e plan for the expected, not the unexpected. Few of us determine in advance which oncologist to see if we develop cancer, which endocrinologist to visit in the event we get diabetes, which neurologist to consult if epilepsy creeps into our lives. Working with healthcare providers my entire life made me feel I was prepared and ready for any medical need. I was confident I could find the right doctor if and when I needed one. What I didn’t anticipate was how overwhelmed I would feel trying to find the best care and how hard it would be to feel comfortable when making a decision that affects my child. Several years ago, I was told to seek the opinions of specialists for my daughter. What a daunting task. I had worked with every type of pediatric specialist before, but still felt incredibly overcome. • Where do I start? • How do I find the best?

At Bergen Medical Associates, we are owned and operated by physicians. Our approach to medicine puts the doctor-patient relationship front and center. We strive to offer excellent customer service and provide high-quality care. Having many types of different doctors and specialties allows for tremendous flexibility in our services. Comprehensive care is offered, covering various aspects of a given condition. For example, patients with diabetes can have endocrinology, cardiology, podiatry and nutrition in one place, while gastrointestinal patients have a similar opportunity with GI, colorectal surgery and nutrition.
Having numerous locations in Emerson, Paramus, Montvale, Midland Park, Northvale, Ridgewood, and Pearl River, NY, our physicians are able to provide care to patients when they need it. We are able to accommodate patients on short notice due to our extended office hours, which include evening and weekend hours. Our doctors accept most insurance plans so we can maintain relationships with our current patients and accommodate newcomers.
Q: What are the key challenges within your practice? Advancements in medical technology have allowed us to better diagnose and treat our patients. Innovation and the continuous development of technology constantly challenge us to find ways to improve our practice and patient care. As technology in the world of healthcare continues to evolve, rules and regulations concerning its use must be established and adjusted to adapt to the new methods of administering care. Ensuring safety when choosing to implement and integrate new products and technologies into our practice is a top priority.

Patients can take comfort that when the exam door closes, they have our complete attention. We approach patient care by understanding not only the biology of the patients’ conditions, but also the various components of their life that need to be considered when developing a treatment plan. In addition to thinking about the present issues, we anticipate future ones that may be associated with the disease or the treatment being selected. Because of this, we are able to arrive more judiciously at a treatment plan. All of our doctors always end an office visit with “Do you have any questions?” and “Are you comfortable with the plan?” It’s an exciting time with the emergence of personalized medicine. In inflammatory bowel disease, there are blood tests that help determine future prognosis and need for surgery, plus lab tests that can noninvasively monitor disease activity and response to medication. Some particular new technologies that we use include gene testing for hereditary cancer syndromes and stool transplant to treat C. difficile infections.

Q: How do you approach patient care and concerns?
We always put our patients’ needs first. Our team is committed to helping patients conquer their pain, overcome disease and revitalize damaged bones, joints and soft tissues. Q: What are the key challenges you face?
One is trying to communicate new technologies to patients who come for second opinions. Sometimes the patient will question my skills and expertise because I am the only one providing this type of treatment. Patients can doubt the legitimacy of these technologies since they are more familiar with doctors who are set in their ways. Another key challenge are the changes in the healthcare climate, especially with insurance companies pushing us to follow their rules and guidelines, which are not always in the patient’s best interest.
Q: What is most impressive about your training and education?
Prior to opening the Kayal Orthopaedic Center, I earned my medical degree from UMDNJ–NJMS (now Rutgers New Jersey Medical School) and then completed a five-year orthopaedic surgery training in New York. I am a board-certified orthopaedic surgeon and a Fellow of the American Academy of Orthopaedic Surgeons.
Q: Do you use any new technology at your practice?
During my time at the Kayal Orthopaedic Center, I’ve pioneered numerous technologies, including a cutting-edge advancement in customized total knee-replacement surgery. To complete the procedure, I use Zimmer® Patient Specific Instrumentation (PSI) and MRI technology to create a three-dimensional blueprint of a patient’s arthritic knee. Essentially, this PSI technology enables me to perform a virtual knee replacement in advance of surgery and then create a new knee designed specifically for a patient’s body and lifestyle. This procedure not only speeds rehabilitation and optimizes surgical outcomes, but also greatly improves patient satisfaction.
Q: What do you enjoy most about your job?
I firmly believe that being an orthopaedic surgeon is, without question, the most rewarding profession. Each day, I’m given the opportunity to develop strong relationships with people, change their lives and restore their quality of life.

From the initial consultation to subsequent follow-up visits, each doctor is personally involved in seeing and caring for patients. During the initial visit, by taking a careful history, performing a thorough physical exam and utilizing the most advanced imaging techniques, we are able to make an accurate diagnosis. This allows us to define an appropriate course of treatment for each patient, and because each patient is unique, this treatment is specific for each one and their condition.
Q: How do you approach patient care and concerns?
Thorough patient evaluation and a focus on continuing education are paramount in our practice. Our doctors are conservative by nature and exhaust non-operative treatment options before considering surgical intervention. Each patient participates in the decision-making process regarding their care, and treatment options are tailored to meet their needs. In order to address patient concerns and to expedite care, our office has three clinical staff members who under the doctor’s direction help achieve this goal.

A key challenge is taking the time to properly educate our patients about their conditions and the treatment options available to them. We work hard to remain current with the latest literature and technologies and pass this information on to patients whom we help educate during their consultations.

Q: What is most impressive about your training and education?
Our physicians at NJSS have all completed orthopedic residencies followed by additional fellowship training specializing in spinal surgery. Dr. Reiter completed an accelerated six-year combined BS/MD honors program and then an orthopedic residency, both at the University of Miami. He completed his spine fellowship training at Emory University. Dr. Drzala graduated NJ Medical School at UMDNJ, where he completed his orthopedic residency. He did his spine fellowship training at the St. Mary’s Spine Fellowship program in San Francisco.
Q: Do you use new technology at your practice?
We are very cautious before introducing any new procedure or technology into our practice, and only after a favorable safety profile and improved efficacy have been clearly demonstrated. We use minimally invasive techniques to help speed the recovery process and favor motion preparation technologies that may help to minimize future problems in the spine as time passes.
Q: What do you enjoy most about your job?
We enjoy the daily interaction we have with our patients and value our relationships with them. We recognize that it is a privilege to care for others and do not take this for granted. We always do our best to help those who are suffering with ailments of the spine. There is no better feeling than knowing a patient has benefited from our care.

It’s our combination of 19 years in practice and large volume of highly sophisticated surgery with a demanding standard for excellent outcomes, blended with a skilled, professional and friendly support staff.
Q: How do you educate your patients about procedures? Patient education sets us apart. Patients are sent packets that contain information about their concerns as well as about our staff and office. The consultation lasts 45 to 90 minutes and includes a thorough exam and dialogue about the procedures they’re interested in. Patients gain knowledge from my numerous blogs, tweets and written descriptions that accompany the thousands of before-and-after images on our extensive website. All patients return prior to surgery to review everything and answer all questions.
Q: Do you perform surgeries in-house or elsewhere?
All surgeries requiring more than local anesthetic are performed either across the street from our office at St. Barnabas Medical Center or down the street at the Ambulatory Care Center.
Q: What is most impressive about your training and education?
I spent eight years in residency and fellowship from 1988 to 1996 training with world-famous surgeons and plastic surgeons in highly coveted, competitive programs.
Q: What steps do you take to ensure the necessary safety standards?
I have two full-time registered nurses, unheard of in plastic surgery. I perform all non-local anesthesia surgery at hospital-based surgery centers where patients get the best anesthesiologists and surgical nursing staff. We see patients frequently after surgery and are available 24/7.
Q: Do you offer any new noninvasive procedures or alternatives?

HELPFUL TIPS WHEN CHOOSING A DOCTOR

hat do people consider the single most important factor when choosing a doctor? The answer might surprise you. Based on a 2014 survey by the Associated Press and the NORC Center for Public Affairs Research, more than 50 percent of respondents said the physician’s personality is most important. Delivery of care was a distant 29 percent. However, these two factors may not be as mutually exclusive as you think. Physicians who are personable, friendly and don’t interrupt when listening to you also can be highly skilled in healthcare delivery. Going to the doctor makes many people feel uncomfortable, and talking to a physician who is open and friendly can put them at ease, which has genuine benefits during the office visit. Consider “white-

coat syndrome,” the fear of doctors, tests or needles that can hinder preventive healthcare. A patient who gets anxious when meeting with a doctor runs the risk of having a higher blood pressure reading than if he or she remains calm. The last thing you want is to be diagnosed with a condition you really don’t have because you were nervous and uncomfortable during an exam. It doesn’t take much to help patients relax, either. According to Derek A. Preece, president of Enhancement Dynamics Inc., a practice management consultancy, “Patients like it when a doctor greets them by their name and remembers their hobbies [and other things about them]. This reassures patients that they are getting individualized care.” Besides personality and delivery of care, there are other critical factors to consider when choosing your doctor.

Determine your needs. “Do you have special medical needs, such as cardiac problems or rheumatology problems, or do you just need routine checkups?” asks Dr. Pamela F. Gallin, author of How to Survive Your Doctor’s Care. “Do you have diabetes? Does your lifestyle put you in a certain category of risk?” Consider choosing a primary-care physician who also is certified in the subspecialty that matches your ailment or specific area of concern. Many women choose a gynecologist as their regular doctor.

Make sure the doctor is in your insurer’s network. Your insurer’s list of approved doctors can vary over time, so it’s a good idea to call the doctor’s office to verify coverage before making an appointment.

Many people rely on word of mouth when selecting a physician. Although you shouldn’t rule out references from family and friends, a doctor who is right for someone you know might not be right for you. You wouldn’t buy a car or choose a college for your child without doing your research or checking online consumer ratings. So why not take the same steps when choosing a doctor, one of the most important decisions of your life? Popular physician-rating websites include Vitals and Healthgrades. And of course there is the “Book of Lists” you hold in your hands.

Choose a primary-care doctor who not only can help you with day-to-day health problems but also can refer you to specialists. If you have a primary-care physician you trust, you will be more comfortable about seeing the specialists they recommend. “It may seem like a burden or restrictive to have to choose a primary-care provider,” says Orly Avitzur, M.D., medical adviser to Consumer Reports. “But everyone needs a project manager, so to speak, to help you navigate our confusing healthcare system.”

Contact your state’s medical board to confirm the physician’s license is valid. Make sure the doctor is board certified through the American Board of Medical Specialties. You can check by going to certificationmatters.org.

Check whether the doctor has evening and weekend hours, whether he or she can schedule same-day appointments if you require urgent care, and how long patients are kept in the waiting room. Many doctors can schedule an appointment within a week of your phone call. Typically, the wait for annual checkups is longer, since these require more of the doctor’s time than other visits.

Assess the support staff. They schedule appointments, check you in and out of the office, and often meet with you prior to your doctor to ask questions and take your blood pressure. Are they professional, friendly and attentive? You also should feel comfortable discussing with your doctor the same questions asked by support staff and having him or her take your blood pressure as well, especially if you or your family have a history of hypertension.

Check for malpractice and other claims by consulting state medical boards. If you see a pattern, you may want to consider another provider. If the doctor has been cited for substance abuse, be aware that many states allow physicians to practice while they are receiving treatment.

Determine the doctor’s hospital affiliation and its location. There are plenty of online state-by-state listings that can tell you how the hospital compares to other health facilities in your area. Check to see if the hospital is renowned for treating the specific disorder or ailment you or your family are seeking treatment for.

If you’re not insured or the physician you prefer is not part of your health plan, see if you can negotiate fees. Some health plans are not nearly as strong in the choices they offer for mental and oral care, so you may want to go out of network. However, bear in mind you likely will have to pay in full for all medications, lab tests and procedures such as X-rays. Fortunately, many hospital clinics and community centers that supply these services charge on a sliding scale based on the patient’s economic situation.

Beginning at age 50, both men and women should have a colonoscopy every 10 years.
PROSTATE CANCER
Starting at age 50, men should talk to a doctor about the pros and cons of testing so they can decide if it is the right choice for them. If they are African-American or have a father or brother who had prostate cancer before age 65, men should have this talk with a doctor starting at age 45. How often men are tested will depend on their PSA level.
BREAST CANCER
Yearly mammograms should start at age 40 and continue for as long as a woman is in good health. Clinical breast exams should be done about every three years for women in their 20s and 30s and every year for women 40 and older.

e’re all familiar with Ben Franklin’s quote “An ounce of prevention is worth a pound of cure.” What many people don’t realize is Franklin was referring to fire safety when he coined his famous phrase. With the transition from candles and gas-lit lamps to electricity, disastrous fires, though still a concern, thankfully are not so common today as they were in Franklin’s time. However, his sage advice still bears listening to. Today, many believe it was healthcare, not fire safety, he was referencing. And why not? For many years healthcare has been at or near the top of the list of Americans' biggest concerns. Whatever one’s feelings about ObamaCare and the fact that the U.S. is the only civilized western nation that does not provide its citizens with national healthcare, everyone agrees preventive care, especially when it comes to cancer, is crucial to staying healthy and the best way to obviate the need for Franklin’s proverbial “pound of cure.” Experts say the key to prevention are diagnostic procedures that help in early detection. Following are procedures for early detection of certain cancers. Included are recommended age ranges and frequencies.
The American Cancer Society does not recommend tests to screen for lung cancer in people who are at average risk of this disease. However, here are the guidelines for individuals who are at high risk of lung cancer due to cigarette smoking: 55 to 74 years of age, in fairly good health and have at least a 30 packyear smoking history AND are either still smoking or have quit smoking within the last 15 years.
CERVICAL CANCER
Screening should begin at age 21. Women under age 21 should not be tested. Women between ages 21 and 29 should have a Pap test every three years. HPV testing should not be used in this age group unless it is needed after an abnormal Pap test result. The preferred approach is for women between the ages of 30 and 65 to have a Pap test plus an HPV test every five years. Women over age 65 who have had regular cervical cancer testing with normal results should not be tested for cervical cancer. Women with a history of a serious cervical pre-cancer should continue to be tested for at least 20 years after that diagnosis, even if testing continues past age 65.

Q: What inspired you to pursue cosmetic dentistry? I was always a good student in all subjects, which made it difficult for me to decide on a career path. I had an aptitude for math and science and wanted to pursue a career in the medical field. However, I also wanted to be an artist. Orthodontics has been a great choice for me as it allows me to do both. I love having to come up with a plan to fix a patient’s bite while at the same time focusing on an aesthetic outcome.
Q: How do you stay current with technology? I attend many continuing-education courses and read the latest journals on orthodontics to keep abreast with the latest techniques and advances in treatment.
Q: What is the most rewarding aspect of your job? I love the dual nature of my profession, the fact that it’s both an art and a science. Q: Do you use any new technology?
Doctors at our centers learn the latest cutting-edge techniques to resolve even the toughest spine and health-related conditions. Both of our facilities have recently acquired a laser-assisted, bipedal analysis computer system to evaluate lower-extremity influence on gait. Imbalances are digitally reformatted and orthotic biomechanic corrections are utilized to enhance the efficacy of spinal therapy.
