7.16.2019 Dear Residents & Family members of Keiro Rehab and Care Center (KRCC), You are receiving this letter because you or someone you are responsible for is a resident at Keiro NW

Rehab and Care Center. This letter is to advise you that the Board of Directors has made the difficult decision to discontinue operations due to the continued financial pressures of operating as a skilled

nursing facility. This letter serves as an official 60 day notice. Written notice of impending closure has also been sent to the Department of Social and Health Services (DSHS), Center of Medicare and Medicaid Services (CMS) and WA LTC Ombudsman Program.

The facility closure plan has been reviewed and approved by DSHS. Be assured that all residents will be given ample support and time to make decisions about relocation to a new residence. Residents will be transferred to the most appropriate facility or other setting in terms of quality, services, and location, taking into consideration the needs, choice, and best interests of each resident. Residents have the right to choose where they would like to be transferred. No new residents will be admitted to the facility effective the first day of the 60 day notice, and CMS has been notified of KRCC’s intent to voluntary terminate Medicare and Medicaid contracts. 60 Day Regulatory Requirement Under federal law, nursing homes are required to provide residents and legal representatives of residents a 60 day notice prior to impending closure. Resources available to assist with the closure and transition are Home & Community Service (HCS), Residential Care Services (RCS) and State LTC Ombudsman Office. Along with the Keiro NW staff, each agency will be working closely with residents and families to transition residents to appropriate long term care settings. The Home and Community Services (HCS) Division promotes, plans, develops and provides longterm care services for persons with disabilities and the elderly who may need state funds (Medicaid) to help pay for them.
