipient of over $1.25 million in grant funding for research in liposuction, fat grafting, stem cells and tissue engineering.

“We use saline-filled implants with people who are concerned about the risk of silicone implants,” Dr. Watson explains. “A silicone gel implant is as soft as normal breast tissue, but over time, usually around 10 years, that implant leaks and the gel can get into your surrounding tissue.” With a cohesive gel implant, “it’s made with silicone; but the gel sticks to itself so if the shell ruptures, it doesn’t leak out,” says Dr. Watson, noting that this option is also known as the gummy bear implant. A “tissue expander” is another option that involves the use of a balloon-like device with a valve that’s inserted. “We can inflate it through the valve and fill it with salt water— saline,” Dr. Watson explains. “Most commonly, we do this for somebody who loses some of the skin of the breast with the mastectomy. We’d have to stretch things out to compensate for missing skin.” He notes that the most common skin removed is the nipple, “because it’s not always possible to save it. The most common patients who lose the nipple include those with very large breasts— double D—or those with a lot of drooping.” With a tissue expander, after the skin is stretched over approximately six weeks, “we remove the expander and then we put in the implant of their choice,” says Dr. Watson, further noting that another option involves the expander balloon followed by a flap. Those who elect this option can have their fat removed from areas of the body, including the back, buttocks, love handles and inner thigh. “All require a stay in the hospital,” he says. We need to decide to thrive, and not just survive. This lifetime is not a trial run, yet so many of us eat, drink and do things that we know are

This year I have some friends who are battling cancer and I’m sad to say, we’ve lost some dear friends. My heart goes out to their families. As a volunteer through my radio show with Making Strides against Breast Cancer and Susan G. Komen, as well as several other foundations, I meet lots of families that are struggling with cancer in some way and are trying to do the best they can with as much knowledge as they can take in. There are many battles we must go through in life, and this one is an extremely hard one to experience. I have friends with breast cancer and throat cancer that I've talked with and know they are fighting for their life. I'm sure you also know someone who has experienced a battle with cancer. Every year I participate and support these foundations in their continued educational efforts and new research, ensuring they can help one more person. Thank you for allowing me to share my stories and help support this fight. You can all make a difference, and I thank you for your efforts. 66
