Letter to the Community BY ALLISON NIDAY

Dear Community Members,

S

ummer is here, along with a feeling that things might just be returning to “normal.” And although the pandemic seems to be subsiding, or at least becoming endemic, the importance of the philanthropic mission of the Dominican Hospital Foundation has not changed. Through our partnership with you, and our many other supporters, we make an impact on patient care and services here at Dominican Hospital. Dominican Hospital is so much more than emergency, in-patient, or out-patient care! The comprehensive services the hospital offers are laser-focused on improving community health through a wide range of prevention and health improvement programs, many in collaboration with our community partners. Our goal with these investments is to address the social determinants of health and provide the tools and training to keep our community healthy and out of the hospital. As you know, Dominican serves anyone in need – regardless of ability to pay. In fiscal year 2021 alone, Dominican Hospital provided $41,535,053 in patient financial assistance, unreimbursed Medicaid costs, and community health improvement services. Of that, nearly $13 million was for medically necessary hospital services to patients and community benefit programs that met certain criteria. Dignity Health, Dominican Hospital’s parent organization, recently announced a $1 million Homeless Health Initiative grant to Housing Matters to build a permanent supportive housing residence. This grant provides the critical funding needed to begin construction on a 120+ unit residence for individuals experiencing chronic homelessness in Santa Cruz County. It also acknowledges that a key factor in maintaining good health is safe and secure housing. In this FOCUS, we welcome our incoming board member Tom Brezsny. Made up of volunteers within our community, members of the Foundation’s Board of Directors generously donate their time and expertise to raise funds for Dominican Hospital. We thank our outgoing directors Susan MacMillan, MBA, and Roy Martinez, MD, for their years of partnership. Elsewhere in the magazine, you’ll find stories from grateful patients, as well as educational and wellness opportunities for you, your family, and your neighbors.

Elevating Health Care

The Dominican Hospital Foundation honors Dr. Michael and Toby Alexander as respected community members and longtime supporters of health care, education, and the arts.

Lifetime Champions of Excellence

F

rom their professional accomplishments to their work on behalf of the Dominican Foundation, to their concern for the well-being of the community, Toby and Dr. Michael Alexander are high achievers of the first order. For the past 46 years, Dr. Alexander has provided life-saving cancer care to patients in the Santa Cruz region and is presently Dominican Hospital's Medical Oncology Director. Toby has been a leader in numerous charities. Together, they are champions of excellence in education, the arts, and quality health care. The Alexanders first came to Santa Cruz in 1976 when Dr. Alexander joined in partnership with colleague and friend, hematologist Dr. Jim Poth. Dr. Alexander became the first medical oncology specialist in Santa Cruz. “Our commitment

was to provide the highest level of cancer care from day one, ensuring patients received the same level of care found at Stanford School of Medicine, where we both trained,” remembers Dr. Alexander. “Over the years, we both taught at Stanford and still maintain strong ties there to this day. Local patients benefited from our ability to tap into Stanford oncologists for informal advice or formal consultations and referrals as needed.” Dr. Alexander served as an adjunct faculty at Stanford and volunteered in its oncology clinics. He currently serves as Clinical Associate Professor Adjunct Faculty (Emeritus). “When it was time to expand our practice, we recruited Stanford-trained fellows, which brought a new set of professional connections.” Dominican also participates in national cancer research clinical trials centered at Stanford. “Our Stanford ties run deep with several physicians at Dominican,” explains Dr. Alexander. “It has brought a university-level of excellence and quality to the hospital. Santa Cruz is fortunate because this community has exceptional doctors in many specialties. With the support of administration and the philanthropic Foundation, Dominican has kept up with innovative technologies in all areas of medical care, ensuring we can offer the most advanced treatments.” “Helping patients with cancer is fulfilling, rewarding work,” says Dr. Alexander. “The best thing about medicine is when patients express gratitude for the care provided during the challenges of cancer. Past patients still call years later to say thank you or seek new advice.” Dominican’s cancer program is certified by the American College of Surgeons, which requires data collection and studies to document the high quality of the program. After 36 years in practice, Dr. Alexander stopped seeing patients in 2012 and became Medical Director of the Dominican Cancer Program. He attends the tumor board, oversees the cancer committee and certification, and directs the cancer lecture series for visiting oncologists. Dr. Alexander also coordinates a lung cancer screening program, which has screened 2,000 patients since 2012. Active in supporting philanthropic work throughout Santa Cruz County, Toby is a founding member and current chairperson of the Foundation’s Women of Wellness (WoW) Council. WoW provides annual funding of $75,000, or more which is allocated from membership dues of $1,000 each. Members vote and distribute contributions to key initiatives and projects focused on women’s health at the hospital.

As we look ahead, we need to continue to modernize our facility and stay equipped with the most advanced tools of medicine. Dominican Hospital is in your future. If you experience any serious illness, heart attack, stroke, or another acute event, it will be treated at Dominican. We are the primary hospital in the region with the full capabilities and range of specialists for life-saving care. –Dr. Michael Alexander, Emeritus Foundation Board Member

Dr. Alexander served on the Foundation board and continues as an emeritus director. He and Toby together have recruited many dynamic leaders to advocate for the hospital’s fundraising. The Alexanders are longtime donors and members of the Bob Swenson Legacy Society, which honors benefactors who have included Dominican in their estate plans. (They are currently involved in the early leadership planning for a potential future hospital expansion.) With her own master’s degree from Stanford University, Toby taught high school math while raising two children. She managed Dr. Alexander’s practice and served on the school board for six years where she helped start a foundation that raised funds for public education. Their children are continuing their parents legacy. Daughter, Jane, is an occupational therapist at Dominican and their son Jonathan Alexander, M.D., Ph.D., is a pulmonologist in San Francisco. Five grandchildren add tremendous joy to their family. Toby and Dr. Alexander have generously helped to further enhance medical care, supported local schools, and raised funds for the local arts; including Santa Cruz Shakespeare and the Kuumbwa Jazz Center. With passion and a genuine concern for the community, their engagement and investment in Santa Cruz County will impact the quality of living in this region for decades to come.

The Circle Of Care

Sharing of Appreciation

T

ouched by the high-level of care he received during a recent stay at Dominican Hospital, retired laboratory director Steve McCarthy penned a letter to the hospital president/CEO, Dr. Nanette Mickiewicz, MD, to communicate his deep gratitude to the doctors and nurses who saved his life. “I wanted to convey my sincere appreciation to a bunch of Dominican folks for their fine care during my recent admission,” writes Steve. “Dominican is a stellar facility in our community and should be recognized as such.” After graduating college in the 1970s, Steve moved to Santa Cruz and served as a laboratory manager first at Watsonville Hospital for a decade, and then landed his dream job as lab director at Dominican Hospital. “I could’ve taken a job in a private biotech firm after completing school, and got paid really well, but working in health care appealed to me more,” says Steve. “The hospital business is ultimately a service business and there’s inherently a lot of altruism that goes into it.”

Since retiring from his 25 year-long career at the hospital in 2011, Steve has maintained ties with Dominican Hospital not only through the friends he made during his long career, but also through his family. “I still have a place in my heart for Dominican. One of my daughters works there, and I have a lot of friends there,” says Steve. “I’m still pretty attached at the hip to the hospital, not only in terms of my loyalty because it was such a great place to work. It’s one of the only medical facilities in the county that I consider truly nonprofit.”

I can’t tell you the number of times the nurses and physicians came in to check in and see how I was doing. They put so much into their work, and I can’t thank the caregivers enough. –Steve McCarthy, grateful patient

The selflessness and professionalism Steve experienced during his recent admission to the hospital and the emergency room across the nursing staff, doctors, and specialists—especially during the COVID-19 pandemic— only cemented his decades-long loyalty to the hospital.

Sharing gratitude for the caregivers at Dominican Hospital goes a long way. You can express your appreciation or make a gift in their honor by contacting the Foundation. Submit a note today and we will be sure to share with your caregiver! 831.462.7712 | supportdominican.org

A Gift of Service

For me, this is something I do every week that really enriches the experience of my retirement life. I think it is a great transition for folks who retire and want to connect with the community or just need something to do. It’s worked out for me so well. -Mike Lambert

Sharing the Joy of Volunteering at Dominican

B

y his own admission, 81-year old retired potter Mike Lambert is not someone who is inherently compelled to help others in an altruistic way, but after he started to volunteer at the Dominican Hospital on a whim 15 years ago, he found that he truly enjoyed giving his time and energy to others. “I was used to working 12 hours a day as a potter, and had a lot of time on my hands after I retired. My neighbor across the street was volunteering at Dominican Hospital and I thought I’d try it,” says Mike. Having owned his own business, Mike wasn’t used to being told what to do, but soon found that he enjoyed his new responsibilities and following the direction of

others rather than leading. He started as a volunteer in the surgery waiting room, and after a few years, and with some encouragement, transferred to the lobby of the Emergency Room (ER). “I was nervous that I wasn’t prepared to be in the ER, but I loved it. Eventually, I was there three afternoons a week, and have trained many students to do the job over the years,” says Mike. Through his 15 years of volunteer work at Dominican Hospital, Mike now knows how simple it is to be of service to others. He enjoys the task-at-hand even as simple as directing visitors to the right room, or pushing patients out in a wheelchair when they’re discharged. “We serve a really helpful function that's hard for the paid staff to do,” says Mike. “We do things that don’t have much to do with medicine but really need to be done. As a volunteer, you’re a part of the hospital.” Because Mike and other volunteers help keep the hospital running smoothly, the doctors, nurses, and other medical staff are very appreciative of their contribution, especially after the volunteers weren’t allowed in the hospital during the first year of the COVID-19 pandemic. “When we were finally allowed to come back, all of the doctors and nurses and staff that we hadn’t seen in a year were so thrilled that we were there,” says Mike. “The relationships I have with the staff are really gratifying.” After 15 years, volunteering has become an important part of Mike’s weekly schedule, and he recommends it to anyone in the community, even if they do not have previous volunteer experience.

Volunteering is just one more way you can give back to the community, if you are interested in learning more please contact Dominican’s Volunteer Services at 831.462.7740.

Leave A Legacy

Tax-Savvy Giving

W

hen planning to make the most of one’s financial assets in retirement and beyond, former Vice President of Philanthropy at the Dominican Hospital, Bob Semas, urges those considering a legacy gift to Dominican to join him and do so now in the form of a tax-savvy charitable gift annuity. “The most common way to make a legacy gift is to name a charity in a will or trust, or as beneficiary of a life insurance policy or IRA. While annuities, in general, might have a dubious reputation in the financial world, charitable gift annuities are a different animal and can be a creative way to make a legacy gift to Dominican Hospital,” Bob says. A charitable gift annuity provides you with income now, and benefits Dominican in the future. You receive a lifetime income at a fixed rate based on your age at the time of the gift, and Dominican receives the residual upon your passing. A two-life variation provides income for as long as either of

Bob Semas, member of the Bob Swenson Legacy Society. two individuals survive – usually, but not necessarily, a spouse. Annuities can be funded with cash, stock or real estate. “Charitable annuities have a number of advantages. Payout rates are higher than what you would receive from other fixed-income options. Only a portion of the income you receive each year is taxed as regular income, and donors itemizing contributions receive a substantial tax deduction for the year of the gift.

A charitable gift annuity can be a smart choice for seniors who no longer wish to maintain income property or want to reduce portfolio risk in a fluctuating market. Funding charitable annuities with appreciated stock or real estate, bypasses the capital gains tax normally due upon sale, which for longheld assets is a huge benefit. Charitable gift annuities can be a very tax-wise way to support Dominican,” says Bob. If you’d like to consider joining Bob as an annuitant, the Foundation office would be happy to prepare a no-obligation example specific to your age(s) and funding source. Giving a legacy gift to the Dominican Hospital Foundation, whether through a charitable gift annuity or otherwise, ensures that the Dominican Hospital will continue to provide a high level of care to the community for decades to come. “I hope the community continues to support the Foundation and that those considering a legacy gift explore their options with a charitable gift annuity and its many benefits,” Bob says.

Charitable gift annuities ensure the Dominican Hospital will thrive for years to come. Learn more at dominicangiving.org or call 831.462.7712.

Retirement After 40 Years

All new advances in medical technology are expensive. Contributors fund key equipment such as surgical robots or endoscopic ultrasound. We need these tools to care for critically ill patients and to help attract new doctors who have trained on this advanced equipment. – Dean Kashino, MD

Devoted to Family Care and Excellence

S

anta Cruz residents needing basic health care often start with a visit to their primary care doctor. In the past 40 years, there is a good chance that the doctor was Dean Kashino, MD, a devoted general practitioner widely known as “everyone’s physician.” Dr. Kashino cared for generations of local patients and their families until his recent retirement. In service to the larger community, Dr. Kashino, and his wife Frances played an active role in the growth of Dominican Hospital since he first started seeing patients in 1980.

1 8 | FOCUS ON WELLNESS

“I enjoyed being a primary care and obstetrics physician. When you care for patients, deliver their babies, take care of their kids, and watch the entire family grow, you get close to them and manage their care better,” says Dr. Kashino who delivered more than 1,000 babies in the 1980s. He offered primary care along with sports medicine, minor surgeries, and chronic disease management. “My work guiding patients to better health was rewarding.”

Dr. Kashino reflects on four decades of care. “The family structure certainly saw changes and so did the delivery of health care,” reports Dr. Kashino. “Along with tremendous advances in medicines, treatments, and technology that save lives, there also have been efforts to make better use of health care dollars. Everyone is looking for efficiency in providing care at an affordable cost.” He also saw the development of electronic medical records which help coordinate care for medical groups and the patients they serve. Forty years in health care gave the Kashino’s a front-row view of the growth and development of Dominican from an independent hospital to an expansive medical center. “The affiliation with Dignity Health, and now CommonSpirit Heath, is beneficial to patients as large systems offer more resources to help operate efficiently and add key medical services needed by the community. Our advancements in heart care and the upgrade of our Level III Neonatal Intensive Care Unit (NICU) are two examples. We also have strong ties to physicians and programs at Stanford and Sutter Health, which brings high-quality standards to patient care.” Despite a busy practice, Dr. Kashino found time to serve on the hospital and foundation boards, collaborating with other leaders. “Issues came up in the physician lounge, and I took pride in bringing concerns forward to make improvements. The input from physicians, nurses, and other staff is essential as leadership works towards a shared goal of better patient care.” Dr. Kashino commends Dominican’s strong commitment to the broader community, as evidenced by its participation in the local safety net coalition that helps uninsured or underinsured patients. He served on the Health Improvement Partnership Council assesses community needs, learns what others are doing, provides outreach services, while avoiding duplication of services. In addition, Santa Cruz County ranks first in the country for controlling costs for all Medicare patients while providing good outcomes. Dr. Kashino encourages physicians to welcome appreciation and to refer grateful patients to the Foundation where they can learn about projects that are in need of funding. He remains active both on the Hospital and Foundation boards and as a philanthropic physician leader. “All new advances in medical technology are expensive. Contributors fund key equipment such as surgical robots or endoscopic ultrasound. We need these tools to care for critically ill patients and to help attract new doctors

who have trained on this advanced equipment.” states Dr. Kashino. Supporting outstanding care is the vital work of the Foundation and hospital donors. Frances raised their family and worked as office manager for Dr. Kashino’s office. She also was Director of Physician Services at Dominican for 14 years before retiring in 2016. Frances continues to serve at the hospital as a volunteer. “Serving beside the Adrian Dominican sisters is an inspiration. Our mission is essential to healing and we welcome the support of the community,” says Frances. “Donors are generous, but we can always do more.” With retirement, Dr. Kashino and Frances look forward to traveling and spending time with their family. Their son Sean works in accounting and lives in San Bruno, while daughter Stephanie, with her passion for baking, is home in Santa Cruz. “The hardest part of retirement is leaving patients we’ve known for decades.” Dr. Kashino’s very first patient back in 1980 also booked his last appointment time, bringing full circle his decades of devotion, quality care, and concern for the broader community. “It certainly made the last patient visit a special moment,” reflects Dr. Kashino. “Together with Frances, we have a deep sense of accomplishment in our service to patients and Dominican Hospital.”

The Dominican Hospital Foundation expresses true gratitude for the devotion of Dr. Dean Kashino who has cared for our community over the last forty years. We have been privileged to work alongside Dr. Kashino and now honor him in his retirement.

he old saying, “Move it or Lose it” can apply to both activity and balance. The GOOD news is that balance, strength, endurance, and flexibility can all be IMPROVED at any age! So NOW is the time to get moving. Balance is a complex task for our bodies. We use multiple systems to keep us upright and moving. Our vestibular (inner ear), our visual (eyes), and our proprioceptive (body movement sensory and muscle) systems all work together to keep us balanced. Beyond doing these simple exercises, you can join a balance class to make it more fun. Be sure to make time to get your hearing and vision tested. Even a few exercises added to your daily routine can help improve your balance.

1

HEEL RAISES 1. Standing tall and using a solid support for safety, rise up onto your toes, hold for 1 count and then slowly lower. 2. Use your hands lightly for support. 3. Repeat 10 times.

2 SIT TO STAND 1. Scoot toward the end of your chair, place your feet hip width apart, lean forward while looking forward (you can also reach forward). 2. Push through your legs and heels to come to standing. 3. Stand tall, pressing arms back and hips forward. 4. Then slowly lower to return to sitting, leading with your hips. 5. Repeat 1O times.

2 4 | FOCUS ON WELLNESS

Read This First! These next exercises can get tricky– you want to be able to challenge your balance but at same time remain safe. Always have solid support to be able to steady yourself as needed. Any exercise can be made harder by closing your eyes, or adding a softer surface, but make sure to progress slowly and safely, listen to your body and rest when needed.

3

4

TANDEM BALANCE WITH EYES CLOSED 1. On foam or on firm ground. 2. Use a solid support surface for your hands (railing or counter top) and place one foot in front of the other in a heel toe position as able, 3. Keep equal weight in both feet and stand tall, activating abdominals and hips, 4. Hold 30 seconds 5. Repeat with the other leg in forward position.

5

SINGLE LEG STANCE 1. Use a solid surface for your hands, shift your weight to one leg and then slowly lift other foot off the ground. 2. Stand tall on your standing leg with a slight bend in your knee (don't lock your standing knee). Hold for 30 seconds 3. Repeat 3 times on each leg.

ADVANCED TANDEM BALL TOSS ON FOAM 1. Stand near a solid support surface (counter or railing), place feet in tandem position (one foot in front of other) on foam. 2. Toss ball for 10 catches. 3. Repeat with other leg forward. *You can also try this activity on firm ground.

There are many balance exercises classes available through the PEP program; check out Balance Basics, Balance Bones and Strength, Boxing for Balance and Your Bones, and various Tai Chi classes. You deserve to live life fully and enjoy it along the way!

Pregnancy – The Fourth Trimester and Beyond!

H

aving a baby is an exciting time that can inspire women to make healthier life choices. Many of these choices will not only help with the pregnancy, but will be great for the development of the baby and can set the whole family up for a lifetime of good health. The “fourth trimester” is a time of major transition and growth that can be a challenge even with good preparation.

2 8 | FOCUS ON WELLNESS

Dominican Hospital is here to help you through all stages of pregnancy, and offers comprehensive expert support. Dr. Sienna Titen, MD, obstetrics & gynecology specialist with Dignity Health Medical Group states, “It’s important not to forget the fourth trimester where you learn to balance your needs with that of your newborn, and create your new regimens to start your new family on the right track to health.”

Trimester Tips

Through the Four Trimesters of Pregnancy: 1st Trimester Tips (conception-12 weeks) □ Take prenatal vitamins □ Exercise regularly □ Drink lots of water □ Eat a diet high in fruits, vegetables, protein, and fiber □ Schedule prenatal appointments

2nd Trimester Tips (13 week-27 weeks) □ Continue trimester 1 tips □ Keep your teeth and gums healthy □ Take prenatal education classes (pg. 44) Birthing, breastfeeding, and newborn care □ Tour Birth Center at Dominican Hospital □ Make nursery or space in home for baby

3rd Trimester Tips ( 28 week- 40 weeks) □ Continue with your regular exercise routine, but don’t exhaust yourself □ Review early signs of labor □ Pick a pediatrician □ Meet with your local IBCLC (International Board Certified Lactation Consultant) □ Make birth plan with partner □ Pack hospital bag

4th Trimester Tips (birth-12 weeks postpartum) □ Eat food that is comforting, nutritious, and nourishing □ Be gentle with yourself and practice self-compassion, finding opportunities for rest and recovery □ Ask for help as needed; you’ve done an amazing job bringing this little being to the planet and you deserve support □ Get sleep when baby sleeps, or do comforting activities for yourself □ Follow your baby's feeding cues, which may be as often as every 2-3 hours initially □ Alternate nighttime waking/feeding schedule with partner, if available and if possible □ Practice skin-to-skin contact with your baby □ Be breast aware and meet with your lactation consultant for infant feeding support (pg. 47) □ Be aware of postpartum depression symptoms and seek medical guidance if needed □ Schedule regular pediatrician check-ups for baby and postpartum check-ups for birth parent

Healthy Moms Raise Healthy Babies: New mothers tend to forget that they need to care for themselves as well as the baby. However, it is essential to take care of your needs as well as those of your infant.

Nutrition and Hydration: Overall health is dictated by nutrition and activity, both essential components for a healthy pregnancy. During pregnancy you need more vitamins and minerals such as folate, iron, and calcium. You may need to change your eating habits including amounts and times, all of which should be discussed with your doctor. If you are breastfeeding, there is an additional need for calories and good nutrition for you and your baby. • Fruit and vegetables are essential to provide vitamins, minerals, and fiber. • Lean protein provides essential amino acids that your body can’t manufacture on its own. Healthy sources include beans, peas, quinoa, soy, lean meats, and seafood low in mercury. • Whole grains such as brown rice, quinoa, and oatmeal provide necessary fiber for a healthy breastfeeding diet. • Healthy fats like olive oil, walnuts, almonds, and avocados provide beneficial nutrients such as vitamin E, omega-3, omega-6, and antioxidants. • Low-fat milk and milk products provide needed calcium to protect your own bones and teeth. You can also try non-dairy milks like soy, almond, or rice milk that have added calcium and vitamin D. • Drink at least 8 cups of water each day to reduce the risk of dehydration, especially if breastfeeding. FOCUS ON WELLNESS | 2 9

New Mom Health

Physical Activity: Most postpartum women need the same amount of physical activity as they did before becoming pregnant and should gradually work up to at least 150 minutes a week of moderate-intensity aerobic activity. It is important to choose moderate activities that aren’t likely to hurt you. A great idea is to choose specific prenatal group exercises where you can gain peer support as well as vital education. After childbirth it is important to check in with your doctor about your specific needs for resuming your activity levels. If you exercised during pregnancy and had an uncomplicated vaginal delivery, it's generally safe to begin light exercise as soon as you feel ready. If you had a C-section or a complicated birth, talk to your health care provider about when to start an exercise program. • Start slowly with simple exercises that strengthen major muscle groups, including your legs, abdominal, and back muscles. • Finding the time to exercise can be hard, so include your baby in your routine, and make sure to take breastfeeding into account. Feed your baby right before you exercise to avoid discomfort caused by engorged breasts and wear a supportive bra. • Joining a postpartum exercise class can help to motivate you while providing more guided exercise progression. • Make sure to drink fluids before, during, and after physical activity. • Wear comfortable clothing that fits well and supports and protects your breasts. • Stop exercising if you feel nauseated, dizzy, excessively tired or short of breath or experience pain. This may be a sign of overexertion.

Weight Management:

Emotional and Mental Health:

Gaining a healthy amount of weight during pregnancy is vital to help your baby grow to a healthy size, but gaining too much weight can increase your risk for pregnancy related high blood pressure and gestational diabetes. Your doctor can guide you through what is an appropriate amount of weight for you, helping you have an easier pregnancy and delivery. While the initial weight loss after childbirth can be fast as you shed retained fluids, the necessary stored fat may take longer to lose. It is important to set realistic goals for your overall fitness and health. Appreciate all that your body has done to bring this new person into the world, and be gentle with yourself as you accept the changes in your body.

There are many highs and lows during the transition of welcoming a new baby into the world. Most new moms experience postpartum "baby blues" after childbirth, which can include mood swings, crying spells, anxiety and difficulty sleeping. Sometimes new moms can experience a more severe, long-lasting form of depression known as postpartum depression. Talk with your doctor if you are experiencing symptoms of postpartum depression. Things that can help improve emotional health include: • Build a support system • Get help from family, friends and professionals as needed • Rest when you can • Talk about your feelings • Take time for yourself • Prepare for weaning • Learn the signs of postpartum depression

Many pre- and post-natal classes are offered by health professionals at the Dignity Health Wellness Center; please see the Childbirth & Parenting section of the PEP catalog for details (pg. 45)

Newborn Feeding

Preparing to Feed Your Baby

T

he Dignity Health Lactation Program provides patients with comprehensive clinical lactation support that starts prenatally and continues through the weaning process. Housed in the Pediatric Clinic, it is a dynamic, team-based program that bridges multiple specialties, and is built upon the highest clinical standards including the Gold Standard of International Board Certified Lactation Consultants. In addition to holistically supporting our patients, the Lactation Program complements, enriches, and expands upon the top notch care that our physicians provide. Each program component positions the Dignity Health Medical Foundation as a leader in Maternal Child Health in our community. Breastfeeding is very healthy for both you and your baby. Breast milk is the only food or liquid your baby needs for about the first 6 months after birth. At about 6 months, you’ll start to feed your baby complementary foods in addition to breast milk. It is recommended that you continue to breastfeed your baby for at least the first 12 months. Breastfeeding is natural, but that doesn’t mean it’s always easy. You and your baby may need practice, and some help in the beginning.

3 4 | FOCUS ON WELLNESS

Why Breastfeed? • Breast milk is the best source of nutrition for most babies. As the baby grows, the mother’s breast milk will change to meet her baby’s nutritional needs. • Breast milk shares antibodies from the mother with her baby. These antibodies help babies develop a strong immune system to protect them from illnesses. Breastfed babies have a lower risk of asthma, obesity, Type 1 diabetes, and sudden infant death syndrome (SIDS). Breastfed babies will experience less frequent ear infections and stomach bugs. • Breastfeeding has health benefits for the mother. It can reduce the mother’s risk of certain cancers, like breast and ovarian cancer, as well as reduce the incidence of Type 2 diabetes, and high blood pressure. • Mothers can breastfeed anytime and anywhere. They can feed their babies on the go without worrying about having to prepare formula and bottles. Breastfeeding can also provide a source of comfort for babies when normal routines are disrupted.

Steps for Breastfeeding Prenatal Care:

Talk to your doctor or midwife about breastfeeding. Get connected with an International Board Certified Lactation Consultant to schedule prenatal lactation and pump consult appointments. • Take a Prenatal Breastfeeding class through the Wellness Center (pg. 44) • Prenatal Lactation Appointment • Orientation to Dignity Lactation Programs, both inpatient and outpatient and how to access services. • Answer any questions about breastfeeding. • Review any individual clinical risk factors. • Discuss breast pumps and review the process for obtaining one through your insurance plan. • Prenatal Pump Consult • Bring in your breast pump to receive instruction provided on proper set up, use, and cleaning. • Patients will be fitted with proper flange size.

Steps for Breastfeeding Postpartum Care:

Let your doctor, nurse or midwife know you want skin-toskin contact with your baby right away. Try to keep baby skin to skin and enjoy that "golden hour", and nurse whenever your baby is showing feeding cues. Seek help from your nurse or Lactation Consultant if you have any concerns or challenges with feeding your baby. • Newborn Feeding Assessment • In conjunction with the Newborn Well-Child Check with the pediatrician, a comprehensive feeding assessment is scheduled with our Lactation Consultant right after or before your appointment with the doctor. • If scheduling doesn't allow for both of these to happen at the same time, the feeding assessment will be scheduled within 1-2 days of the appointment with the pediatrician. • Infant weight gain, milk transfer assessment, proper positioning and latching, breast care, sore nipples, and milk supply are addressed. • Feeding assessments and consultations can be accessed by any parent-baby couplet, not just breastfeeding patients. Our Lactation Consultants help with bottle feeding, safe formula preparation, proper feeding volumes, as well as breast care during the engorgement period.

Sarah Fulop-Furlano, BS, IBCLC and Erin Respicio, IBCLC, RLC, are both International Board Certified Lactation Consultants (IBCLC) who are available both in- and out- patient to help you manage breastfeeding your infant.

• Lactation Consultation • Any infant feeding issues can be addressed at these visits including: low milk supply, clogged milk ducts, mastitis, slow infant weight gain, painful feeding, weaning, bottle feeding, over-supply of breastmilk and induced lactation. • Patients can self-refer for these appointments and/ or infants’ or parents’ medical providers can refer for these visits. • Back to Work Consult • 2-3 weeks before going back to work. • Discuss lactation accommodation rights in the workplace. • Create a sample pumping plan based on your work schedule. • Provide strategies for maintaining parents’ lactation goals while working. • Discuss tips for working with childcare providers on the handling and storage of human milk, as well as appropriate feeding volumes for infants while in their care.

Be Breast Aware:

Make sure to look at your breasts in the mirror, and check them regularly for signs of tenderness. The proper bra, hygiene, latch and routine can reduce many problems. Make sure to talk to your provider if you have flu-like symptoms and painful breasts that can be symptoms of mastitis.
