Navigator Nook The Care Management Program enables people living with MS to stay connected to the world and move their lives forward. The National MS Society, Arizona Chapter developed the Care Management Program to address the challenges of people living with multiple sclerosis. The goal of the Care Management Program is to promote enhanced quality of life and to help people living with MS achieve the highest level of independence and autonomy. The services provided by the Care Manager vary depending on each individual’s goal, but may include: individual care management, advocacy, navigation of community services, disease education, referral and emotional support.
