A B C A) first 3 numbers are the school code, B) next 2 numbers are the year of graduation, and C) the remaining 4 numbers are a member’s ID code. Note: For dentists graduating in 2006 and later, ADA numbers are randomly assigned.
Membership Records: This directory captures membership records and status through August 6, 2013. Payments received after this date or alterations made to a membership status, eg, regular membership to life membership or life membership to retired life membership, will not be reflected. With regard to the DENPAC status of members, only contributions made between September 1, 2012 – August 23, 2013 are reflected. Addresses and Phone Numbers: The directory publishes office address and phone numbers when possible. Some members have only provided their home address and have not provided the TDA with their home phone numbers. Consequently, the directory does not list a phone number for them. Alphabetization of Names: The names in the directory are sorted using a letter-by-letter system in which spaces and other characters are ignored. For example, “Dahse” precedes “D’Alfonso.” The abbreviation “St.” is alphabetized as “Saint.”
Definition of an Interim Member: A dentist who has completed the application with dues payment and meets the qualifications for active membership in this Association, shall become an interim member of this Association until the selection process by the component society is completed. Interim Members are designated with the symbol of =.
