FLYIT Simulators Inc. delivers AS350B3 cockpit specific professional helicopter simulator to hospital wing At Heli-Expo 2011 in Orlando Florida - FLYIT Simulators will be presenting Hospital Wing of Memphis, Tennessee the title to their new FLYIT AS 350 B3 Professional Helicopter Simulator. The Hospital Wing AS 350 Simulator will be giving demo flights at the FLYIT Simulators booth at HELI The AS 350B3 Professional Helicopter Simulator (PHS) was built based on Hospital Wing actual Instrument panel including the VEMD. The AS350 PHS features a dual G430W & Auto Pilot along with the other instrumentation that comes standard to the AS 350B3. Hospital Wing operates several AStar AS 350B3’s equipped to transport critically ill and injured patients. Hospital Wing is a non-profit air medical transport which provides inter-hospital transfers as well as emergency scene calls within 150 mile radius of Memphis.

This area includes West Tennessee, Arkansas, Mississippi, Missouri, Alabama, and Kentucky. Hospital Wing aircraft are configured to carry a flight crew of three and one patient. FLYIT Simulators was pleased to have been selected to work with Hospital Wing in providing a Flight Simulator that would mesh well with Hospital Wing’s training program. FLYIT’s AS 350 PHS will provide Hospital Wings flight crews a platform to maintain VFR, IFR, and emergency procedures skills through scenario based training to a very high level.

Van Horn Aviation announces expansion to accommodate manufacturing of main rotor blades TEMPE, AZ—Composite rotor blade manufacturer Van Horn Aviation (VHA) recently signed an offer to purchase on a 23,000+ sq. ft. facility in Tempe, Ariz., significantly expanding the company’s available manufacturing space compared to its current location. According to VHA president James R. Van Horn, the expansion is required to accommodate current and future rotor blade programs. “With in-house manufacturing of the 206 and UH-1 tail rotor blades while also designing and prototyping composite main rotor blades, we’ve outgrown our current leased facility,” said Van Horn. “Over the past six months VHA has added staff and equipment to meet increased demands for our tail rotor blades, plus we obtained a 25foot autoclave to accommodate main rotor blades. We’re already out of room and must be established in the new facility before we start main rotor blade production next year.” VHA’s current facility houses three machining centers, an automated fabric cutter, water jet, clean room, paint booth, inspection center, autoclave, industrial oven, and assorted design

and test equipment. The new facility will allow VHA to run simultaneous tail and main rotor blade production lines while continuously developing new products. “Our goal is to accomplish the move this summer with minimal impact to the tail rotor blade production schedules while also moving forward with main rotor blade flight and fatigue testing,” said VHA general manager Dean Rosenlof. “We have a long list of blade programs we intend to tackle over the next ten years, and this new facility will provide a good launch pad for new developments far into the future.”

Direct operating costs have been higher due to the frequency of Compressor Case removals as a result of cracking, lifting and departure of this inferior plastic lining formula. the challenge: In order to substantiate a valid alternative to the OEM outdated plastic case lining, AvidAir set out to design a lining that would utilize modern day Carbon Fiber Composite Technology. Carbon fibers are classified by the tensile modulus of the fiber. Tensile modulus is a measure of how much pulling force a certain diameter fiber can exert without breaking.

With this technology in mind, as a Compressor Case endures thousands of thermal cycles, our challenge was to reinforce this resin matrix to withstand these (pulling force) cycles, thus increasing the life of the lining.

the solution: In the spring of 2010, our team launched a long awaited Carbon Fiber Composite Compressor Case Lining test program. After months of design, formulation and production tests, AvidAir employed a well respected OEM engineer that had extensive compressor case plastic lining experience to review our data, write our test program and prepare for FAA Certification. Sophisticated thermal cycle testing (1750 cycles) and accelerated thermal cycle testing along with on aircraft field testing were prescribed and executed. The solution became clear.

the results: The results are a next generation, AvidAir proprietary, high performance carbon fiber reinforced composite lining that proves to be revolutionary. There are currently several units successfully operational in the field. Our goal was to provide a superior fatigue resistant composite lining that performs out to the 1750 hour inspection interval with reduced crack propagation. North American surveillance systems offers new simulator training McCourt, NASS President. The NASS simulator will enable pilots and operators to “train to proficiency”. “This means that each student can receive the amount of instruction necessary to become proficient with the system on the ground, saving costly flight time. 200 Amp generator approved for the Soloy sd2 OLYMPIA, WA - Soloy Aviation Solutions announces the approval and availability of a 200 amp generator for installation on the Honeywell LTS101-700D-2 engine used in the Soloy AS350SD2 engine conversion kit for AS350B2 helicopters. It is approved as an option for the standard 150 amp unit on the existing engine conversion STC (SR01647SE). The STC optional kit is based on the use of a Skurka Aerospace 200 amp Starter Generator. The approval provides 200 amps without affecting the existing performance at altitudes and outside air temperatures where the operation is already torque limited. It is only necessary to ensure that power checks result in a “plus” power available, i.e. T4 allowable is higher than T4 recorded. The only limiting factor is that at Ng of less than 90%, the maximum draw should be 150 amps or less. Soloy is utilizing Skurka Aerospace to manufacture the Soloy STC’d starter/ generator at Skurka’s Camarillo,

California facility. The catalyst for developing a higher amperage generator is a need to power more electrical equipment onboard today’s helicopters. Dave Stauffer, Soloy’s CEO has heard from specific operators

The first Pilot of the Year Award, HAI’s oldest honor, was presented for 1960 to Don Carson of Petroleum Helicopters, Inc. “Since helicopters first flew, this industry’s members have been using exceptional skill and dedication to aid and support the public greater good,” HAI President Matt Zuccaro said. “The latest honorees continue that outstanding tradition. We and our award sponsors applaud their efforts, which go above and beyond.”

This award honors “an outstanding single feat performed by a helicopter pilot during the year or extraordinary professionalism over a period of time.” Sikorsky Humanitarian Service Award: The crew of U.S. Coast Guard Helicopter 6605, Air Station New Orleans, La.—Lt. Cdr. Thomas G. Hickey, aircraft commander; Lt. Cdr. Craig Murray, copilot; flight mechanic/hoist operator and Avionics Electrical Technician 2nd Class Scott Lloyd, and Aviation Survival Technician Chief Petty Officer Kurt Peterson (rescue swimmer). This Eurocopter MH-65C was the first to arrive at the Deepwater Horizon drilling rig after its explosion April 20, 2010 in the Gulf of Mexico; its crew assumed on-

scene command of rescue and recovery efforts there. After an initial search, 6605 lowered Peterson to the Offshore Supply Vessel Damon B. Bankston, where he took charge of a makeshift medical team and triaged 115 survivors. The crew remaining on 6605 flew five of those survivors to safety. This award honors those who best demonstrate the value of helicopters to society by saving lives, protecting property and aiding those in distress. Bell Helicopter Lifetime Achievement Award: Michael C. “Mike” Hurst. Currently chief pilot for PHI, Hurst has over his 40-year career left an indelible mark on the world of aviation, aeronautics and safety management within the U.S. and around the world. This award salutes excellence in management and leadership and is presented to an individual who is recognized for long and significant service to the international helicopter community. Eurocopter Golden Hour Award: Capt. Daniel Aufdenblatten of Switzerland’s Air Zermatt, rescue specialist Richard Lehner and Capt. Sabin Basnyat of Nepal’s Fishtail Air. On April 29, 2010, a newly formed helicopter rescue team of Air Zermatt and Fishtail Air carried out what was arguably the highest longline rescue in history, evacuating three climbers from 22,800 feet (6,950 meters) up on Nepal’s Mount Annapurna. This award recognizes the efforts of those who, through a particular activity or contributions over time to the air medical industry, advanced the use of helicopters in the vital mission of air medical transport. W. A. (Dub) Blessing Certified Flight Instructor of the Year Award: Lawrence “Larry” Graves, chief pilot for AgustaWestland in Philadelphia. He is honored for providing, in the words of one nominator, “the highest standard of professional instruction to international and U.S. helicopter pilots” for more than 30 years. This award recognizes superlative contributions by a helicopter flight instructor in upholding high standards of excellence. AgustaWestland Safety Award: Keith Johnson, Airborne Law Enforcement Association. Johnson has volunteered

hundreds of hours to support helicopter safety initiatives through the Airborne Law Enforcement Association, the HAI Safety Committee and the International Helicopter Safety Team (for which he chairs the Safety Management System working group). This award acknowledges outstanding contributions in the promotion of safety and safety awareness throughout the international helicopter community. Rolls-Royce Excellence in Helicopter Maintenance Award: Aviation Maintenance Technician 2nd Class Nicholas H. Baisden, U.S. Coast Guard, Air Station New Orleans, for his exceptional leadership in guiding 72 mechanics assigned temporarily to Air Station New Orleans to support the Coast Guard’s aerial response to the April 20, 2010 Deepwater Horizon disaster. This award recognizes those who have contributed to the promotion and advancement of helicopters in support of law enforcement activities. Excellence in Communications Award: Guy R. Maher of Vertical magazine and Lanier Media. A widely known and respected aviation photojournalist, Maher has spent 30 years providing a vital and unique firsthand perspective on the industry, while tangibly promoting a focus on safety. This award is given to the individual or organization achieving the most creative and distinct dissemination of information about the helicopter. HAI is the trade association of the international helicopter community, representing more than 2,700 members in 70 nations. HAI members safely operate more than 5,000 helicopters each year, totaling 2.3 million hours of flight. SRT completes China’s rescue salvage bureau training program BAKERSFIELD, CA—SRT Helicopters, a leading provider of high-risk operational services and training for rotorcraft users and aviation organizations, has successfully completed a major contract, implemented in February 2009, to provide advanced aerial search and rescue (SAR) training for the Chinese Ministry of Transport’s Rescue & Salvage Bureau (MOT/SRB). SRT delivered comprehensive advanced training under the auspices of the U.S. Department of State for MOT/SRB pilots, crew chiefs and rescue support personnel. After the MOT/SRB acquired four new Sikorsky S-76C++™ helicopters, doubling the Bureau’s SAR fleet to eight S-76C rotorcraft, the agency requested

Now Quantum forces the mechanics to barcode in and out at the beginning and end of each step, and we know exactly how long it takes to complete each work order. This tracks our labor hours and allows us to more accurately quote customer projects.”

Unmanned k-max achieves numerous “firsts” during recent demonstration flights Yuma, AZ—Kaman Aerospace Corporation, a subsidiary of Kaman Corporation [NASDAQ GS: KAMN] and the U.S. Army Natick Soldier Research, Development and Engineering Center (NSRDEC) recently made aviation history with the Unmanned K-MAX helicopter by successfully completing multiple guided airdrops via sling load at 10,000 ft above sea level. In four separate flights conducted January 24-25, 2011, at the Army’s Yuma Proving Ground near Yuma, Arizona, the K-MAX successfully airdropped 16 payloads; 10 GPS guided Joint Precision Aerial Delivery Systems (JPADS), two of which were triggered remotely from the Unmanned K-MAX ground control station. Payloads included medical equipment, food, simulated leaflets and bulk cargo. Among the “firsts” achieved by the optionally piloted aircraft: • Largest payload, 4,400lbs, airdropped via sling load from a helicopter (four 1,100 lb payloads). • The highest altitude for payloads airdropped from a sling load (10,000 ft above sea level).

ntsb reports NTSB Identification: *CEN11CA152* Date: January 01, 2011 Location: Albert Lea, MN Aircraft: EUROCOPTER DEUTSCHLAND GMBH MBB-BK 117 C-2 Injuries: 3 Uninjured. The helicopter emergency medical services (HEMS) pilot stated that he had completed a pre-flight inspection at the beginning of his shift. He noted that earlier in the day, prior to the accident flight, he verified some fuel control settings at the request of one of the mechanics. Both of these required that the engine cowling doors be opened. The pilot stated that prior to the accident flight, he “visually checked for security of doors and cowlings, leaking fluids, tie downs, and overall condition of the aircraft.” He reported that after starting

the number one engine, the flight medic commented that she heard an “unusual rattle.” The pilot subsequently asked her to check the security of the cowling door latches. When the flight medic returned, she informed him that the latches appeared to be secure. The pilot stated that the flight proceeded without incident until about one-half mile from the destination helipad, when he heard a “loud bang and thump from the rear of the aircraft.” From that point in time, a “four per rev[olution] vibration had developed.” He recalled thinking that due to the noise and vibration something might have separated from the rotor system. He elected to continue the approach to the destination helipad and subsequently landed without further incident. A post accident examination

revealed substantial damage to all four main rotor blades. The lower portion of the left engine cowling door was separated. No anomalies were observed with respect to the airframe side of the cowling door latches. The separated portion of the door was not recovered. The helicopter was being repositioned in order to pick-up a patient, and only the pilot and medical crew were on-board at the time of the accident. NTSB Identification: *ERA11WA120* Date: January 14, 2011 Location: Ocho Rios, Jamaica Aircraft: BELL 206B III Injuries: 3 Uninjured. On January 14, 2011, about 1600 eastern standard time, a Bell 206B III was substantially damaged during a forced landing in Ocho Rios, Jamaica. The pilot and two passengers were not injured. Visual meteorological conditions prevailed for the flight that departed Kingston Jamaica, destined for the Rooms Hotel in Ocho Rios. According to information obtained from an official with the Government of Jamaica, the helicopter was on short final approach to the Rooms Hotel, about 50 feet above ground level, when it experienced a total loss of engine power. The pilot performed an autorotation to a grass area on hotel property; however, the helicopter slid on the grass and struck a coconut tree. The helicopter sustained damaged to the nose section, both main rotor blades, and the skid. The helicopter was equipped with a Rolls Royce (Allison) 250-C20B engine. The engine compressor was retained for further examination. This investigation is under the jurisdiction of the Government of Jamaica. NTSB Identification: *ERA11LA116* Date: January 15, 2011 Location: Stuart, FL Aircraft: SCHWEIZER 269C-1 Injuries: 1 Uninjured. On January 15, 2011, about 1705 eastern standard time, a Schweizer 269C-1 was substantially damaged after it became airborne and rolled over on the ramp at Witham Field Airport, Stuart, Florida. The certificated private pilot not injured. According to the operator, the helicopter was involved in a training exercise with another company helicopter. After landing on the ramp at SUA, the flight instructor assisted the student pilot in setting the collective friction and the student pilot set the cyclic friction. The instructor then transferred control to the student pilot and exited the helicopter. The student pilot remained in the cockpit to allow the engine to cool down for shutdown. As the instructor was being driven off the ramp on a line cart, he observed the helicopter become airborne and roll over on its right side. A pilot witness estimated that the helicopter attained a height of 30 feet prior to the rollover. The student pilot reported that, after the flight instructor had left the cockpit, he noticed that the helicopter got ?light on [the] skids? and began a slight yaw to the right. He confirmed that the collective was all the way down and pushed slightly on the ?already stiff cyclic (because of the friction)? to avoid the other helicopter. The helicopter then climbed, rolled right, and the main rotor blades began hitting the tarmac. The engine was still running after the rollover and he shut the engine down and exited the cockpit. An inspector with the Federal Aviation Administration responded to the accident site and inspected the wreckage. He reported that control continuity was confirmed to the collective, cyclic, and antitorque controls. The friction controls operated normally. The main rotor blades sustained substantial damage


from impact with the ramp surface. The fuselage, landing gear skids, and tail boom all exhibited substantial damage. The second helicopter received minor damage from flying debris. NTSB Identification: *CEN11IA156* Date: January 19, 2011 Location: Galliano, LA Aircraft: SIKORSKY 92 Injuries: 18 Uninjured. On January 19, 2011, at 1145 central daylight time, a Sikorsky 92A helicopter experienced a flight control malfunction during initial descent for landing to Air Logistics Galliano Heliport (2LS0), Galliano, Louisiana. Visual meteorological conditions prevailed and a company flight plan was filed for the 14 Code of Federal Regulations Part 135 non-scheduled passenger flight. The three crewmembers and 15 passengers were not injured. The flight originated from an off-shore helipad in the Gulf of Mexico with 2LS0 as its intended destination. During the initial approach into 2LS0, as airspeed reduced through approximately 70 knots, the helicopter began a right yaw of more than 100 degrees that could not be stopped by application of left pedal. The crew immediately lowered the nose to regain airspeed and streamline the aircraft. They diverted to the South Lafourche Airport (GAO), Galliano, Louisiana and performed a successful roll on landing, touching down at approximately 72 knots, utilizing differential braking to maintain runway heading. An after landing inspection showed that the tail rotor pitch change beam retaining nut was missing. Recent maintenance had been performed. NTSB Identification: *WPR11GA115* Date: January 31, 2011 Location: Marana, AZ Aircraft: MCDONNELL DOUGLAS HELI Injuries: 1 Fatal, 2 Serious, 1 Minor. On January 31, 2011, about 1115 mountain standard time, a McDonnellDouglas 369FF helicopter was substantially damaged during an attempted pinnacle landing on Waterman Peak near Marana, Arizona. The pilot received fatal injuries, two passengers received serious injuries, and one passenger received minor injuries. Visual meteorological conditions prevailed, and no flight plan was filed for the flight. The purpose of the flight was to enable PCWIN personnel to conduct a site

survey for the planned installation of a communications-repeater tower. The helicopter departed Tucson International Airport (TUS), Tucson, Arizona, about 1050, with the PCSD pilot/deputy in the left front seat, two Pima County employees in the right front and rear seats, and a private contractor in the left rear seat. Initially, the flight was in communication with, and being tracked by, TUS local and TRACON air traffic control (ATC) facilities as it headed for the peak, located about 30 miles westnorthwest of TUS. The 1053 TUS recorded weather observation included winds from 300 degrees at 9 knots with gusts to 16 knots; visibility 10 miles; and a broken cloud layer at 7,000 feet. The passengers reported that during the landing attempt, the helicopter either bounced or the pilot lifted off again, the nose pitched down, and then the helicopter began to spin to the right. The helicopter tumbled and slid about 120 feet down the northeast face of the peak before it was halted by rocks and scrub vegetation. A ground-based witness located about 1,000 feet west of and below the peak stated that the helicopter completed about four or five rotations before it disappeared from his view. Federal Aviation Administration (FAA) records indicated that the pilot held a commercial pilot certificate with rotorcraft-helicopter and instrumenthelicopter ratings, and a private pilot certificate with airplane single and multiengine land ratings. According to the pilot’s personal flight log, he had approximately 11,500 total hours of flight experience, most of which was in helicopters. His first recorded flight in the accident helicopter make and model was in August 2008, and he had logged about 186 total hours in that equipment. In January 2011, excluding the accident flight, the pilot logged 6 flights, for a total of 7.5 hours, in the accident helicopter make and model. His most recent FAA second-class medical certificate was issued in February 2010. According to PCSD information, the pilot joined PCSD in November 2008, and had about 30 years experience flying helicopters for the Arizona Department of Public Safety and the Maricopa County Sheriff’s Department. The helicopter was manufactured in 1998, and was registered to Pima County in 2008. The helicopter’s most recent annual inspection was completed in April

2010, and it had accumulated about 115 hours in service between that inspection and the accident. The helicopter was equipped with an Allison (Rolls-Royce) 250-C30 series turbine engine. During the follow-up investigation, the engine was removed and prepared for a test-run. During the test run, the engine developed rated power, and engine performance exceeded minimum values for overhauled engines, and no anomalies were noted.

Q & A with Question: After performing preventive maintenance on my aircraft, am I required to make an entry in the airplane’s maintenance logbooks? If so, how should I record it?

Answer: Yes, according to 14 CFR 43.5, a maintenance record entry must be made by the person approving the aircraft for return to service. Section 43.9 requires the logbook entry to include a description of the work performed, the date the work was completed, and the signature, certificate number, and kind of certificate held by the person approving the work. Overall the aviation insurance market is flat. While there isn’t a single underwriter who does not want to raise premiums, over capacity and the sluggish economy are keeping rates in check. In addition, the poor performance of other Property and Casualty segments of the industry appears to be dampening any tendency by the reinsurance market to move money to other market segments. Still, as reinsurance agreements are negotiated, those underwriting companies who are operating in the red could see a rise in the rates they pay which will likely impact consumer rates. Last year saw what may be the beginning of some consolidation which is necessary to reduce the number of markets in the aviation segment. One major company left the aviation market all together and another sold its aviation

Insurance Outlook for 2011 – Are Rates on the Rise? book to an existing underwriting company. This eliminated the two most aggressive underwriting companies as regards to pricing. However, that was offset by a couple of underwriting companies either re-entering the marketplace or expanding their underwriting appetite. The major difference between 2011 and 2010 is subtle but significant. While underwriters are willing to do what it takes to keep their preferred clients – those with good loss histories, strong flight departments and excellent risk management practices—they aren’t pursuing new business opportunities as aggressively as they have in the past. Simply stated, they have stopped chasing business they have little chance of keeping.

Again, they are willing to competitively bid on the preferred operators, but will simply pass on risks that would have to be priced aggressively in order to win. The only exception to this seems to be the preferred operator who is looking for a better home and long term relationship. Commercial helicopter and EMS operators saw a significant premium increase last year driven primarily by the segments’ loss history. That trend could continue for other than the preferred operators. Municipal and government operators appear to remain competitive. Owner-flown turbine and piston helicopters appear flat at this time. Like EMS operators, ag operators are seeing rate increases driven by losses in

the past couple of years. Those operators with poor loss histories could see fewer choices and 20 to 30% increases at renewal. The “preferred risk” –those operators flying turbine aircraft, with good risk management practices and very few small losses over the last several yearscould see at or near “expiring terms”. One anomaly is the “pristine risk”—the best of the best. They benefited most from the soft market with several new entrant underwriting companies outbidding one another for their business. Like the rest of the industry segments, the underwriters have quit chasing business and are no longer offering terms “at a loss.” The small rate increase is seen as a market correction to get premiums more in line

with what a “pristine risk” should be, which is still considered a preferred rate. A key indicator of a return to a hard market could be in pilot training and pilot qualification requirements as the underwriters try to stem losses through tighter risk management requirements. However, this will likely be accompanied by a modest rate increase. The bottom line is, if the economy improves there is a likelihood that premiums could start a modest rise. Even without a single triggering event or the reinsurance market tightening, all the ingredients for higher rates are in place. It may be prudent for all aviation operators to budget accordingly.
