LETTER TO THE COMMUNITY

Dear Community Members, Happy Holidays to you and your loved ones!

As this year comes to an end, we must first pay tribute to Sister Julie Hyer, OP, who passed away this summer. As the tenth and final Adrian Dominican Sister to lead Dominican Hospital, Sister Julie (June 23, 1948 – July 27, 2022) was president from 1985 to 2007. The hospital flourished under her skilled leadership in so many ways, from expanded medical services to the integration of new equipment and technologies, elevating our community hospital to new heights of excellence. Our goal is that our ongoing partnership with you, and our community, will continue to fulfill the legacy of Sister Julie, and all of the Adrian Dominican Sisters, to continue to offer the highest level of patient care and services here at Dominican Hospital.

Over the last few months, we have been happy to see a continued reduction in negative impacts of COVID-19 and its overall effect on our community. In the past months, we’ve seen many able to safely travel and see extended family. Students have safely returned to school and extracurricular activities, and, here at the Dominican Hospital Foundation, we’ve begun to safely gather, in-person, with our community of supporters.

In early Fall, former Chief Philanthropy Officer, Drew A. Gagner, MBA, became the Northern California Division Vice President of Philanthropy, overseeing eight Foundations located in our region. I’m delighted to announce that, stepping into the role of Chief Philanthropy Officer, is Anthony Pagliaro, MBA, who has been with Dominican for eight years – six with the Foundation. Join us in working with him to continue to further our mission to support the hospital in providing excellent medical support to our community.

In this issue of FOCUS on Wellness you’ll find the full PEP Program schedule, located at the new Wellness Center in Live Oak. You’ll also see some photos of events held at the new Center, and if you have not had the chance to visit, it’s a must see. I hope you enjoy this issue of FOW as you read about the new CT Precision Imaging Campaign, grateful patient stories, honor our Partners for Patient Care, and take advantage of the educational and wellness opportunities like the Therapy Corner

With Gratitude and Appreciation

Sister Julie Hyer, OP, led the hospital for 22 years with great foresight and purpose while always viewing the work of all involved in the hospital as a ministry. As the tenth and final Adrian Dominican Sister to lead Dominican Hospital, Sister Julie was President of Dominican Hospital from 1985 to 2007. The hospital flourished under her skilled leadership in countless ways, including expanding numerous medical services that brought the medical center to new heights of excellence.

During Sister Julie’s tenure and oversight, Dominican opened the level III neonatal intensive care unit (NICU), opened its first cardiac catheterization laboratory in 1988, welcomed the openheart surgery program in 1990, and brought the first MRI to the community. Sister Julie oversaw the purchase and transformation of the Dominican Rehabilitation Services and developed innovative programs there, including the Acute Rehabilitation Unit (ARU) and pediatric outpatient clinic.

Sister Julie Hyer, OP, directed significant growth and development in vital patient care services at the hospital for two decades. Dominican’s imaging services will be named in Sister Julie’s honor to memorialize her distinguished service. The hospital’s comprehensive imaging services encompass breast imaging, ultrasounds, MRI (magnetic resource imaging), PET (positron emission tomography), x-ray and fluoroscopy, nuclear medicine, and interventional radiology. And, soon, outstanding leading-edge CT scanners, learn more on pg. 8.

She also spearheaded the building of Dominican Oaks and expanded the hospital’s emergency department to its current location. Over the years, Sister Julie also guided Dominican Hospital in joining Catholic Healthcare West, the predecessor to Dignity Health and now CommonSpirit Health.

Following her retirement from Dominican Hospital 15 years ago, Sister Julie Hyer, OP, remained active on various boards and committees with the Diocese of Monterey. Following her years of service at Dominican Hospital, she served on the Dignity Health Board of Directors and several Dignity Health system committees and was a leader of Salud Para La Gente in Watsonville.

INNOVATIVE TECHNOLOGY

CT Precision Imaging Campaign

Dominican Hospital Foundation is excited to enhance patient care with the launch of our CT Precision Imaging Campaign to acquire two new CT (computerized tomography) scanners for Dominican Hospital. This fundraising campaign will bring the most advanced CT scanner technology to Dominican Hospital and honor Sister Julie Hyer, OP, the hospital’s past president.

The CT Precision Imaging Campaign will help fund two Siemens Somatom Definition Edge 128-slice computerized tomography (CT) scanners. Two new machines will support patient care daily through multi-purpose utilization across the hospital departments.

“As the most heavily used imaging technology in community hospital settings, and the case here at Dominican, CT scans are a fundamental tool in a patient's care plan,” explains Dr. Nanette Mickiewicz, MD.

“As the need increases within our services, we request the support of our generous benefactors and donor community.”

8 | FOCUS ON WELLNESS
–Nanette Mickiewicz, MD, President/CEO, Dominican Hospital
“ ”
State-of-the-art CT scanners will aid our medical staff in accelerating clinical decisions while further improving patient quality of care. Using the latest technology in simultaneous imaging of blood, bone, and soft tissue will enhance our capabilities in diagnosing trauma and pain, as well as early detections of cancer and stroke.

Dominican Hospital performs approximately 64 CT scans per day for both in- and out-patient needs, across departments including emergency, cardiology, oncology and other critical care areas. Patients who come to Dominican with serious, complex, or rare health challenges benefit from advanced technology and exceptional care.

“Sister Julie was devoted to addressing the changing health care needs of the community,” explains Dr. Mickiewicz. “She was a forward-thinking champion of modern technology, ensuring our patients had access to the latest advances in health care technology which continue to improve each year. CT scanning is the centerpiece of diagnostic services at Dominican Hospital, a fitting tribute to Sister Julie’s relentless pursuit of improved health outcomes.”

$4.3M Philanthropic Goal

To make this vision a reality, $4.3 million in funding is necessary. To date, $3.3 million is secured through a $1.5 million investment from CommonSpirit Health and $1.8 million allocated funds from Dominican Hospital Foundation.

High-quality CT Imaging is Essential to Patient Care

Diagnostic imaging can be crucial to a patient’s care plan as it helps determine the type of trauma and the precise location in a noninvasive and timely manner. New CT

scanners will advance the hospital’s capability to diagnose disease and injury, plan medical treatment and provide improved image quality with lower doses of radiation more quickly. This will enable clinicians to continue to expand clinical capabilities for cardiology and vascular disease, improve emergency medicine through process optimization, and advance treatment of cancer with greater access to precise imaging.

Whether diagnosing abnormalities, such as tumors or blood vessels, giving a clear picture of an area before surgery, or guiding doctors on where to take biopsy tissue samples, access to modern CT imagery is a critically important technology for patient care. Diagnostic imaging with CT has become a crucial part of the care plan for many patients, particularly in the specialties of emergency, surgical, and cancer medicine.

Make a Year-End Gift to Our CT Precision Imaging Campaign Today

We can accomplish remarkable things, together, with generous donors like you, who value health care close to home.

Caring For Your Heart

Today, heart disease has become the leading cause of death for women in the United States, responsible for about one in four deaths among women. Stroke is the number four cause of death in women and kills more women than men. In fact, one in five women will have a stroke.

That is why Dominican Hospital is committed to providing the latest advances in heart and stroke care for women right here in Santa Cruz County. Our highly trained staff includes a rapid-response team, emergency physicians, and experienced nurses to offer heart and stroke patients the critical care they need. From innovative diagnostics to the latest in rehabilitation techniques, we offer the most advanced and efficient services available.

Creating awareness is also important since studies show too few women realize the danger of heart disease.

Less than 50 percent of women recognize the risks of heart attacks and strokes.

Supported by the Dominican Hospital Foundation, the Women of Wellness Council (WoW) voted this past spring to support our “Women Have Strokes Too” program. Now we can help educate women on how to recognize female-patten heart disease studies show women differ from men in the structure of their hearts, the types of heart problems they have, and even the risk factors for heart disease they carry.

We invite you to join the campaign for women’s health and help us fund additional vital women’s health programs and services - that need our group’s support. Our council members take immense pride in helping support and expand women’s health services, acting as advocates for specialized health care for women and children close to home.

We ensure that 100 percent of all donations fund modern technologies and programs that our members vote on each year. WoW has raised close to $300,000 in membership dues and private support to fund many lifesaving health care initiatives.

With your membership, you will learn more about women’s health, and serve as a community leader keeping Dominican Hospital at the forefront of excellence for all the patients we serve.

Sharing of Gratitude to the Life-Saving Treatment at Dominican

The journey to good health can feature life-saving moments. Alison Olsen, a 35-year-old Santa Cruz mom, was finally on her road to good health. She had suffered from severe pelvic pain for years and was happy to be home, recovering from surgery at Dominican Hospital. Alison had endured a total robotic hysterectomy required to treat endometriosis, a painful disorder in which scar tissue grows

outside the uterus. Alison’s surgery was a success, performed by Sienna Titen, MD, a surgical obstetrician specializing in robotics, who was instrumental in diagnosing her pelvic pain and guiding her through surgery to better health.

Alison and her husband, Chris, had good reasons for her to be healthy. Waiting at home were Ben and Sam, their two recently adopted one-year-old twin boys.

14 | FOCUS ON WELLNESS
GRATEFUL
PATIENT
Chris and Alison Olsen, joined by their twin boys Ben and Sam.

Three days into her recovery from surgery, Alison was preparing for a walk and suddenly felt dizzy. She had shortness of breath, and her heart was pounding with knife-like pain in her chest and head. An ambulance was called to rush Alison back to Dominican Hospital. In the Emergency Department, an ultrasound revealed a blood clot in her leg, not an uncommon complication after major surgery. In Alison’s case, the blood clot that formed in her thigh during her healing from surgery broke off and lodged in her lungs.

Each year, two million Americans experience deep vein thrombosis. The condition arises when a clot develops in a vein surrounded by dense connective tissue, typically in the arms or legs, and begins to block blood flow. About a third of the time, a piece of the clot breaks off and travels to the lungs, becoming a pulmonary embolism (PE) that interferes with breathing or causes worse problems. In this country, pulmonary embolisms kill about 60,000 people a year.

Alison was now under the care of the Pulmonary Embolism Response Team at Dominican, the first in the Monterey Bay region. Only three days after major surgery, Alison could not receive standard blood clot treatment for blood thinners. She spent the night under observation in the intensive care unit. With signs of cardiac decline, Juan Rodriguez, MD, an

interventional radiologist, was called to utilize new technology to remove the clot from Alison’s body.

Alison benefitted from a new treatment called the FlowTriever Retrieval/Aspiration System, which can quickly resolve a pulmonary embolism and reduce the risks of side effects with traditional therapies. If a large clot stays in place, the heart must work extra hard to push blood to the lungs. That can strain the heart and may even cause it to stop working. Tests showed that Allison’s heart was under strain, and her clot needed to be addressed immediately.

The FlowTriever system quickly removes even large clots. A catheter, a thin, flexible tube, is navigated through a large vein in the groin, up to the heart, and into the pulmonary arteries. With the catheter in position, clots are removed by suction into a syringe. The treatment mechanically removes clots more immediately than medication, though medication may also be used. FlowTriever treatment spares patients large doses of thrombolytic medicines, which may lengthen the time spent in the intensive care unit and increase the risk of bleeding.

Alison and her family have expressed gratitude for the fast-acting PE Response Team and acknowledge the lifesaving care she received at Dominican Hospital. A return to good health lets everyone focus on doing what they love.

Share your gratitude. There are many ways to say thank you to those who care for us at Dominican Hospital. Contact the Foundation at 831.462.7712 to learn how you can impact the care received for future patients.

An Opportunity to Give Back

Growing up in Mexico, Manuel Mercado, learned the value of hard work and the importance of giving back. He came to the United States at age 18, harvesting strawberries in the fields of Watsonville. After several years, he was introduced to Delores Medina, who he would later marry and build a life together in Watsonville.

From harvester to tractor driver, to crew leader and ranch manager, Manuel worked his way up the ranks for 14 years. He was then given the opportunity to become a co-owner with Tom Driscoll, a shareholder and independent grower for Driscoll’s. “If you have good fields and take care of your people, those are basically what you need to be successful,” says Manuel. Twenty-one years later, Manuel

continues to dedicate himself to those basics under his own growing operations at River Valley Farms.

In 2021, Manuel joined the Board of Directors at Dominican Hospital Foundation, to support the healthcare needs of farmworkers. “I am incredibly proud to be part of the Foundation’s work to bring direct mobile clinic outreach with high-quality healthcare directly to our farmworker community.” The Foundation’s efforts to promote health services in the South County led Manuel to become a direct sponsor. “I wanted my three children, and especially my youngest son, Gabriel, to see that whatever you can do to help your community, it’s well worth it, no matter what you are able to contribute. Whatever we can do, we need to offer that back.”

Exercise Rx

If you could package exercise in a pill, it would be the most powerful medication in the world. Exercise helps primary risk factors for heart disease, such as:

• Lowering blood pressure

• Improving your cholesterol

• Reducing depression

• Lowering the risk of type II diabetes

Exercise helps you manage stress and is a key component in maintaining a healthy weight. There are many immediate benefits as well. For example, a single bout of moderate-to-vigorous activity can:

• Improve sleep quality

• Reduce feelings of anxiety

• Lower blood sugar levels

• Increase your metabolism

24 | FOCUS ON WELLNESS THERAPY CORNER

also help curb cravings for nicotine and alcohol, help maintain youthful skin, and even boost libido.

So if exercise is medicine, what’s the prescription?

First, just going from nothing to something is powerful. An article published in the New York Times last year shared results from a large-scale study conducted in the UK. Researchers found that just moving from the “least-active group” to the “not-quiteas-inactive group” dropped the risk of heart disease by almost 30 percent. And they found no upper limit to the benefits.

n First things first.

Check with your doctor before starting any exercise program or if you have any heart issues, diabetes, high blood pressure, or arthritis. If at any point you feel symptoms such as pain or discomfort in your chest, arm, jaw, shoulder, or feel dizzy or faint during exercise - stop and contact your doctor.

n Start with small steps.

Try going for a 10-20 min. walk 3x/week. Then work on gradually increasing your exercise duration and frequency. Progress slowly, allowing your joints and muscles time to adapt. If you cannot walk or have knee or hip problems, consider cycling, stationary bike, elliptical, seated step machines, and/or water

exercise. For those who are wanting to pursue the optimal health benefits of exercise, there does appear to be a realistic “sweet spot.”

n Exercise Rx: “Sweet Spot”

⊲ 150 min. per week of moderate-intensity aerobic physical activity (example: 30 min., 5x/week)

⊲ Or, 75 min. per week of vigorous-intensity aerobic physical activity - if you are cleared by your doctor to perform vigorous or strenuous activities (example: 25 min, 3x/week)

⊲ Moderate-intensity exercise tends to be safe for most people. However, please contact your doctor if you are unsure on the level of exercise intensity just right for you.

⊲ Complement with resistance exercise and/or balance exercises 2-3x/week with at least one rest day in between bouts. Resistance training is extremely beneficial, especially as we age and lose muscle mass. Examples are: hand weights, weight machines, bands, and floor exercises resisting your own body weight.

⊲ Warm up (5-10 min.), and cool down (5-10 min.) before and after your exercise routine, followed by some stretching. Stretching is best performed after aerobic exercise when your muscles are warm and can prevent injury and soreness.

Based on the American College of Cardiology and the American Heart Association’s recommendations for the prevention of heart disease.

Dominican Hospital has many resources and exercise professionals available to assist you with the specific exercise program that is best for you. Check out the Lifestyle Management programs which include cardiac rehab and medical fitness or the PEP exercise classes held at the Dignity Health Wellness Center. Group classes such as these are a great option especially if you have health issues or if you simply need help with motivation. See page 47

PULMONARY EMBOLISM AWARENESS

Clot Warriors

PPulmonary embolism (PE) is the third-leading cause of cardiovascular death (following heart attack and stroke) in the United States, with an annual mortality of approximately 100,000 people per year.1 Acute pulmonary embolism is a medical emergency and should be addressed immediately by your doctor or local emergency department.

A pulmonary embolism is caused when a blood clot breaks free in the deep veins of the body and travels through the venous system to become lodged in the pulmonary arteries. The blockage of blood flow results in significant strain on the right ventricle of the heart and it is unable to push blood past the clot. In severe cases, heart failure may occur leading to a fatal event.

⊲ The most common symptoms of PE include:

• Sudden shortness of breath

• Chest pain, usually worse with breathing

• A feeling of anxiety

• A feeling of dizziness, lightheadedness, or fainting

• Irregular heartbeat

• Palpitations or heart racing

• Sweating

• Low blood pressure

• Coughing up blood

⊲ Risk factors for PE include:

• Genetic conditions that increase the risk of blood clot formation

• A family history of blood clotting disorders

• Surgery or injury (especially to the legs)

• Situations in which mobility is limited, such as extended bed rest, flying or riding long distances, or paralysis

• Previous history of clots

• Cancer and cancer therapy

• Certain medical conditions, such as heart failure, chronic obstructive pulmonary disease (COPD), high blood pressure, stroke, and inflammatory bowel disease

• Certain medicines, such birth control pills and estrogen replacement therapy

• During and after pregnancy, including after cesarean section

• Obesity

• Enlarged veins in the legs (varicose veins)

• Cigarette smoking

PULMONARY EMBOLISM

PE Prevention

Because PE (pulmonary embolism) is often caused by a blood clot that originally formed in the legs, and because it is often difficult to detect a DVT (deep vein thrombosis) before problems start, the prevention of DVTs is vital for the prevention of PE.

⊲ Maintaining a healthy lifestyle can help with PE prevention, including:

• Engaging in regular exercise

• Maintaining a healthy weight

• Eating a balanced diet

• Taking medicines as prescribed

• Not smoking

⊲ After surgery the following methods are used to help prevent DVT’s:

• Compression stockings that squeeze or compress the veins and prevent blood from pooling

• Pneumatic compression devices which are sleeves on the legs that provide alternating pressure on the legs to keep blood moving.

• Getting up and moving as soon as possible after surgery or illness. Movement can help keep clots from forming by muscle pump action to assist with blood circulation.

• Medicine, anticoagulants and aspirin are often given to help prevent DVT.

Many people remain at risk for developing DVTs for a period after they are discharged from the hospital. It is important that treatment to prevent DVTs continues until the risk has been resolved, usually about 3 to 6 months.

This team comprises emergency medicine, pulmonary/ critical care medicine, interventional radiology, cardiology and cardiothoracic surgery in offering additional strategies to treat and resolve acute PE.

Treatment for pulmonary embolism include anti-coagulation (blood thinners) but patients that exhibit symptoms such as blood pressure instability and require supplemental oxygen to maintain adequate blood oxygen levels may be at high risk for death and may benefit from the additional therapies available at Dominican Hospital.

These additional therapies include catheter-directed therapy (blood thinner is delivered directly to the blood clot in the lung), mechanical thrombectomy (the clot is removed from the pulmonary artery directly via catheter inserted into a peripheral blood vessel), surgical embolectomy (clot is removed in surgery) and extracorporeal membrane oxygenation (ECMO) where blood is circulated through an external oxygenation device while the PE is treated.

“These therapy advancements would not be possible without strong collaboration among our emergency, radiology, intensive care and cardiothoracic surgical teams here at Dominican,” comments Juan Rodriguez, MD, Medical Director of Interventional Radiology. “We’ve seen tremendous positive outcomes for many patients with very severe cases of PE.”

PULMONARY EMBOLISM AWARENESS

Alison’s Story

As you have read on pg. 14, Alison Olsen experienced a PE post-surgery and subsequent life-saving treatment. She also shares her experience of the symptoms to be aware of.

“The discomfort and pain that I was in can only be described as torturous. While recovering from surgery, the pulmonary embolism caused extreme chest pain and I had a searing headache causing me to be rushed to the emergency department and hospitalized. I was extremely anxious, and just wanted to get home to my twin two-year old sons.

Unfortunately, because my body was receiving so little oxygen, my heart needed to keep up its increased activity, and I could not take anything to help me calm down and sleep. I was so weak that I was rendered breathless just by lifting up my cell phone in one hand to try to text my husband updates. I had been under observation for 24 hours to then be taken to Interventional Radiology for the thrombectomy.of my saddle pulmonary embolism.

The relief was immediate and there may be no better sensation than the absence of pain. I am very grateful for Dr. Juan Rodriguez and the PERT team, and I want to thank you for helping me. I know that your work takes years of dedication, education, and training. Thank you for your commitment to your specialty, to our hospital, to our community, and to me as your patient."—Alison

Stroke Prevention

On a sunny Sunday afternoon Molly was walking on West Cliff with her friend. All of a sudden her left arm and left side of her face go numb, and she is unable to lift her left arm. Her friend, Rosa, calls EMS and she is transported to Dominican Hospital.

This is fortunate for Molly as she suffered from a stroke, in particular a large vessel occlusion. Stroke is the leading cause of long term disability. Thus, getting Molly to the hospital in a timely fashion is crucial. Both Molly and Rosa are fictional characters, but this is a potential scenario of an emergency resulting in a stroke.

Luckily, Dominican Hospital is a Primary Stroke Center (PSC). This means that we can treat Molly and others that present with advanced therapy if they come in a timely manner. The advanced therapy available here at Dominican is tissue plasminogen activator (tPA) when administered quickly after stroke onset can help restore blood flow to the brain regions affected by a stroke, thereby limiting the risk of damage and functional impairment.

Although Dominican is a PSC, the best option, as with most health issues, is prevention. Stroke prevention can be categorized broadly as physical and mental.

WELLNESS
MINDFULNESS

Physical

A great way to protect yourself against stroke is by protecting your cardiovascular health. Stroke prevention can be summed up with the American Heart Association's “Simple 7”, which includes seven risk factors for improvement of an individual's cardiovascular health.

“Simple 7” Lifestyle Changes

• Don’t smoke cigarettes or use other tobacco products

• Reach and maintain a healthy body weight (body mass index < 25/kg/m2)

• Get at least 150 minutes of moderate-to-intense physical activity or 75 minutes of vigorous-intensity activity (or a combination) each week

• Eat a healthy diet consistent with the American Heart Association

• Get your cholesterol checked and talk to your doctor about your number and they impact your health

• Keep your blood pressure below 120/80

• Keep your fasting blood sugar < 100 mg/dl

Mental

The mental aspect of stroke prevention is addressing the psychological stress of stroke. According to the American Institute of Stress, the current stress level experienced by Americans is 20 percentage points higher than the global average. Unfortunately, in the past few years, there have been increased levels of stress due to COVID-19 pandemic. In evidence-based research, mindfulness-based interventions are showing promise to help with psychological, psychosocial, and physical limitations of strokes. By creating a routine of mindfulness practices, you can help to reduce stress and how you cope with stressful situations.

Mindfulness Practices

• Meditation

• Walking

• Deep breathing

• Gentle yoga or other exercises

Developing a mindfulness practice is advantageous if you have had a stroke and/or for prevention. For our friend, Molly, such a technique would help her remain calmer and more balanced as she is transported to Dominican Hospital compared to if she did not have such practices.

Adopting both physical and mental activities can help greatly with stroke prevention. Dominican hospital offers a variety of options for both the physical and mental stroke prevention. Including many PEP exercise classes; yoga for stress, Tai Chi, and Mindfulness Stress Reduction Program. Starting with just one from each category can help you along the pathway and start the journey without making the new habit too cumbersome!
