(CBR). I understand CBR retains superior title to all foster dogs, and may exercise its superior claim if at any time the conditions of this agreement are not being met. Please sign your initials after each item in the brackets [ ] to show your agreement and sign at the bottom. If you have any questions, please ask them before signing. 1.I agree that I will only take in a dog as a foster that has been approved in writing by an officer of the rescue “Executive Director, Director, or Intake Coordinator”. If I take in a dog without permission of an officer of the rescue I understand that this dog will be considered a personal dog whom I am responsible for and the rescue will have no responsibility for this dog. In the event that I ask the rescue for help placing this dog for adoption I understand that this dog will be considered an owner surrender and will be subject to the owner surrender guidelines including but not limited those listed below. In addition to these guidelines I understand that I will be responsible for fostering this dog in the event the rescue agrees to place this dog up for adoption through the rescue. [ ] Owner Surrender Guidelines include but are not limited to: a. Dog must be a pure bred boxer- picture required b. Dog must be up to date on all vaccines c. Dog must have heartworm test and be up to date on heartworm medicine d. Dog must be spayed or neutered e. Proof of vaccinations and spay/neuter must be submitted to the rescue f. Dog must not be known to be aggressive g. Owner surrenders are only taken in as space and funds allow 2.
I agree to take my foster dog(s) to any and all pre-arranged veterinary appointments, administer any medication prescribed by a veterinarian for said dog, and guarantee that said dog will remain healthy and cared for in my home. [ ]
3.
I understand that any and all vet visits require pre-approval and a written vet authorization form. I understand that if I take my foster to the vet and do not receive pre-approval that I may not be reimbursed for any out of pocket expenses. [ ]
4.
In the event of an emergency you may take your foster to the ER vet in your area. However, I understand that if I take my foster to the ER vet and the situation is not deemed an emergency I may be responsible for partial or all costs incurred. [ ] Emergency situations are those that would put the foster dog’s life in danger if not seen by a vet immediately and include but are not limited to: • Hit by a car • Excessive bleeding • Labored breathing or difficulty breathing • Broken Bones • Symptoms of bloat Examples of non-emergencies include but are not limited to: • Runny nose • Sneezing/Coughing • Failure to eat or drink • Constipation • Lethargic • Diarrhea • Small cuts and abrasions

I am aware that a foster dog may have tested heartworm positive and if so, will require heartworm treatment while in my care. I guarantee that said dog will receive this treatment and all veterinary instructions will be followed and upheld. [ ]
6.
I agree not to abuse or neglect my foster dog(s), and to return said dog to Carolina Boxer Rescue, Inc. or its agent in the event I can no longer foster or properly care for said dog. [ ]
7.
I agree to keep my foster dog(s) as an indoor foster and to provide a fenced yard or exercise area, proper shelter, food, water, crate and socialization. [ ]
8.
I agree to obey all leash laws and to always leash when my foster dog when not in a fenced area or indoors. [ ]
9.
I agree to restrain my foster dog when riding in an open vehicle. Dogs may NOT ride in the back of a pick-up truck. [ ]
10.
No dog may be euthanized or destroyed while in my care without consent of all directors. [
11.
I agree that I am responsible for my foster dog’s actions while in my care. [
12.
I agree never to chain or tether my foster dog without supervision. [
13.
I agree to allow Carolina Boxer Rescue, Inc. or its agent examine my foster dog in my home at any time. [ ]
14.
I agree to provide Carolina Boxer Rescue with updates on the dog’s personality, health and behavior weekly until said dog is adopted. Updates and notification should be submitted via our Volunteer website: http://www.carolinaboxerrescue.org/home [ ]
15.
I agree to notify Carolina Boxer Rescue at least 3 week in advance if I move or if I change my telephone number. Failure to do so may result in the breach of this contract. [ ]
16.
I agree not to change my foster dog's name if the name indicated on this contract is a name the dog already answers to. [ ]
17.
I agree to keep a Carolina Boxer Rescue, Inc. tag and rabies tag affixed to the dog's collar at all times. [ ]
18.
I agree that should my foster dog become aggressive towards another dog in the home or towards people, that the dog will be immediately confined to a crate. I understand that fosters may become aggressive with the pets in the home. Fosters are under a lot of stress initially and may behave in a manner unlike their norm. Don’t hesitate to ask for advice. If a foster dog is aggressive to the point of biting a person I must report this to a CBR Director immediately. [ ]
19.
I understand and agree to Carolina Boxer Rescue, Inc.’s policy that no dog is to be corrected physically. Swatting, smacking, kicking, jerking, or any other such physical correction is strictly prohibited and will result in the immediate return of the dog to a CBR agent. Prong, choke and ecollars are forbidden unless written approval is given and the tool selected is needed to manage the dog. [ ]
20.
I agree to properly inform CBR (webmaster, directors etc) of the dog’s progression and personality for the dog’s page on the CBR website, and to provide digital photos for the website as this is what gets the dog attention and gets it adopted. [ ]

21.
The foster home will not be compensated for expenses incurred in the normal daily care of the animal. Expenditures incurred by the foster family other than emergency care and treatment require prior written approval by one of the rescue directors. The foster family will be held responsible for unapproved expenditures. Carolina Boxer Rescue, Inc. will pay for pet sitters or kennels for the foster dog only, if the foster must be out of town. [ ]
22.
I understand the foster home will not be reimbursed for vet care unless pre-approved by one of the directors. Some vet costs and procedures may be over and beyond what is normally paid or required by the rescue. These need to be approved by the directors before incurred. [ ]
23.
I release Carolina Boxer Rescue, Inc. of any damages to my property brought on by the behavior of the foster dog. Crate training is required as is heavy supervision. An unattended, anxious or bored dog may become destructive towards physical property such as furniture, curtains, and outdoor items. [ ]
24.
I assume all risk of injury or harm to myself, my family members, my guests or visitors, and agrees to release, indemnify, and forever discharge Carolina Boxer Rescue Inc., it’s directors, volunteers, and agents from and against any and all liability, claims, or demands for personal injury, sickness, death, property damage and expenses; including without limitation attorney fees and court costs; and any and all other liabilities of any nature whatsoever which may be incurred by foster dog. [ ]
25.
I agree that my foster dog is to be adopted to its permanent home only under the authorization of the CBR adoption coordinator. You may be needed for answers to specific questions asked by the approved adoptive family prior to the dog’s adoption. If so, honest and timely answers are expected & please let us know what was said. [ ]
26.
I agree that should I want to adopt foster dog, I will sign Adoption Contract and pay any adoption fee that has been negotiated in a timely manner. [ ]
27.
I agree to and acknowledge that as a Foster Volunteer, I have no authority to make sole decisions regarding a foster dog, to move, relocate, adopt or transport said dog, nor do I have permission or authority to make promises to potential adopters of said dog. [ ]

and correct and that Carolina Boxer Rescue, Inc. or it’s agent will not be held liable or chargeable for any false information, or any misrepresentations, that I have submitted on this form.
