Rhinoplasty (Nose Job)

Science says your ears and nose never stop growing, which means as the candles on your birthday cake multiply so do the centimeters sitting front and center on your face. The actual size increase is minimal, but a droopy tip that results in an apparent dorsal hump is a common side effect of aging. If a new shape is what you seek, so be it, but if you’re focused on a simple, youthful appearance, stick to these age-defying adjustments. “Although historically, rhinoplasty has been an operation for young women, the aging nose needs to be fixed along with the aging face. The biggest complaint is: my nose looks bigger and is drooping, just like other things on my body. There is a loss of support to the nasal tip as well as some growth of the nose as the years pass, so now the request is for better support of the nasal tip and for reduction in size of the nose as a whole.” Mitchell Blum,

As you age, gravity has a tendency to rear its ugly head, which can result in an overall “sagging” appearance. Start with your eyes. As the years tick by, eyelids and brows both begin to droop. There are a couple of viable solutions for the excess skin on your lids. Breast Augmentation

Whether gravity is getting you down, or years of childrearing have changed your lady parts, there’s no need to live with the results. A breast augmentation has many purposes—reducing size, increasing size, or simply giving the girls a lift. If youth is the goal, go perky—just not obnoxiously so. And while boob jobs are often synonymous with a boost in size, the opposite could hold the key to the fountain of youth. After all, your boobs were likely a cup size or two smaller when you were younger. “A breast lift, or mastopexy, is commonly recommended in addition to augmentation to create a more youthful contour as well as enhanced size.” –Thomas McNemar, MD, McNemar Cosmetic Surgery

Abdominoplasty

(Tummy Tuck) This cosmetic procedure tends to go hand-in-hand with weight loss. The ultimate goal is to look slimmer, right? Sure. But it goes beyond that. Here’s the skinny: When women give birth, their abdominal muscles can separate, causing their stomach to look bigger. They can also develop softer bellies and looser skin. A tummy tuck can correct these unsightly issues, resulting in a smaller, firmer belly. A modified abdominoplasty can even address an unattractive postpartum belly button, (just make sure it’s not a hernia first).

Stretch Mark Removal

Defined as streaks or stripes on the skin (often red or purple in color at first but fade to white or silver with time), stretch marks are a common side effect of pregnancy. Laser treatment can, however, reduce or eliminate the appearance of these marks. Several sessions are often required.

Vaginaplasty

If you had a natural birth, your vagina underwent some substantial physical trauma. Many women aren’t bothered by their postpartum lady parts; however, if you are, there’s a surgery to address your concerns. This cosmetic procedure—also often used to treat related cancers—reconstructs the vaginal canal, its mucous membrane, and damaged vulvo-vaginal structures. The result? A kitty that purrs like it used to.

Knee Lift

As elastic fibers, muscle mass, and collagen leave your body at an alarming rate, knees begin to droop. If you’ve maintained face and torso with cosmetic procedures, don’t let knees give away your real age! Instead, opt for a knee lift, which not only raises the knee itself but also reduces the appearance of wrinkles and skin folds. This particular procedure can be hard to come by, and is not quite as common as the other contenders on this list.

Varicose Vein Removal

Varicose veins (usually most apparent in the legs and feet) are gnarled, enlarged veins that often appear blue, purple, or red in color. Spider veins are a less serious version of the condition. While for some these veins cause actual discomfort, they are usually a cosmetic concern. The real zinger is that there isn’t much to be done in terms of prevention (they are literally the result of pressure put on your lower body from standing and walking upright). Sclerotherapy, injections that collapse the veins, is a viable treatment, as is laser treatment.

Babies are born with the natural urge to suck. They may suck their thumbs, fingers, fists, or other items like pacifiers and blankets. Around six months of age this urge tends to decrease, but many children continue to suck their thumbs as a way of achieving comfort when they are hungry, tired, afraid, or bored.

Razi, a pediatric dentist who practices in Tracy, this behavior in young children (less than four years of age) should not be cause for concern. “Sucking thumb is a normal behavior for babies and young children, therefore early intervention with mouth appliances is usually not recommended,” she explains.    Problems related to thumb sucking don’t generally occur until a child is four to six years old. If intense sucking (often characterized by a callus on the thumb) is still a frequent habit at this age, it puts them at risk for speech or dental problems. When a child sucks their thumb, the teeth can become progressively 20

misaligned. The position the thumb sits in the mouth can also push teeth forward. It should correct itself, at least partially, when the habit stops, but permanent damage that requires orthodontic treatment may occur when prolonged thumb sucking is at play.    In terms of speech, a child who sucks their thumb may push their tongue out too much when they talk, develop a lisp, or struggle to make “t” and “d” sounds, requiring speech therapy to correct. If your child starts displaying dental or speech complications, it’s time for intervention.    “Parents can encourage [children] to stop by a reward system for the time not engaged in the habit,” Dr. Razi suggests. Providing distractions, creating appropriate time periods for thumb sucking (maybe just before bed), or placing adhesive bandages or gloves on your child to deter the habit can all work. It is important, however, that parents not shame their child for the behavior as that can negatively affect self esteem and lead to other issues. Instead, use positive reinforcement, and reward your child when they don’t suck their thumb.    When children are older, explaining the risks may help deter them. “For older patients, there are children’s books to help them see the possible effects of sucking: open bite, vaulted palate, [and] protrusion of maxillary incisors,” Dr. Razi says. If the habit continues after the permanent six-year molars are in, a mouth appliance can be used.    In some cases, older children who suck their thumbs may be exhibiting signs of emotional distress or an underlying disorder. Stop and think if there are any stresses that could be affecting your child’s emotional state. If you suspect your child may be experiencing anxiety, check with your pediatrician.

You’ve just given birth to your brand-new baby, and you should be on cloud nine, but what if you can’t stop feeling down? Your hormones are waging war on your body, you’re still recovering from the birthing experience, and you aren’t quite feeling yourself. While many women experience a range of

emotions in the weeks post-birth, there could be more to your feelings. If you can’t quite shake the funk, consult your doctor. You may be suffering from Postpartum Depression (PPD).   A Centers for Disease Control (CDC) survey found that 8-19 percent of women suffered from frequent postpartum depressive symptoms, including trouble sleeping while baby sleeps, having scary or negative thoughts about the baby, and feeling guilty about not being a good mom or ashamed that you cannot care for your baby (in addition to traditional depression symptoms). Some women are at higher risk for developing PPD than others. “Patients at risk are those with external stresses such as financial issues, lack of support, abusive relationships, and previous history of postpartum depression,” says Ben Rodgers, OB, MD, at San Joaquin General Hospital in French Camp.    So what can you do? Bonding with your baby is one way doctors suggest treating PPD. “Staying close to the baby and especially breastfeeding when you have PPD helps to release a natural hormone called oxytocin. Oxytocin helps with bonding with the baby and developing warm, loving emotions toward your baby,” says Dr. Eugenia Hurlbut at Gill OB/ GYN Medical Group, Inc. in Stockton.   The first step to recovery is diagnosis, and the earlier a mother can get help, the better chance she has to overcome PPD. While in the hospital, patients are assessed during the immediate postpartum period in daily rounds. During this time, hospital staff will pay special attention to potential stressors that could trigger postpartum depression. Patients are also screened for PPD risk during their pregnancy, according to Dr. Rogers. A short follow-up one or two weeks after discharge is often used to reassess.

WISDOM TEETH REMOVAL There’s a reason why we have the old phrase “Like pulling teeth.” No one enjoys it, and if they tell you otherwise they’re lying… through their teeth. Or maybe lack thereof. But when it comes to your wisdoms, sometimes you’ve just got to roll with the punches, especially when the compounding pressure on your pearly whites wreak havoc on your oral wellbeing.

So when is the right time to schedule your appointment? Truth be told, it varies for all! We turned to Dr. Suzuki Yusuke of Suzuki DMD & Associates to find that answer. “Wisdom teeth, also known as third molars are the last adult teeth to come in, usually between age 17 and 25. Most people have four (one in each quadrant) but some people never see their wisdom teeth, either because they are fully impacted and never erupt, or they were born without them. 26

Frequent checkups with your dentist will help to decide the right time to schedule your appointment, but your late teens to early twenties is best, due to the increasing density of the bones in your mouth. The harder they are, the more work it takes to remove the teeth. When it comes time to have those four furthest molars extracted, your dentist will refer you to an oral surgeon for the procedure. As Dr. Suzuki explains, “Oral surgeons may hold an MD degree in addition to their dental DDS or DMD degree. Oral surgeons specialize not only with extractions but also with placement of dental implants and maxillofacial surgery.”    Your oral surgeon will determine the type of anesthetic needed by the work necessary to remove the teeth. “Treatment can be done with local anesthesia only, but most patients elect to have treatment performed under sedation or general anesthesia, so getting all [four teeth] removed at once makes sense.”

Once the procedure is complete you’ll continue feeling a little groggy for the next few hours. Follow the directions your oral surgeon provides to minimize any bleeding, swelling, and pain you’ll experience from the extraction. Have a friend or family member drive you home and expect slight discomfort over the next few days. Call your dentist if you experience excessive bleeding, fever, or if the swelling does not subside after the first few days. Your recovery time should last no more than a week before you’ll be ready to head on out and back into the world!

Staving off heart disease with preventative care  BY NORA HESTON TARTE

Heart disease was ranked the leading cause of death amongst Californians in 2013. In San Joaquin County alone, an average of 696.3 people died each year from the disease. So how can you save yourself? Preventative heart attack care is key.

Heart attacks are caused by plaque (hardening of the arteries) that is present in the coronary arteries and becomes unstable, leading to an acute obstruction of the blood vessel. This means blood flow is interrupted and the heart muscle essentially dies, resulting in a heart attack, or myocardial infarction.    A person’s medical history as well as lifestyle can contribute to the likelihood of having a heart attack. “There are several factors that we consider to help evaluate a person’s heart health and risk for developing heart disease,” explains Samira Jahangiri, MD, family practice physician with Dignity Health Medical Group Stockton. “We check for high cholesterol, high blood pressure, diabetes, and a family history of heart disease. We also look at lifestyle factors including whether or not the patient exercises regularly or uses tobacco.”    “People who consume diets that are low in fresh fruits and vegetables in addition to excess amounts of unsaturated fat are at risk for developing plaque in the coronary arteries,” says Steven LaViola, MD, at Lodi Health. “People who are sedentary are also at risk.”   Without the ability to control genetic markers that increase susceptibility, the most important step is to take care of yourself, especially when high blood pressure and diabetes is involved. “The presence of any of these indicators alone can mean that a person is at risk to develop heart disease, and the presence of multiple risk factors increases this possibility exponentially,” Dr. Jahangiri points out.   Aside from potential medical interventions, the treatment is the same—healthy diet, healthy lifestyle. So what does living a healthy lifestyle entail? Put simply, drink lots of water, get plenty of rest, exercise, meditate, eat healthy—including lots of fruits, vegetables, and brightly colored foods, — reduce stress, don’t smoke or hang around others while they smoke, get outside, and take care of your teeth. 28

How to make orthodontic investment successful  BY NORA HESTON TARTE Braces are a BIG investment. Not only do they cost a pretty penny, parents who pay for their childrens’ braces are making an investment into their future, setting them up for success and positive oral health later in life.

Science shows that a healthy smile can give children confidence, as well as help them to make a positive impression on others. Aesthetics aside, proper oral care at a young age leads to better oral health as an adult, especially when steps are taken to maintain your mouth.   Cameron Wheeler, DDS, of Wheeler Orthodontics in Stockton, says a successful orthodontic investment can be broken down into three steps. First, choose the right place. “I think there is a misconception held by many that all orthodontic treatment must be equal,” he says. “Not all braces are the same. Not all practitioners are the same.”    What many parents don’t realize is that orthodontists take two to three years of additional training after dental school to master their specialties. “In essence, they learn and perfect the tools and techniques to accurately diagnose and treat any case… to ‘get the job done right.’”

Secondly, children and parents must follow a practitioner’s directions to achieve the best results. Regardless of the request, the orthodontist is the professional and their advice comes with merit. From making all scheduled appointments to following good oral hygiene, the suggestions they make lead to a healthier, happier mouth. “Poor hygiene causes inflammation,” Dr. Wheeler says. “Inflammation can result in drastically slower tooth movement and can accelerate damage to adjacent soft and hard tissues which can compromise the result.”

Finally, review all payment plans and expectations with your orthodontist’s office. Hidden fees and al la carte services can rack up big bills. Records, exam and diagnosis charges, treatment that reaches beyond the contract terms, and fees for late appointments, broken appliances, emergency visits, and premium up sells all add up. To make orthodontics as affordable as possible, some practitioners offer monthly payment plans, and will even forego interest.    After care it’s still important to continue on the path laid out by your orthodontist. Step two doesn’t end when your braces are removed. Continued use of retainers, proper brushing and flossing, and regular dental checkups are all required to preserve the results you’ve achieved through orthodontics. Shift happens when you don’t follow directions. Staying on top of your oral health is a lifelong need.

right for you?  BY NORA HESTON TARTE When you’re contemplating going under the knife, there is a lot to consider. From a breast augmentation to a rhinoplasty, choosing to have an elective surgery is a big decision. Consider the following before signing on the dotted line; it could bring added confidence to your choice, or help you realize why waiting is a better option.

Are you a good candidate?

This is the most important question to ask before undergoing an elective cosmetic procedure. Good candidates for cosmetic surgery are overall healthy, understand the risks associated with the procedure, and have realistic expectations. Making a sound decision means you’ve considered both why you are having the procedure, and what to expect afterward. A reported 20 percent of people who undergo cosmetic surgery end up disappointed with the results. While some of these can be chalked up to botched operations, many are the result of unrealistic expectations, or underlying mental conditions like body dysmorphia.   Talking at length with your doctor—and seeing a credible surgeon—can help shape practical expectations. If the procedure will not reap the right results, contemplate other options. Surgery is a lot to endure only to be displeased in the end.

Additional preparation

Before surgery, most doctors recommend at least two to four weeks of no smoking, as well as an additional two to four weeks post-op. This also applies to secondhand smoke. So, if your spouse tends to light up inside, consider staying elsewhere for a few weeks.    In the days leading up to surgery some suggest dying your hair if needed (which is not recommended after a facial surgery), not shaving to reduce risk of infection, taking an herbal supplement to reduce post-op swelling (consult your doctor first), and preparing your support system. Anyone who has a surgical procedure needs aftercare in some form. Doctors will require a driver to transport you to and from your appointment. 34

When to say “no”

Those in poor health may not be good candidates for cosmetic surgery. “Elective cosmetic procedures should not be done when a patient is either medically or emotionally unstable,” advises Kimberly Cockerham, MD. “As an example, sudden loss of a partner or job shouldn’t be treated with an invasive surgical procedure.” If a serious health condition such as diabetes, high blood pressure, a bleeding disorder, heart disease, or depression is underlying, consult your doctor. They may recommend forgoing the procedure all together, or at least waiting until you are healthier.   Certain medications—over the counter and prescription—can lead to complications during surgery, as well. Always be 100 percent honest with your surgeon about the medications you are taking, and heed their advice if discontinuing something is recommended or if surgery needs to be postponed due to use. Non-steroidal anti-inflammatory

medications, such as Aspirin, Motrin, Ibuprofen, and Advil are typically not advised. It is possible to be a good or bad candidate for a specific surgery, as well, which is why getting at least one professional opinion is important. For example, someone with an abnormal dental bite that requires jaw realignment is not likely a good candidate for a chin implant. Weight, lifestyle, age, and other factors can all determine how a specific procedure will affect you.

This summer Dignity Health’s St. Joseph’s Medical Center in Stockton proudly opened a new state-of-the-art cardiac hybrid suite, a first in San Joaquin County. The hybrid suite is a combination

room that includes the technology of a cardiac catheterization laboratory and an operating room. As Joann Marks, Cardiovascular Service Line director explains, the suite is designed for cases that are minimally invasive, or of higher complexity, requiring highly specialized imaging equipment and surgical techniques.    For patients that are at high risk, or too sick for open heart valve surgery, the suite is a true game-changer, as it supports trans catheter aortic valve replacement (TAVR). “Instead of traditional open heart surgery, where the heart is exposed, we go through a catheter using the groin (one of the three access sites for that procedure). With this procedure the recovery time is much less, and has an average length of two to three days hospitalization, whereas open heart surgery hospital stay is at times seven days,” Joann continues, “Because this procedure is minimally invasive, high risk patients who would not be eligible for open heart surgery, can have their valve replaced and have an opportunity to live many more years of quality life.”    The hybrid suite brings together cardiologists, cardiac surgeons and staff from both the operating room and cardiac catheterization lab, in a true collaboration between independent departments. “The benefit of having a hybrid suite is that we can start all procedures as minimally invasive as possible, while still having all of the tools and staff available to support a more invasive procedure, if needed,” Joann explains. The St. Joseph’s Medical Center Foundation led fundraising efforts to bring this state-of-the art option to San Joaquin residents. “There are so many people that have donated,” Joann says, “from patients that have had care here to our physicians who practice here. Dr. James Morrissey and Dr. Richard Waters really took the lead to educate and campaign for funds.”    In addition to the cardiac hybrid suite, the nationally recognized Heart & Vascular Institute at St. Joseph’s Medical Center is a designated STEMI heart attack receiving center, certified Chest Pain Center, and an accredited Stroke Center, truly offering advanced cardiac care to San Joaquin County.

For any ear, nose, and throat issues that stump your family doctor, head over to an ENT—a ear, nose, and throat specialist. If you are concerned about disorders of the auditory and vestibular system portions of the ear head to an AU (Audiologist) who specializes in identifying, diagnosing, treating, and monitoring those portions of the ear. And if you need to go under the knife, consider a doctor with MS after his name, meaning Master of Surgery.   When you see abbreviations with more than three letters they most likely represent an affiliation with a Medical Association. Dameron Hospital is thrilled to offer Central Valley residents suffering from Peripheral Artery Disease (PAD) the FDA-cleared Ocelot catheter system. As the first hospital in

Sacramento, San Joaquin, and Stanislaus counties to invest in and offer this state-of-the-art technology, Dameron hopes to tell a story to the community, explains Lorraine Auerbach, Dameron Hospital CEO. “We are a community hospital, we are here for the long run to serve our population…You don’t have to leave the Central Valley for quality care.” 44

Dr. Daren Primack, senior cardiologist with Pacific Heart and Vascular Group and Dameron Hospital Medical Director for Peripheral Vascular Program and chief of staff-elect, says PAD—defined as a blockage in the lower extremities—is an under diagnosed and undertreated condition. However, it leads to a lot of disability, especially for those with diabetes (diabetics, those who smoke, and the elderly are most susceptible). “We can avoid amputation and save people’s limbs with this technology,” Auerbach says.   There are several procedures available to patients with PAD including balloons, stents, shaving blockages out,

and surgery. “What was really attractive about this system was we can treat [patients] through a small puncture hole instead of cut them open,” Primack says. For elderly patients, this is especially helpful because they tend not to tolerate surgery well. “[It] gives us a new non-surgical way to relieve their symptoms.”   The recovery time is also minimal, one night in the hospital and a one-week “no heavy lifting” period. “That’s about it,” Primack says. Specifically, the Ocelot system (implemented nearly six months ago by Dameron) allows for better imaging so doctors can view the arteries in 3D—a new technology for cardiologists. “Three dimensions is so helpful because it lets you know what direction to go cross through the blockage,” Primack explains.    It’s an upgrade to the two-dimensional equipment the team was using in its cath lab. “You are looking down the artery as though you were in there,” Auerbach explains. “I saw the demonstration and said, ‘why haven’t we been doing it this way before?’”    Despite being packaged as an imaging technology, the Ocelot isn’t used to find blockages in arteries. Instead, it is used to open blocks that were previously located, through a non-surgical approach. In addition, these advances allow physicians to not rely on x-rays. “X-rays can be cancer causing. We try to use minimal at all times,” Primack explains. It also means less x-ray contrast, which can cause kidney problems for patients. “The accuracy of this treatment is great and we’ve had phenomenal results,” Auerbach says.

Whether you were blessed with 20/20 vision or dubbed “four eyes” as a child, you’re vision is likely to change once you hit 40. Presbyopia, or blurred near vision, is considered inescapable even if you’ve never experienced vision issues, or are nearsighted.   No need to worry, as presbyopia is wide-

spread and considered perfectly normal. As Dr. Joseph Zeiter Jr. of Zeiter Eye Medical Group explains, “If you notice you have to hold a menu or cell phone at arm’s length to see clearly, then this is likely the change that is occurring in you as well.”    So what causes this change? It’s an age related process that differs from other optical issues like farsightedness, nearsightedness, and astigmatism—which are factors related to the shape of your eyeball or genetic and environmental factors. “Unfortunately as with all things, as we get older the lens sus46

pended in the middle of our eye will begin to undergo changes,” Dr. Zeiter Jr. explains, “The lens begins to become more stiff, and it becomes unable to change into a sphere. When this happens people notice difficulty in seeing objects up close.”    Treatment options are aplenty and range from cheap fixes like over the counter readers that can be purchased at a drug store and worn only when needed, designer frames with progressive addition lenses, no line bifocal lenses (lenses with to point of focus), and contact lenses, to surgical options. “People often say they don’t want to wear the glasses because they don’t want to become dependent on them,” Dr. Zeiter Jr. notes, “I say, give the body what it wants. Wearing readers does nothing bad or harmful to your eyes in any other way than to allow it to see things better.”    If you already wear contact lenses, your eye doctor can custom prescribe reading

glasses to be worn while also wearing your contacts, or you can opt for multifocal contact lenses, or monovision lenses—which offers one contact lenses in a distance prescription and another for near vision. In this instance, your brain learns to favor the appropriate eye and lens for what is needed—amazing!    To discover the best option for you, visit your optometrist or ophthalmologist to ensure there isn’t a larger issue at hand, and proceed from there. As Dr. Zeiter notes, presbyopia is a natural and unfortunately inevitable change, so there is no need to worry—but regular check ups to keep your peepers pleased is a must for all.

WHAT IS YOUR

TUMMY TELLING YOU?  BY NORA HESTON TARTE Nausea, vomiting, bloating, cramping, pain – a 2013 survey revealed that 74 percent of Americans are living with gastrointestinal (or GI) symptoms. While some of these issues can be normal, or

a temporary response to diet or illness, others are cause for concern. So how do you know what’s ailing you? Listen to your stomach (and your doctor).

IBS

IBS, or irritable bowel syndrome, is an intestinal disorder that results in abdominal pain, gas, diarrhea, bloating, or constipation. More than 200,000 causes are reported in the U.S. each year, and while the condition affects those of all ages, it peaks in people ages 14-60. Diagnoses are commonly made based on symptoms, and it is considered chronic. There is no cure, but treatment like over-the-counter medications and diet changes can help. In some cases counseling is necessary.

Food Allergies

Food allergies or intolerances can present in a myriad of ways. Many people conjure up images of anaphylactic shock or an itchy rash when they consider food allergies, but symptoms can be GI related. In fact, GI symptoms—abdominal cramps, nausea, vomiting, and diarrhea—are among the most common for food allergies, however it is hard to diagnose based on these symptoms alone. GI symptoms related to food allergies can be treated with over-the-counter or prescription medications. The best course of action is to avoid the distress-causing food. The symptoms of food intolerance, as opposed to food allergy, are often the same. However food intolerance is typically much less severe and limited to digestive symptoms. This can include sensitivities to food additives, lactose intolerance, IBS, Celiac’s disease, and more. Treatment can be as simple as taking a pill before consuming a specific food, or avoiding that food altogether.

Heartburn

Heartburn is caused by acid regurgitation into the esophagus, a form of indigestion that causes a burning sensation in the chest. Gastro esophageal reflux disease (or GERD) affects 60 percent of Americans each year, and 20-30 percent experience weekly symptoms. Lifestyle and diet changes can help prevent instances of heartburn. Medications can be administered to relieve symptoms. Always check with a doctor if your symptoms are recurring; ignored GERD symptoms can turn serious.

Stomach Ulcer (i.e. Peptic Ulcer)

A stomach ulcer is a sore that develops on the lining of the esophagus, stomach, or small intestine and causes upper abdominal or chest pain. Common symptoms are indigestion, nausea, gas, and vomiting. More than 200,000 cases are diagnosed in the U.S. each year and most resolve within months. Treatment by a medical professional is necessary for diagnoses and labs and images are common. Bacteria causes some ulcers (and require antibiotics to resolve) while others are caused by stomach acid, in which case a medication that reduces acid production is needed. If pain is unusual, persistent, or not relieved by medication, consult a physician. Vomiting blood, bloody or black stools, dramatic weight loss, persistent sore throat, diarrhea, or constipation, difficult or painful swallowing, or a sensation of food caught in the chest or throat, also warrant a call.

Troubled skin seems to go hand-in-hand with the teen years. Most adolescents suffer from some

(socially) debilitating skin problem or another. Aman Samrao, MD, chief of dermatology at Kaiser Permanente Central Valley Area, offers hope. “The three most common teen skin problems we treat are: acne, eczema, and fungal infections,” she says. While identifying some of these common skin issues faced by teens (did you hear, teens? You’re not alone!), she offers solutions and tips for putting your best face forward. Read on for more.

For those with acne, prescription medication can offer relief (topical cream or pills). “We also recommend gentle face washing with mild skin cleansers, and urge patients to avoid daily use of scrubs as they can exacerbate acne,” Dr. Samrao says. PREVENTION: Preventing acne is hard, although following the regimen set up by your dermatologist may help. You can, however, limit breakouts and prevent acne scarring. “Picking at and popping pimples worsen acne,” Dr. Samrao says.

SNIP, SNIP: VASECTOMY 101  BY NORA HESTON TARTE Many men opt for a vasectomy—a male sterilization technique that dates back to 1899—as a permanent form of birth control. In fact, 500,000 men in the U.S. undergo one every year.

But what is a vasectomy? And how effective is it? Get ready for a crash course in this pregnancy preventing procedure.

What happens during a vasectomy?

During a vasectomy, an urologist seals off the vas deferens (ducts that carry the sperm away from the testes to the ejaculatory ducts). Then, sperm can’t enter the seminal stream, which effectively prevents fertilization.   To do this, the patient is given a local anesthesia via injection to numb the area. The doctor then makes one or two small incisions in the scrotal skin to access the vas deferens. The ducts can be cauterized, tied off, or clipped. Sutures or skin glue are sometimes used to hold the skin together as it heals. In all, it takes a physician 20-30 minutes to complete the procedure, and it is typically done in office. Anti-anxiety medicine can be used if a patient is feeling on edge.

How Effective Is It?

Hailed as the most effective form of birth control (other than abstinence), men who undergo a vasectomy are unlikely to get a woman pregnant. In fact, the Centers for Disease Control report less than one pregnancy per 100 women in the year following a vasectomy. It is, however, recommended that another method of birth control be used for the first three months following the procedure. About 1 in every 2,000 vasectomies will fail later because the vas deferens has grown back together.

How long is recovery?

Recovery happens at home. Strict bed rest for two days following the procedure is a common recommendation. After that, strenuous or sexual activity should be avoided for one week. An icepack or frozen bag of peas can be placed on the scrotum to reduce pain and swelling. Pain medication can be taken, but isn’t always necessary. Most men find they can return to work after two to three days.

What are the risks?

Risk of serious complication are low. Many men will experience tenderness, but that should resolve itself within a week. However, like all surgical procedures, complications can occur. Bruising, bleeding into the scrotum, and infection are among the most common short-term complications. Longterm, a tender nodule can form at the vasectomy site, and in some cases, a second procedure will be needed to remove it. Some studies have claimed that undergoing a vasectomy increased a man’s risk for developing prostate cancer and heart disease. Other studies have failed to find a link.

Is reversal possible?

A vasectomy can be reversed, and the success rate is high, however, not all reversals result in successful pregnancies. It requires another outpatient surgery—this one is typically two to four hours long and requires spinal or general anesthesia—to reattach the vas deferens so sperm can again flow into the seminal stream.   Reversals are most effective in the first three years after the original procedure. Overall, pregnancy rate is about 50 percent after a reversal. If more than 10 years have passed, the rate of pregnancy drops to about 30 percent.

If given intravenously in the first three hours after the start of stroke symptoms, tPA has been shown to significantly reduce the effects of stroke and lessen the chance of permanent disability. SJGH earned the award by meeting not only the specific quality achievement measures for the diagnosis and treatment of stroke patients, but by performing at set levels for a designated period of time.   The quality measures set forth are designed to help hospital teams provide the

most up-to-date, evidence-based guidelines with the goal of a quicker recovery and reduced instances of death and disability for stroke patients. “With a stroke, time lost is brain lost, and this award demonstrates our commitment to ensuring patients receive care based on nationally respected clinical guidelines,” says David Culberson, CEO of SJGH.

Healthy teeth... long happy life

Leading researchers have long touted the benefits of well cared for healthy teeth, to include extending life by up to 15 years. However, rarely does the general public hear about the psychological benefits of a great smile. Americans spend countless hours ensuring they get regular exercise and eat properly. How often do we think of the importance of our smile? It is the most powerful and positive form of communication we have in defining us when we interact with others. A confident smile makes everyone around you comfortable and lets them know that you are healthy confident and optimistic. Many people are unhappy with their smiles and this can unknowingly lead to a self-conscious or unknowing lack of confidence and awkwardness in public. Are you someone who tries to hide your smile? I interviewed Dr. Maywell L. Inong, a leading Stockton dentist and private practitioner at the head of her field in smile rehabilitation and oral health technology research. Arriving at her office I found a cozy waiting room. It is what you would expect from a small private practice that spends more dedicated time on the individual care of their patients. On my tour I found each of the two exam rooms sported comfortable leather dental chairs with built-in neck and back massagers! After a hectic commute on I-5’s endless construction projects I am tempted to ask the doctor if we can do our interview with me in the dental chair. I manage to hold back my request for now and instead make it to her back office for our meeting. I asked Dr. Inong what types of her treatments are most common for someone that would like to rehabilitate their smile and correspondingly their health and confidence? The doctor stated that for many, porcelain veneers and crowns can be used to transform

twisted, overlapping or crooked teeth into a perfect smile. To straighten front teeth simple orthodontic care may be used which consists of ceramic brackets that only need to remain in place for just a few short months. This is a great option for adults who are mainly concerned about their front teeth and do not want to have long-term conventional braces. Invisalign can also be a great option to straighten teeth without the use of conventional braces. Another way to enhance your smile is to change the appearance of teeth by replacing discolored fillings and crowns. This can be done by replacing older worn out dental work with a tooth-colored composite material or porcelain. Many patients are asking to change out old silver filings with newer more natural materials. Dr. Inong stated that she can also change the shape of teeth, close spaces, change tooth color and straighten crooked teeth with veneers or bonding. As the season for Stockton’s new professional hockey team the Stockton Heat is well underway I simply had to ask the doctor about missing teeth, something hockey players have in abundance! She stated that missing teeth not only affects your smile but also your oral health as it can lead to changes in your mouth by causing teeth to shift, which in turn can cause pain in the jaw muscles and joints. These types of issues may be easily treated with implants or bridges without others ever suspecting real teeth were ever missing. Dr. Inong smiles as she says, “If you ever see a professional hockey player off the ice after the game in a nice suit giving interviews with all of their teeth magically present and accounted for, it would be due to the modern miracle of implants and bridges!” Creating a healthy smile can also address serious oral health problems associated with the gum tissue and gum recession. if not addressed usually leads to tooth loss and more serious health issues later in life. Dr. Inong offers her patients a new medical technology called Pinhole Surgical Technique (“PST”) in order to bring recessed gum tissue back to its normal healthy position. Unlike traditional gum repair, PST requires no painful gum grafts! PST instead uses collagen to help the body regenerate its own gum tissue. Doctors Hospital of Manteca strives to exceed community need, keep abreast of the latest technology, and offer the community quality, personal care.

With this mission in mind, over the last decade, Doctors Hospital has grown to offer many new medical services at their Manteca Wound Center, Imaging Center, and Surgery Center.   The Wound Center, an outpatient program that focuses on chronic and non-healing wounds, uses advanced treatment methods to help improve patients’ quality of life. The center offers Hyperbaric Oxygen Therapy (HBOT), a treatment that exposes patients to 100 percent oxygen to breathe, 62

while under pressure in a hyperbaric chamber. This combination of pure oxygen and pressure dissolves more oxygen in your bloodstream, reaching troublesome wounds that are in need. This treatment is monumental in the healing process for those with diabetes that face a slow or non-healing wound, someone that has undergone radiation therapy, has a skin graft with low blood supply, or anyone with a wound that has shown no improvement in a four week period.    To further serve the community, Doctors Hospital opened their Manteca Imaging Center in 2012. This center has both patients’ comfort and health in mind, and houses a High Field Open MRI, PET/

CT and ultrasound scans, and a walk-in, digital lab. “The open MRI is set up so that if a child needs an MRI, the parents can sit with them throughout,” explains Carmen Silva, Doctors Hospitals’ Chief Operating Officer. “The machine also has a great stereo system, so you can bring your own music. It gives a personal touch.”    Their newest addition, the Manteca Surgery Center fills a serious community void, because, as Carmen explains, many residents had to travel outside of the area for out patient surgeries.    “The center is an easy and convenient place for out patient surgery, and you can avoid the hospital environment,” she continues. The Surgery Center is equipped with gurney operating room tables, an Infusion Room for IV therapies, and the Alcon LenSx Laser System (an advanced laser-surgical system for eye surgeries).   Though Doctors Hospital of Manteca has over 50 years of high quality health care, the hunger to grow, expand, and evolve never ceases as the hospital continues to nurture community need.

Periodontal disease, also known as gum disease, is the major cause of tooth loss in adults and children. There are several types and stages of the disease, all of which start with an infection of the gums that can move into the bones and ligaments that support the teeth. If you plan to make your teeth last a lifetime, it’s important to understand the causes, treatment, and prevention of periodontal disease. What causes Periodontal Disease? When plaque is not removed from the teeth regularly, it forms a hard, porous substance called calculus, or tartar. If calculus forms on the roots of the teeth below the gum line, it irritates the gums even further and contributes to even more plaque collection and disease. How can Periodontal Disease be treated? New advances in medications for the treatment of periodontal disease exist. The best method is to schedule an appointment with your dentist, and

follow his/her advice. If severe disease exists, a specialist may be necessary. Your dentist will know how to treat your disease. How can Periodontal Disease be prevented? The best way to prevent periodontal disease is to practice good oral hygiene. It is never too early to start prevention. SURVIVING Co-RETIREMENT How To Plan For and Enjoy Life Together  BY NORA HESTON TARTE Retirement is an adjustment. It’s freedom—freedom to do as you please, live where you want, and spend unlimited hours with your significant other—masked as the light at the end of a very long tunnel.

But this causes some people to overlook the challenges they are hurtling toward. Transitioning from a life of evenings and weekends spent together to a 24/7 one-on-one relationship can be a startling wake-up call for couples. Don’t be blindsided. Instead, prepare for the emotional aspect of retirement as you would prepare for the financials. Communication and proper planning is key.

Talk it out:

Retirees have the sudden ability to do what they’ve always dreamed of, and live where they’ve always wanted to live. Does a bungalow on the beach sound like pure bliss? A cottage on the lake? A suburban home close to grandkids? Any choice you make is fine, as long as your wants align with your partner’s. A 2013 field study revealed that nearly 40 percent of couples disagreed on the retirement lifestyle they desired.   So many couples get caught up in what they want their own retirement to look like that they forget their spouse may have different ideals. Get ahead of the problem and start planning for the emotional shift while you’re planning for the financial one. It’ll give you time to compromise and adjust expectations for a more seamless transition.    It’s also important to note that approximately 80 percent of couples do not retire at the same time. That means there will be at least some period of time where you’ll be at different points in your life—a first for many couples. If this is the case, don’t fret; just make it part of the plan. Calculate when each of you will retire and pay special attention to how you’ll handle these transitions. 98

Have hobbies:

Couples who want to spend their retirements traveling, golfing, gardening, and spending all of those lost hours together may welcome the change in schedule (or sudden lack thereof), but consider how spending 24 hours each day together may affect your relationship. If the idea is daunting, make plans to spend time apart. In addition to shared passions, have a few of your own—whether that’s a weekly lunch with the girls, or a round of golf with old colleagues, make it a “no spouses” space.


Keep working (if you want to):

This may sound contradictive of retirement, but some people (mainly men) grieve the loss of their careers. Even if they have been counting down the years, they may be hit with a sudden need to mourn their former life. This gender-specific emotion seems to stem from the idea that men are more focused on their work, where as women, who are natural multitaskers, see their careers as just one aspect of their lives.   If your job offers a chance to serve on a board and attend monthly meetings, it could be a good transition for those nervous about leaving. Other options include picking up part time work, clocking volunteer hours, or participating in community events and organizations you feel partial to. Retirement is freedom—and that includes the freedom to work if you want it to.

Take it slow (and take it all in):

Don’t rush to check off every item on your bucket list. You have time. Leave room for the unexpected, and to adjust to your new life—together. It’s impossible to know how retirement will affect you until you live it. Published by San Joaquin Magazine

Talking loss of privilege with your aging parents  BY NORA HESTON TARTE Consider feelings

Whether the discussion is about losing their driver’s license for safety reasons, or transitioning to a senior care facility, the core of the problem is the same. Your parent is getting older and they can no longer complete basic tasks themselves. “It is important to be empathetic and patient when having these types of conversations. Children should make sure their parents know that they are still loved and still have a great deal to offer their family and society,” Melanie Toutai, director of business development at Arbor Rehabilitation & Nursing Center, explains. “Just because they need more help doesn’t make them worth any less.”

Reduce guilt

While the added responsibilities may not be weighing on your mind, they may plague your parent’s. Seniors don’t want to become a burden on your life by requiring rides to the grocery store or financial help to afford a living facility that has become necessary. Public transportation can be a suitable solution, but approach the topic carefully. And of course consider if public transit is really a viable solution. When memory problems are also present, it can create more issues than it solves.

Don’t change what you don’t have to

Too much change at once can be overwhelming. When a medical diagnosis is given, children may rush to 104

make all of the necessary arrangements to unburden their parents as quickly as possible, but seniors need time to adjust to their new limitations. When time permits, ease into changes. “Even if an elderly parent needs to move into a place where more assistance is offered, some things don’t have to change. Families should encourage activities and ways to stay connected such as family outings and visits,” Melanie explains.

Don’t take over

Let your parent make choices as long as they are lucid enough to do so. In a time when they feel they are losing their sense of self, being in control will help them cope. When it comes to where they will live, give them a say. If they are adamant about paying for a professional driver instead of calling you every time they need a ride, let them. They are still their own person, and your parent.

As people age—their body undergoes a variety of changes—from decreased mobility and eyesight to loss of mem-

ory. Transitioning from child to caregiver is hard, but when a major change is necessary in your aging parent’s life, it’s important to take it seriously. Loss of freedom is painful. It is a reminder of the inevitable aging process and requires seniors to turn to others for help—whether it is you, or another caregiver.
